ABSTRACT

ABSTRACT Objective: To report outcomes from the largest multicenter series of penile cancer patients undergoing video endoscopic inguinal lymphadenectomy (VEIL). Materials and Methods: Retrospective multicenter analysis. Authors of 21 centers from the Penile Cancer Collaborative Coalition-Latin America (PeC-LA) were included. All centers performed the procedure following the same previously described standardized technique. Inclusion criteria included penile cancer patients with no palpable lymph nodes and intermediate/high-risk disease and those with non-fixed palpable lymph nodes less than 4 cm in diameter. Categorical variables are shown as percentages and frequencies whereas continuous variables as mean and range. Results: From 2006 to 2020, 210 VEIL procedures were performed in 105 patients. Mean age was 58 (45-68) years old. Mean operative time was 90 minutes (60-120). Mean lymph node yield was 10 nodes (6-16). Complication rate was 15.7%, including severe complications in 1.9% of procedures. Lymphatic and skin complications were noted in 8.6 and 4.8% of patients, respectively. Histopathological analysis revealed lymph node involvement in 26.7% of patients with non-palpable nodes. Inguinal recurrence was observed in 2.8% of patients. 10y- overall survival was 74.2% and 10-y cancer specific survival was 84.8%. CSS for pN0, pN1, pN2 and pN3 were 100%, 82.4%, 72.7% and 9.1%, respectively. Conclusion: VEIL seems to offer appropriate long term oncological control with minimal morbidity. In the absence of non-invasive stratification measures such as dynamic sentinel node biopsy, VEIL emerged as the alternative for the management of non-bulky lymph nodes in penile cancer.

ABSTRACT

ABSTRACT Prostate cancer is the most common invasive cancer in men. Radical prostatectomy (RP) is a definitive treatment option, but biochemical recurrence can reach 40%. Salvage lymphadenectomy is a relatively recent approach to oligometasis and has been rapidly diffused primarily due to improvement in imaging diagnosis and results showing possibly promising therapy. A systematic literature review was performed in March 2020, according to the PRISMA statement. We excluded studies with patients with suspicion or confirmation of visceral and / or bone metastases. A total of 27 articles were included in the study. All studies evaluated were single arm, and there were no randomized studies in the literature. A total of 1,714 patients received salvage lymphadenectomy after previous treatment for localized prostate cancer. RP was the most used initial therapeutic approach, and relapses were based on PET / CT diagnosis, with Coline-11C being the most widely used radiopharmaceutical. Biochemical response rates ranged from 0% to 80%. The 5 years - Free Survival Biochemical recurrence was analyzed in 16 studies with rates of 0% up to 56.1%. The articles do not present high levels of evidence to draw strong conclusions. However, even if significant rates of biochemical recurrence are not evident in all studies, therapy directed to lymph node metastases may present good oncological results and postpone the onset of systemic therapy. The long-term impact in overall survival and quality of life, as well as the best strategies for case selection remains to be determined.

Resumo Objetivo: Analisar a densidade endotelial das córneas doadas para transplante nas diferentes faixas etárias. Métodos: O estudo avaliou 244 córneas de 122 doadores de ambos os sexos no Banco de Olhos de Cascavel-PR a partir da análise de prontuários médicos contendo a microscopia especular dos doadores. Resultados: A maioria dos doadores de córnea era do sexo masculino (67,2%). A idade média dos doadores foi 53,8 anos com variações entre 5 e 70 anos. A faixa etária dos 61-70 anos com o maior número de doações, correspondendo a quase 41% e a densidade média endotelial foi de 2645 células/mm² (variando de 1897 a 4201 células/mm²). Na primeira década de vida, a densidade endotelial média foi 2923 células/mm²; na segunda década 2757 células/mm²; na terceira 2846 células/mm²; na quarta 2627 células/mm²; na quinta década, 2830 células/mm²; na sexta 2605 células/mm² e na sétima década de vida 2570 células/mm². Conclusão: Este estudo demonstrou que a densidade endotelial média das córneas doadas para transplante na primeira década de vida (2923 células/mm²) quando comparada isoladamente à sétima década (2570 células/mm²) teve redução. Já na avaliação da segunda até a sexta década não houve um padrão, ora havendo acréscimo, ora decréscimo da densidade endotelial média.


Abstract Objective: To analyze the endothelial density of corneas donated for transplant in different age groups. Methods: The study evaluated 244 corneas from 122 male and female donors in Cascavel Eye Bank -PR from the analysis of medical records containing specular microscopy of the donors. Results: Most corneal donors were male (67.2%). The average donors age was 53.8 years, ranging from 5 to 70 years. The age group of 61 -70 years with the largest number of donations, corresponding to almost 41% and the average endothelial density was 2645 cells / mm² (ranging from 1897 to 4201 cells / mm²). In the first decade of life, mean endothelial density was 2923 cells mm²; in the second decade 2757 cells / mm²; in the third 2846 cells / mm²; in the fourth 2627 cells / mm²; in the fifth decade, 2830 cells / mm²; on the sixth 2605 cells / mm² and on the seventh decade of life 2570 cells / mm². Conclusion: This study demonstrated that the mean endothelial density of corneas donated for transplant of the first decade of life (2923 cells / mm²) when compared to the seventh decade alone (2570 cells / mm²) was reduced. In the evaluation from the second to the sixth decade, there was no pattern, sometimes there was an increase, sometimes a decrease in the average endothelial density.

