ABSTRACT

The pathogenesis of autoimmune diseases(AID)involves the inflammatory response of the human body, and inflammatory pathways are also shared by mechanisms of aging.Clinical observations of AID have revealed evidence of aging-related diseases in younger adults.This article reviews the literature related to research progress on AID combined with aging-related diseases, such as cardiovascular diseases, diabetes and osteoporosis, and briefly explains the underlying mechanisms, in order to promote interdisciplinary research between the two fields.

ABSTRACT

Objective:To assess the predictive value of histogram parameters derived from synthetic MRI for extramural venous invasion (EMVI) of rectal cancer.Methods:Totally 76 patients with pathologically proven rectal adenocarcinoma were enrolled in this retrospective study from November 2018 to December 2019 in Cancer Hospital, Chinese Academy of Medical Sciences. All participants underwent preoperative rectal MRI examination including MAGiC within 4 weeks before surgery. The status of EMVI on MRI was independently assessed by one junior and one senior radiologist. Histogram parameters were extracted from T 1, T 2 and proton density (PD) mapping, including mean, variance, maximum, minimum, 10 th percentile, median, 90 th percentile, energy, kurtosis, entropy and skewness. With postoperative pathological result as the gold standard, the patients were divided into EMVI-positive group ( n=18) and EMVI-negative group ( n=58). The Mann-whitney U test was used to compare the differences in histogram parameters between the two groups. The ROC curves were used to explore the predictive performance for assessing EMVI. The logistic regression analysis was used to combine the assessment of radiologists with parameters whose area under the ROC curve (AUC)>0.7. The Delong test was used to analyze the differences of diagnostic efficacy between different methods in predicting EMVI. Results:Significant differences of the energy of T 1, T 2 and PD mapping and skewness of PD mapping were observed between the EMVI-positive and EMVI-negative group ( P<0.05), with the AUC of 0.744, 0.728, 0.708 and 0.652. The AUC of junior radiologist in evaluating EMVI was 0.711, and the AUC of the combination with energy of T 1, T 2 and PD mapping was 0.817, showing a statistically significant difference ( Z=2.281, P=0.023). The AUC of senior radiologist in evaluating EMVI was 0.837, and the AUC of the combination with energy was 0.856. There was a significant difference in AUC between junior and senior radiologists in assessing EMVI ( Z=2.587, P=0.010), while there was no significant difference between junior radiologist combined with energy and senior radiologist ( Z=0.578, P=0.563). Conclusion:The histogram parameters based on quantitative mapping of synthetic MRI were useful for predicting EMVI of rectal cancer.

