ABSTRACT

Ulcerative colitis (UC) is a common inflammatory bowel disease (IBD) in clinical practice, characterized by symptoms such as abdominal pain, diarrhea, and bloody mucus in the stool. It is difficult to cure and has a high recurrence rate. The pathogenesis of UC is related to abnormal immune response, oxidative stress in intestinal tissues, and inflammatory reactions. As reported, the abnormal activation of the NOD-like receptor pyrin domain-containing protein 3 (NLRP3) inflammasome is involved in the pathological process of UC. This activation triggers pathological mechanisms such as oxidative stress, pyroptosis, and inflammation in intestinal epithelial cells. Therefore, blocking the abnormal activation of NLRP3 is beneficial for alleviating UC. Currently, western medicine treatment for UC mainly includes salicylic acid derivatives, corticosteroids, and biologics, but the overall efficacy is unsatisfactory. Traditional Chinese medicine (TCM) treatment of this disease has the advantages of significant efficacy and low recurrence rate. In recent years, great advances have been made in the basic research of using TCM methods to treat UC. Studies have found that TCM intervention targeting the NLRP3 inflammasome can significantly promote intestinal mucosal healing and treat UC, and the mechanism of action involves multiple targets, levels, and pathways. This article summarized the experimental research on the impact of TCM targeting the NLRP3 inflammasome on UC in recent years, and found that NLRP3 interacted with factors such as Caspase-1 and nuclear factor-κB (NF-κB), thereby promoting the release of pro-inflammatory factors and cell pyroptosis in intestinal epithelial cells. This activation triggered oxidative stress, inflammatory reactions, and other pathological mechanisms. TCM acted on the NLRP3 inflammasome and its upstream and downstream factors to block the pathological process of UC, inhibit the pathological damage to the intestinal mucosa, and thereby alleviate colonic ulcers. The findings of this study provide a theoretical basis for the prevention and treatment of UC and further drug development.

ABSTRACT

Objective:To study the prevalence and distribution of adult thyroid diseases in urban and rural areas of Hebei Province.Methods:A multi-stage stratified cluster sampling method was used to select Renqiu City and Licun Town, Luquan City of Hebei Province as the urban and rural survey sites, respectively. Questionnaire survey, physical examination and thyroid B ultrasound examination were conducted on local permanent residents (≥ 5 years of residencies) over 18 years old. The fasting venous blood sample was collected to determine the serum thyroid function indicaters.Results:A total of 2 650 adults were surveyed, including 1 393 urban residents and 1 257 rural residents (1 357 males and 1 293 females). A total of 435 patients with thyroid diseases were diagnosed, the detection rate was 16.42%. There were seven thyroid diseases, including subclinical hypothyroidism (60.92%, 265/435), Hashimoto's thyroiditis (34.02%, 148/435), hypothyroidism (4.83%, 21/435), simple goiter (3.22%, 14/435), hyperthyroidism (2.53%, 11/435), subclinical hyperthyroidism (2.53%, 11/435), and thyroid cancer (1.84%, 8/435). The detection rates of thyroid diseases in urban and rural areas were 21.18% (295/1 393) and 11.14% (140/1 257), respectively. The detection rates of thyroid diseases in males and females were 11.42% (155/1 357) and 21.66% (280/1 293), respectively. The detection rates of thyroid diseases in 18-< 30, 30-< 40, 40-< 50, 50-< 60 and ≥60 years old were 13.46% (91/676), 14.81% (81/547), 15.42% (89/577), 20.94% (85/406) and 20.05% (89/444), respectively. There were statistically significant differences between different areas, gender and age groups (χ 2 = 48.54, 50.53, 14.68, P < 0.05). Conclusions:The detection rate of subclinical hypothyroidism in adults in urban and rural areas of Hebei Province is relatively high, followed by Hashimoto's thyroiditis. Attention should be paid to the screening, evaluation, and intervention of thyroid function among urban female populations.

