ABSTRACT

Background In the context of global warming, the impact of meteorological factors on human health has gradually become a research hotspot at home and abroad. Objective To describe the distribution of children's bronchopneumonia in Huzhou City, and explore the influence of diurnal temperature range (DTR) on children with bronchopneumonia, so as to provide guidance for identifying vulnerable populations and developing targeted measures. Methods A distributed lag nonlinear model (DLNM) was used to explore the potential nonlinear lag effect of DTR on admission of children with bronchopneumonia in Huzhou City from 2014 to 2019. Then a generalized additive model (GAM) was used to calculate the hospital admission risk of exposure to DTR in total population and sex-, age-, season-stratified populations. Results A total of 17658 hospitalized children with bronchopneumonia were included in the study. When DTR exceeded 7.5 ℃, the relative risk rose abruptly. When DTR reached 17 ℃, the risk of admission to hospital of children with bronchopneumonia was the greatest. In terms of the single-day lag effect, the admission risk of DTR for bronchopneumonia in children began on the same day and persisted until lag day 2; it peaked on the same day, and RR was 1.353 (95%CI: 1.220-1.502). The cumulative lag effect occurred from lag0 to lag0-6, and the highest RR value was at lag0-3, which was 1.938 (95%CI:1.483-2.533). The results of stratified analysis showed that the maximum effect values for boys and girls appeared at lag0-3 (RRboys=2.301, 95%CI: 1.671-3.169) and lag0-2(RRgirls=1.566, 95%CI: 1.152-2.129) respectively, and the effect value and duration of DTR in boys were higher and longer than those in girls. Among different age groups, both children of 0-3 years old and 4-14 years old had the maximum effect value at lag 0-3, the RR values were 1.734 (95%CI: 1.454-2.572) and 1.998 (95%CI: 1.226-2.254) respectively, and the effect value and duration of DTR on the children of 4-14 years old were higher and longer than those of the children of 0-3 years old. As to seasons, no significant increase was found in cumulative effect in summer and autumn (P>0.05); in winter and spring, the maximum effect value appeared at lag0-10, and the RR value was 4.164 (95%CI:1.191-14.561). Conclusion The impact of DTR on bronchopneumonia in children is acute. Boys and children aged 4-14 years old are more sensitive to DTR changes. Therefore, we should be alert to severe DTR changes and take protective measures in advance

ABSTRACT

Objective To explore the role of lung dendritic cells (DCs) and Th17/regulatory T cells (Treg) pathway in the pathogenesis of chronic obstructive pulmonary disease(COPD).Methods COPD patients who received lobectomy from Sep.2015 to Mar.2016 in our hospital were enrolled and classified into non-smoking non-COPD group,smoking without COPD group and COPD group.The expression of CD80,chemokine recepter-6 (CCR6),interleukin-17A (IL-17A) and fork-head transcription factor P3 (FoxP3) were detected by immunohistochemistry (IHC) in lung tissue.Mature DCs (mDCs),immature DCs (imDCs),Th17 cells and Treg cells in lung tissue were detected by flow cytometry (FCM) and the correlation between Th17/ Treg cells with lung function was analyzed.Results (1) The expression of CD80 and FoxP3 in COPD group was decreased,while the expression of CCR6 and IL-17A was increased (P<0.05).(2) The percentage of mDCs and Treg in lung tissue of COPD group was significantly decreased.In contrast,the proportion of imDCs and Th17 cells in COPD group was significantly increased (P<0.05).(3) The imbalance of Th17/Treg ratio in lung tissue was seen in patients with COPD,suggesting the potential mechanism of Th17 cell-mediated proinflammatory response.(4) The percentage of Th17 cells and Th17/Treg ratio in COPD patients was negatively correlated with forced expiratory volume in the first second (FEV1) as a percentage of predicted value (FEV1% pred),forced vital capacity(FVC) as a percentage of predicted value (FVC% pred),FEV1/FVC.On the other hand,the percentage of Treg cells was positively correlated with FEV1% pred,FVC% pred,FEV1/FVC.Conclusions The data in this study demonstrate the maturation disorder of dendritic cells in lung tissue of COPD patients.The imbalance of Th17/Treg ratio suggests that Th17 cell-mediated proinflammatory response may be involved in the pathogenesis of COPD.

ABSTRACT

@# Objective To explore the factors related with the infection after cerebrospinal fluid shunt (CFS) and nursing interventions.Methods 115 patients receiving CFS were reviewed. Results From November, 2009 to December, 2011, infection was found in 11 of 60 patients receiving CFS (18.3%). After intensive intervention, no infected case was found from January, 2012 to August, 2013 in 55 cases receiving CFS. Factors related with the infection included non-antibiotic-soaked shunt, non-laminar flow operating room, one-glove surgical procedure, and too many persons in the operating room. Conclusion Intensive nursing interventions can prevent the infection after CFS.