ABSTRACT

Objective@#To evaluate the value of T2WI signal intensity related parameters that can be obtained by magnetic resonance imaging (MRI) for predicting pathological complete response (pCR) after neoadjuvant chemoradiotherapy (nCRT) in patients with locally advanved rectal cancer (LARC).@*Methods@#Signal Intensity of Tumor (SIT) and Signal Intensity of Tumor/Muscle (SIT/M) of MR T2WI before and after neoadjuvant chemoradiotherapy of 101 patients with locally advanced rectal cancer were evaluated by two experienced readers independently. Signal Intensity of Tumor Reduction Rate (SITRR) and Signal Intensity of Tumor/Muscle Reduction Rate (SIT/MRR) were calculated. The difference of related parameters of T2WI tumor signal intensity between the pCR and the non-pCR group were analyzed. Receiver operating characteristic (ROC) analysis was used to assess the diagnostic performance for predicting pCR.@*Results@#Of the 101 patients, 18 were in pCR group and 83 were in non-pCR group. In all patients, the SITpre, SITpost, SITRR, SIT/Mpre, SIT/Mpost and SIT/MRR measured by reader 1 were 197.0 (133.0), 144.2 (69.7), 0.4% (0.5%), 2.6 (0.6), 3.0 (2.3) and 0.4 (0.2)% in pCR group, and 227.0 (99.0), 205 (95.4), 0.1% (0.6%), 2.6 (0.6), 2.6 (1) in non-pCR group, respectively. SITpre, SITpost, SITRR, SIT/Mpre, SIT/Mpost and SIT/MRR measured by reader 2 were 193.0 (135.0), 143.0 (69.8), 0.4% (0.2%), 2.6 (0.6), 1.5 (0.5) and 0.39% (0.2%) in pCR group, and 234.0(108.0), 203(96.5), 0.1% (0.3%), 2.6 (0.6%), 1.7 (0.7) and 0.25% (0.2%) in non-pCR group, respectively. Between the pCR and non-pCR group, there were significant differences in SITpost, SIT/Mpost and SIT/MRR measured by both readers (all P<0.01), but there was no significant differences in SITpre and SIT/Mpre (P>0.05). The difference of SITRR measured by reader 1 was not statistically significant (P=0.415), while the difference of SITRR measured by reader 2 was statistically significant (P=0.001). In patients with rectal non-mucinous adenocarcinoma, SITpost, SIT/Mpost, SITRR and SIT/MRR measured by two physicians were still statistically significant between the pCR and non-pCR group (all P<0.01), but SITpre and SIT/Mpre had no significant difference (P>0.05). ROC curve analysis showed that in all patients, the area under curve (AUC) of SITpost, SIT/Mpost and SIT/MRR for predicting pCR to neoadjuvant chemoradiotherapy in locally advanced rectal cancer was 0.694-0.762, the sensitivity was 68.2%-77.3%, and the specificity was 63.6%-77.3%. In rectal non-mucinous adenocarcinoma patients, the AUC, sensitivity and specificity was 0.704-0.764, 62.7%-78.9% and 66.2%-84.2%, respectively.@*Conclusions@#T2WI signal intensity related parameters are potential predictors for pCR in locally advanced rectal cancer after neoadjuvant chemoradiptherapy. The predictive value is higher in non-mucinous adenocarcinoma.

ABSTRACT

Objective To explore the relationship between plasma adiponectin, visfatin, leptin, and resistin levels, and the onset of colonic polyps in prediabetes subjects. Methods A total of 468 prediabetes subjects, who received colonoscopy examination, were enrolled in this study, including 248 cases of colon polyps (polyps group with prediabetes) and 220 cases without colonic mucosal lesions ( polyps-free group with prediabetes). According to the clinical characteristics of colonic polyps, colonic polyps patients with prediabetes were subdivided into single polyp group, multiple polyps group, low-risk polyps group, and high-risk polyps group, respectively. In addition, 108 subjects with normal glucose tolerance, who were matched with prediabetes subjects on gender and age, were selected as control group, and 46 cases of them were refered to polyps group with normal glucose tolerance and 62 cases were refered to polyps-free group with normal glucose. Plasma adiponectin, visfatin, leptin, and resistin levels were measured in all subjects, and related risk factors of colonic polyps in prediabetes patients were analyzed. Results Not only in normal glucose tolerance subjects, but also in prediabetes subjects, plasma visfatin levels in polyps group were significantly higher than those in polyps-free group (P<0.05), and plasma adiponectin levels were significantly lower than those in polyps-free group [normal glucose tolerance (9.8±4.8 vs 13.3±3.9)mg/L, P<0.05; prediabetes (5.6 ± 3.7 vs 9.2 ± 4.4)mg/L, P<0.01], respectively. However, no significant difference in the plasma leptin and resistin levels were observed between polyps-free group and polyps group ( both P>0. 05), respectively. In addition, in prediabetes subjects, plasma visfatin levels increased (P<0.05) and adiponectin levels decreased significantly [(4.3 ± 2.6 vs 6.7 ± 3.9) mg/L, P<0.05] in multiple polyps group than in single polyp group. Nevertheless, there were no significant differences in plasma leptin and resistin levels between two groups (both P>0.05). Moreover, plasma adiponectin levels decreased significantly in high-risk polyps group with prediabetes than in low-risk polyps group with prediabetes[(3.7±2.9vs7.4±3.5)mg/L,P<0.05].Meanwhile,noneofplasmavisfatin,leptin,andresistinlevels had shown significant difference between two groups (all P>0.05). The multivariate logistic regression analysis found that adiponectin was an independent protective factor for colon polyps, multiple colon polyps and high-risk colon polyps. Conclusion The changes of plasma adiponectin levels might be associated with onset of colonic polyps in prediabetes.

