ABSTRACT

In recent years, a number of policies have been implemented to strengthen the cultivation of general practitioners in China. However, the development of community-level health professionals is still lagging behind, the development is uneven among regions, the overall number of general practitioners is insufficient, and the quality of medical services needs to be improved. Based on the Shanhai (Mountain and Sea) promotion project, with the close cooperation between the Second Affiliated Hospital and the medical consortium unit Suichang County People′s Hospital, a " Joint Cultivation by Dual-Teachers " model has been applied in training general practitioners for Suichang county community since March 2021. In this article we discuss the optimization and integration of medical and health resources through the outreaching assistance of high-quality personnel, management and system of medical service, to comprehensively upgrade the primary care and the quality of grass-roots general practitioners in remote mountainous areas.

ABSTRACT

OBJECTIVE To investigate the distribution of killer cell immunoglobulin-like receptors (KIR) and their specific ligands human leukocyte antigen-I (HLA-I) gene in northern China. METHODS One hundred and eighty-four unrelated northern Chinese Han individuals were recruited. Genotypes of the KIR and HLA-ABC genes were studied by sequence-specific primer polymerase chain reaction (SSP-PCR). RESULTS Sixteen KIR genes were detected among the 184 unrelated individuals. In all individuals, the four framework genes were present. The frequencies for those carrying the remaining 12 KIR genes have ranged from 16.3% to 99.5%. Twenty-four KIR genotypes were identified, for which half were detected in a single individual. A new genotype comprised of KIR2DL3, 3DL1, 2DP1 and the framework genes was detected in one subject. Respectively, 12, 27 and 11 specificities of HLA alleles were identified on the HLA-A, B, C loci. CONCLUSION The distribution of polymorphisms of KIR and its ligand HLA-ABC genes among northern Chinese Han population have been ascertained. The frequencies of 9 KIR/HLA combinations in the above population have been determined for the first time.

ABSTRACT

Objective To explore the mechanism ofHuanshuai Oral Liquid in the process of hypoxia-microvascular waste-renal interstitial fibrosis in mice with atherosclerosis renal artery stenosis.Methods The atherosclerosis renal artery stenosis model was established on Apoe-/- mice by high fat feed and uninephrectomy. After 2 weeks of renal ablation, model mice were randomly divided into TCM group (Huanshuai Oral Liquid) and model group. C57BL/6J mice were taken as the blank group. The mice of TCM group receivedHuanshuai Oral Liquid 0.01 mL/(g?d) for gavage, while the blank group and the model group received normal saline 0.01 mL/(g?d) for gavage. Renal function, atherosclerosis and renal interstitial fibrosis were assessed in the 12th week after taking medicine via immunohistochemical method to detect the expressions of the collagen typeⅣ, alpha smooth muscle actin (α-SMA) andⅧ factor related antigen (vWF), and via RT-PCR to detect the expressions of HIF-1α and TGF-β.ResultsCompared with the blank group, the model group suffered severe renal artery stenosis and renal fibrosis, and the expressions of HIF-1α, TGF-β, collagen typeⅣ andα-SMA significantly increased (P<0.05), while the expression of vWF decreased (P<0.05). Compared with the model group, the expressions of HIF-1α, TGF-β, collagen typeⅣ andα-SMA group, the expressions of HIF-1α, TGF-β, collagen typeⅣ andα-SMA significantly decreased in the TCM group (P<0.05), while the expression of vWF significantly increased (P<0.05).ConclusionHuanshuai Oral Liquid could significantly improve the renal function and the renal tubular-interstitial fibrosis. The mechanism might be associated with regulating the expressions of HIF-1α and TGF-β.

ABSTRACT

Objective To observe the photoprotective effect and possible mechanisms of resveratrol for ultraviolet A (UVA) irradiated HaCaT cells. Methods HaCaT cells under UVA irradiation with 5 J/cm2 were interfered with 0.01 mmol/L and 0.1 mmol/L resveratrol. The testing objects were divided into a control and a UVA irradiation group, and then we detected the proliferation capacity with methylthiazdyl tetrazolium (MTT) and superoxide dismutase (SOD), glutathione peroxidase (GSH-Px) activity, content of maleic dialdehyde (MDA) with hydroxylamine, colorimetric, thiobarbituric acid (TBA) methods. The ultrastructure was observed under electron microscope. Results Resveratrol could enhance the proliferation activity, SOD, GSH-Px activity of HaCaT cells under UVA irradiation, decrease the content of MDA in dose-dependent manner (P<0.05). The electron microscope revealed that resveratrol could relieve the injury of HaCaT cells'ultrastructure. Conclusion Resveratrol can relieve the inhibition to HaCaT cell proliferation, injury of their ultrastructure and oxidation by UVA irradiation. The protection is dose-dependent. That resveratrol raises the oxidase activity and clears the oxyradical may account for these results.

ABSTRACT

Objective To establish a method for the determination of chlorogenic acid in Fuyanqing perfusate (FP). Methods The sample was extracted with methyl and the chromatographic condition were as follows: C18 chromatographic column (250 mm? 4.6 mm,5 ? m),a mobile phase of methyl alcohol and water and methyl acid(30 ∶ 70 ∶ 1),the detection wavelength at 327 nm and the flow rate being 0.5 mL/min.Results A linearity of chlorogenic acid in FP was obtained in the range of 0.16 ? g~ 0.58 ? g,r=0.9996(n=6).The average recovery was 98 % and RSD =1.0% (n=6).Conclusion This method is easy,sensitive and accurate for the determination of chlorogenic acid in FP.
