ABSTRACT

Abstract The aim of this study was to evaluate the root canal shaping effect of ProTaper Gold (PTG) versus ProTaper Next (PTN) instrumentation systems, and of a manual #15 K-type file (K15) versus the ProGlider (PG) mechanized instrument for glide path creation, in severely curved mesial canals. Twenty-four mandibular molars with two separate mesial canals were anatomically matched using computed tomographic scanning, and then divided into two groups (n=12) according to the glide path instrument used, either K15 or PG. In all teeth, the PTG system was used to prepare the mesiobuccal canal, and the PTN, the mesiolingual canal. The teeth were scanned by computed microtomography, before and after root canal preparation, and the values of the initial volume, final volume, volumetric variation, untouched walls, and canal transportation variables were determined. The data were analyzed using the two-way ANOVA test, and the Tukey test for multiple comparisons. There was no significant difference among the study groups regarding volumetric variation or root canal transportation, either in the cervical, middle or apical thirds, or in the entire root canal (p>0.05). In the apical third, the percentage of untouched walls was significantly higher in groups using K15 than in those using PG (p<0.05), namely 33.144% and 23.285%, respectively, irrespective of the instrumentation system. In the other regions, there was no difference between K15 and PG regarding this variable. It was concluded that PG was associated with a lower rate of untouched walls in the apical region than K15.

Abstract This study evaluated the correlation between root canal curvature and the effects of ultrasonic irrigation in the following parameters: volume of uncontrolled dentin removal (UDRVol), maximum depth of dentin defects, removal of accumulated hard tissue debris (AHTD), and canal transportation in prepared curved root canals. Twenty-four human permanent mandibular molars were divided into two groups according to root canal curvature: moderate curvature (MC: mean 25°); and severe curvature (SC: mean 48°). The specimens were scanned using an X-ray microcomputed scanner (Skyscan 1172) before and after cleaning and shaping and after the final irrigation protocol with ultrasonic irrigation. There was a moderate correlation between the degree of root canal curvature and the volume of remaining AHTD (p<0.05) and between the degree of root canal curvature and maximum depth of defects due to uncontrolled removal of dentin (p<0.05). The teeth in the SC group had a greater maximum depth of defects on the dentin wall in the apical third than the teeth in the MC group (p <0.05). Both groups had a significant reduction of AHTD in all canal thirds, but the amount of remaining AHTD in the middle and apical thirds and the whole canal was significantly greater in the SC than in the MC group (p <0.05). Canal transportation was not influenced by the canal curvature in all thirds (p >0.05). This study concluded that root canal curvature affects significantly the uncontrolled removal of dentin and remaining AHTD volume after the final irrigation protocol with ultrasonic irrigation.


Resumo Este estudo avaliou a correlação entre a curvatura do canal radicular e os efeitos da irrigação ultrassônica nos seguintes parâmetros: volume de remoção não controlada de dentina (UDRVol), profundidade máxima do desgaste de dentina, remoção de debris de tecido duro acumulado (AHTD) e transporte em canais radiculares curvos. Vinte e quatro molares inferiores humanos permanentes foram divididos em dois grupos de acordo com a curvatura do canal radicular: curvatura moderada (MC: média de 25°); e curvatura acentuada (SC: média de 48°). Os espécimes foram digitalizados usando um scanner microcomputador de raios X (Skyscan 1172) antes e depois da limpeza e modelagem e após a irrigação ultrassônica. Houve uma correlação moderada entre o grau de curvatura do canal radicular e o volume de AHTD remanescente (p<0,05) e entre o grau de curvatura do canal radicular e a profundidade máxima de defeitos devido à remoção descontrolada de dentina (p<0,05). Os dentes do grupo SC apresentaram maior profundidade máxima de defeitos na parede dentinária no terço apical do que os dentes do grupo MC (p<0,05). Ambos os grupos tiveram uma redução significativa de AHTD em todos os terços do canal, mas a quantidade de AHTD remanescente nos terços médio e apical e em todo o canal foi significativamente maior no grupo SC do que no grupo MC (p <0,05). O transporte do canal não foi influenciado pela curvatura do canal em todos os terços (p > 0,05). Este estudo concluiu que a curvatura do canal radicular afeta significativamente a remoção descontrolada de dentina e o volume remanescente de AHTD após a irrigação ultrassônica.

ABSTRACT

Resumo O objetivo deste estudo é investigar a associação entre iniquidades raciais e condição de saúde bucal. Trata-se de revisão sistemática com protocolo cadastrado na plataforma prospero (CRD42021228417), com buscas realizadas em bases de dados eletrônicas e na literatura cinzenta. Identificou-se 3.028 publicações e após aplicação dos critérios de elegibilidade e análise do risco de vieses, 18 estudos foram selecionados. Os resultados indicam que indivíduos de raça/cor da pele preta/parda apresentam condições de saúde bucal desfavorável, representada principalmente pela autoavaliação de saúde bucal, perda dentária, cárie e periodontite. Os resultados evidenciaram iniquidades raciais em saúde bucal em diferentes países, para todos os indicadores analisados, com maior vulnerabilidade da população negra.