ABSTRACT

Objective To compare the predictive value of radiomics signature extracted from MRI plain and enhancement sequence for the disease-free survival (DFS) of rectal cancer. Methods We retrospectively analyzed fifty-one patients with rectal adenocarcinoma confirmed by biopsy from October 2010 to December 2013 in Cancer Hospital Chinese Academy of Medical Sciences.All patients underwent neoadjuvant chemotherapy(nCRT)followed total mesorectal excision(TME),and MRI scans were performed before nCRT.Follow-up time for the survival patients were more than 3 years.The image segmentation was performed on the T2WI sequence of the small FOV and the multi-phase enhancement sequence venous phase,respectively.Least absolute shrinkage and selection operator(LASSO)Cox regression was applied to extract radiomics features and the imaging signature was constructed. According to the radiomics score of each patient,the patients were divided into the high risk group with shorter DFS and the low risk group with longer DFS. A 3-year DFS was calculated for radiomics signature using the Kaplan-Meier product limit method with univariate log-rank analysis testing for differences in the training and validation cohort, respectively. And the predictive ability of the model was evaluated by concordance index (C-index). Results The training set and the validation set were 36 and 15 cases, respectively. During follow-up 32 patients experienced relapse(26 distant,3 local and 3 both),and 19 cases were censored.Twelve features were extracted in the enhanced sequence.The radiomics signatures were significant for DFS in the training set and the validation set(P=0.000 2 and 0.009 1,respectively).The C-index of the model were 0.904 and 0.700 in the training set and the validation set, respectively. The model has the better ability to predict survival.Two features were extracted in the plain sequence.The radiomic signature was significant for DFS in the training set(P=0.005 0),while the radiomics signature was not significant for DFS in the validation set (P=0.767 0). The C-index of the model were 0.711 and 0.500 in the training set and the validation set, respectively.Conclusions Radiomics signature extracted from MRI venous phase enhancement sequence superior to plain sequence for predicting the DFS of rectal cancer before nCRT.

Colon cancer stem cells have uncontrolled self-renewal capabilities,a potential of forming colon cancer and resistance to conventional anti-tumor therapy.Insulin resistance related factors mainly include adiponectin,leptin and various inflammatory factors.Various insulin resistance related factors have different effects on the proliferation,apoptosis,migration,carcinogenesis of colon cancer stem cells through different mechanisms.Understanding the relationship between various insulin resistance related factors and colon cancer stem cells can provide a new treatment for colon cancer.

ABSTRACT

Objective:To optimize the forming process of Luomai granules. Methods: With the qualified granule yield and the moisture absorption as the indicators, the excipients were screened, and through the study of granulation effect, the proportion of excip-ients was optimized. With the normalized value of qualified granule yield, solubility, hygroscopicity and sensory evaluation as the eval-uation indices, and excipients times, ethanol concentration and amount of citric acid as the main influencing factors with five levels for each, the forming process of Luomai granules was optimized by central combination design-response surface methodology. Results:The option formula for Luomai granules was the follows:the excipients amount was 5-fold of the extract, 85% ethanol was used as the wet-ting agent, and citric acid content was 0. 3%. Conclusion:Optimized by the central combination design-response surface method, Lu-omai granules has such advantages as high qualified yield, low hygroscopicity, good solubility, soft taste and appropriate dosage, sug-gesting the optimized preparation process of Luomai granules is reasonable and feasible.