ABSTRACT

Objective:To explore the effect of applying standardized patient teaching based on Calgary-Cambridge communication model in nurse patient communication training for junior college interns, and to enrich the teaching methods of nurse patient communication training in domestic medical institutions.Methods:This study was a quasi-experimental study. In July 2022, 78 students were selected from 335 junior college interns in the First Affiliated Hospital of Shandong First Medical University by random coding method, and were randomly divided into the experimental group (39 students) and the control group (39 students) by lot. The control group received routine training. The experimental group received standardized patient teaching based on Calgary Cambridge communication model: teaching the key points of communication, guiding demonstration based on Calgary Cambridge communication model, and guiding reflection and exploration. The nurse patient communication ability, nurse patient communication practice skills, communication self-efficacy, and teaching satisfaction of the two groups of interns were compared between the two groups after 8 weeks of training.Results:After training, the total score of nurse patient communication ability evaluation in the experimental group was (91.41 ± 5.35) points, higher than that in the control group (88.08 ± 7.40) points, there was significant difference ( t=2.24, P<0.05); after training, the communication self-efficacy score of the experimental group was (30.21 ± 4.28) points, higher than that of the control group (27.94 ± 5.09) points, there was significant difference ( t=2.09, P<0.05); the total score of communication practice skills in the experimental group was (173.59 ± 18.48) points, higher than that in the control group (158.44 ± 15.57) points, there was significant difference ( t=3.82, P<0.05); the total score of communication teaching and training satisfaction in the experimental group was (16.77 ± 2.94) points, higher than that in the control group (15.22 ± 1.90) points, and there was significant difference ( t=2.68, P<0.05). Conclusions:The standardized patient teaching based on Calgary Cambridge communication model can effectively improve the practical skills of nurse patient communication of junior college interns, and promote the improvement of their nurse patient communication self-efficacy, which is conducive to the improvement of nurse patient communication ability junior college intern.

ABSTRACT

Objective:To observe any effect of aerobic exercise on lipid deposition in the liver and monoacylglycerol O-acyltransferase 1 (MGAT1) signaling in non-alcoholic fatty liver disease (NAFLD).Methods:Male Sprague-Dawley rats were fed a 45% diet fat for 6 weeks, after which they were confirmed to have NAFLD. The rats were then randomly divided into an exercise group, a sedentary group and a diet adjustment group. The exercise and sedentary groups remained on the high-fat diet, but the exercise group underwent 8 weeks of aerobic exercise, while the diet adjustment group returned to a normal diet without any exercise. After the intervention, lipid accumulation in liver tissues was detected by hematoxylin-eosin staining and hepatic steatosis indices were calculated. Liver MGAT1 and the expression of peroxisome proliferator activated receptor γ (PPARγ) protein were detected using western blotting.Results:Liver steatosis indices in the exercise and the diet adjustment groups had decreased significantly after the eight weeks. The expression of MGAT1 protein had decreased significantly in the exercise group and the expression of PPARγ protein had increased significantly. Compared with the sedentary group, no significant changes were observed in the expression of MGAT1 protein in the livers of the diet adjustment group, though their average PPARγ protein expression had increased significantly. Compared with the diet adjustment group, the average liver steatosis index had increased significantly in the exercise group, but the expression of MGAT1 protein had decreased significantly.Conclusions:Aerobic exercise can significantly improve liver lipid deposition in NAFLD, at least in rats. The mechanism may be related to inhibition of the MGAT1 signal pathway. Aerobic exercise may be a rehabilitation intervention for NAFLD patients.

