ABSTRACT

OBJECTIVE@#To establish the aGVHD mouse model,and investigate the regulatory effect and its mechanism of low-dose GSI combined with BMSC on aGVHD mice.@*METHODS@#C57BL/6 (H-2b) and BALB/c (H-2d) were selected as donor and recipient of allogeneic transplantation to establish the aGVHD mouse model. BALB/c mice were randomly divided into 6 groups, which were the bone marrow cell infusion after irradiation (BM) group; the bone marrow cells + spleen cells after irradiation (BM+SC) group; the bone marrow cells + spleen cells + DMSO (BM+SC+DMSO) (transplant control) group; bone marrow cells + splenocytes +GSI after irradiation (BM+SC+GSI) group; bone marrow cells + spleen cells + bone marrow mesenchymal stromal infusion after irradiation cell (BM+SC+BMSC) group; bone marrow cells + spleen cells + bone marrow mesenchymal stromal cells +GSI infused after irradiation (BM+SC+BMSC+GSI) group. The mice in the two groups containing GSI were intraperitoneally injected with GSI at 5 μmol/kg on day 1, 2, and 3 after transplantation with DMSO as a control. The general conditions, survival time and hematopoietic recovery of mice were observed, cytokines were detected by ELISA, and histopathological changes were detected by immunohistochemistry. The effects of low-dose GSI combined with BMSC on hematopoietic reconstruction and aGVHD development after allo-BMT were investigated.@*RESULTS@#The survival rate of the mice in BM+SC+BMSC+GSI combination group was 80% during the observation period, which was significantly higher than that in the other groups; the incidence of aGVHD was reduced in the BMSC GSI or their combination groups after 21 days of transplantation. GSI could partly promote the recovery of leukocytes, and show no significant delayed effect on the recovery platelets. Moreover, the level of Th1 cytokines (IFN-γ) in BM+SC+BMSC+GSI combined group was lower than that in BM+SC+GSI group (P<0.01), the level of Th2 cytokines (IL-4) in the combination group was higher than that in BM+SC+GSI group (P<0.01), also the level of IL-17 was significantly lower than that in the corresponding control group (P<0.001).@*CONCLUSION@#Low dose GSI combined with BMSC can promote hematopoietic reconstruction and regulate cytokines secretion including IFN-γ, IL-4 and IL-17. GSI combined with BMSC achieve the goal of synergistically inhibiting the occurrence and progression of aGVHD.

ABSTRACT

Aim To observe the protective effects of total flavonoids from Anchusa italica Retz, and four compounds on hypoxia/reoxygenation (H / R) injury in myocardial cells and to explore the mechanisms. Methods Primary neonatal rat cardiomyocytes were isolated, cultured and were randomly divided into different groups. The H / R model of cultured neonatal rat cardiomyocytes was developed. The related indexes were determined after administration. Results Compared with model group, the total flavonoids and the four compounds could increase the survival rate of H / R cardiomyocytes (P < 0. 01), reduce the leakage of LDH (P < 0 . 0 1), reduce the content of M DA (P < 0 . 0 1), significantly increase the activity of SOD (P <0. 01), significantly decrease the early apoptosis index and increase the mitochondrial membrane potential (P < 0. 01) . The activation of key proteins in NF-KB signaling pathway was inhibited, the proportion of Bax / Bc l - 2, beclin-1 and p62 significantly decreased (P < 0. 01), and the expression of apoptosis related proteins NLRP3, IL - lß and GSDMD significantly decreased (P <0. 0 1) . Conclusions The total flavonoids and four compounds can alleviate the damage of H/R on cardiomyocytes. The mechanism is related to the inhibition of the activation of NF-KB signaling pathway and the excessive autophagy of cardiomyocytes, alleviation of the scorch death of cardiomyocytes, and reduction of the apoptosis of cardiomyocytes.