Resumo Objetivo: Avaliar o astigmatismo induzido pela cirurgia de catarata através da técnica de facoemulsificação (FACO) guiada pelo planejador cirúrgico VERION®, em um serviço oftalmológico do Paraná. Métodos: O estudo tem caráter observacional com avaliação individualizada de prontuários de forma retrospectiva, analisando 37 olhos de 20 pacientes operados de catarata pela técnica de FACO com a utilização do VERION® e implantação de lente não-tórica no Hospital de Olhos de Cascavel - PR no período de maio de 2016 a novembro de 2018. Resultados: Dentro de nossa amostra composta por 37 olhos abordados com assistência do VERION®, 43% dos participantes do estudo (n=16) apresentaram redução do astigmatismo inicial, inclusive com eliminação de graus mais graves de astigmatismo (≥2.5 D). Conclusão: O impacto do VERION® foi significativo uma vez que permitiu a correção do astigmatismo prévio de uma parte da amostragem. Em olhos que ocorreram astigmatismo induzido cirurgicamente essa complicação foi menos relevante clinicamente em comparação com incisões corneanas da técnica convencional.


Abstract Objective: To evaluate the astigmatism induced by cataract surgery through the phacoemulsification (PHACO) technique guided by the VERION® surgical planner, in an ophthalmological service of Paraná. Methods: This is an observational study with retrospective individualized evaluation of medical records, analyzing 37 eyes of 20 patients who underwent cataract surgery using the PHACO technique using VERION® and non-toric intraocular lens implantation at the Hospital de Olhos de Cascavel - PR in May 2016. Results: Within our sample of 37 eyes approached with VERION® assistance, 43% of study participants (n=16) had reduced initial astigmatism, including elimination of more severe degrees of astigmatism (≥2.5 D). Conclusion: The impact of VERION® was significant as it allowed the correction of previous astigmatism of a part of the sample. In eyes that had surgically induced astigmatism, this complication was less clinically relevant compared to conventional technique corneal incisions.

ABSTRACT

Resumo A Síndrome de Wolfram consiste em uma patologia neurodegenerativa de caráter genético, também conhecida pela sigla DIDMOAD que traduz os principais achados dessa doença, Diabetes Insipidus, Diabetes Mellitus, Atrofia Óptica e Surdez. O artigo visa relatar ocaso de um paciente diagnosticado clinicamente com essa síndrome em um ambulatório geral de oftalmologia. Tendo em vistaque os pacientes portadores dessa alteração genética apresentam mais de um par craniano afetado e quadro clínico sem históricode meningite ou outras alterações neurológicas, tem-se que pensar em alterações raras, como é o caso dessa síndrome. A partir dodiagnóstico, aplicou-se o questionário WRUS em consulta, o qual permitiu a comparação do paciente abordado com dados obtidosinternacionalmente disponíveis na literatura.


Abstract Wolfram Syndrome consists of a neurodegenerative pathology of genetic character, also known by the acronym DIDMOAD that translates the main findings of this disease, Diabetes Insipidus, Diabetes Mellitus, Optic Atrophy and Deafness. The article report the case of a patient diagnosed clinically with this syndrome in a general ophthalmology out patient clinic. Considering that patients with this genetic alteration have more than one cranial nerve affected by the disease and clinical history without meningitis or other neurological alterations, one has to think about rare alterations, as is the case with this syndrome. From the diagnosis, the WRUS questionnaire was applied in consultation, which all owed the comparation of the patient with concepts obtained internationally available in the literature.

ABSTRACT Introduction Penile cancer (PC) occurs less frequently in Europe and in the United States than in South America and parts of Africa. Lymph node (LN) involvement is the most important prognostic factor, and inguinal LN (ILN) dissection can be curative; however, ILN dissection has high morbidity. A nomogram was previously developed based on clinicopathological features of PC to predict ILN metastases. Our objective was to conduct an external validation of the previously developed nomogram based on our population. Materials and methods We included men with cN0 ILNs who underwent ILN dissection for penile carcinoma between 2000 and 2014. We performed external validation of the nomogram considering three different external validation methods: k-fold, leave-one-out, and bootstrap. We also analyzed prognostic variables. Performance was quantified in terms of calibration and discrimination (receiver operator characteristic curve). A logistic regression model for positive ILNs was developed based on clinicopathological features of PC. Results We analyzed 65 men who underwent ILN dissection (cN0). The mean age was 56.8 years. Of 65 men, 24 (36.9%) presented with positive LNs. A median 21 ILNs were removed. Considering the three different methods used, we concluded that the previously developed nomogram was not suitable for our sample. Conclusions In our study, the previously developed nomogram that was applied to our population had low accuracy and low precision for correctly identifying patients with PC who have positive ILNs.