Abstract The present study aimed to investigate the association between racial iniquities and oral health status. This is a systematic review with a protocol registered on the Prospero Platform (CRD42021228417), with searches carried out in electronic databases and in gray literature. Our study identified 3,028 publications. After applying the eligibility criteria and risk of bias analysis, 18 studies were selected. The results indicate that individuals of black/brown race/skin color have unfavorable oral health conditions, mainly represented by self-rated oral health, tooth loss, caries, and periodontitis. The results showed racial iniquities in oral health in different countries, for all analyzed indicators, with a greater vulnerability of the black population.

SUMMARY: The livers of reptiles are being studied as a model for the link between the environment and hepatic tissue. There have been few investigations on the histology of reptile livers, and very few or no studies have examined the histology of liver of veiled chameleon (Chamaeleo calyptratus). This paper describes the histomorphological, histochemical and ultrastructural characterization of the liver of veiled chameleons in southern Saudi Arabia. Seven Chamaeleo calyptratus were captured in the summer season in Abha City, Aseer region, southern Saudi Arabia. Chamaeleon liver samples were processed for histomorphology, histochemistry and ultrastructure analyses. Morphologically liver of Chamaeleo calyptratus was observed as a large dark brown organ with lighter speckles, which represent melanin deposits. It located at the ventral part of abdominal cavity forward of the stomach. Its dimensions approximately were 3.7 x 2 cm. The liver was a bilobed organ divided into two lobes, right and left lobes. The right one was bigger than the others. The gallbladder was well developed and had an elongated shape, situated between the two lobes and contained the bile for the digestion. Microscopically, the liver was found to be covered by a thick layer of connective tissue, which formed the hepatic capsule. Hepatic parenchyma probably appeared in cross sections as hepatic glandular-like alveoli "acini" or follicular structures with various diameters, each acinus contains approximately four to six hepatocytes, surrounded by sinusoidal capillaries filled with abundant melanomacrophages, which are absent in birds and mammals. Melanomacrophages are common in the hepatic parenchyma's perisinusoidal areas, particularly near portal spaces. Hepatocytes are polyhedral or pyramidal with and mostly contained large, rounded nuclei mostly peripherally located, with prominent dark oval nucleoli. Some of nuclei are eccentric or central position. The cytoplasm appeared spongy or vacuolated and more eosinophilic when stained by hematoxylin-eosin and strongly reactive to PAS staining technique, indicating abundant glycogen content. The reticular fibers that surround hepatocytes, blood arteries, and sinusoids supported the hepatic parenchyma. The blood sinusoids are seen interspersed among hepatocytes of varying sizes. The sinusoidal lumen was bordered by flattened endothelial cells and includes elliptical nucleated erythrocytes and liver macrophages as phagocytes, which are also known as Kupffer cells. Branches of the portal vein, hepatic artery, small bile duct, and lymph vessels were detected in the hepatic portal area "tract" or triad which made up of connective. Hematopoietic tissue was observed in subcapsular region and portal triads. Ultrastructurally, the hepatocyte appeared polyhedric containing a single large rounded basal or eccentric vesicular nucleus with prominent nucleolus. Extensive network of rough endoplasmic reticulum (RER) often arranged in an array parallel to the nuclear membrane with many mitochondria, and Golgi apparatus were described. The cytoplasm contained glycogen granules, vesicles or vacuoles scattered throughout the cytoplasm especially at the apical region were reported. The bile canaliculi and the hepatic "Kupffer" cells were also discussed. This is the first study on the histological characterization of the healthy liver of Yemen veiled chameleon in southern Saudi Arabia. The findings reported here should be used as a reference to compare with the pathological abnormalities of the liver in this animal.

ABSTRACT

Background: Neoadjuvant chemotherapy (NACT) in carcinoma stomach was introduced in an effort to eliminate micro-metastasis and to improve resectablity before surgery which improves R0 resection rates. The aim of the current study was to evaluate the effect of neoadjuvant chemotherapy on downstaging and resectability rate in locally advanced gastric cancer. Material & Methods: This was a single-center quasi-experimental study conducted in the Department of Surgical Oncology in collaboration with the Departments of Medical Oncology, Radiation oncology, and Pathology at the National Institute of Cancer Research and hospital, Dhaka, which is a tertiary care cancer hospital in Bangladesh, between January 2021 and June 2022.Patients with locally advanced adenocarcinoma stomach staged by contrast-enhanced computed tomography (CECT) were randomly included in this study by purposive sampling. Patients in Group I underwent upfront surgery Patients in Group II were started on neoadjuvant chemotherapy, either XELOX or FLOT regimen. Surgery was done following the response assessment CECT. We assessed R0 resection rate, age, sex, comorbidities, tumour size, TNM stage and complications were compared between the two groups. Response to NACT was assessed in Group II. Results: The mean age of patients in groups 1 & 2 was 56 ± 11.06 and 55.70 ± 10.46 years of age respectively (p > 0.05). Majority of the respondents (55/74) were male and 19 patients (26%) were female. Male to female ratio was (24/37 &31/37) in group 1 and (31/37 & 6/37) groups respectively (p > 0.05). Out of 37 patients who received NACT, in 9 patients (24.32%) complete response was noted. Partial response was found in 20 cases (54.05%), p-value (<.0001) while a stable disease was reported in three (8.1%) cases. 5 patients (13.51%) had progressive disease. In the upfront surgery group, R0 resection was feasible in 16 (43.2%) cases, and in the NACT plus surgery group, R0 resection was done in 29 (78.4%) cases. In group 1, R1 resection was done in considerable numbers (19/37) compared to group 2 (5/37), P=0.001. Three patients (8.1%) in group 2 and one (2.7) in group 1 had irresectable lesions. Conclusion: In this study it can be concluded that neoadjuvant chemotherapy could downstage tumour and increase tumor resectability rate in patients with locally-advanced gastric adenocarcinoma. However, further studies are necessary to confirm the effect of this modality on patients’ overall survival. We await survival analysis to further validate the role of NACT.