ABSTRACT

A method was developed for the determination of four sulfa antibiotics in groundwater, soil and excreta using solid phase micro extraction disks coupled with high performance liquid chromatography. The influence of eluent, different solid phase micro extraction membranes on the recovery of sulfa antibiotics in groundwater was investigated and it was found that when using the mixture of methyl alcohol and 1 . 0% formic acid as eluent, HLB ( divinyl benzene-N-vinyl pyrrolidone polymer ) as extraction membranes, an optimal enrichment effect was obtained. Different pretreatment methods for the 3 kinds of samples abovementioned were also examined. It was found that the signal response values obtained by using mixture of methyl alcohol and 1 . 0% formic acid as base solution of standard or sample solution was higher 8-10 times than that by using methyl alcohol only. Under the optimal conditions, good linear relationships were obtained in the sulfa antibiotics concentrations of 0 . 005-10 . 0 mg/L with the correlation coefficients>0 . 9999;The detection limits of sulfathiazole ( ST ) , sulfadiazine ( SM ) , sulfamethazine ( SM2 ) , sulfamethoxazole ( SMX ) were 1 . 08 , 3. 56, 4. 63 and 1. 84 ng/L(S/N=3), respectively. The enrichment factors for four sulfa antibiotics were 4000 times with solid phase micro extraction disks. The RSD of matrix spiked samples were 0. 1%-0. 4%(n=7). The proposed method was applied to the determination of the four sulfa antibiotics in groundwater, soil and excreta with spiked recoveries of the four sulfa antibiotics in the range of 69 . 80%-117 . 60%.

@#ObjectiveTo observe the effect of holistic nursing care by nurses in operation room on post-operative rehabilitation of patients with cerebral palsy accepting selective posterior rhizotomy.MethodsThe retrospective study was performed to compare the difference of pre-operative Rate Pressure Product (RPP) and post-operative Pain-Relieving Drug Request (PRDR), Incidence of Urinary Retention (IUR) after the removal of urinary catheter, and Starting Time for Rehabilitation Training (STRT) between patients with cerebral palsy accepting selective posterior rhizotomy under common nursing care and holistic nursing care by nurses in operation room.ResultsPre-operative RPP and post-operative PRDR, IUR, and STRT in the group of patients under holistic nursing care by nurses in operation room were significantly lower than those in the control group (P<0.05).ConclusionThe holistic nursing care by nurses in operation room can effectively reduce patients' fear to the operation, decrease undesirable factors and facilitate the advanced rehabilitation training as early as possible.

ABSTRACT

@#Objective To report the experience of nursing during operation on spina bifida and related diseases.Methods 168 patients with spina bifida and related diseases undergoing spine surgery in the hospital from October, 1989 to October, 2008 were reported. They were 76 males and 92 females, the averaged age was 5.6 years old (from 1 month to 23 years old). There were two follow-ups one month and 6 months to 2 years after operation. Results According to the second follow-up, the neural function improved in 123 cases, didn't change in 32 cases, and deteriorated in 13 cases. Nursing experience included: carefully observed the patients, especially managed respiratory tract and blood pressure; prepared good conditions for surgon to protect the neural function; sponge cushion for the pressured positions.Conclusion Perfect preparation of nursing can facilitate spine surgery on spina bifida and related diseases.

ABSTRACT

Objective To study the association of transcription factor 7-like 2(TCF7L2)polymorphisms with tvpe 2 diabetes mellitus in Chinese Han population. Methods Two polymorphisms (rs7903146 and rs12255372)of TCF7L2 gene were genotyped in 446 patients with type 2 diabetes mellitus(T2DM group)and 303 normal subiects (NC group) by PCR-restriction fragment length polymorphism(PCR-RFLP).Waist circumference.body mass index,plasma glucose,serum insulin,lipid profiles,high-sensitivity C-reactive protein and non-esterified fatty acid were measured.Homeostasis model assessment of insulin resistance(HOMA-IR)and β-cell function(HOMA-β)were calculated.Results (1) In T2DM group,T allele frequency and CT,TY geno tvpe frequeneies of rs7903146 were significantly higher than those in NC group(0.093,0.150,0.018 vs 0.043, 0.079,0.003,respectively,a11 P<O.O 1).Logistic regression analysis showed that the CT/TT genotype was a risk factor of tvpe 2 diabetes(OR=2.25,95%CI 1.39-3.62,P=0.001)and was associated with the decrease of insulin secretion. (2) No significant association was observed in vs12255372 alleles and genotypes with type 2 diabetes mellitus.Conclusion These results indicate that TCF7L2 might be one of the candidate genes for confe ring susceptibi lity to type 2 diabetes mellitus in the Chinese Han population.

ABSTRACT

Objective To study the expression of KISS-1 in normal endometrium and in ectopic endometri-um patients with endometriosis. Methods The expression of KISS-1 in 55 cases of normal endometrium as well in 46 cases of ectopie endometriurn with endometriosis were detected by immunohistochemicai EliVision plus method. Results The positive expression rates of KISS-1 in ectopic endometriotic tissues and in normal endometri-urn were 30.43 % and 63.64 % respectively ,and the positive expression rates in ectopic endometriotic tissues signifi-cantly lower than those in the normal endometrium( P< 0.01, P < 0.05). Conclusion The decreased expression of KISS-1 may be related to the mechanism of endometriosis.