ABSTRACT

Objective:To observe any effect of combining the anti-asthmatic drug montelukast with exercise therapy on the pulmonary function, clinical symptoms, functional exercise capacity and quality of life of children with asthma.Methods:Seventy children (between 7 and 14 years old) with mild asthma were randomly divided into an observation group ( n=35) and a control group ( n=35). Both groups were given the anti-asthmatic drug montelukast, while the observation group was additionally provided with breathing training and aerobic pedaling training. Before and after the intervention, both groups′ forced expiratory volume in 1 second, forced vital capacity and 6min walking test (6MWT) distance were tested. Their asthma symptoms were scored using the pediatric asthma quality of life questionnaire (PAQLQ). Any adverse events during the study were recorded. Results:After 8 weeks the observation group′s average 6MWT distance, total PAQLQ score, as well as the scores on each domain of the PAQLQ (symptoms, activity restriction, emotions) had improved significantly. The observation group′s average heart rate and perceived exertion rating after the 6MWT had also improved significantly, as had their average daytime and nighttime asthma symptom scores. The control group also demonstrated significant improvement in their PAQLQ symptom scores and their daytime and nighttime asthma symptom scores after the intervention. There was no significant difference in the incidence of adverse reactions between the two groups.Conclusions:Supplementing montelukast with exercise is effective and feasible in the treatment of children with mild asthma, with good safety and tolerance. Such combined therapy is worthy of further research and promotion.

ABSTRACT

Objective To observe the effects of inhalation salbutamol combined with psychological intervention on the treatment of patients with severe asthma, and evaluate its clinical curative effect. Methods 162 cases with severe asthma were selected from February 2014 to February 2017 in hospital of traditional Chinese medicine in Zhoushan as the research object, which were randomly divided into the control group and the study group with 81 cases in each group. All patients were given nebulized salbutamol, the control group in the perioperative were given routine nursing intervention, at this basis, the study group were given targeted psychological nursing. The effect in the two groups was compared. Results The total effective rate in the study group was 91.36% (74/81) was significantly better than that in the control group 76.54% (72/81), the difference was statistically significant (P<0.05). After treatment, cough, shortness of breath, wheezing, chest tightness, lung wheezing were improved significantly , and all the symptoms in the study group was obviously better than those in the control group (P<0.05). Conclusion Aerosol inhalation of salbutamol combined with psychological intervention on the treatment of severe asthma patients has significant curative effect, which can significantly improve the clinical treatment effect, improve the clinical symptoms in a timely manner, and has the value of application and promotion.

ABSTRACT

Objective To investigate the expression and significance of E-cadherin and β-catenin in type Ⅰ endometrial carcinoma.Methods The expression of E-cadherin and β-catenin were detected by immunohistochemistry in 60 patients with type Ⅰ endometrial carcinoma and 10 normal endometrial tissues.The patients were followed-up postoperatively for the survival rate.Results Expression of E-cadherin and β-catenin in 10 normal endometrial tissues was all high expression,while those of both abnormal expression in type Ⅰ endometrial carcinoma with expression rates with 60.0 % (36/60) and 55.0 % (33/60),respectively,which were difference compared with both in normal endometrial tissues (P < 0.05).The expression rates were markedly decreased in carcinoma with lymph node metastasis than those without lymph node metastasis.The proportions of abnormal expressions of E-cadherin and β-catenin were positively correlated with tumor grade and myometrial invasion in type Ⅰ endometrial carcinoma (P < 0.05).The postoperative two-year survival rate in the patients with abnormal expression of E-cadherin and β-catenin were much lower than those with normal expression (P < 0.05).Conclusions Abnormal high expression of E-cadherin and β-catenin may play important roles on generation and metastasis in type Ⅰ endometrial carcinoma.Both of them may be promising method for predicting invasion and metastasis and can be prognostic predictors in type Ⅰ endometrial carcinoma.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the differences between the Bayesian interim analysis and the classical interim analysis.</p><p><b>METHODS</b>To compare the means of two independent samples between control and treatment, superior hypothesis test was established. In line with the data requirements for group sequential design, Type Iota error of Bayesian interim analysis based on various prior distributions, Power, Average Sample Size and Average Stage were estimated in the interim analysis.</p><p><b>RESULTS</b>In the Pocock and O' Brien & Fleming designs, the Type Iota errors in the Bayesian interim analysis based on the skeptical prior distribution and the handicap prior distribution were controlled at around 0.05. When the powers of these two classical designs were both 80%, Bayesian powers of the skeptical prior distribution and the handicap prior distribution were markedly lower. The powers of the non-informative prior distribution and the enthusiastic prior distribution were distinctly higher than 80%.</p><p><b>CONCLUSION</b>In the Bayesian interim analysis based on the skeptical prior distribution and the handicap Prior distribution, the Type Iota errors can be well controlled. Bayesian interim analyses using these two prior distributions, compared with the analysis adopting the O' Brien & Fleming method, can markedly increase the possibility of ending the clinical trials ahead of time. The Bayesian interim analyses based on these two distributions do not have practical value for group sequential design of the Pocock method.</p>