Objetivo: Investigar os fatores de risco intrínsecos para queda entre idosos de duas Instituições de Longa Permanência (ILP) no interior de Minas Gerais/Brasil. Métodos: Foram avaliados 20 idosos com idade média de 79 anos (entre 60-100 anos). Foram aplicadas as escalas de Tinetti e Barthel para avaliação do equilíbrio corporal e independência funcional, respectivamente. A escala de Downton para análise do risco de quedas; a estesiometria, a dinamometria e o teste manual de força muscular para estimar respectivamente, a sensibilidade das mãos e pés, a força de preensão palmar e a força dos músculos de membros inferiores. A análise estatística utilizada foi o teste t-student, o teste de correlação de Pearson, a análise de variância (ANOVA-one way), considerando nível de significância de 5%. Resultados: A média geral do escore da escala de Dowton foi de 4,68 (p<0,05); a do equilíbrio corporal foi de 14,57 pontos (p<0,05), a escala de Barthel foi de 72,36 pontos (p<0,05); a força de preensão palmar foi de 2,73 kg/m² (±3,64) e a média de força em membros inferiores foi de 3,7 kg/m2 (p<0,05). Conclusão: Conclui-se que os idosos avaliados apresentam alto risco de quedas sendo os parâmetros mais comprometidos e responsáveis por este risco, a polifarmácia, desequilíbrio, fraqueza muscular, perda de sensibilidade e dependência funcional.


Objective: The objective of the study is to investigate the intrinsic risk factors for falls among elderly people from two Long Stay Institutions (ILP) in the countryside of Minas Gerais/Brazil. Methods: Twenty elderly people with a mean age of 79 years (between 60-100 years) were evaluated. Tinetti and Barthel scales were applied to assess body balance and functional independence, respectively. The Downton scale for the analysis of the risk of falls; esthesiometry, dynamometry and manual muscle strength test to estimate, respectively, the sensitivity of the hands and feet, the hand grip strength and the strength of the muscles of the lower limbs. The statistical analysis used was the t-student test, Pearson's correlation test, analysis of variance (ANOVA-one way), considering a significance level of 5%. Results: The general mean score on the Dowton scale was 4.68 (p<0.05); that of body balance was 14.57 points (p<0.05), the Barthel scale was 72.36 points (p<0.05); the handgrip strength was 2.73 kg/m² (±3.64) and the mean strength in the lower limbs was 3.7 kg/m2 (p<0.05). Conclusion: It is concluded that the evaluated elderly have a high risk of falls and the parameters most compromised and responsible for this risk are polypharmacy, imbalance, muscle weakness, loss of sensitivity and functional dependence.

ABSTRACT

Background: Urinary tract infections (UTIs) are amongst the most common bacterial infections, affecting about 150 million individuals per year.Urinary tract infection (UTI) is one of the most common diseases in human societies which occur in women more than men. Women of all ages are more likely to experience such an infection than men, while half of them may be affected by a uropathogen once in their lives, and 25-30% may develop recurrent UTIs not associated with any functional or anatomical abnormality. The aim of this study was to explore the prevalence of antibiotic resistance of urinary tract infections in immunocompromised patients. Material & Methods: This was a retrospective observational study and was conducted in the Department of Medicine of Dhaka Central International Medical College and Hospital, and National Institute of Kidney Diseases & Urology, Dhaka, Bangladesh during the period from June, 2021 to June, 2022. We included 70 immunocompromised patients with UTI. Results: In our study we found 44% patients were aged between 18-30 years old and 83% of our patients were female. Among all respondents, 58 patients were culture positive. We found 38% & 62% and 33% & 67% had history of upper & lower UTI in female and male patients respectively, 29% & 36% and 17% & 25% had urethritis & cystitis in female and male patients respectively. We found culture positive patients with E. coli (41%), Klebsiella spp.(19%), Enterobacter spp.(16%) and 12% with Pseudomonas spp in our study. We found amoxicillin showed resistance to E. coli (58.3%), Staphylococcus (75%), & Klebsiella (36.4%) and amikacin showed resistance mostly to E. coli(50%), Klebsiella (54.5%) & Pseudomonas (42.9%). Conclusion: In our study, we found the most common MDR pathogen among patients with UTI in our institution was E. coli & Staphylococcus. We discovered that being above 40 years old, having DM, CKD and previously received antibiotic medication inappropriately were risk factors for the development of MDR infection. Given the high prevalence of multidrug resistant uropathogens among immunocompromised patients, the growth in Penicillin resistance is alarming. We discovered the significant incidence of MDR bacterial strains, adherence to currently suggested empiric therapy is extremely challenging and is linked with high failure rates.

