ABSTRACT

Objective@#To explore the value of quantitative CT radiomics features in predicting the anaplastic lymphoma kinase (ALK) mutation status in lung adenocarcinoma patients.@*Methods@#This retrospective study reviewed one hundred and ninety-five lung adenocarcinoma patients (including 60 patients with ALK mutation) whose ALK genetic test results were available from Nov 2015 to May 2018 in PUMCH. VOIs were labeled by an automatic pulmonary nodule detection and segmentation algorithm and were later revised and confirmed by two senior radiologists. The PyRadiomics tools were used to resample the labeled regions, followed by image pre-processing (Wavelet filter or Laplacian of Gaussian (LoG) filter) and feature extraction. Normalized features were selected based on their representativeness on Dr. Wise research platform. Multivariate logistic regression was performed to develop prediction models of ALK mutation gene based on different image pre-processing techniques and different radiomics feature types. The results were validated by ten runs of five-fold cross validation. ROC curve analysis and Delong test were used to compare the predictive performance among models.@*Results@#Fifteen radiomics features with the highest representativeness were selected from the original 1 232 features. The prediction model based on these radiomics features showed good performance (AUC=0.88 in the training set and 0.78 in the validation set) and was not significantly different from the prediction models based on radiomics features of different pre-processing images (AUC=0.76, P=0.1, original CT images; AUC=0.75, P=0.3, Wavelet-filtered images; AUC=0.76, P=0.2, LoG-filtered images). Among the models built with radiomics features of different types, the one based on GLCM feature (a subtype of texture feature) showed the best performance in predicting ALK genetic status (AUC=0.83, accuracy=0.74, sensitivity=0.85 and specificity=0.69). The model based on first-order statistic features had an AUC of 0.80.@*Conclusion@#Quantitative CT radiomics features have a good potential to anticipate the expression of ALK fused gene in patients with lung adenocarcinoma.

ABSTRACT

Objective@#To investigate the value of CT wavelet texture analysis based on primary tumor of papillary thyroid carcinoma (PTC) in predicting central lymph node metastasis (CLNM).@*Methods@#A retrospective analysis was performed to 250 patients (307 nodules) who pathologically confirm with PTC in the First Affiliated Hospital of Kunming Medical University from December 2013 to August 2019. Thyroid dual phase scanning was performed in all patients within two weeks before surgery. All patients underwent central or total cervical lymph node dissection, among which 160 cases (189 nodules) were classified as training sets, while 90 cases (118 nodules) were in the verification sets. Besides, all patients were divided into CLNM group and no CLNM group according to pathology. The DeepWise software were used to manually delineate PTC primary nodules on venous phase CT images, and 576 wavelet texture features were extracted. The differences of texture feature parameters between the two groups were compared. The top 10 wavelet texture features of the area under curve (AUC) value were manually selected as the best parameters. Multivariable logistic regression analysis was used to establish and verify the model, the optimal cutoff value was found by using receiver operating characteristic curve analysis.@*Results@#Totally 124 features were statistical difference between the two groups. The top 10 characteristic parameters for manual diagnosis with AUC values ranged from 0.599 to 0.630 (P<0.05), multi-collinearity test and multi-logstic regression analysis showed that there was no collinear correlation between the above 10 features, and small-area low-gray emphasis was an independent predictor of risk factors. The AUC value, sensitivity, specificity, and accuracy of the predictive model for the diagnosis of CLNM in the training set were 0.693, 62.84%, 60.47%, 62.96% and validation set were 0.602, 64.95%, 33.33%, and 59.32%, respectively.@*Conclusion@#Wavelet texture analysis in CT venous phase may allow detection of CLNM of PTC.