ABSTRACT

ABSTRACT: Various equipment, mainly manual knapsack sprayers, are used to apply agrochemicals by smallholder farmers. They are highlighted by their versatility of use and mainly by their low cost for farmers. Few operator-related studies were reported in the literature regarding the effort required to operate the knapsack sprayer lever. Knapsack sprayers were evaluated regarding the effort required to move their hand levers, reach and maintain the 300 kPa pressure, and return to the initial position. The objective of this study was to evaluate the operator's effort to operate the hand lever using the "Super magro" biofertilizer as fluid. It was concluded that all knapsack sprayers are within the Consolidation of Labor Laws (CLT) standards regarding the effort required to operate the hand lever. However, restrictions have been reported in some manual knapsack sprayers regarding international standards.


RESUMO: Existem diversos equipamentos utilizados na aplicação de agrotóxicos em propriedades familiares, principalmente os pulverizadores costais manuais. Estes ganham destaque pela sua versatilidade de uso e, principalmente, o baixo custo para o produtor rural. Na bibliografia foi constatado poucos estudos relacionados ao operador, quando analisado o seu esforço ao acionar a alavanca do pulverizador costal. Os pulverizadores costais foram avaliados quanto aos esforços necessários ao deslocamento da alavanca: para atingir a pressão de 300 kPa, na manutenção desta e em seu retorno para a posição inicial. Este estudo teve como objetivo avaliar os esforços do operador ao acionar a alavanca utilizando como fluido o biofertilizante composto Super Magro. Conclui-se, no que tange ao esforço para acionamento da alavanca, que todos os pulverizadores estão dentro das normas da CLT (Consolidação das Leis Trabalhistas), porém existem restrições a alguns pulverizadores costais manuais em relação às normas internacionais.

ABSTRACT Standard radical cystectomy (RC) in women involves removal of the distal ureters, bladder, proximal urethra, uterus, ovaries, and adjacent vagina. Furthermore, pelvic organ-preserving RC to treat selected women has become an accepted technique and may confer better postoperative sexual and urinary functions than standard RC, avoiding complications such as incontinence, prolapse, neobladder-vaginal fistula (NVF), and sexual dysfunction, without compromising oncological outcome. This article reports a different surgical approach: a patient who underwent a cutaneous continent reservoir and neovagina construction using a previous ileal orthotopic neobladder after RC. Patient presented no complications and she has no evidence of recurrent disease and is sexually active, with a satisfactory continent reservoir. This case is the first report of this procedure that was able to treat concomitant dyspareunia caused by short vagina and neobladder-vaginal fistula. In conclusion, standard radical cystectomy with no vaginal preservation can have a negative impact on quality of life. In the present case, we successfully treated neobladder fistula and short vagina by transforming a previous ileal orthotopic neobladder into two parts: a continent reservoir and a neovagina. However, to establish the best approach in such patients, more cases with long-term follow-up are needed.

ABSTRACT Background and Purpose: Recent advances in cancer treatment have resulted in bet- ter prognosis with impact on patient's survival, allowing an increase in incidence of a second primary neoplasm. The development of minimally invasive surgery has provided similar outcomes in comparison to open surgery with potentially less mor- bidity. Consequently, this technique has been used as a safe option to simultaneously treat synchronous abdominal malignancies during a single operating room visit. The objective of this study is to describe the experience of two tertiary cancer hospitals in Brazil, in the minimally invasive treatment of synchronous abdominal neoplasms and to evaluate its feasibility and peri-operative results. Materials and Methods: We retrospectively reviewed the data from patients who were submitted to combined laparoscopic procedures performed in two tertiary hospitals in Brazil from May 2009 to February 2015. Results: A total of 12 patients (9 males and 3 females) with a mean age of 58.83 years (range: 33 to 76 years) underwent combined laparoscopic surgeries for the treatment of at least one urological disease. The total average duration of surgery was 339.8 minutes (range: 210 to 480 min). The average amount of intraoperative bleeding was 276.6mL (range: 70 to 550mL) and length of hospitalization was 5.08 days (range: 3 to 10 days). Two patients suffered minor complications regarding Clavien system during the immediate postoperative period. Conclusions: Combined laparoscopic surgery for the treatment of synchronous tumors is feasible, viable and safe. In our study, there was a low risk of postoperative morbidity.

The most important design requirements and their specifications are related to technical performance and production cost.


RESUMO: O presente trabalho, por meio de metodologia de projeto sistemática, objetivou hierarquizar os requisitos de projeto e estabelecer as especificações de projeto de uma semeadora puncionadora à tração humana para milho e feijão. Os requisitos de projeto mais importantes e respectivas especificações estão relacionados ao desempenho técnico e custo de produção.