ABSTRACT

O exame foi uma tecnologia disciplinar destacada por Michel Foucault desde os estudos realizados acerca do sistema carcerário. O exame se apresentou como um ponto de desvio nas reformas penais do século XVIII, conforme nos mostra Foucault, em que a lógica punitiva se afasta das propostas dos reformadores para voltar-se à correção dos infratores e ao controle das virtualidades. No Brasil, o denominado exame criminológico, instrumento de inserção do trabalho da psicologia no âmbito prisional, apresenta-se como a peça-chave dessa tecnologia de poder. Inserido na legislação nacional desde a Lei de Execuções Penais, datada de 1984, o exame criminológico se mantém resistente nas práticas penais e nas discussões legislativas, mesmo que tenha sido questionado em sua credibilidade e atestada, por pesquisas, a arbitrariedade das orientações presentes na prática dos laudos. Assim é que o presente artigo pretende fazer uma análise histórica e social da inserção do exame criminológico na legislação brasileira, assim como nas práticas judiciais, levando em consideração a permanência do instituto, mesmo após a retirada da previsão legal em 2003. A metodologia utilizada foi a análise do discurso com um viés pragmático e dialético, assim como utilizamos a pesquisa documental e qualitativa de leis e projetos de lei, além de pesquisa bibliográfica e análise comparativa.

ABSTRACT

O estudo descreveu as alterações comportamentais durante a pandemia de COVID-19 e sua asso-ciação com o sobrepeso e obesidade atual de crianças e adolescentes. Participaram do estudo 1.004 crianças e adolescentes de comunidades rurais em áreas de remanescentes de quilombos de Feira de Santana, Bahia (50,3% meninos; Idade: 9,04 ± 1,52 anos). Desfechos: sobrepeso e obesidade, com base nos valores de Índice de Massa Corporal para idade e sexo, avaliados nos pontos de corte da In-ternational Obesity Taskforce. Alterações comportamentais (apetite, sono, psicológicas, sociais) foram avaliadas por meio de questionário preenchido pelos pais. A análise de dados incluiu estatística des-critiva, bivariada e regressão de Poisson múltipla, com estimador robusto de variância, para estimar Razões de Prevalência (RP) e Intervalos de 95% de Confiança (IC95%). A significância estatística foi fixada em valores de p<0,05. Covariáveis: idade, sexo, infecção por COVID-19 e insegurança ali-mentar. Após o período pandêmico, 24% dos participantes estavam com excesso de peso (sobrepeso = 15,5%; obesidade: 8,5%). As principais alterações comportamentais ocorreram no apetite (comer muito mais do que o habitual = 45,6%) e no sono (ir dormir e acordar mais tarde do que o habitual, em 74% e 62,2%, respectivamente). Comer mais que o habitual foi fator associado ao sobrepeso (RP = 1,63; IC95%: 1,14 - 2,32) e à obesidade (RP = 2,00; IC95%:1,22 - 2,25) entre os participantes, após ajuste por sexo, idade, infecção por COVID-19 e insegurança alimentar. As prevalências de sobrepe-so e obesidade subsequentes ao período pandêmico se associaram às alterações comportamentais no apetite ocorridas durante a pandemia entre escolares Quilombolas


The aim of the study was to describe behaviors changes during COVID-19 pandemic and its association with subsequent overweight and obesity among Quilombola schoolchildren. Participants in this study were 1,004 children and adolescents from rural communities in the Quilombo areas of Feira de Santana, Bahia (50.3% boys; Age: 9.04 ± 1.52 years). The outcomes were overweight and obesity prevalence based on age-and-sex Body Mass Index values, evaluated according to the International Obesity Task Force cut-offs. Behavior changes (appetite, sleep, psychological, and social) were measured by a proxy report questionnaire. The analysis included descriptive statistics and multiple Poisson regression models, using robust variance to estimate Prevalence Ratio (PR) and 95% Confidence Intervals (95%CI). Co-variables: age, sex, COV-ID-19 infection, and food insecurity. The results presented 24% of overweight subsequent to the pandemic period (overweight without obesity = 15.5%; obesity = 8.5%). Main behavior changes occurred in diet (eating more than usual = 45.6%) and sleep (time to go to bed/time to wake up later than usual, among 74% and 62.2% of participants, respectively). Eating mor than usual was associated to overweight without obesity (PR = 1.63; 95%CI: 1.14 - 2.32) and obesity (PR = 2.00; 95%CI: 1.22 - 2.25), when adjusted by sex, age, COVID-19 infection, and food insecurity. Being overweight and obese subsequent to the COV-ID-19 pandemic was associated with behavior changes in the appetite during the pandemic period among Quilombola schoolchildren