ABSTRACT

Objective To explore the relationship between apoptosis promoting effector molecules caspase-3,caspase-9 and lubar intcrvertebral disc protrusion.Methods 99 of operation patients with lubar intervertebral disc protrusion in their mater nity ward were recruited.Among them,single segment of lubar intervertehral disc protrusion were 70 (Group A),more than one segments of lubar intervertebral disc protrusion were 29(Group B).In addition,40 unrelated healthy people from physi cal examination center were enrolled as controls.Enzyme linked immunosorbent assay (ELISA) was used to examine serum caspase-3 and caspase-9 levels in lubar intervertebral disc protrusion patients.Results The level of caspase-3 in control group,group A and group B,respectively were 11.24±0.41,14.31±0.67 and 17.43±1.86 pmol/L.The caspase-3 activity in each group was statistically significant difference (F=8.47,P<0.01).The level of caspase-9 in control group,group A and group B respectively were 18.54±2.19,30.57±3.63 and 43.68±5.15 pmol/L.The caspase-9 activity in each group was statistically significant difference (F=7.85,P=0.001).Compared with control group,the caspasc 3 and caspase-9 ac tivity in group A (q=3.08.3.29,all P<0.05),group B (q=5.78,4.50,all P=0.014) was statistically significant differ ence.The caspase-3 and caspase-9 activity in group A,group B was statistically significant difference (q=3.21,3.22,all P<0.05).Conclusion The augment of caspase-3 and caspase-9 promoted apoptosis of lubar intervertebral disc protrusion.It was connected with quantity of protrusive segments.The more segments of protrusion,the higher caspase-3 and caspase-9 levels of examination would be.

Spinal cord injury (SCI) is a disabling disease usually caused by trauma. The treatment and nursing of SCI patients has brought great economic burden to the society. This article introduced the mechanism of riluzole in treating spinal cord in-jury, including blocking Na+channels, reducing glutamate-mediated excitotoxicity, promoting the expression of neuro-trophic factors, and alleviating cellular oxidative stress damage and apoptosis, and the research progress on clinical trials of riluzole.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the efficacy of three-dimensional anal and endorectal ultrasound in identifying the internal opening and tracing the tract of the anorectal fistula.</p><p><b>METHODS</b>From November 2008 to January 2010, 127 patients suffering anorectal fistula were managed with three-dimensional endoanal and endorectal ultrasound. The internal opening, the tract of the fistula and fistula trace were identified by the ultrasonography with three-dimensional imaging. All results were confirmed and compared with findings from the operation.</p><p><b>RESULTS</b>The internal opening of the fistula was specified in 116 patients, the accuracy rate was 91.3% (116/127). The internal opening of the fistula was located above the dentate line in 112 patients, and located in rectal ampulla in 4 patients. The main fistula tract was identified in all the patients, the accuracy rate was 100%. In this group, the fistula tunneled as follows: trans-sphincteric in 47 patients, intersphincteric in 75 cases, supra sphincteric in 2 cases, extra sphincteric in 3 patients. Secondary extension was found in 37 patients, the accuracy rate was 100% (37/37).</p><p><b>CONCLUSIONS</b>Three-dimensional anal and endorectal ultrasound is an effective way for localizing the internal opening and the tract of anorectal fistula. It can provide valuable information for curative operation.</p>

ABSTRACT

The method of event-related potentials (ERP) was used to study the gender difference in face recognition. The stimuli in the experiment were 10 upright and 10 inverted face photos. The subjects, half female and half male, were asked to judge whether the face was upright or inverted. The results showed that the N170 wave forms were observed in the occipito-temporal regions and they were found exhibiting brain's right hemispheric dominance. The stimuli of different gender photos were noticed to have no significant impact on the N170 wave forms, but there were significant differentce in the amplitude and latency period of N170 between different gender participants. Moreover, there was marked difference in the latency period of the male participants watching the same gender face and different gender face. Similar results were not found in the ERP's latency period of female participants.

ABSTRACT

The VEPs evoked by different colors are differently distributed on human cerebral cortex. This study sought to shed light on the spatio-temporal distribution of chromatic VEPs and compare the distribution characteristics of chromatic VEPs evoked by different colors in different cerebral cortex regions. The subjects were asked to stare at the red, green, black gradient bars against a white background. The results show that the latency periods and the amplitudes of P1 and N1 peaks from different chromatic VEPs exhibit significant differences (P<0.001). The speed and intensity of cognitive task of red are higher than those of green. The latency periods and the amplitudes of P1 and N1 peaks from different cerebral cortex regions also display remarkable differences (P<0.001). The intensity of reaction in the temporal and occipital regions is stronger than that in other regions, and the latency periods in frontal area and central area are shorter than those in temporal and occipital regions.