ABSTRACT

RESUMO O pseudotumor esclerosante de órbita é um subtipo raro de pseudotumor inflamatório idiopático de órbita. É mais comum em adultos e apresenta diagnóstico de exclusão. A primeira linha de tratamento são os esteroides. O subtipo esclerosante apresenta resposta moderada aos esteroides devido à predominância de fibrose e colágeno na histologia. Relatamos o caso de um paciente com diagnóstico histológico de pseudotumor esclerosante de órbita que teve boa resposta ao tratamento com corticoide associado à azatioprina.


ABSTRACT Sclerosing orbital pseudotumor is a rare subtype of idiopathic orbital inflammatory pseudotumor. It's more common in adults and presents diagnosis of exclusion. Steroids represent the first option of treatment. The sclerosing orbital pseudotumor subtype shows moderate response to steroids due to the predominance of fibrosis and collagen in its histology. We report on a case of a patient with histologic diagnosis of sclerosing orbital pseudotumor successfully treated with corticosteroid associated with azathioprine.

RESUMO A toxoplasmose é uma zoonose que tem como agente etiológico o Toxoplasma gondii, que se caracteriza por ser uma doença infecciosa de distribuição mundial. Considera-se que no mundo existam mais de 2 bilhões de pessoas infectadas pela toxoplasmose. É bem conhecido que a toxoplasmose é uma causa frequente de cicatriz coriorretiniana. Nesse grupo de doenças, o exame angiográfico (ou angiofluoresceinografia de retina) é de fundamental importância para o diagnóstico. Objetivo: Realizar um levantamento da prevalência de cicatrizes coriorretinianas em angiografias em serviço privado na região de Cascavel (PR), Brasil. Métodos: Realização um estudo retrospectivo, transversal, com levantamento de documentos fonte, onde foram analisados 8719 laudos de angiografias de retina realizadas no Instituto da Visão na cidade de Cascavel (PR), Brasil, entre os anos de 2000 a 2011, sendo selecionados como primeiro diagnóstico de cada paciente, completando um total de 4928 exames válidos. As alterações encontradas foram classificadas de acordo com o tipo de cicatriz visualizada, e divididas conforme frequência percentual. Resultados: Observou-se uma prevalência de cicatrizes de 6,38%, sendo que o percentual de cicatriz coriorretiniana encontrado foi de 6,14%. Conclusão: A baixa prevalência de cicatrizes coriorretinianas encontrada na região de Cascavel (PR), Brasil, quando comparada à de Erechim (RS), Brasil, se deve a diversos fatores, tanto climáticos, socioculturais e institucionais. Além das cicatrizes coriorretinianas, que representaram o maior número entre as cicatrizes, foram encontrados outros subtipos cicatriciais com prevalências menores.


ABSTRACT Toxoplasmosis is a zoonosis whose etiologic agent is Toxoplasma gondii, which is characterized as an infectious disease of worldwide distribution. There are thought to be more than 2 billion people globally infected with toxoplasmosis. It is well known that toxoplasmosis is a frequent cause of chorioretinal scarring. In this group of diseases, the angiographic examination (or retinal fluorescein angiography) is very important for diagnosis. Objective: To perform a survey on the prevalence of chorioretinal scars in angiography at a private clinic in the area of Cascavel, Paraná State, Brazil. Methods: This was a retrospective, cross-sectional study with collection of data from source documents, involving the analysis of 8,719 reports of retinal angiograms performed at the Instituto da Visão of Cascavel, Brazil, between the years 2000-2011. The first diagnosis of each patient was selected, completing a total of 4,928 valid exams. The changes were classified according to the type of visualized scarring, and divided as percentage frequency. Results: We observed a prevalence of 6.38% scars, and the percentage of chorioretinal scars was found to be 6.14%. Conclusion: The low prevalence of chorioretinal scars found in the region of Cascavel, Brazil, when compared with the region of Erechim (Rio Grande do Sul State), Brazil, is due to climatic, sociocultural and institutional factors. Besides the chorioretinal scars, which represent the largest group of scars, other subtypes of scars with lower prevalence were found.

ABSTRACT

A hipertensão pulmonar é uma complicação comum em pacientes com anemia falciforme. A despeito das elevações leves das pressões pulmonares, a morbimortalidade desses pacientes é alta e a hipertensão pulmonar é um fator de risco muito importante nestes pacientes. A patogênese da hipertensão pulmonar relacionada à anemia falciforme é multifatorial e inclui a hemólise, baixos níveis de óxido nítrico, hipóxia crônica, tromboembolismo, doença hepática crônica e asplenia. Na maioria dos pacientes, hipertensão arterial pulmonar é a causa principal para elevações na pressão arterial pulmonar, mas hipertensão pulmonar venosa também é um fator contribuinte em alguns pacientes. Existem poucos estudos específicos avaliando os efeitos de tratamento para a hipertensão pulmonar em pacientes com anemia falciforme. É provável que a intensificação da terapia para a anemia hemolítica em todos os pacientes e tratamento específico para a hipertensão pulmonar em pacientes com hipertensão pulmonar severa seja benéfica...