Abstract Background: The emergence of multidrug-resistant NDM-1-producing enterobacteriaceae strains has become a threat to inpatients, especially to immunosuppressed ones, such as kidney transplant recipients. NDM-1 is a carbapenemase that makes gram-negative bacteria resistant to many types of antibiotics. The incidence of carbapenemase-producing enterobacteria infection in solid organ transplant recipients is around 3 to 10%, with a mortality rate of up to 30%. Methods: We present a case series of 4 patients with NDM-1-producing enterobacteria isolated in urine cultures or rectal swabs. We also conducted a cross-sectional study 30 days after patient identification, collecting surveillance cultures (rectal swab) from all inpatients to assess the extent of spread of this resistance mechanism; a total of 101 patients were included. Results: Two patients were adequately treated with negative control cultures. The other two patients were not treated because they were asymptomatic and had subsequent negative urine cultures. No new colonization was identified in the cross-sectional screening, and no new cases of urinary NDM-1 infection were recorded after a 4-year follow-up. Conclusion: Surveillance for infections caused by multidrug-resistant strains in hospitals treating immunosuppressed patients should be continued and prompt action should be taken in cases of outbreaks of multidrug-resistant infections.


Resumo Histórico: O surgimento de cepas multirresistentes de enterobacteriaceae produtoras de NDM-1 tornou-se uma ameaça para pacientes hospitalizados, especialmente para os imunossuprimidos, como os receptores de transplante renal. NDM-1 é uma carbapenemase que torna as bactérias gram-negativas resistentes a muitos tipos de antibióticos. A incidência de infecção por enterobactérias produtoras de carbapenemas em receptores de transplante de órgãos sólidos é de cerca de 3 a 10%, com uma taxa de mortalidade de até 30%. Métodos: Apresentamos uma série de casos de 4 pacientes com enterobactérias produtoras de NDM-1 isoladas em culturas de urina ou esfregaços retais. Também realizamos um estudo transversal 30 dias após a identificação do paciente, coletando culturas de vigilância (esfregaço retal) de todos os pacientes internados para avaliar a extensão de disseminação deste mecanismo de resistência; foram incluídos um total de 101 pacientes. Resultados: Dois pacientes foram tratados adequadamente com culturas de controle negativo. Os outros dois pacientes não foram tratados porque eram assintomáticos e tiveram culturas de urina negativas subsequentes. Não foi identificada nenhuma nova colonização na triagem transversal, e não foram registrados novos casos de infecção urinária por NDM-1 após um acompanhamento de 4 anos. Conclusão: A vigilância de infecções causadas por cepas multirresistentes em hospitais que tratam pacientes imunossuprimidos deve ser continuada e devem ser tomadas medidas imediatas em casos de surtos desses tipos de infecções.

RESUMO Objetivo: Analisar os fatores associados ao aumento da violência doméstica entre professores no período inicial na pandemia. Métodos: Estudo transversal e analítico, do tipo websurvey , realizado com professores da educação básica pública de Minas Gerais, Brasil. A coleta de dados ocorreu de agosto a setembro de 2020, via formulário digital. A variável dependente foi o autorrelato da violência doméstica. Utilizou-se a Regressão de Poisson. Resultados: Participaram do estudo 539 professores em situação de violência doméstica na pandemia, dos quais 6,3% (n = 34) diminuíram um pouco e muito, 58,3% (n = 314) permaneceram o mesmo e 35,4% (n = 191) aumentaram um pouco e muito. Na análise ajustada, observou-se maior prevalência do aumento da violência doméstica entre os professores que referiram dificuldade com o trabalho docente (RP = 1,38; IC95% 1,01;1,89); com adesão total ao distanciamento social (RP = 1,33; IC95% 1,01;1,76); piora no estado de saúde (RP = 1,70; IC95% 1,23;2,36) e que se sentiram tristes ou deprimidos muitas vezes ou sempre (RP = 1,57; IC95% 1,15;2,16). Conclusão: O aumento da violência doméstica da pandemia em professores foi associado a características laborais e condições de saúde mental.


ABSTRACT Objective: To analyze the factors associated with the increase in domestic violence among teachers in the initial period of the pandemic. Methods: Cross-sectional and analytical study, of the websurvey type, carried out with teachers of public basic education in Minas Gerais, Brazil. Data collection took place from August to September 2020, via a digital form. The dependent variable was self-reported domestic violence. Poisson regression was used. Results: 539 teachers participated in the study in situations of domestic violence in the pandemic, of which 6.3% (n = 34) decreased a little and a lot, 58.3% (n = 314) remained the same and 35.4% (n = 191) increased a little and a lot. In the adjusted analysis, a higher prevalence of increased domestic violence was observed among teachers who reported difficulty with teaching work (PR = 1.38; 95%CI 1.01;1.89); with full adherence to social distancing (PR = 1.33; 95%CI 1.01;1.76); worsening of health status (PR = 1.70; 95%CI 1.23;2.36) and in teachers who felt sad or depressed many times or always (PR = 1.57; 95%CI 1.15;2.16). Conclusion: The increase in domestic violence of the pandemic in teachers was associated with work characteristics and mental health conditions.

