ABSTRACT

Objective:To investigate the effects of cognitive intervention in an attribution study on psychological resilience and cancer-related fatigue in older adult patients with lung cancer.Methods:A total of 88 older adult patients with lung cancer who received treatment in Ningbo Ninth Hospital between January 2018 and January 2020 were included in this study. These patients were randomly divided into a study group ( n = 44) and a control group ( n = 44). The control group was given routine nursing intervention, and the study group was given cognitive intervention under an attribution study. All patients were treated for 6 consecutive months. After the intervention, the changes in psychological resilience and improvements in cancer-related fatigue symptoms compared with before intervention were observed. The Coping Style Questionnaire (CSQ) was applied to evaluate the change in coping styles after intervention relative to before intervention. Before and after intervention, patient compliance was compared between the two groups. Results:After 6 months of intervention, the scores of various dimensions of the Chinese version of Connor–Davidson Resilience Scale (CD-RISC) in the study group were (7.74 ± 1.36) points, (23.67 ± 1.94) points, and (41.28 ± 1.47) points, respectively, which were significantly higher than (6.92 ± 1.12) points, (12.45 ± 2.76) points, and (32.34 ± 1.69) points respectively in the control group ( t = 3.08, 22.06, 26.47, all P < 0.05). The scores of various dimensions of Piper Fatigue Scale-Revised (PFS) in the study group were (4.85 ± 1.15) points, (4.71 ± 1.05) points, (4.85 ± 1.05) points, (5.24 ± 1.05) points, respectively, which were significantly lower than (6.33 ± 1.25) points, (6.21 ± 1.52) points, (6.39 ± 1.17) points, (6.72 ± 1.23) points respectively in the control group ( t = 5.78, 5.38, 6.49, 6.07, all P < 0.05). The score of positive coping style in the study group was (28.51 ± 2.65) points, which was significantly higher than (24.84 ± 2.52) points in the control group ( t = 6.65, P < 0.05). The score of negative coping style was (12.39 ± 2.53) points, which was significantly lower than (14.81 ± 2.12) points in the control group ( t = 4.86, P < 0.05). The patient compliance with medication, review, diet, and physical exercise in the study group were 100.00%, 90.91%, 90.91%, and 81.82%, respectively, which were significantly higher than 81.81%, 68.18%, 75.00%, and 61.36%, respectively in the control group ( χ2 = 6.73, 6.98, 3.93, 4.52, all P < 0.05). Conclusion:Cognitive intervention in an attribution study can enhance psychological resilience, improve the symptoms of cancer-related fatigue, promote a positive change of coping style, and enhance treatment compliance in older adult patients with lung cancer.

ABSTRACT

OBJECTIVE:To observe therapeutic efficacy and safety of budesonide and formoterol in the treatment of acute exacerbation of mild to moderate asthma.METHODS:A total of 89 patients with acute exacerbation of mild to moderate asthma were randomized divded into study group (45 cases) and control group (44 cases).Study group was given Budesonide and formoterol dry powder inhalation,one inhalation,q6 h,gargling 5 times after inhalation,6 inhalation per day at most+Montelukast tablet 10 mg orally,once a day.Control group received Prednisone tablet 25 mg orally after breakfast,once a day,d1-5+Theophylline sustained-release capsule 0.2 g,twice a day+Montelukast tablet 10 mg,once a day in the evening.Both groups were treated for 5 d.Acute AQLQ score,FEV1,PEF%pred and SpO2 were observed in 2 groups before and after treatment,and the occurrence of ADR was recorded.RESULTS:Before treatment,there was no statistical significance in acute AQLQ score,FEV1,PEF%pred or SpO2 between 2 groups(P>0.05).After treatment,acute AQLQ score,FEV1,PEF%pred and SpO2 of 2 groups were significantly higher than before treatment,with statistical significance (P<0.05),but there was no statistical significance between 2 groups (P> 0.05).There was statistical significance in the incidence of ADR between 2 group(P<0.05).CONCLUSIONS:Budesonide and formoterol can improve pulmonary ventilation function and prognosis in patients with acute exacerbation of mild to moderate asthma with good safety.