OBJECTIVE@#To investigate the effect of arsenic disulfide (AS@*METHODS@#The human DLBCL cell OCI-LY3 was treated with different concentrations of AS@*RESULTS@#The DLBCL cell viability was decreased significantly at 24, 48 or 72 h as cultured with itraconazole. Along with the increasing of itraconazole concentration, the DLBCL cell viability was significantly reduced as compared with that in control group, and the results showed statistically significant(r=-0.690,r=-0.639, r=-0.833, r=-0.808, r=-0.578). The inhibitory and apoptosis rates of the cells were significantly increased as compared with those of the single drug-treated group after treated by the combination of itraconazole and AS@*CONCLUSION@#Itraconazole can inhibit proliferation of DLBCL cells in a concentration-and time-dependent manner. ABSTRACT

Objective To establish a rat model of Cryptococcus neoformans meningitis. Methods Sprague-Dawley rats were divided into four groups (group A to D). The load of Cryptococcus neoformans was inoculated by intracisternal injection to the animals in Group A (1×105 cfu), Group B (1×106 cfu), Group C (1×107 cfu), and 0.9% NaCl solution to Group D. The rats were observed after inoculation for their clinical symptoms. On day 14 and day 21 after inoculation, cerebrospinal fluid was sampled and cultured for counting of bacterial colonies. The 28-day mortality of the animals was calculated. Results All the animals in group A (1×105 cfu) survived without any apparent clinical symptoms, and associated with decreasing bacteria load. The animals in group B (1×106 cfu) had mild symptoms associated with low mortality rate and slightly increased bacterial load. The animals in group C (1×107 cfu) showed a lot of symptoms and associated with high mortality rate and significantly increased bacteria load. All the animals in group D (0.9% NaCl solution) survived with normal activity. No bacterial colony was cultured from the cerebrospinal fluid. Conclusions Intracisternal injection of 1×107 cfu Cryptococcus neoformans to rats could cause apparent clinical symptoms of meningitis. The 28-day mortality rate of such a rat model of Cryptococcus neoformans meningitis is greater than 80%. An ideal rat model of Cryptococcus neoformans meningitis is established successfully.

ABSTRACT

Objective To summarize the clinical experience of patient diagnosed with Niemaoh-Pick disease being pregnant after liver transplantation. Methods Clinical data of one case of type B Niemaoh-Pick disease being pregnant after liver transplantation were retrospectively analyzed. Results The patient successfully underwent liver transplantation combined with splenic artery ligation on July 8, 2011. She was well recovered postoperatively. After surgery, she received conventional anti-rejection treatment, and gradually switched to use of tacrolimus at a dosage of 2.5 mg/d. The serum drug concentration was maintained at 2 ng/mL. In September 2015, she was successfully pregnant. On June 2, 2016, she delivered a male infant through cesarean section. She could breastfeed the infant in a low quantity early after delivery. Both the mother and infant were followed up until submission date. The mother was physically stable and the infant grew normally. Conclusions Patients diagnosed with Niemaoh-Pick disease can obtain favorable clinical outcomes of pregnancy and delivery after liver transplantation.

ABSTRACT

The Notch signaling pathway is a highly conserved cell signaling system that plays an essential role in many biological processes. Notch signaling regulates multiple aspects of hematopoiesis, especially during T cell develop-ment. Recent data suggest that Notch also regulates mature T cell differentiation and function. The latest data show that Notch also plays an essential role in alloreactive T cells mediating acute graft-versus-host disease (aGVHD), the most severe complication of allogeneic hematopoietic stem cell transplantation (allo-HSCT). Notch inhibition in donor-derived T cells or blockade of individual Notch ligands and receptors after transplantation can reduce GVHD severity and mortality in mouse models of allo-HSCT, without causing global immunosuppression. These findings indicate Notch in T cells as an attractive therapeutic target to control aGVHD. In this article, the pathophysiology of aGVHD, the Notch signal pathway and aGVHD are reviewed.