ABSTRACT

As a kind of physiological signals, the electroencephalogram (EEG) represents the electrical activity of the brain. Because of its higher time-varying sensitivity, EEG is susceptible to many artifacts, such as eye-movements, blinks, cardiac signals, muscle noise. These noises in recording EEG pose a major embarrassment for EEG interpretation and disposal. A number of methods have been proposed to overcome this problem, ranging from the rejection of various artifacts to the effect estimate of removing artifacts. This paper reviews many kinds of methods for artifact rejection in the EEC recently, including regression-based methods, artifact subtraction, principal component analysis (PCA), independent component analysis (ICA) and wavelet transform. The specific assumptions of each method and its advantage/disadvantage are also summarized.

ABSTRACT

The rapid development of systems biology and especially the advances in the high-throughput and comprehensive research technologies and research idea in metabonomics provide new strategies in the analysis of active components in the formula of traditional Chinese medicine (TCM) and pharmacokinetic analysis of their metabolites in vivo. Furthermore, the initiatives of metabonomics may pave a new way to explain the action mode of TCM in the light of modern sciences, while the metabonomic research achievements may contribute to the establishment of a new technique platform for evaluating the efficacy of the formula of TCM.

ABSTRACT

Objective To examine ERP components related to different processings of emotional expression of fury, neutral and smile during face-map recognition. Method Twelve undergraduate students (7 male, 5 female), 19~24 years old, were served as subjects in the experiment. The visual stimuli consisted of six kinds of pictures: three kinds of facial pictures (neutral, smile, fury) and three kinds of non-facial ones. The subjects were asked to complete two tasks, i. e., face recognition (FR) and facial expression classification (FEC). During FR task, two runs of 210 stimuli (duration: 50 ms) with each (3 facial and 3 non-facial pictures) were randomly presented with equal probability (ISI: from 1 500 ms to 2 000 ms randomly), and subjects were asked to react to facial stimuli and non-facial stimuli by pressing the left button and right button respectively as quickly as possible. During FEC task, 210 stimuli (3 facial stimuli and 3 non-facial stimuli) were randomly presented with equal probability (duration: 50 ms, ISI: from 1 500 ms to 2 000 ms), and subjects were asked to react to different facial expressions by pressing 3 different buttons respectively as quickly as possible. Electroencephalogram (EEG) was recorded with NeuroScan-32 cap (10/20 system), in reference to bilateral mastoids. Result: 1) Compared with the ERP elicited by facial stimuli during FR task, the responses during FEC task evoked a P580 component clearly. 2) A positive-potential difference after 450 ms post-stimulation between ERP during FEC task and during FR task was observed, which was significant at central and parietal sites. According to the difference of task demand between two tasks, this wave was termed the expression processing positivity (EPP) to reflect the expression processing during face recognition. 3) The peak latency of furyEPP was the shortest and smile-EPP the longest. Conclusion The mode and site of the EPP are likeIv to represent brain function in late processinq of the expression.

ABSTRACT

Objective To study early face processing of internet addiction patients by using ERP method. Methods Ten healthy young man 18~21 years and 10 internet addiction patients 17~22 years served as subjects in experiment. Two runs of 300 stimuli (duration: 50 ms) of 3 facial and 3 non-facial pictures were randomly presented with equal probability (ISI: from 1 000 ms to 1 500 ms randomly), and the subjects were asked to react to facial stimuli and non-facial stimuli by pressing the left button and right button respectively as quickly as possible. Thirty two channel electroencephalogram (EEG) was recorded by Neuroscan Nuamps System. Results 1) Specific-face component Nd170 (face N170 minus object N170) was found in both groups, which was distributed at the temporal-occipital region in control groups but at occipital region in internet addiction group; 2) Compared with Nd170 in internet addiction group, Nd170 was significantly lower at occipital negion and delayed in control group. Conclusion The early face processing mechanism of internet addiction patients may be different from normal people.