ABSTRACT

Objective:To analyze the volatile components in compound Shenqiyizhi powder. Methods: The technique of head-space solid phase micro extraction( HS-SPME) with gas chromatography-mass spectrometry ( GC-MS) was applied to study the vola-tile components in the compound powder. The compound essential oil was analyzed by steam distillation( SD) with GC-MS. Results:A total of 117 kinds of volatile components were detected, and among them, 70 kinds of chemical compositions were identified from the compound powder, accounting for 94. 46% of the total volatile components. The component with the highest content wasβ-himachalene followed by ylangene (15. 56%) and β-asarone (7. 36%). Totally 66 kinds of volatile components were detected from the compound essential oil by SD-GC-MS, and among them, 43 kinds were identified, accounting for 77. 41% of the total volatile components. The results indicated that the main volatile components were β-asarone (16. 73%), eugenol methyl ether (6. 74%) and ylangene (5. 81%). Conclusion:The main volatile components in Shenqiyizhi powders areβ-cedrene, ylangene,β-asarone,α-asarone and el-emicin etc. The research can provide scientific basis for the analysis of volatile components.

ABSTRACT

Objective:To analyze and compare the volatile components in vinegar-processed schisandrae sphenantherae fructus and vinegar-processed schisandrae chinesis fructus.Methods:The volatile components in vinegar-processed schisandrae sphenantherae fruc-tus and vinegar-processed schisandrae chinesis fructus were extracted by headspace solid phase-microextraction (HS-SPME) and quali-tatively analyzed by GC-MS.Results:Totally 20 kinds of constituents were identified from vinegar-processed schisandra sphenanthera fructus, which accounted for 99.55%of the total volatile components , and totally 21 kinds of constituents were identified from vinegar-processed schisandra sphenanthera fructus, which accounted for 99.90% of the total volatile components .Conclusion: The type and content of volatile components in vinegar-processed schisandrae sphenantherae fructus and vinegar-processed schisandrae chinesis fructus are quite different , and the study can provide scientific basis for the two traditional Chinese medicinal materials .

ABSTRACT

Objective To observe the clinical effect of maintenance chemotherapy by pemetrexed in advanced non-small-cell lung cancer. Methods 60 patients with advanced non-small-cell lung cancer (stage Ⅲb~ Ⅳ) were randomly divided into observation group and control group , all the patients showed complete or partial remission after receiving 4 cycles of induction chemotherapy. Observation group was given pemetrexed maintenance (500 mg/m2 on days 1 every 21 days) until first disease progression, control group was given best supportive care . Progression free survival ( PFS ) , overall survival ( OS ) and adverse reaction were observed . Results PFS and OS of observation group were significantly higher than control group, PFS was (7.3 ± 1.20) vs (3.5 ± 1.13) months, OS was (12.2 ± 1.14) vs (7.1 ± 1.23) months, the difference was statistically significant. The main side effects were fatigue and leucopenia which were tolerable. Conclusion Maintenance therapy with peme-trexed following initial therapy was feasible, and extended the PFS and OS, and shows a favorable toxicity profile.

ABSTRACT

Background The mitochondrial Na+/Ca2+exchanger, NCLX, plays an important role in the balance between Ca2+influx and efflux across the mitochondrial inner membrane in endothelial cells. Mitochondrial metabolism is likely to be affected by the activity of NCLX because Ca2+activates several enzymes of the Krebs cycle. It is currently believed that mitochondria are not only centers of energy produc-tion but are also important sites of reactive oxygen species (ROS) generation and nucleotide-binding oligomerization domain receptor 3 (NLRP3) inflammasome activation. Methods&Results This study focused on NCLX function, in rat aortic endothelial cells (RAECs), induced by glucose. First, we detected an increase in NCLX expression in the endothelia of rats with diabetes mellitus, which was induced by an injection of streptozotocin. Next, colocalization of NCLX expression and mitochondria was detected using confocal analysis. Suppression of NCLX expression, using an siRNA construct (siNCLX), enhanced mitochondrial Ca2+influx and blocked efflux induced by glucose. Un-expectedly, silencing of NCLX expression induced increased ROS generation and NLRP3 inflammasome activation. Conclusions These findings suggest that NCLX affects glucose-dependent mitochondrial Ca2+signaling, thereby regulating ROS generation and NLRP3 in-flammasome activation in high glucose conditions. In the early stages of high glucose stimulation, NCLX expression increases to compensate in order to self-protect mitochondrial maintenance, stability, and function in endothelial cells.