ABSTRACT

OBJECTIVE:To study the bioequivalence of Levodopa micro-capsule floating tablets in Beagle dogs after multi-dose administration. METHODS:6 dogs were collected and divided into Levodopa micro-capsule floating tablets group and Com-pound levodopa preparation group (Benserazide tablet,reference preparation). They were given levodopa 200 mg intragastrically, every 8 h,for consecutive 4 day. In two-period crossover test,HPLC method was established to determine the concentration of le-vodopa in dog. The pharmacokinetic parameter,bioequivalence and plasma concentration fluctuation of steady state were calculated. RESULTS:The main pharmacokinetic parameters of Levodopa micro-capsule floating tablets and reference preparation were as that cmax were(4.23±0.75)and(8.47±1.18)μg/ml;AUC0-∞ were(12.18±1.16)and(13.81±2.12)μg·h/ml;tmax were(1.83±0.26) and(0.67±0.13)h,respectively. 90% confidence intervals for the geometric mean ratio of AUC0-∞ for test and reference prepara-tion were 80.61%-97.90%,and that for cmax were 42.75%-57.63%,respectively. There was statistical significance in tmax between test and reference preparation. Degree of fluctuation of test and reference preparation at steady state were(283.914±43.217)% and (506.489±78.965)%,and fluctuation coefficient were(177.463±7.873)% and(187.405±1.650)%,respectively. The degree of fluctuation of test preparation was significantly less than that of reference preparation. CONCLUSIONS:Levodopa micro-capsule floating tablets show good sustained-release property,and are bioequivalent with reference preparation in absorption after multiple dose administration. It also has lesser fluctuation of blood concentration.

ABSTRACT

OBJECTIVE:To reduce the risk of single dispensing,and to improve the quality of pharmaceutical care in emergen-cy pharmacy. METHODS:Single dispensing environmental factors,human factors and social factors of emergency pharmacy in our hospital were analyzed;the suggestions and measures were put forward,risk intervention mechanism was established and manage-ment effects were evaluated. RESULTS:Through improving the pharmacy hardware and software,carrying out the relevant sys-tem,enhancing training,improving the process of dispensing and window order,promoting the drug management and carrying out near error management. Compared with before management(Oct.-Dec. 2014),the times of near error and disputes significantly re-duced after management(Feb.-Apr. 2015)(P<0.05),and medication error reduced to zero. CONCLUSIONS:As single dispens-ing,risk management and the improvement of prescription audit ability can improve pharmaceutical care in emergency pharmacy.

OBJECTIVE:To regulate the management of operation drug and promote medication security. METHODS:Based on the relevant principles in the standard of JCI,the practice about establishment of operation pharmacy in our hospital was intro-duced,and it were stated from two aspects which included working mode and effects. RESULTS:The working mode were format-ed by definiting the duties and working flow of pharmacist,anesthetist and liaison man. The establishment of operation pharmacy, which brought operation drugs into the unified management system of our hospital,saved manpower and improved work efficien-cy,refined the management of drugs in all aspects,improved the management of drug safety level,strengthened drug expiration date management,implemented the reporting of adverse drug reactions(event)and collecting of medication(potential)errors. The established mode conformed to the relative standard of JCI including preparation,grant,storage and collection of ADR of drug. CONCLUSIONS:The established drug management of operation pharmacy promotes standardized administration of surgery drug, which guarantees the drug use safety of patients.

ABSTRACT

OBJECTIVE:To provide baseline data of safe and rational drug use in pediatrics. METHODS:The pediatrics outpatient prescriptions were randomly selected from pediatric department of The Second Affiliated Hospital of Xinjiang Medi-cal University during Mar. 2014-Feb. 2015. The behavior of off-label drug use was determined according to the content of drug instruction. The type of off-label drug use,the relationship of different age groups with various drugs were analyzed. RE-SULTS:A total of 2400 outpatient prescriptions were collected from pediatric department,5115 medication records were ana-lyzed,involving 171 kinds of drugs. By the number of prescriptions,medication records and drug types,the incidence of off-la-bel drug use were 59.08%,40.87% and 80.70%,respectively. The main categories of drug off-label use were off-label dosing frequency(43.03%),off-label dosage(29.55%)and off-label indication(18.83%). The incidence of off-label drug use in in-fants and adolescents ranked the top 2 places,being 67.58%,63.01%. The top-four drugs in the list of the incidence of off-la-bel drug use were drugs for external use(55.56%),drugs for nerve system(54.12%),drugs for respiratory system(48.95%) and nutritional agents(48.09%). CONCLUSIONS:The phenomenon of drug off-label use is widespread in pediatric outpatient prescriptions.