ABSTRACT

Objective To investigate effects of driving fatigue on auditory involuntary attention.Method Between-groups design was used.The control group included 13 taxi drivers after adequate rest while the fatigue group included 13 taxi drivers who had been driving for 10 h.Auditory oddball pattern was adopted.The standard stimulus was 800 Hz,probability 80%;target stimulus was 1 000 Hz,probability 10%;novel stimulus was sound generated by computer or other sound,probability 10%.Subjects were asked to press the mouse upon hearing the target sound.Result The distribution of P3a was mainly around the frontal-central area of the subjects in control group;the amplitude of P3a was evidently lowered in subjects after driving fatigue.Conclusion The ability of auditory involuntary attention declines after driving fatigue.

ABSTRACT

<p><b>OBJECTIVE</b>Wilms' Tumor Trial (WT-99) of Shanghai Children's Medical Center was designed and conducted by applying therapeutic regimens stratified by stage and histology in accordance with National Wilms' Tumor Study (NWTS) criteria of U.S.A. The main aim of WT-99 was to reduce treatment of low-stage, favorable-histology (FH) tumors without impairing survival and to improve prognosis of stage III and IV (FH) and unfavorable-histology (UFH) tremors with more intensive chemotherapy.</p><p><b>METHODS</b>Diagnosis and treatment was decided by the multi-disciplinary team including oncologists, surgeons, pathologists, radiologists and diagnostic radiologists. Twenty consecutively diagnosed patients were recruited between October 1998 and October 2002. The regimen for patients at favorable-histology (FH) stage I and II and anaplastic stage I was vincristine (Vcr) and dactinomycin (Act-D) only, while for those at focal anaplastic stage II to IV and FH stage III and IV the regimen was Vcr, Act-D and adriamycin (Adr). Patients at diffuse anaplastic stage II to IV and clear cell stage I to IV received four-drug regimen including Vcr, etoposide (VP16), Adr and cytoxan (CTX). For those at rhabdoid stage I to IV the regimen was carboplatin, VP-16 and CTX. Un-resectable patients received 2 courses of Ifosfamide, Vcr and VP-16 as pre-surgery therapy. No radiation therapy was used for patients at stage I and FH stage II.</p><p><b>RESULTS</b>Twenty patients, from 7 months to 12 years old, were enrolled. Pathologic analysis showed fourteen cases were at their FH, three at unfavorable-histology (UFH), two at clear cell and one at rhabdoid stage. Five patients were at stage I, five at stage II, six at stage III, three at stage IV and one at stage V. Eighteen reached complete response (90%), and two failed. One relapsed after 24 months of CCR and reached the second CR after intensive chemotherapy. No therapy-related death happened. Survival rate (SR) was 90% (18/20) and event-free survival (EFS) was 85% (17/20) at 11-45 months, average 27 months.</p><p><b>CONCLUSION</b>Multi-disciplinary team work model and protocol WT-99 are safe and effective for Wilms' tumor.</p>

ABSTRACT

Objective To study the response characteristics of left and right brain to signals from left(LVF) and right(RVF) visual fields during cognitive activity. Method ERPs at 9 locations to LVF and RVF signals were compared in 23 normal subjects in 3 task conditions:looking forward only (VC); making switch response to target signals(T) only(SR); making switch response to T differentially(DR). Result (1)Significant difference in ERPs was found between that induced by LVF and RVF signals on left brain especially at frontal location(F5), which appeared as slow negative deflection induced by T and NT from RVF in SR and DR;(2) Condition-dependent unsymmetrical features between left and right brain were found: the slow potential at F5 was significantly more negative in T and contralateral spatial relation(i.e., left brain for RVF) but more positive in NT and ipsilateral relation than that at F6.Conclusion Signals with psychological meaning in RVF might cause more psychological load as suggested by the results obtained in this study.