In spite of the mild elevations in pulmonary artery pressures in these patients, morbidity and mortality is high and in adult patients with sickle cell disease, pulmonary hypertension is emerging as a major risk factor for death. The pathogenesis of pulmonary hypertension is likely multifactorial, including hemolysis, impaired nitric oxide bioavailability, chronic hypoxemia, thromboembolism, chronic liver disease and asplenia. In the majority of patients with sickle cell disease, pulmonary arterial hypertension is the cause of elevated pulmonary artery pressures, but also that pulmonary venous hypertension does contribute to pulmonary hypertension in a subgroup of patients. There are currently limited specific data on the effects of any treatment modality for pulmonary hypertension in patients with sickle cell disease. ABSTRACT

O milho e o feijão são semeados em praticamente todas as pequenas unidades familiares de produção, normalmente, em sistema convencional de cultivo. Há interesse no sistema de semeadura direta, porém não estão disponíveis semeadoras adequadas às suas necessidades. As semeadoras manuais, utilizadas por esses agricultores, inclusive para semeadura direta, possuem restrições de capacidade e de desempenho operacional, além de problemas ergonômicos. É necessário o desenvolvimento de máquinas desse porte, que garantam maior qualidade na operação de semeadura dessas culturas. O objetivo do presente trabalho foi estabelecer a estrutura funcional de uma semeadora puncionadora de tração humana para semeadura direta de milho e feijão. Foi aplicada uma metodologia sistemática, contemplando análise de especificações, identificação de restrições e estabelecimento das estruturas funcionais em diagrama de blocos. Foi eleita a estrutura funcional de empurrar com um dosador e múltiplos abridores de solo.


Maize and beans are grown on small farms usually tillage system. There is interest in migrating to no tillage, but are not available to small planters suitable for this purpose. The portable hand seeder (saraquá), massively used by these farmers, including no tillage, have capacity and operating performance constraints, besides ergonomic problems. It is necessary to develop machines of this size, ensuring higher quality at sowing these crops and reduce the painfulness of work these farmers. The aim of this research was to establish the functional structure of a human-powered drill for direct seeding of corn and beans. A systematic design methodology was applied, covering analysis of specifications, identifying constraints and establishment of functional structures as block diagram. Functional structure with a push seed meter and multiple soil openers was chosen.

ABSTRACT

Purpose Little is known about the effects of literacy levels on prostate cancer screening. This study evaluates the association between literacy, compliance with screening, and biopsy findings in a large Brazilian screening study. Materials and Methods We analyzed 17,571 men screened for PCa with digital rectal examination (DRE) and total and free prostate-specific antigen (PSA) from January 2004 to December 2007. Of those, 17,558 men had information regarding literate status. Full urological evaluation in a specialized cancer center was recommended in the case of: a) suspicious DRE, b) PSA > 4.0 ng/mL, or c) PSA 2.5-3.9 ng/mL and free/total PSA (f/tPSA) ratio < 15%. Transrectal ultrasound guided prostate biopsy (14 cores) was performed upon confirmation of these findings after the patient's consent. Patients' compliance with screening recommendations and biopsy results were evaluated according to literacy levels. Results an abnormal PSA, a suspicious DRE, or both were present in 73.2%, 19.7%, and 7.1% of those men who underwent biopsy, respectively. PCa was diagnosed in 652 men (3.7%). Previous PSAs or DREs were less common among illiterate men (p < 0.0001). Additionally, illiterate men were less prone to attend to further evaluations due to an abnormal PSA or DRE (p < 0.0001). PSA levels > 10 mg/mL (p = 0.03), clinical stage > T2a (p = 0.005), and biopsy Gleason > 7 (p = 0.02) were more common among illiterate men. Conclusions In a screened population, literacy levels were associated with prior PCa evaluations and with compliance with screening protocols. Illiterate men were at higher risk of being diagnosed with more advanced and aggressive PCa. .

ABSTRACT

PURPOSE: To analyze the correlation between the "International Consultation on Incontinence Questionnaire-Short Form" (ICIQ-UISF) survey and the urodynamic findings in men with urinary incontinence (UI) following radical prostatectomy (RP). METHODS: 88 men who presented post-RP UI for a minimum of 1 year were enrolled prospectively. All answered the ICIQ-UISF survey and underwent urodynamic testing. Patients were divided in 3 Groups according to their urodynamic diagnosis: Group 1, patients with sphincteric incontinence (SI) alone; Group 2, patients with mixed UI (SI + Bladder Dysfunction (BD)); and Group 3, patients with BD alone. Data were analyzed using SPSS v16.0 software. RESULTS: There were 51 men in Group 1 (57.9%); 30 in Group 2 (34%); and 7 (7.9%) in Group 3. BD was found in 37/88 patients (42%), but it was the main cause of UI in only 14 patients (15.9%). There was no statistically significant difference among the mean ICIQ-UISFs values from groups 1, 2, or 3 (p>0.05). The symptoms of stress incontinence correlated with the urodynamic finding of SI (r = 0.59), and complaints of urinary urgency correlated with the presence of detrusor overactivity (DO) (r = 0.37), but these complaints did not predict the main cause of UI. CONCLUSION: The etiology of UI following RP cannot be predicted by the ICIQ-UISF survey. Symptoms of stress and urge incontinence predict the findings of SI and DO on urodynamic tests, but they cannot ascertain the main cause of UI. Urodynamic testing remains the gold standard to assess the etiology of post-RP UI.