ABSTRACT

Objective To evaluate the relationship between glucose-regulated protein 78 (GRP78) and diabetes mellitus-induced influence on myocardial protection provided by remifentanil postconditioning in vitro.Methods H9c2 cells were cultured in DMEM/F12 culture medium supplemented with 10% fetal bovine serum.The cells were seeded in 96-well (100 μl/well) or 6-well (2 ml/well) plates at the density of l05 cells/ml.The cells were then randomly divided into 6 groups (n =24 each) using a random number table:normoglycemic control group (group NC),normoglycemic hypoxia/reoxygenation (H/R) group (group NHR),normoglycemic remifentanil postconditioning group (group NRP),hyperglycemic control group (group HC),hyperglycemic H/R group (group HHR),and hyperglycemic remifentanil postconditioning group (group HRP).In NC,NHR and NRP groups,the cells were cultured in normoglyccmic culture medium (5.5 mmol/L) for 48 h.In HC,HHR and HRP groups,the cells were incubated in hyperglycemic culture medium (25.0 mmol/L) for 48 h.In NHR,NRP,HHR and HRP groups,after changing the culture medium for Tyrode solution,the cells were exposed to 95% N2-5% CO2 in an incubator at 37 ℃ for 5 h.Subsequently,in NHR and HHR groups,the culture medium was changed to DMEM/F12 culture medium supplemented with 10% fetal bovine serum and glucose at the corresponding concentration,and the cells were incubated for 1 h;in NRP and HRP groups,the cells were incubated for 1 h in the DMEM culture medium containing remifentanil at the final concentration of 1 μmol/L.At 1 h of reoxygenation,the cells of 9 wells in each group were selected to measure the cell viability by CCK8 assay,the cells of 12 wells in each group were selected to determine the activity of lactic dehydrogenase (LDH) released in the supernatant using colorimetric method,and the cells of 3 wells in each group were selected to detect the expression of GRP78 by Western blot.Results Compared with group NC,the cell viability was significantly decreased,the LDH activity was increased,and the expression of GRP78 was up-regulated in group NHR.Compared with group NHR,the cell viability was significantly increased,the LDH activity was decreased,and the expression of GRP78 was down-regulated in group NRP,and the cell viability was significantly decreased,the LDH activity was increased,and the expression of GRP78 was up-regulated in group HRP.Compared with group HC,the cell viability was significantly decreased,the LDH activity was increased,and the expression of GRP78 was up-regulated in group HHR.There was no significant change in the parameters mentioned above between group HRP and group HHR.Compared with group NRP,the cell viability was significantly decreased,the LDH activity was increased,and the expression of GRP78 was up-regulated in group HRP.Conclusion Up-regulation of GRP78 expression may be involved in the mechanism by which diabetes mellitus negates myocardial protection induced by remifentanil postconditioning in vitro.

ABSTRACT

Objective To observe the effects and toxicity of paclitaxel liposome combined with cisplatin in the treatment of ad-vanced non-small cell lung cancer .Methods 80 cases of patients of advanced non-small cell lung cancer were randomly divided into study group and control group ,40 cases in each group .Study group was treated with paclitaxel liposome (135 mg/m2 ,IV drip 3 h , first days) ,control group received paclitaxel(135 mg/m2 ,IV drip 3 h ,first days) ,the two groups combined with cisplatin 75 mg/m2 ,IV drip ,for 3 days ,take 21 days as one cycle .Results The study group CR 0 .0% (0/40) ,PR 27 .5% (11/40) ,RR 27 .5% (11/40);control group CR 0 .0% (0/40) ,PR 25 .0% (10/40) ,PR 25 .0% (10/40) ,(P>0 .05) .The research group of rash ,muscle and joint pain ,peripheral neuropathy was significantly lower than that of the control group (P0 .05) .Conclusion Paclitaxel liposome combined with cisplatin in treatment of advanced non-small cell lung cancer clinical efficacy and long-term efficacy of taxol and quite ,but with less allergic reaction .