ABSTRACT

OBJECTIVE:To establish a method for the content determination of chloramphenicol and salicylic acid in Acne tinc-ture. METHODS:HPLC method was adopted with the column of Shim-pack CLC-ODS with the mobile phase of methanol-water (50∶50,V/V)at the flow rate of 0.8 ml/min. The detection wavelength was 278 nm,column temperature was 35 ℃,and the vol-ume was 10 μl. RESULTS:The linear range was 2.0-10.0 μg/ml(r=0.999 2)for chloramphenicol and 5.0-25.0 μg/ml(r=0.999 1) for salicylic acid;the RSDs of precision,stability and repeatability tests were lower than 1%;the average recoveries chlorampheni-col and salicylic acid were respectively 99.77%(RSD=0.35%,n=9) and 99.29%(RSD=0.48%,n=9). CONCLUSIONS:The method is simple,rapid,accurate,specific,precise,and can be used for quality control of Acne tincture.

ABSTRACT

<p><b>OBJECTIVE</b>To explore an optimal anesthesia method with less impact on hemodynamics and electrocardiogram (ECG) of old patients with coronary artery disease (CHD) during abdominal operation.</p><p><b>METHODS</b>The 133 CHD patients waiting for abdominal operation were randomly divided into continuous epidural anesthesia (EA) group, general anesthesia group (GA) and combined spinal-epidural anesthesia (CSEA) group. Continuous monitoring was carried out during operation and mean arterial pressure (MAP), heart rate (HR), oxygen saturation (SaO2), abnormal ECG were measured respectively at different time for comparison and the differences of the above hemodynamic parameters and abnormal ECG features were compared among the 3 groups.</p><p><b>RESULTS</b>At the 15 min and 30 min point after anesthesia, SaO2 in GA group was significantly increased compared to that in the EA group (P < 0.05). At 15 min, 30 min and 60 min point after anesthesia, MAP in CSEA group was significantly increased compared to that in the EA group (P < 0.05). At 30 min point after anesthesia, HR in CSEA group was increased significantly compared to the EA group (P < 0.05). At 15 min and 30 min point after anesthesia, SaO2 in the CSEA group was increased significantly compare to the EA group (P < 0.05). Compared with preanesthesia (T0) in EA group, MAP, HR and SaO2 decreased significantly at 15, 30 and 60 min after anesthesia (P < 0.05). The fluctuation of the three parameters in GA and CSEA groups were relatively small (P > 0.05). As well as the comparison of abnormal ECG among the 3 groups was concerned, the incidence of ST-T changes in GA and CSEA groups were significantly lower than that in EA group at the time of 15 min, 30 min and 60 min after anesthesia and at the time of surgery termination (P < 0.05, P < 0.01). The incidence of arrhythmia in GA and CSEA groups were significantly lower than that in EA group at the time of 15 min, 30 min and 60 min after anesthesia (P < 0.05, P < 0.01). Compared with T0 in the same group, the incidences of ST-T changes and arrhythmia in GA or CSEA group at the time of 15, 30 and 60 min after anesthesia and at the time of surgery termination were significantly lower than that before anesthesia (P < 0.05, P < 0.01).</p><p><b>CONCLUSION</b>GA and CSEA have less impact on hemodynamics and have smaller incidence of abnormal ECG of old CHD patients with abdominal operation.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the tumor growth inhibition of gamma secretase inhibitor MRK003 on human multiple myeloma xenograft mice by inhibition of AKT and Notch1 expression.</p><p><b>METHODS</b>NOD/SCID mice were injected with human multiple myeloma cell lines RPMI8226 to establish a xenograft mouse model. Mice were randomized into two groups:the experimental group were injected with MRK003 at a dose of 5 mg× kg⁻¹×d⁻¹ for 14 days; the inhibitor was replaced by an equal saline in the control group. Mice were sacrificed by cervical dislocation on the next day after the last injection and tumor tissue was removed to detect the expression of Notch1 and AKT by immunohistochemistry.</p><p><b>RESULTS</b>After subcutaneous injection with RPMI8226, mice had tumor formation in 5-7 days and the largest tumor block in 10-12 days. Before RPMI8226 injection, the mean sizes of tumor block in the experimental and the control groups were 509.2 mm³, 511.2 mm³(P>0.05). 9 days after injection, the mean sizes of tumor tissue in the experimental and the control groups were 636.6 mm³, 691.2 mm³(P<0.01). On the next day after the last injection, the tumor sizes of the experimental and the control groups were 683.5 mm³ and 1798.7 mm³(P<0.01). The size of tumor block in the experimental group was significantly smaller than that of the control group(P<0.01). Immunohistochemistry staining showed that the positive expression rates of Notch1(11.1%, P<0.01) and AKT(13.3%, P<0.01) in experimental group were significantly decreased compared with the control group(Notch1: 95.6%; AKT: 93.3%). Western blot results showed that Notch1 and AKT protein in experimental group were significantly lower than those in the control group.</p><p><b>CONCLUSION</b>MRK003 could inhibit the tumor growth of human multiple myeloma xenograft mice by downregulated expression of Notch1 signaling pathway.</p>