OBJETIVO: Analisar a relação entre as queixas clínicas mensuradas pelo "International Consultation on Incontinence Questionnaire-Short Form" (ICIQ-UISF) e os achados urodinâmicos em homens com incontinência urinária (IU) após a prostatectomia radical (PR). MÉTODOS: 88 homens que apresentavam IU por um período mínimo de 1 ano após a PR foram incluídos prospectivamente. Todos responderam o questinário "ICIQ-UISF" e foram submetidos a avaliação urodinâmica. Os pacientes foram categorizados em 3 grupos de acordo com o diagnóstico urodinâmico: Grupo 1, pacientes com incontinência esfincteriana isolada (IE); Grupo 2, pacientes com IU mista (IE + disfunção vesical (DV)); e Grupo 3, pacientes com DV isolada. Os dados foram analisados utilizando o software SPSS v16.0. RESULTADOS: Dos 88 pacientes avaliados, após a avaliação urodinâmica, 51 homens (57,9%) apresentaram IE isolada (Grupo 1); 30 homens (34%) apresentaram IE associada a DV (Grupo 2) e 7 homens (7,9%) tinham somente DV (Grupo 3). A DV foi encontrada em 37/88 pacientes (42%), mas foi a principal causa de IU em apenas 14 pacientes (15,9%). Não houve diferença estatisticamente significativa entre os valores das médias do "ICIQ-UISFs" entre os grupos 1, 2 ou 3 (p> 0,05). Os sintomas de incontinência de esforço se correlacionaram com o diagnóstico urodinâmico de IE (r = 0,59), e as queixas de urgência miccional se correlacionaram com a presença de hiperactividade do detrusor na avaliação urodinâmica (r = 0,37), entretanto apesar da correlação encontrada, os sintomas não foram capazes de identificar a principal causa da UI . CONCLUSÃO: A etiologia da UI após a PR não pode ser previsto pelo escore de sintomas obtidos através do "ICIQ-UISF". Os sintomas de perda urinária as manobras de estresse e de urgência miccional estão relacionados a presença de IE e hiperatividade detrusora na avaliação urodinâmica, entretanto estes sintomas não conseguem identificar com segurança qual é o principal fator da IU após a PR.O teste urodinâmico continua sendo o padrão ouro para avaliar a etiologia da IU após a PR.

ABSTRACT

PURPOSE: Simple diversions are underutilized, mostly for unfit, bedridden, and very self-limited patients requiring palliative surgical management due to life-threatening conditions. Experience with cutaneous ureterostomy (CU) as palliative urinary diversion option for unfit bladder cancer patients is reported. METHODS: We retrospectively reviewed clinical and operative parameters of 41 patients who underwent CU following RC in three specialized Cancer Centers from July/2005 to July/2010. Muscle-invasive disease (clinical Stage T2/worse), multifocal high-grade tumor, and carcinoma in situ refractory to intravesical immunotherapy were the main indications for RC. Double-J ureteral stents were used in all patients and replaced every 6 months indefinitly. Peri-operative morbidity and mortality were evaluated. RESULTS: Median age was 69 years (interquartile range - IQR 62, 76); 30 (73%) patients were men. Surgery in urgency setting was performed in 25 (61%) of patients, most due to severe bleeding associated with hemodynamic instability; 14 patients (34%) showed an American Society of Anesthesiologists score 4. Median operative time was 180 minutes (IQR 120, 180). Peri-operative complications occurred in 30 (73%) patients, most Clavien grade I and II (66.6 %). There was no per-operative death. Re-intervention was necessary in 7 (17%) patients. Overall survival was 24% after 9.4 months follow-up. CONCLUSIONS: CU with definitive ureteral stenting represents a simplified alternative for urinary diversion after palliative cystectomy in unfit patients. It can be performed quickly, with few early and late postoperative complications allowing RC in a group of patients otherwise limited to suboptimal alternatives. Future studies regarding the quality of life are warranted.


