ABSTRACT
A instalação de implantes contíguos na maxila anterior representa um grande desafio de planejamento e execução. O objetivo deste estudo foi apresentar um relato de caso, com acompanhamento em longo prazo, em que uma paciente de 23 anos teve implantes dentários inseridos em substituição ao dente 11, que apresentava reabsorções internas e externas, e ao dente 12, que havia sido avulsionado. Tais alterações foram decorrentes de um traumatismo sofrido pela paciente aos 13 anos de idade. Inicialmente, um retalho mucoperiosteal foi elevado, o dente 11 extraído e foi realizada regeneração óssea guiada com enxerto autógeno particulado associado a uma membrana com reforço de titânio. Após sete meses, realizou-se a instalação dos implantes com hexágono externo e, posteriormente, os pilares cerâmicos e as coroas cerâmicas. Adicionalmente, optou-se por realizar facetas cerâmicas nos dentes vizinhos (13 e 21), para harmonização estética, e gengiva cerâmica, para restaurar a ausência parcial da papila entre os implantes. Após seis anos, o nível dos tecidos moles e duros peri-implantares se manteve estável e a paciente relatou satisfação com o resultado estético final
The installation of adjacent dental implants in the anterior maxilla represents a great challenge concerning both treatment planning and execution. The aim of this study is to present a clinical case, with long-term follow-up, in which a 23 years-old patient had dental implants inserted to replace tooth 11, presenting internal and external resorption, and the avulsed tooth 12. These alterations were due to a traumatism suffered from the patient at his 13 years-old. Initially, a mucoperiosteal flap was raised, the tooth 11 extracted, and guided-bone regeneration performed with particulated autogenous material and a titanium-reinforced barrier. After seven months, external hex dental implants were placed along with ceramic abutments and crowns. Additionally, adjacent teeth (#13 and #21) had ceramic veneers for esthetic harmonization, and pink porcelain was built-up to restore the partial papillary loss between implants. After six years, the level of soft and hard peri-implantar tissues maintained stable over time, and the patient reported satisfaction with the esthetic outcome