ABSTRACT

Background and purpose:The greastest diameter of cervical cancer with stage Ⅰ_(b2) disease was more than 4 cm in diameter. surgery as the fi rst priority was diffi cult in these patients, bleeding was the most frequent adverse effect. This article studied the effect of the cervical cancer with stage Ⅰ_(b2) disease underwent neoadjuvant chemotherapy with gemcitabin plus cisplatinum(DDP). Methods:23 cases (A groups) stage Ⅰ_(b2) cervical cancer were treated with gemcitabin 1.5 g/m2 iv infusion at d1, plus cisplatinum(DDP) 20 mg/m2 iv infusion at d1-3. The interval between the two cycles was two weeks.19 cases (B groups) were treated with cisplatinum(DDP) 20 mg/m2 iv infusion at d1-3,plus VCR 1.5 g/m2 iv infusion at d1, and BLM 10 mg/m2 im at d1-3, The interval between the two cycles was three weeks. The assessment for clinical effect and side effect were conducted for the patients with completion of at least two cycles of chemotherapy. Results:42 cases were enrolled in this trial. There was signifi cant(P=0.004) difference between the two groups with the shrinkage of the greatest diameter after neoadjuvant chemotherapy. The main toxicities were myelosuppression. ABSTRACT

To construct a hepatic stellate cells (HSCs) subtracted cDNA library to find differentially expressed genes in normal mice and mice infected with Schistosoma japonicum (S. japonicum). Suppression subtractive hybridization (SSH) was used. The cDNA fragments of normal mouse were compared to those of schistosoma-infected mice to find differentially expressed genes. Then differentially expressed cDNA fragments were directly inserted into T/A cloning vector to set up the subtractive library. Amplification of the library was carried out with transformation of DH5alpha. The amplified library contained more than 400 positive bacterial clone, which were then hybridized with forward and backward subtracted probes for differential screening. One hundred positive bacterial clones were randomly selected for sequencing and BLAST analysis. Finally, virtual Northern Blot confirmed such differential expression. The subtracted cDNA library of differentially expressed genes of HSCs was constructed successfully, the library is efficient and lays foundation for screening and cloning new and specific genes of schistosomiasis.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the possible effect of transforming growth factor-beta(1) (TGF -beta(1)) on the development of renal fibrosis in human mesengial proliferative glomerulonephritis (MsPGN).</p><p><b>METHODS</b>Immunohistochemistry method, sirius red staining polarization microscopy and the computer imaging analysis system were used to detect the expression of TGF-beta(1), the distribution of collagen I, collagen III and collagen IV.</p><p><b>RESULT</b>In MsPGN with renal fibrosis, collagen IV was increased markedly,and collagen I and collagen III appeared in the expanded mesengial matrix abnormally. Collagen III and collagen IV were increased markedly in tubulointerstitium. TGF-beta(1) expression was positively correlated with the expression of collagen I, collagen III and collagen IV in tubulointerstitium (r=0.82 0.92,P<0.01), and negatively correlated with I/III, I/IV and III/IV (r=-0.83,-0.92, P<0.001).</p><p><b>CONCLUSION</b>Abnormal increase of TGF-beta(1) may be one of the important factors associated with glomerular sclerosis and tubulointerstitial fibrosis through the increment and abnormal distribution of collagen I, collagen III and collagen IV.</p>

