ABSTRACT

The discharge of infants from NICU increased in recent years, it is considered to have the discharge plan modified with the aim of increasing survival rate and decreasing morbidity and mortality. Accordingly, the purpose of this qualitative study was to describe the preparations for discharge of infants from neonatal intensive care unit. A qualitative study [Conventional content analysis approach] was used. Data collection was done through interviews with 18 nurses, physicians and mothers and participant observation in neonatal intensive care units of university hospitals in Isfahan and Tabriz, Iran. MAXQDA, a qualitative data analysis package, was used for coding. Two themes and six categories were identified including: Teaching mothers [motivation, educational needs, learning recourses and enabling teachings] and providing discharge criteria [maintaining neonate`s health and trust to mother`s ability]. The result of the study revealed that not all, but some of the preparations elaborated during the process of discharge are incomplete and must be improved by developing a discharge plan to reach desired level. The result of this study can be beneficial to promote neoned at intensive care.

ABSTRACT

Pain Management is an important children right and a priority in treatment. Pain alleviation prevents severe complications and side effects and provides the children, a normal growth and development. Nurses are the key personnel who have the most interactions with pain experiencing children. They can prevent further problems by assessing and managing pain. Studying their experiences could play a valuable role in managing pain. The present study aimed at exploring the experiences of nurses of pain management in hospitalized children. A phenomenology study was recruited to conduct the study. In-depth interviews were undertaken with a theoretical purposive sample of 19 nurses working in medical-surgical and infection pediatric wards in a hospital in Esfahan. The data analysis using Coallizzi seven-stages method. Nurses experiences were studied in two main areas of assessments and interventions. Three major themes were identified namely: the ability of detection, reaction and beliefs. The sub-themes included: being aware of pain reality, caregiver role, medication dependency, having tolerance, interpersonal interaction, beliefs regarding placebo, negative attitude to mother's attendance during procedures. Nurses' experiences reflect their abilities and knowledge of pain, the influential factors and understanding the mother role. Studying their experiences is a valuable contribution in better pain management. So, it's necessary to pay more attention to the pain subject in nursing curriculum and providing pain management educational programme to train experienced nurses. The role of physician and their participation should be considered in managing children's pain. The required facilities and condition for applying standards of pain management should be provided as well

ABSTRACT

Pain is a health problem and major part of the life in childhood, pain associated with different illnesses is one of the most common symptoms that children experience in the hospitals. Relieving pain is always an essential part of nurses caring role, but yet they feel weakness in pain management. The purpose of this study was to determine pediatric pain management by nurses, in the education hospitals of Shahrekord, Iran. This research was a descriptive and analytic study which conducted on 100 nurses. The data gathering tool was a questionnaire with for subscales: demographic data, knowledge [20 items], attitude [20 items], and practice. Data were analyzed by the independent T-test, one way ANOVA test, and Pearson correlation. The finding showed that the mean knowledge, attitude and practice of pediatric pain management by nurses were 13.09 +/- 1.9, 76.1 +/- 7.2, 8.42 +/- 2.9 and majority of nurses demonstrated knowledge [73.1%], attitude [69.1%] and practice [71.3%] moderated. There was not a significant difference between practice with knowledge and attitude of nurses about management of pain in children. This study demonstrated that the knowledge, attitude and function of the nurses about the pediatric pain management were intermediate. It showed that they need to become more aware of pharmacologic pain management with opioids, risks of addiction, non pharmacologic pain management, and respiratory depression and assessment tools of pain

ABSTRACT

Intravenous [IV] insertion is a common painful procedure in pediatric care and it is a source of distress in children. Many children especially those suffering from chronic diseases such as thalassemia experince treatment procedures without any pain relief. This study was performed to compare the effect of distraction and eutectic mixture of local anesthetic [EMLA] cream on pain intensity of IV catheterizing in children with thalasemia. In this study a clinical trial was performed on 32 thalassemic children aged 5-12 years selected by convenient sampling method. The methods of distraction, EMLA cream and control [routine intervention] were used for each case in three stages with 14 days intervals. Data was collected by Ocher scales and CMFS questionnaire and was analyzed using t- and Wilcoxon test. There was a significant difference between mean pain intensity [numeral and photographic] of distraction and EMLA cream methods compared to the control, but not between distraction and EMLA cream Our findings showed that both distraction and EMLA cream procedures decreased pain of IV catheter with the same level of efficacy. However, since distraction method is more economical than EMLA ointment, it is recommended
