The major specific steps of modern planned extracapular cataract extraction are anterior capsulotomy, nucleus delivery and cortical clean-up The difficulties that are met with-in these steps are discussed with special emphasis on anterior capsulotomy. Our scheme for managing the inadequate anterior capsulotomy is mentioned together with new concepts of removal of large capsular flaps
