ABSTRACT

Objective:To evaluate the significance of copy number variation (CNV) and metagenomic next-generation sequencing (mNGS) of cerebrospinal fluid (CSF) in the diagnosis of meningeal carcinomatosis (MC).Methods:Ten patients with MC diagnosed in the Department of Neurology of Peking Union Medical College Hospital from March 2022 to June 2022 were consecutively enrolled in this study. The patients were diagnosed according to the criteria of the Chinese expert consensus on the diagnosis of MC by the Chinese Society of Infectious Diseases and Cerebrospinal Fluid Cytology, and the diagnosis of MC was confirmed by CSF cytology. The control group included 10 patients who were diagnosed as autoimmune encephalitis or viral encephalitis. CSF mNGS and CNV analysis were performed simultaneously in all the patients. Results:Of the 10 patients with MC, 6 had lung adenocarcinoma, 4 had breast cancer. CSF mNGS and CNV analysis detected large CNV in 8 of 10 patients with MC, including 4 patients with breast cancer and 4 patients with lung cancer. The results of pathogenic microorganism analysis of CSF mNGS in all the patients were negative. Meanwhile, large CNV was not detected in the control group. Conclusions:CSF CNV can serve as a diagnostic marker for MC. The combination of mNGS and CNV analysis has demonstrated a high positive rate in the diagnosis of MC. The dual-omics analysis of pathogenic microorganisms and CNV has been proposed as a potential strategy to further expand the clinical utility of CSF mNGS in the realm of auxiliary diagnosis.

ABSTRACT

Objective To investigate the clinical value of hysteroscopy in helping identify the etiology of intrauterine fluid collection in postmenopausal women. Methods One hundred and thirtyfour postmenopausal women with intrauterine fluid collection, detected via transvaginal sonography, were admitted in the department of gynecology and obstetrics at Beijing ChaoYang Hospital from May 2014 to October 2016.All clinical data were retrospectively analyzed. All patients underwent hysteroscopy and biopsy of the endometrium. The results of hysteroscopy were analyzed against biopsy results. Results Hysteroscopy revealed that 116 of the 134 cases had uterine fluid(86.6%).Based on hysteroscopy results,44 (37.9 %) had endometritis,10 (8.6 %) had atrophic endometritis,5 (4.3 %) had endometrial carcinoma, and 7(6.0 %)had endometrial polyps, but the case numbers and rates for these conditions from microscopic examination were39 (33.6%),15 (12.9%),6 (5.2%),and 7 (6.0 %),respectively. When microscopic examination was used as the gold standard, the sensitivities and specificities of hysteroscopy in the diagnosis of endometritis, atrophic endometritis, endometrial cancer and endometrial polyps were 82.0 % and 85.7 %,66.7 % and 97.1%,80.0 % and 98.2 %,and 100.0% and 97.3%,respectively.The diagnostic coincidence rates were 84.6%,93.2%,94.4% and 97.4%,respectively.Conclusions Postmenopausal women with intrauterine fluid collection shown on transvaginal sonography should receive hysteroscopy for early detection of endometrial lesions.

ABSTRACT

Objective To study the relationship between gestational glucose, lipid metabolism parameters and fetal distress. Methods Retrospectively,82 cases of primipara with fetal distress and 246 cases of primipara without any obstetric complications were analyzed. The latter were treated as control group. All the patients were from the same hospital between January,2014 and January,2016.Factors as fasting plasma-glucose (FPG),triglyceride (TG),total cholesterol (TC),high-density lipoprotein (HDL-C),low-density lipoprotein (LDL-C) parameters during early pregnancy, blood glucose of fasting,1 hour and 2 hours in 75 g oral glucose tolerance test (OGTT),at 24 to 28 weeks of gestation, hemoglobin and blood pressure parameters during ante partum were recorded for all the cases while parameters as glucose and lipid, hemoglobin, blood pressure and cesarean section rate were compared between the 2 groups. Results FPG during early gestation, blood glucose of fasting,1 hour and 2 hours 75 g OGTT during mid gestation, systolic blood pressure and diastolic blood pressure during late gestation in the 'fetal distress' group were significantly higher than that of the control group.Hemoglobin during late gestation in the fetal distress group was lower than that of the control group, with statistically significant difference (P<0.05).The rate of cesarean section in the fetal distress group was higher than that in the control group (x2=4.489,P=0.034).Conclusions High BMI at pre-pregnancy,high blood glucose during early and mid-gestation,high blood pressure and anemia during late gestation were related to the fetal distress group.Cesarean section rate was high for pregnancy women with fetal distress.