OBJETIVO: Relatar a experiência do emprego da ureterostomia cutânea (UC) como forma de derivação urinária definitiva em pacientes portadores de neoplasia vesical avançada, em más condições clínicas e que necessitam de tratamento paliativo. MÉTODOS: Foram analisados retrospectivamente os parâmetros clínicos e operatórios de 41 pacientes submetidos a cistectomia radical e UC em três centros oncológicos especializados. A UC foi a derivação urinária escolhida quando os pacientes não apresentavam condições clínicas de serem submetidos a outro tipo de derivação . Foram avaliados a morbidade peri-operatória e a sobrevida global. RESULTADOS: A idade média dos pacientes foi de 69 anos (intervalo interquartil - IQR 62, 76); 30 (73%) pacientes eram do sexo masculino. Vinte e cinco pacientes (61%) foram submetidos a cirurgia de urgência sendo a maioria devido a hemorragia grave associada a instabilidade hemodinâmica. O tempo cirúrgico médio foi de 180 minutos (IQR 120, 180). As complicações peri-operatórias ocorreram em 30 (73%) pacientes sendo a maioria classificadas como "Clavien" graus I e II (66,6%). Não houve óbito per-operatório. A reabordagem cirúrgica foi necessária em 7 (17%) dos pacientes e a sobrevida global foi de 24% após 9,4 meses de seguimento. CONCLUSÕES: A UC com implante de "stent" ureteral é uma alternativa simples de derivação urinária, após cistectomia paliativa, em pacientes sem condições clínicas de serem submetidos a procedimentos cirúrgicos mais complexos. A UC é um procedimento rápido e apresenta taxas de complicações aceitáveis. Essa alternativa cirúrgica permite melhorar a qualidade de vida dos pacientes portadores de tumores vesicais localmente avançados.

ABSTRACT

The current study aimed to analyze the histochemical and morphological characteristics of the levator ani muscle in rats. For this, we used 10 Wistar rats (5 males and 5 females), weighing between 200 and 765g. The animals were dissected fresh and in formalin for the levator ani muscle anatomical observation. Muscle fragments were collected and frozen in n-Hexane previously cooled in liquid nitrogen. Then, the muscles were transferred to a microtome cryostat (HM 505 E Microm), being fixed in metal mounts with the adhesive Tissue Freezing Medium. Histological sections of 6.0um were removed and subjected to HE staining. Other sections were subjected to NADH-TR and SDH reactions. After being dissected and fixed, the architecture of the female pelvic floor revealed the presence of two muscles: iliocaudalis and pubocaudalis. The anatomical inspection in male rats revealed, pronouncedly, the presence of the levator ani muscle: ischiocavernosus and bulbocavernous. We therefore observed a marked anatomical difference between animals of the same species, which does not occur with humans. The HE staining revealed muscular fibers with preserved morphology, contours ranging from polygonal to rounded, acidophilic cytoplasm, one or more peripheral nuclei with rounded shape and dense chromatin aspect. The fibers were organized in fascicles arranged by a dense connective tissue, the perimysium, and each fiber surrounded by the endomysium, composed of loose connective tissue. The sections subjected to NADH-TH and SDH, whose reactions show the activity of oxidative or glycolytic muscle fibers, allowed the identification of the two types of fiber. The fast-twitch fiber shows weaker reactivity, whereas the slow-twitch fiber has small diameter and intense reactivity, especially in the subsarcolemmal, presenting a highly oxidative metabolism. El estudio tuvo como objetivo analizar las características histoquímicas y morfológicas del músculo elevador del ano en ratas. Para esto, se utilizaron 10 ratas Wistar (5 machos y 5 hembras), con un peso entre 200 y 765g. Los animales fueron disecados frescos y en formol para la observación anatómica del músculo elevador del ano. Fragmentos de músculo fueron recogidos y congelados en n-Hexano, previamente enfriado en nitrógeno líquido. Luego, los músculos fueron trasladados a un micrótomo criostato (Microm HM 505 E), fijados en soportes metálicos con adhesivo Tissue Freezing Medium. Cortes histológicos de 6,0 um fueron retirados y sometidos a tinción de H-E. Otras secciones fueron sometidas a las reacciones de NADH-TR y SDH. Después de haber sido disecado y fijado, la arquitectura del suelo de la pelvis de las ratas hembra, reveló la presencia de dos músculos: m. iliocaudalis y m. pubocaudalis. La inspección anatómica de las ratas macho mostró, marcadamente, la presencia del músculo elevador del ano: isquiocavernoso y bulbocavernoso. Por lo tanto, observamos una marcada diferencia anatómica entre los animales de la misma especie, lo que no ocurre con los seres humanos. La tinción HE reveló fibras musculares con morfología conservada, contornos que van desde el esquema poligonal al redondeado, citoplasma acidófilo, uno o más núcleos periféricos con forma redondeada y un aspecto denso de la cromatina. Las fibras se organizaban en fascículos compuestos por un tejido conectivo denso, perimisio, y cada fibra rodeada por el endomisio compuesto por tejido conectivo laxo. En las secciones sometidas a NADH-TH y SDH, cuyas reacciones muestran la actividad oxidativa o glicolítica de las fibras musculares, permitió la identificación de los dos tipos de fibras. Con los avances en la tecnologuia y la investigacion llevada a cabo en neonatología, ya es posible ver más allá de lo que podamos para salvaguardar las vidas de recién nacidos (RN), proporcionando una calidad de vida. Para esto necesitamos tener una visión amplia que se ve más allá del individuo, el medio ambiente y el equipo que le rodea. Humanización debe impregnar todas las acciones tomadas en la Unidad de Cuidados Intensivos Neonatales (UCIN). Objetivo: En este contexto, el ruido ambiental emerge como un objeto de estudio de esta investigación tiene como objetivo identificar los niveles de ruido a que están expuestos los recién nacidos ingresados en una unidad específica de cuidados intensivos neonatales y discutirlos a la luz de las normas vigentes país. Materiales y Métodos: Teniendo en cuenta el tema de la parte delantera de esta investigación considera que es importante conocer la legislación que aborda este tema, específicamente el procedimiento estándar de Higiene Profesional Técnica (NHO 01) y la norma 15 (NR15).De acuerdo con la Norma 15 (NR 15) promulgado por el Ministerio de la Ordenanza N º 3214/1978 del Trabajo, actualizado por el Decreto 203/2011, el ruido que se clasifica como el impacto del ruido o el ruido continuo también llamado intermitente, que se caracteriza los picos de energía acústica de menos de 1 (un) segundo a intervalos superiores a 1 (un) segundo. Resultados: Comenzamos la presentación de los resultados que describen el campo de estudio, los resultados de la investigación discutidos a la luz de las normas vigentes en el país. Discusión y Conclusiones: La institución por área de estudio es un centro de referencia para cuidados intensivos neonatales del micro-región de Barbacena recibe pacientes de varios lugares. De acuerdo con la Resolución del Directorio N º 7 (RDC 07) se clasifica como un nivel de unidad de cuidados intensivos II.