Abstract Introduction Phagocytosis of autoantibody-sensitized coated platelets through Fc gamma receptors on phagocytic cells is an important mechanism of thrombocytopenia in primary immune thrombocytopenia (ITP). Objective We aimed to investigate the contribution of the FcγRIIa and FcγRIIIa genes polymorphism to the risk of ITP and their association with disease characteristics in Egyptian children. Methods A case control study was conducted on eighty children with primary ITP and eighty age and sex healthy matched subjects as a control group. The FcγRIIa and FcγRIIIa genes polymorphism was detected using the polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). Results We found that the FcγRIIa‐131H and ‐131R allele frequencies were 51.3 % and 48.7%, respectively, in children with ITP, versus 75% and 25%, respectively, in controls (p= 0.002). The compound heterozygous HR genotype was significantly higher in ITP patients (p < 0.05). The FcγRIIIa-158F and ‐158V allele frequencies were 46.3% and 53.7%, respectively, in children with ITP, versus 70% and 30%, respectively, in controls (p= 0.002). The compound heterozygous VF genotype was significantly higher in ITP patients (p < 0.05). The combined HR/FV genotype was 47.5% in ITP patients, versus 10% in controls (p < 0.001). No significant difference was found between children with newly diagnosed ITP and those who developed chronic ITP, regarding the frequency distribution of the FcγRIIa and FcγRIIIa alleles and genotypes (p > 0.05). Conclusion There is a possible association of the FcγRIIa and FcγRIIIa genes polymorphism with the risk for, and genetic susceptibility to ITP in Egyptian children, but large-scale studies are still needed to support our findings.

INTRODUCTION: After the onset of the Covid-19 pandemic, oral health care in Primary Health Care (PHC) services in Brazil focused on emergency care. OBJECTIVE: To evaluate the impact of the Covid-19 pandemic on the number of emergency dental visits in PHC in Brazil. METHODS: This was an analytical and ecological study with data from the Health Information System for Primary Care referring to the number of attendances performed from March to December 2018 to 2020 in PHC services throughout Brazil for toothache, abscess, and dentoalveolar trauma. Differences between the monthly medians of the number of attendances before the pandemic (April to December 2018 and 2019) and during (April to December 2020) were analyzed using the independent-samples Mann-Whitney U test considering the interquartile ranges (IQR). RESULTS: Approximately 14 million cases were analyzed, with almost one-third of them occurring during the pandemic. There was a reduction in the median of the monthly number of emergency department visits in Brazil (-16.5%; p<0.031). Attendances for toothache reduced from a monthly median of 448,802.0 to 377,941.5 (IQR before [IQRa]: 416,291.7-506,150.5; IQR during [IQRd]: 310,251.0-454,206.5), dentoalveolar abscess attendances reduced from 34,929.0 to 27,705.5 (IQRa: 30,215.0-37,870.5; IQRd: 22,216.0-30,048.2) and to dentoalveolar trauma from 16,330.5 to 10,975.0 (IQRa: 14,800.0-18,472.7; IQRd: 8,111.2-13,527.5). CONCLUSION: Significant reductions were observed in the performance of emergency dental procedures in PHC during the COVID-19 pandemic.


INTRODUÇÃO: Após a instalação da pandemia da Covid-19, a atenção em saúde bucal nos serviços de Atenção Primária à Saúde (APS) no Brasil se concentrou nos atendimentos de urgência. OBJETIVO: Avaliar o impacto da pandemia da COVID-19 no número de atendimentos a urgências odontológicas na APS no Brasil. MÉTODOS: Trata-se de um estudo analítico e ecológico com dados do Sistema de Informação em Saúde para a Atenção Básica referentes ao número de atendimentos realizados de março a dezembro de 2018 a 2020 nos serviços de APS de todo o Brasil para dor de dente, abscesso e traumatismo dentoalveolares. Diferenças entre as medianas do número de atendimentos mensais antes da pandemia (abril a dezembro de 2018 e 2019) e durante (abril a dezembro de 2020) foram analisadas usando o teste não paramétrico de Mann-Whitney U para amostras independentes considerando os intervalos interquartílicos (IIQ). RESULTADOS: Foram analisados aproximadamente 14 milhões de atendimentos, sendo quase um terço deles ocorridos durante a pandemia. Houve redução na mediana do número mensal de atendimentos a urgências no Brasil (-16,5%; p<0,031). Atendimentos a dor de dente reduziram de uma mediana mensal de 448.802,0 para 377.941,5 (IIQ antes [IIQa]: 416.291,7-506.150,5; IIQ durante [IIQd]: 310.251,0-454.206,5), atendimentos de abscesso dentoalveolar reduziram de 34.929,0 para 27.705,5 (IIQa: 30.215,0-37.870,5; IIQd: 22.216,0-30.048,2) e a traumatismos dentoalveolares de 16.330,5 para 10.975,0 (IIQa: 14.800,0-18.472,7; IIQd: 8.111,2-13.527,5). CONCLUSÃO: Foram observadas reduções significativas na realização de procedimentos odontológicos de urgência na APS durante a pandemia de COVID-19.