ABSTRACT

<p><b>OBJECTIVE</b>Sepsis and septic shock remain a common problem that results in significant mortality and morbidity in pediatric intensive care units (PICU). According to literature, the use of more physiologic steroid replacement therapy is associated with hemodynamic and survival benefits in adult patients with relative adrenal insufficiency (RAI) and catecholamine-resistant septic shock. But little information is available in children. The aim of the current prospective study was to determine the prevalence of adrenal insufficiency in children with sepsis and septic shock using a low-dose adrenocorticotropic hormone (ACTH) stimulation test (1 microg/1.73 m2) in children.</p><p><b>METHODS</b>The authors performed cortisol estimation at baseline and after low-dose (1 microg/1.73 m2) ACTH stimulation at 30 mins in children during the first 24 hours in patients with sepsis or septic shock admitted to our PICU. Adrenal insufficiency was defined as a response < or = 90 microg/L. Absolute adrenal insufficiency (AAI) was further defined as baseline cortisol (T0) < 200 microg/L and RAI insufficiency by T0 > or = 200 microg/L.</p><p><b>RESULTS</b>Sixty-two consecutive cases with sepsis and septic shock admitted to PICU of Shanghai Jiaotong University Affiliated Children's Hospital from April, 2006 to March, 2007. The median age was 37.6 months (range, 2 - 168 months), and their gender distribution was 42 (67.7%) males and 20 (32.3%) females, 53 cases had sepsis (85.5%) and 9 had septic shock (14.5%). The mean pediatric critical illness score (PCIS) was 79.3 +/- 9.2 and median pediatric risk of mortality score (PRMSIII) 11.3 (5 - 19), respectively. Overall mortality of sepsis and septic shock was 27.42%. The evaluation of adrenal insufficiency was conducted as follows. (1) The mean cortisol levels at baseline (T0) and 30 mins after ACTH stimulation (T1) were (318.6 +/- 230.4) microg/L, (452.3 +/- 230.7) microg/L and (454.7 +/- 212.7) microg/L, (579.3 +/- 231.9) microg/L in patients with severe sepsis and septic shock group, respectively. There were no significant difference between the two groups (P > 0.05). (2) The proportion of patients with adrenal insufficiency in the study population was 40.3% as defined by a response < or = 90 microg/L post test. The proportion of patients with adrenal insufficiency in sepsis and septic shock were 39.6% and 44.4%, respectively (chi2) = 0.073, P > 0.05). (3) The serum T0 and T1 levels were (320.5 +/- 223.9) microg/L, (462.3 +/- 212.0) microg/L and (384.3 +/- 258.3) microg/L, (500.7 +/- 470.6) microg/L, respectively, and the proportion of patients with adrenal insufficiency were 37.8% and 47.1% in the survivors and the dead (P > 0.05). The levels of T0 and T1 were related to the PCIS (P < 0.05). The morbidity of adrenal insufficiency was not related to the PCIS, PRISMIII, and number of organ that developed functional insufficiency (P > 0.05).</p><p><b>CONCLUSIONS</b>Adrenal insufficiency may occur in patients with sepsis and septic shock in children. ACTH stimulation test may be helpful to determine whether corticosteroid therapy has a survival benefit in patients with relative adrenal insufficiency. A low-dose ACTH stimulation test can be used to evaluate the adrenal function status of severe sepsis and septic shock in children.</p>