ABSTRACT

<p><b>OBJECTIVES</b>Using receiver operating characteristic (ROC) curve to compare the value of platelet derived growth factor-BB (PDGF-BB), transforming growth factor-beta1 (TGF-beta1), matrix metalloproteinase-1 (MMP-1), tissue inhibitor of matrix metalloproteinase-1 (TIMP-1), hyaluronic acid (HA), type III procollagen (PCIII), type IVcollagen (C-IV) and laminin (LN) in serum and message RNA (mRNA) expression of TIMP-1 and MMP-1 in peripheral blood mononucleocytes (PBMC) to diagnose liver fibrosis.</p><p><b>METHODS</b>Serum samples from 60 chronic hepatitis B patients and 20 healthy blood donors were assayed for serum level of PDGF-BB, TGF-beta1, MMP-1 and TIMP-1 with ELISA, and for serum level of HA, PCIII, C-IV and LN, with RIA. The mRNA expression of TIMP-1 and MMP-1 in PBMC was detected by RT-PCR. Liver biopsy was performed in all those patients. The biopsy materials were examined histopathologically.</p><p><b>RESULTS</b>Through the analysis by ROC curve, serum PDGF-BB is the most valuable marker, and its sensitivity was the highest among eight indices. The marker with the highest specificity was TIMP-1 mRNA in PBMCs. The area under the curve (AUC) of PDGF-BB, TIMP-1, HA, PCIII, C-IV, LN, TIMP-1 mRNA was 0.985, 0.726, 0.318, 0.728, 0.727, 0.583, 0.463, 0.876, respectively. The sensitivity and specificity of combination of PDGF-BB and TIMP-1 mRNA were 97.4%, 95.0%, respectively.</p><p><b>CONCLUSION</b>Serum PDGF-BB is the most potential index among eight markers. The combination of serum PDGF-BB and TIMP-1 mRNA in PBMC might be more efficient to screen liver fibrosis.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the expression of Bcl-2 and Bax in rat liver fibrosis model induced by CCl4 and the role of IFN-gamma.</p><p><b>METHODS</b>Liver fibrosis was induced in rats by subcutaneous injection of CCl4. The rats were divided into fibrosis model group, Bie-Jia-Ruan-Gan-Tablet treatment group and IFN-gamma treatment group (0.2 MU.kg.d, i. m. for 12 weeks), and another 10 rats without any treatment were used as normal control. Bcl-2 and Bax proteins expression were detected by immunohistochemistry.</p><p><b>RESULTS</b>Bcl-2 was expressed weakly in the homogenate of hepatocytes and hepatic sinusoid in normal control rats, and it was expressed stronger in fibrous septae, portal area, hepatic sinusoid, the homogenate and membrane of hepatocytes and central vein in fibrosis model rats (3.87%+/-2.37% vs 9.46%+/-4.29%, t=2.83, P<0.05). Bax was expressed slightly in central vein and the hepatic sinusoid around, and it was expressed stronger in the homogenate of hepatocytes, hepatic sinusoid, fibrous septae, membrane of hepatocytes and epithelial cells of bile duct (3.50%+/-1.88% vs 9.80%+/-3.75%, t=3.72, P<0.01). Compared with that in fibrosis model rats, the expression of Bax was significantly lower in rats treated with IFN-gamma (9.80%+/-3.75% vs 5.85%+/-2.35%, t=2.98, P<0.01), but the expression of Bcl-2 was not significantly different (t=1.49, P>0.05), however it was significantly lower in fibrous septae in IFN-gamma-treated group than in model group (6.58%+/-4.13% vs 9.46%+/-4.29%, t=2.80, P<0.05).</p><p><b>CONCLUSION</b>The expression of Bcl-2 and Bax increases in liver fibrosis model rats, and IFN-gamma can promote myofibroblasts apoptosis</p>

ABSTRACT

<p><b>OBJECTIVE</b>To analyse the factors which influence the four serum fibrosis markers hyaluronic acid (HA), type III procollagen (PCIII), laminin (LN) and type IV collagen (CIV).</p><p><b>METHODS</b>The levels of serum HA, PCIII, LN and CIV were measured by RIA in 141 patients with chronic hepatitis B (CHB), then the patients were divided into two groups according to the serum fibrosis markers, namely consistent group and inconsistent group. the liver biopsy materials were examined pathomorphologically and liver function was detected by automatic biochemistry analyzer, The interior diameters of the portal vein, the spleen vein and the thickness of the spleen were also measured with ultrasonography.</p><p><b>RESULTS</b>16 patients (14.16%) whose serum fibrosis markers were inconsistent with histological stage of liver fibrosis were found. Their serum fibrosis markers were not correlated with staging of liver fibrosis (P>0.05), but were positively correlated with inflammation grade (x(2)=12.07, P<0.05), at same time, the serum levels of alanine aminotransferase (ALT), aspartate aminotransferase(AST), gamma-glutamyltransferase (GGT) and globulin (GLB) decreased obviously, from 89.28 U/L +/- 64.25 U/L to 49.31 U/L +/- 26.75 U/L (t=2.45, P<0.05), 66.10 U/L +/- 42.30 U/L to 40.83 U/L +/- 22.40 U/L (t=2.33, P<0.05), 86.26 U/L +/- 70.36 U/L to 48.99 U/L +/- 29.96 U/L (t=2.08, P<0.05) and 32.13 g/L +/- 5.18 g/L to 28.05 g/L +/- 3.47 g/L (t=3.03, P<0.01) respectively. And the level of albumin (ALB) and the ratio of albumin and globulin (A/G) increased evidently, from 42.34 g/L +/- 4.81 g/L to 46.19 g/L +/- 3.61 g/L (t=3.06, P<0.01) and 1.35 +/- 0.28 to 1.63 +/- 0.26 (t=3.70, P<0.01). But the serum level of alkaline phosphatase (ALP), total bilirubin (TBil), total protein (TP), the width of main portal vein, the width of splenic vein and the thickness of the spleen did not change clearly (P>0.05).</p><p><b>CONCLUSION</b>As diagnostic markers, serum fibrosis markers as well as inflammation grade and liver function should be taken into account.</p>