Introdução: Cuidados paliativos são descritos como um conjunto de terapêuticas que visam minimizar as implicações negativas das doenças graves que ameaçam a vida e o bem-estar do indivíduo acometido. A progressão da doença e a sintomatologia provocam uma deterioração gradual e generalizada do estado do doente, e os sintomas físicos ocasionam a perda progressiva da capacidade funcional que, associados às manifestações emocionais pelo momento em que o paciente se encontra, impactam a sua qualidade de vida. Objetivo: Correlacionar a capacidade funcional com a qualidade de vida de pacientes oncológicos em cuidados paliativos. Método: Estudo transversal de abordagem quantitativa com 225 indivíduos. Os dados foram coletados por meio de questionário sociodemográfico, do índice de Barthel e do European Organization for Research in the Treatment of Cancer Questionnaire Palliative (EORTC QLQ-C15-PAL). Resultados: Dos 225 indivíduos elegíveis, observou-se homogeneidade entre a porcentagem de homens e mulheres, com média de 55,73±15,14 anos de idade. A análise estatística demonstrou associação positiva entre o escore do índice de Barthel e o escore de qualidade de vida do EORTC QLQ-C15-PAL com coeficiente de 0,74 e nível significância de p<0,05, bem como correlação negativa para as subescalas funcional e de sintomas com coeficiente de -0,702 e -0,544, respectivamente. Conclusão: Há uma correlação significativa entre a capacidade funcional e a qualidade de vida em pacientes oncológicos em cuidados paliativos.


Introduction: Palliative care is described as a set of therapies that aim to minimize the negative implications of severe life-threatening illnesses on the well-being of the individual affected. The progression of the disease and the symptomatology cause a gradual and generalized deterioration of the patient's condition and the physical symptoms cause the progressive loss of the functional capacity which, associated with emotional manifestations due to the moment the patient is living through, impact its quality-of-life. Objective: To correlate the functional capacity with the quality-of-life of cancer patients in palliative care. Method: Quantitative investigation with 225 individuals whose data were collected through sociodemographic questionnaire, Barthel Index and the European Organization for Research in the Treatment of Cancer Questionnaire Palliative (EORTC QLQ-C15- PAL) Results: The sample of 225 eligible individuals consisted in men and women evenly distributed with mean age of 55.73±15.14 years. Statistical analysis showed a positive association of 0.74 between the Barthel Index and the EORTC QLQ-C15-PAL quality-of-life with level of significance of p<0.05, as well as a negative correlation of -0.702 and -0.544 for the subscales functional and symptoms, respectively. Conclusion: There is a significant correlation between functional capacity and quality of life in cancer patients undergoing palliative care


Introducción: Cuidados paliativos se describen como un conjunto de terapias que tienen como objetivo minimizar las implicaciones negativas de enfermedades graves que amenazan la vida y el bienestar del individuo afectado. La progresión de la enfermedad y la sintomatología provocan un deterioro paulatino y generalizado del estado del paciente, los síntomas físicos provocan la pérdida progresiva de la capacidad funcional, asociados a manifestaciones emocionales debido al momento en el que se encuentra el paciente, impactando en la calidad de vida del paciente. Objetivo: Correlacionar la capacidad funcional con la calidad de vida de pacientes oncológicos en cuidados paliativos. Método: Estudio transversal con un enfoque cuantitativo en el que participaron 225 individuos. Los datos se recogieron mediante un cuestionario sociodemográfico, el índice de Barthel y el European Organisation for Research in the Treatment of Cancer Questionnaire Palliative (EORTC QLQ-C15-PAL). Resultados: De los 225 individuos elegibles, se observó homogeneidad de género, con una edad promedio de 55,73±15,14 años. El análisis estadístico mostró una asociación positiva entre la puntuación del índice de Barthel y la puntuación de calidad de vida de la EORTC QLQ-C15-PAL con un coeficiente de 0,74, adoptando una significancia de p<0,05, así como una correlación negativa para las subescalas funcional y de síntomas con un coeficiente de -0,702 y -0,544, respectivamente. Conclusión: Existe una correlación significativa entre la capacidad funcional y la calidad de vida en pacientes oncológicos en cuidados paliativos.