ABSTRACT

Objective To compare anti-inflammatory and hepatoprotective effects of Bupleurum marginatum Wall. ex DC. and Bupleurum chinense DC. Methods The anti-inflammatory effect of two kinds of radix bupleuri decoction were compared using the xylene-induced ear edema mice model and the egg white-induced foot swelling rats model. Acute liver injury in mice was induced through intraperitoneal injection of peanut oil with 0. 1%carbon tetrachloride. Alanine aminotransferase ( ALT ) , aspartate aminotransferase ( AST ) , and alkaline phosphatase ( ALP ) in serum, superoxide dismutase ( SOD ) , malondialdehyde ( MDA ) , and glutathione ( GSH ) in liver tissue homogenate were detected. Histopathological changes, transforming growth factor-β ( TGF-β) and nuclear factor-κB ( NF-κB) in hepatic tissues were also observed. Results Ear swelling experiment showed that the decoction of both Bupleurum marginatum Wall. ex DC. and Bupleurum chinense DC. had equal anti-inflammatory effects (P>0. 05). Similarly,the decoction of Bupleurum marginatum Wall. ex DC. and Bupleurum chinense DC. improved levels of ALT,AST,SOD,GSH,and MDA to the same extent compared with the model control group ( P 0. 05 ). Histopathological and immunohistochemical experiments showed that expressions of TGF-β and NF- κB were similar between the two kinds of radix bupleuri. Conclusion Bupleurum marginatum Wall. ex DC. and Bupleurum chinense DC. have similar anti-inflammatory and hepatoprotective effects.

ABSTRACT

OBJECTIVE To establish multiple PCR(M-PCR) assay for simultaneous detection of Treponema pallidum(TP) and herpes simplex virus(HSV),Haemophilus ducreyi(HD).METHODS According to specific gene sequence of the three agents:HSV DNA polymerse gene,TP tpp47 gene,HD 6s rRNA gene;three sets of specific primers were designed and a multiple PCR assay was developed to detect 3 agents in one test.RESULTS The target DNA of 3 agents were specifically amplified by their specific primers,the appropriate size of four DNA fragments was 202bp,252bp,152bp for TP,HSV and HD,respectively.The DNA of other several common microbes of genital tract and human genome could not be amplified by three sets of primers.CONCLUSIONS Primary study indicated that the M-PCR assay can simultaneously,specifically and rapidly detect out HSV,TP and HD.

ABSTRACT

Objective To investigate the clinical significance of antinucleosome antibody (AnuA) in the patients with lupus erythematosus (LE) through detecting the serum level of AnuA in different types of LE patients before and after treatment. Methods Enzyme-linked immunosorbent assay(ELISA) was used to detect the serum level of AnuA in 31 SLE patients, 26 SCLE patients, 7 DLE patients, 6 scleroderma patients, 5 dermatomyositis patients and 30 healthy controls. Among them, the serum levels of AnuA before and after treatment were detected in 15 SLE patients and 10 SCLE patients. Clinical data, laboratory test results and the scores of SLE disease activity index (SLEDAI) were analyzed with SPSS11.0 software. Results The serum levels of AnuA in SLE patients were higher than those in SCLE, DLE, scleroderma, dermatomyositis patients and healthy controls(P
