ABSTRACT

Objective:To evaluate the homogenization effect of standardized residency training base of internal medicine and to provide reference for further improving the standardized residency training system. Methods:Taking all "unit residents" who took part in the standardized residency training test of Beijing resident physicians in 2015 and 2016 as the research objects, the self-assessment questionnaire survey of resident competency was carried out to analyze the training effect on competency. SPSS 21.0 was used to statistically analyze the collected data. Results:There are differences in self-assessment scores among the four system bases. From high to low, the overall self-assessment score are PUMCH, CCMU, the military, PUHSC. Among the eight abilities, the abilities of teaching and scientific research were the lowest. Conclusion:The training effect of Beijing internal medicine training base has not achieved homogenization. In order to improve the comprehensive ability of residents, it is suggested to strengthen the weak links in the standardized residency training from three aspects: teachers, training process and increasing support.

ABSTRACT

Objective To establish a reasonable evaluation system of the training effect of residents and professional master degree graduates in order to meet the needs of the construction of medical education system. Methods A questionnaire study had been conducted by stratified sampling to evaluate the abilities of the residents and medical professional degree graduates who had finished residency training in Beijing from 2015 to 2016.The trainees were evaluated by themselves, their colleagues and patients they had dealt with. Results The results showed that 95.29% of the trainees and 99.00% of their colleagues or patients thought that the overall abilities of the trainees were "excellent" or "better",and the evaluation results of all kinds of personnel above were consistent.Statistical analysis showed that some of the sub items of each ability index were evaluated better than the others, such as professional ethics, team cooperation ability and interpersonal communication ability, while some of the sub abilities need to be improved such as the information and management ability as well as teaching and research ability. The comparative analysis indicated that the postgraduates were superior to the residents in abilities of clinical diagnosis and treatment, information and management ability, and teaching and research ability. Conclusion The study showed that the training effect of residents and professional degree graduates had met the basic requirements, but the comprehensive quality and innovation ability needs to be promoted. The combination of degree education and resident training helps to cultivate the comprehensive ability of medical talents.

ABSTRACT

The treatment of late stage hepatocellular carcinoma (HCC) presently remains a great challenge. A very few drugs have been recently approved for clinical use except sorafenib and lenvatinib. After decades of failure and experience with molecular targeted and immunosuppressive therapy, immune checkpoint inhibitors are becoming one of the potentially effective therapies for patients with HCC, whose tumor is in the middle and late stages. Moreover, immune checkpoint is one of the main mechanisms of tumor immune evasion; of which programmed cell death protein 1 and its ligand (PD1/PD-L1) are important immune checkpoint targets, and its related pathway has shown to have an antitumor effect in a variety of solid or hematologic tumors and its inhibitors can effectively exert antitumor immunosuppressive effects. This review summarizes the current role of PD1/PD-L1 inhibitors in the treatment of late stage HCC, and explores the forecasting value of combined therapy strategy for HCC.

ABSTRACT

Objective Under the guidance of graduate education evaluation theory,to establish the comprehensive quality evaluation standard of medical graduate students and apply it in the research of the scholarship evaluation. Methods Byusing literature study, brainstorming, expert advice and other research methods,we established comprehensive quality assessment scale. In the work of graduate scholarship, the scale was used to carry out the evaluation work, and the application of the scale, the use value and the effect of moral evaluation were studied. Results A comprehensive quality assessment scale for medical graduate students, which included 4 dimensions of moral education, training process, scientific research and social activities was established so that the students' daily performance was quantified and evaluated, which,to a certain extent, eliminated the influence of the subjective judgment to the evaluation work, and had good guidance and practicality. Conclusion Establishing comprehensive quality quantitative evaluation mechanism of the graduate students and conducting the dynamic evaluation to graduates' moral education, their academic and social activities are conducive to stimulating the development of postgraduates' comprehensive quality and improving the quality of training.

ABSTRACT

The reform in cultivating professional-degree programs in Clinical Medicine Education contributes to the transformation of training mode, which surpasses the connotative bounds of the inherent education management. The personalized training of students has made it more challenging for both the tutor and the faculty to involve in education management, the effectiveness of which is highly valued due to the pressing need for sound medical ethics. Furthermore, the development of information technology calls for the reform of education management as well as the emphasis of student-centered approach. This article summa-rizes the existing problems of current education management mode for master degree programs of profes-sional clinical medical education. In addition, a three-level management system composed of medical experts is brewed and put forth. This system, based on compensation and restraint, assessment and incentive, ser-vice as well as security, is meant to facilitate the training of application-oriented medical talents.

ABSTRACT

The radial support capacity of intravascular stent is usually evaluated by the planar compression or the radial compression methods. Based on FEM simulation, the planer and radial compression methods are compared, and the agreement of the evaluation for the radial support capacity between these two methods is found. Moreover, the planer compression method is used to study the geometric parameters' effect on the radial support capacity by numerical simulations and experiments. Results show that, at the beginning of the compression process, the radial support capacity is mainly influenced by the metal-to-artery surface ratio; at large compression rate, the radial support capacity will decrease sharply with the increment of post-expansion diameter and decrement of the thickness and metal-to-artery surface ratio. The results provide guidance to the design and test of stents.

ABSTRACT

@#Objective:To probe the effects of the electroacupuncture on vascular dementia and its P300.Methods:60 patients suffering from vascular dementia were randomly divided into two groups, the electroacupuncture group(30 cases) and the medicine group(dihydroergotoxine,DHET,30 cases).Changes in scoring value of HDS-R(Hasegawa Dementia Scale),FAQ(Functional Activity Questionaire),ADL,injure of nerve function, main symptoms and P300 were observed before and after treatment. The course was 42 days. Results:The results was that effective rate was 60% in electroacupuncture group and 23.33% in medicine group(P<0.01).All the above indices were improved in electroacupuncture group, better than those of medicine group(P<0.05~0.01).Conclusion:It suggests that the electroacupuncture therapy can promote the VD patients in intelligence, functional activity, ability of daily living and quality of life.

ABSTRACT

Objective To obtain the recombinant protein of an antigen gene Ts88 of Trichinella spiralis and identify the characteristics of the recombinant protein. Methods Ts88 cDNA obtained by immunoscreening the cDNA library of adult T. spiralis was subcloned into the pET 28c(+) expression vector and expressed in E.coli . Mice were immunized with the fusion protein incorporated into Freund’s adjuvant and the immune sera were collected. The titers of the Ts88 immune sera and the antigenicity of the recombinant protein were detected by ELISA and Western blotting. Immuno fluorescence test was performed in order to confirm the distribution of Ts88 protein in the worm. Results The fragment of Ts88 gene was expressed successfully in E.coli and a highly purified fusion protein was obtained. Immunization with the recombinant protein in mice produced high titers of antibodies, which recognized some components of native antigens of soluble proteins from adult worm of T. spiralis. Western blotting analysis showed that Ts88 recombinant antigen was recognized by all the positive sera, such as the sera from infected or immunized rabbits, from infected swine and from patients of trichinosis. Immuno fluorescence test confirmed that Ts88 protein mainly distributed in the cuticle surface of the worm. Conclusion The Ts88 antigen gene from T. spiralis was successfully expressed. The recombinant protein presented antigenicity.