With advances in technology and research carried out in the nursery, it is possible to see beyond what we can to safeguard the lives of newborn (NB), providing a quality of life. For this we need to have a broad vision that looks beyond the individual, the environment and the team around him. Humanization must permeate all actions taken in the Neonatal Intensive Care Unit (NICU). Objective: In this context, environmental noise is emerging as an object of this research study aims to identify the noise levels they are exposed to the newly infants admitted to a specific unit and neonatal intensive care discussed in the light of existing rules the country. Materials and Methods: Given the theme of the front of this investigation considers it important to know the law that addresses this issue, specifically the standard procedure of Technical Vocational Hygiene (NHO 01) and Rule 15 (NR15). According with Rule 15 (NR 15) promulgated by the Ministry Order No. 3214/1978 of Labor, updated by Decree 203/2011, the noise impact is classified as noise or continuous noise also called intermittent, which features of acoustic energy peaks of less than 1 (one) second at intervals greater than 1 (one) second. Results: We begin the presentation of results that describe the field of study, research results discussed in the light of existing standards in the country. Discussion and Conclusions: The study area institution is a referral center for neonatal intensive care of the micro-region Barbacena receives patients from several places. According to Board Resolution No. 7 (RDC 07) is classified as a level II intensive care unit.

ABSTRACT

PURPOSE: To verify the correlation between the presence of IH and the intensity of LUTS related to BPH quantified through the International Prostate Symptom Score (IPSS). METHODS: We prospectively selected 52 patients over the age of 55years; Patients were divided into 2 groups. Group 1: composed of 32 patients with IH; Group 2 (control group): composed of 20 patients with no clinical evidence of IH. All patients were assessed using the IPSS, uroflowmetry (Qmax), post-void residual urine volume (PVR) and prostate volume (PV). RESULTS: Groups 1 and 2 presented no difference in PV (p>0.05) and uroflowmetry (Qmax) (p>0.05). There was a statistical significant difference between the PVR mean values between groups 1 and 2. The presence of IH correlated with a higher IPSS score (r=0.38 p<0.05) despite the fact the no difference was detected between the incidence of patients with mild, moderate and severe LUTS in groups 1 and 2. CONCLUSION: Patients with IH present higher IPSS. The role of IPSS as a marker to predict the development of clinical IH still to be determined.


OBJETIVO: Verificar a correlação entre a presença de HI e a intensidade dos STUI quantificados através do escore internacional de sintomas prostáticos (IPSS). MÉTODOS: Foram selecionados prospectivamente 52 pacientes com idade superior a 55 anos; os pacientes foram divididos em 2 grupos. Grupo 1: composto por 32 pacientes portadores de IH, Grupo 2 (grupo controle): composto por 20 pacientes sem evidência clínica de IH. Os parâmetros avaliados foram: sintomas urinários, quantificados através do IPSS; fluxo urinário máximo (Qmax), obtido através da fluxometria livre; resíduo pós miccional (RM) e volume prostático (VP), obtido através do ultrassom transretal. RESULTADOS: Os grupos 1 e 2 não apresentaram diferença significativa em relação ao Qmax e VP (p>0.05). Foi detectada diferença estatística significativa entre em relação ao VR entre os grupos 1 e 2 (p<0.05). A presença de HI se correlacionou com o aumento da intensidade dos STUI (r=0,38; p<0.05). Não foi detectada diferença estatisticamente significativa entre a incidência de pacientes portadores de STUI, leve, moderado ou intenso entre os grupos 1 e 2. CONCLUSÃO: Pacientes portadores de HI apresentam escore de sintomas (IPSS) mais elevados, entretanto o papel dos sintomas urinários como um dos possíveis fatores causadores do aparecimento de HI ainda não foi determinado.