ABSTRACT

Objective: To ascertain the process condition of extraction Mashi Granules(Herba Ephedrae, Gypsum Fibrosum, Radix Puerariae, Herba Methal, Semen Armeniacae Amerum, etc.) Methods: In combination with area under the curve of hypothermy in rabbits, process water, macerating time and boiling times were used as parameters in uniformidy design. And then appropriate excipients were selected on the basis of hygroscopicity and granulation. Results: 20 times amount of water as much as herbs was added, macerating time reached as far as 40 min, and extracted 3 times, 2h a time. Excipient formula consisted of lactose and mannitol(4∶1, w/w). Conclusion: The experimental results provide the basis for the ascertainment of extracting process and formulation of Mashi Granules.

Objective: To evaluated the granules stabilities in four conditions by taking the change of Baricalin content and granules surface color as stability test markers. Methods: The content of baicalin was determined by dual wave length iso absortion method and the granules surface color was determined by the diffuse reflectrance spectroscopy. ① The granules were tested in a simulated summer environment (30 ?C , RH92.5%, 1month) and in a high intensity light condition (3000LX, 10dyas). ② The influence of humidity on the stability of the granules was investigated. ③ The expiry date of the granules was predicted by the mutivariate linear regression analysis and compared with the result of the classical constant temperature method. ④ The influences of temperature and humidity on the granules stability were etudied by orthogonal test. Results: ① There was hardly any change in baicalin content of samples in a simulated summer environment and in a high intensity light condition. ② The results showed that the decrease of baicalin content and charge of granules color were obvious when the relative humidity of environment was high and the granules color was more easily influenced by the humidity. ③ The expiry date was about 2.87 years. ④ The results indicated that both temperature and humidity had effects on the change of baicalin content. Conclusion: We tried to estimate the stability of the granules by two indexes. The results showed that Qingrejedu granules was stable in different conditions. It satisfied the stability requirements of solid dosage form.

ABSTRACT

AIM: To optimize the extraction and purification of water-soluble active ingredient of Ferula sinkiangensis.METHODS: First,the comprehensive scoring system was set up to reflect the change of the extraction rate and ferulic acid coutent.in combination with the uniform design.RESULTS: The optimized extracting process was as follows: adding 8 times water and extracting twice,every time lasting 2.5 h.The optimized centrifugation purificating process consisted of: concentration ratio (1 ∶ 3),35 minutes centrifugal time,4 000 r/min rotation speed.The optimized ZTC1 + l-Ⅱ clarificant purification process was as follows: concentration ratio was 1 ∶ 22,the content of pacificator was 22% ,purification time was 1 h.CONCLUSION: The optimized extraction is available,reasonable,and reproducible.The results of pharmacodynamic test show that centrifugation is suitable to the purification of extract liquid.

ABSTRACT

AIM: To select the prescription of harmine hydrochloride cream and to study the anti-itching effect of harmine hydrochloride cream. METHODS: A comprehensive score system,including appearance,homogeneity,coating performance,viscosity,particle diameter,temperature-resistant test,and centrifugal effect were established as evaluative criteria. Then uniform design was used for selecting the prescription of harmine hydrochloride cream. The histamine phosphate and 5-hydroxytryptamine (5-HT) were used to induce mouse cutaneous pruritus. Then the anti-pruritic effect of harmine hydrochloride cream in contrast to Piyanping Ointment as control group. RESULTS: The optimized prescription was as follows: 8% of monostearin,5% of Vaseline,6. 1% of beeswax, 1. 1% of Span-60,1. 9% of Tween-80. The local application of harmine hydrochloride cream could alleviate the itching induced by histamine phosphate and 5-HT,respectively. And the anti-pruritic effect of harmine hydrochloride cream was stronger than the effect of the positive control group. CONCLUSION: The prescription of the harmine hydrochloride cream is available,reasonable,and reproducible. The harmine hydrochloride cream has the anti-pruritic effect.
