ABSTRACT
Objective To evaluate the efficiency of low molecular weight heparin(LMWH, fraxiparine) given as a single predialysis bolus injection with reused dialyzers in comparison with standard heparin(SH) administered with a continuous infusion. Methods 30 hemodialysis patients were studied in a radomized crossover fashion. Dialyzers fibrer bundle volumes(FBV), predialysis hemocrit and 2 h clearance of urea, creatinine were observed in the first, the fourth dialysis. The plasma heparin activities(anti-Fxa levels) were measured by the chromogenic substrate assay in 0 h, 2 h, 4 h of dialysis. Results Significant increase (P0.05); in addition, the plasma heparin activity(anti-Fxa levels) were comparable in both groups after 2 h of dialysis, however, they were significantly higher after 4 h in the LMWH than those in the SH group (P<0.05). Conclusion LMWH as a single bolus dose can prevent decrease in dialyzer clearance. It is clinically worthy of further popularity.

【Objective】To investigate the role of mycophenolate mofetil (MMF) in the prevention of acute rejection in renal transplantation.【Methods】A total of 106 patients were randomized into two groups.One group received MMF (n=56),the other received azathioprine (Aza) (n=50).The time of the following study was within the first 6 months after transplantation.【Results】The rate of acute rejection of group receiving MMF was 20%,it′s lower than that of the group receiving Aza 44% (P<0.01).The recovery rate of acute rejection treated by methylprednisolone (MP),in MMF group 82% was higher than Aza group 55%.(P<0.05).Meanwhile the hepatotoxicity as well as cytomegalovirus (CMV) infection were lower in MMF group than those in Aza group.【Conclusion】MMF as a new anti-rejection drug could more effectively prevent acute rejection than Aza after renal transplantation,and has lower toxicity and side effect.

ObjectiveTo establish a new method for diagnosis of active human cytomegalovirus (HCMV) i nfection and disease in transplant recipients.MethodsThe expression of HCMV pp67-mRNA in blood was detected by the nucleic acid sequence-based amplification (NASBA), in comparison to detection of pp65 HCMV a ntigen in white blood cells.ResultsIn 50 patients subject to transplantation, 26 were positive for pp65 (≥1/50 000 WBC). Among them, 16 were positive for pp67. In 9 cases of CMV with symptoms, b oth pp65 and pp67 were positive. In the patients negative for pp65, pp67 was not positive. The sensitivity, specificity, positive predictive value and negative predictive value were 100?%, 88?%, 56?% and 100?%, respectively.ConclusionDetection of pp67-mRNA by NASBA is a rapid objective and accurate approach for the diagnosis of HCMV infection.