@#Introduction: A thorough medical history ensures safe dental practice. A good medical history guides clinicians in risk stratification to avoid medical emergencies and improve preparedness to prevent patient morbidity and mortality. This clinical audit aims to analyse the medical history taken by the dental students in patients with hypertension and/or diabetes mellitus (DM) and subsequently, recommend improvements in history-taking components in the dental practice. Methods: Hundred and two patients’ folders from the Faculty of Dentistry were examined by two independent auditors using a validated history-taking evaluation form. Six components of the medical history were classified as good or bad practices. Sociodemographic factors and distribution of the American Society of Anesthesiologists (ASA status) were described. The level of completeness of medical history records with years of study was assessed using the Chi-square test. Results: None of the students met 100% of the components required in medical history taking. Year three undergraduates performed poorly in the completeness of diagnosis and control of the medical condition whereby none of them had a good level of practice. The completeness of records did not differ between years of study except for diagnosis (p=0.026), control (p<0.001) and updating medical history (p=0.009) whereby the postgraduates had the best practice. Conclusion: This study highlighted marked deficiencies in taking a thorough medical history. Adaptation of the European Medical Risk Related History (EMRRH) form is recommended to be implemented in dental schools.

@#Introduction: The Healthier Choice Logo (HCL) was introduced in 2017 by the Ministry of Health Malaysia. This paper analysed acceptance of HCL, effectiveness of HCL in encouraging healthier product reformulation, and factors affecting reformulation among food industries. Methods: An online self-administered questionnaire consisting of four sections utilising multiple choice and 5-point Likert scale questions was distributed to food industries in Malaysia. Sample size calculation yielded 100 respondents. Results: Food industries had a higher acceptance of the processes and requirements involved in HCL implementation. HCL was highly effective in encouraging product reformulation among food industries in Malaysia. Meeting consumer demand, improving brand image, public health, more awareness around nutrition labelling, logo and national nutrition target, more technical knowledge and budget were found to motivate healthier product reformulation. However, product suitability, consumer acceptability, difficulties maintaining taste and shelf life, and limited budget were the challenges faced in product reformulation. There was no correlation between HCL acceptance and factors encouraging or inhibiting reformulation. Conclusion: These findings are expected to help relevant authorities or stakeholders make changes, if necessary, towards processes and requirements involved in HCL application to ensure wider HCL implementation. Future research should identify the relationship between HCL implementation and public health improvement among the Malaysian population.

Background@#The clinical use of sildenafil citrate (Viagra), a drug used to treat erectile dysfunction, is limited because of its many side effects on tissues. In this context, we aimed to investigate the protective effects of hesperidin, a citrus flavonoid, on hepatic and testicular damage induced by a high dose of sildenafil citrate in male rats. Rats were randomly divided into four groups. The first group was used as the control group. The second group was orally administered sildenafil citrate at a high dose of 75 mg/kg thrice a week. In the third group, hesperidin was administered orally at a dose of 50 mg/kg/day. The fourth group was administered 75 mg/kg sildenafil citrate three times a week with 50 mg/kg hesperidin daily. The experiment lasted for 28 days. @*Results@#In the sildenafil-treated groups, blood indices were altered, liver function tests were deranged, and serum testosterone levels were reduced. In the liver and testicular tissue, sildenafil citrate treatment resulted in significant reductions in catalase and total antioxidant capacity; as well as increased malondialdehyde, reactive oxygen species, and nitrous oxide levels. In addition, sildenafil citrate treatment caused abnormal histopathological patterns in both the liver and the testes. Liver vascular endothelial growth factor and testicular steroidogenic acute regulatory protein gene expression were upregulated. @*Conclusions@#Hesperidin attenuated the harmful effects of intensive sildenafil citrate treatment on liver and testicular functions, alleviated oxidative stress and normalized blood indices. Therefore, hesperidin could be protective against sildenafil citrate-induced oxidative damage that may develop over the long term.

Abstract Based on an integrative review, this study aimed to critically discuss the use of focus groups in empirical research in Brazilian Psychology over the last two decades and to qualify the methodological decisions of those who opt for this approach. Scopus, Web of Science, and SciELO were consulted, finding 62 articles in national journals in the area. The primary contexts of use were work, education, health, and social, with wide theoretical diversity. There has been a significant growth of the technique in research in Psychology since 2002, despite the need for improvement in the description of selection procedures, group composition, number of participants, the script for conducting, analysis, and presentation of results. However, it is necessary to ensure alignment between objective, theory, and method to improve the use of focus groups in psychological interventions.


Resumo A partir de uma revisão integrativa, o objetivo do estudo foi discutir criticamente o uso de grupos focais nas pesquisas empíricas da Psicologia brasileira nas últimas duas décadas e qualificar as decisões metodológicas daqueles que optam por essa abordagem. Foram consultadas as bases Scopus, Web of Science e SciELO, encontrando-se 62 artigos em periódicos nacionais da área. Os principais contextos de utilização foram trabalho, educação, saúde e social, com ampla diversidade teórica. Conclui-se que houve um crescimento significativo da técnica nas pesquisas em Psicologia desde 2002, apesar da necessidade de aprimoramento na descrição dos procedimentos de seleção, composição dos grupos, número de participantes, roteiro de condução, análise e apresentação dos resultados. Contudo, é necessário garantir o alinhamento entre objetivo, teoria e método para melhorar o uso de grupos focais em intervenções psicológicas.

Sin embargo, es necesario asegurar la alineación entre objetivo, teoría y método para mejorar el uso de grupos focales en intervenciones psicológicas.