ABSTRACT

Objective To evaluate the appearances of osteochondral fracture of the knee on X-ray and MR imaging. Methods Twelve patients with knee acute injury were examined with X-ray and MRI. The findings of MRI and X-ray were analyzed and reviewed, and the results of each patient were confirmed by arthroscopy and operation. Results Thirteen areas of osteochondral fracture including 9 loose bodies in 12 patients were diagnosed by MRI. Seven locations were found by MRI in the distal lateral femoral condyle, and 6 in patellar. MRI could clearly show the location, the size, and the depth of each osteochondral fracture, and MRI could also differentiate the articular cartilage and the subchondral of the loose body. T 2WI, STIR, and FFE-T 2WI were the best sequences to demonstrate the osteochondral fracture. X-ray showed 5 loose bodies in the knee joint, but could not detect where they came from. Conclusion MRI can accurately reveal and diagnose the osteochondral fracture of knee after trauma, which improves the diagnostic certainty and is very helpful for arthroscopy and operation. X-ray is still the initial modality in detecting the osteochondral fracture of knee and should be combined with MRI to confirm the diagnosis.

ABSTRACT

Purpose:To study the intraperitonal chemotherapy combined with intravenous chemotherapy after surgery for the treatment of ovarian cancer. Methods:The 88 patients of ovarian cancer after cytoreductive surgery were divided into two groups A and B. A group(45 cases) received intraperitonal chemotherapy three times once a week with DDP and 5-FU plus either the MMC or AT-1258, then intravenous chemotherapy, stages Ⅰ/Ⅱ 21 cases(23.86%),stages Ⅲ/Ⅳ 24 cases(27.27%). B groups were given intravenous chemotherapy with CAP protocol once every three weeks,stage Ⅰ/Ⅱ 24 cases(27.27%),stages Ⅲ/Ⅳ 19 cases(21.59%).Results:After three or six courses of chemotherapy there was significant decrease in CA125 both Ⅰ/Ⅱ and Ⅲ/Ⅳ in A and B groups . After three courses of chemotherapy there were 34 cases((38.64%))of A groups and 19 cases(21.59%)of the B groups with CA12535 u/ml(35 cases) who were given three courses of chemotherapy after entire treatment. A and B groups in the Ⅰ/Ⅱ stages showed average survival time (35.05?0.54),(32.38?1.19) months,and in the Ⅲ/Ⅳ stages showed average survival time (31.33?1.41),(28.26?1.88) months.There was no significance (P=0.156) with three year survival rate between A and B groups in the Ⅰ/Ⅱ stages with Log Rank test. There was no significance (P=0.08) in three-year survival rate between A and B groups in the Ⅲ/Ⅳ stages with Log Rank test. There was significance in three-year survival rate between A and B with intraperitonal fluid.Conclusions:The intraperitonal chemotherapy combined with intravenous chemotherapy after surgery for the treatment of ovarian cancer could marketly decrease CA125 than intravenous chemotherapy after three or six courses. Three-year survival ratewas significantly different between CA125 35 u/ml(35 cases) when they were given three courses of chemotherapy after entire treatment. Although the intraperitonal chemotherapy after surgery for the treatment of ovarian cancer had no significant difference between A and B groups in Ⅰ/Ⅱ and Ⅲ/Ⅳ stages with three-year survival rate , it appears longer survival time . There is significant difference with three-year survival between A and B with intraperitonal fluid. The intraperitional chemotherapy combined with intravenous chemotherapy after surgery can be used for the ovarian cancer with intraperitonal fluid and metastases.
