ABSTRACT

Objective@#To examine the dose-response association between cardiorespiratory fitness ( CRF ) and the risk of cardiovascular and cerebrovascular diseases.@*Methods@#A joint search was performed in Chinese and English electronic databases, including China National Knowledge Infrastructure ( CNKI ) , Wanfang Data, VIP, CBM, PubMed, Embase and Web of Science, to retrieve publications reporting the correlation between CRF and the risk of cardiovascular and cerebrovascular diseases until May, 2021. The pooled risk was estimated using the random effects model, and the dose-response association was evaluated using restricted cubic splines. The source of heterogeneity was assessed by subgroup analysis, and the stability of the results was tested by the trim-and-fill method, while the publication bias was assessed using funnel plots.@*Results@#Totally 37 280 literatures were identified, and 23 eligible studies were finally included in the analysis, which covered 2 605 622 subjects. There were 22 publications identified as high-quality. Meta-analysis revealed that the pooled risk of cardiovascular and cerebrovascular diseases reduced by 42% in the highest CRF group relative to the lowest CRF group ( OR=0.58, 95%CI: 0.52-0.65 ), and a one metabolic equivalent ( MET ) increase in CRF caused a 10% reduction in the pooled risk of cardiovascular and cerebrovascular diseases ( OR=0.90, 95%CI: 0.88-0.92 ). There was a negative linear correlation between CRF and the incidence of cardiovascular and cerebrovascular diseases ( P=0.396 ). Subgroup analysis identified gender, sample size and study regions as possible sources of heterogeneity, and sensitivity analysis showed that the study results were stable.@*Conclusions@#There is a negative linear correlation between CRF and the risk of cardiovascular and cerebrovascular diseases, and an increase in CRF may reduce the risk of cardiovascular and cerebrovascular diseases.

ABSTRACT

Objective To study the characteristics of positive expression of integrin β1 in the rat brain tissue of two kinds of traumatic brain injury models and to explore the feasibility of inferring the mode of traumatic brain injury using the positive expression of integrin β1. Methods The occipital region of rats was hit by hydraulic impact method and pendulum striking method to produce two closed brain injury models of linear and rotation acceleration respectively, then 120 SD rats were randomly divided into linear acceleration injury group, rotation acceleration injury group, sham operation group and normal control group. Immunohistochemistry staining and Western blotting method were used to detect the positive expression of integrin β1 in different parts of the brain tissue at 30 min, 3 h, 6 h, 12 h, 3 d and 7 d after rat injury. The data was processed statistically by SPSS 18.0 software. Results The positive expression of integrin β1 was detected 30 min after brain injury and reached the peak 6 h after brain injury. With the extension of injury time, the expression tended to enhance. At the same time points after injury, the differences in the positive expression of integrin β1 between the linear acceleration injury group and the rotation acceleration injury group in the occipital strike point and thalamus had no statistical significance ( P>0.05), but the differences in the expression of integrin β1 in the frontal lobe and brain stem had statistical significance (P<0.05). Conclusion The characteristics of positive expression of integrin β1 in brain tissue can be used to infer the strike point and the manner of injury and has application value for the reconstruction of craniocerebral injury process.

ABSTRACT

Objective To study the iodine nutritional status of pregnant women in Foshan City,Guangdong Province,and to provide basis for iodine supplementation scientifically for them.Methods In 2017,five districts Chancheng,Nanhai,Shunde,Sanshui and Gaoming in Foshan were selected.Each monitoring site was divided into five sections according to stratified random sampling in five directions:east,west,south,north and middle.One township (street) was taken in each section,and 20 pregnant women were taken from each township (street) to detected their family salt iodine and urinary iodine levels.Results A total of 500 edible salt samples were collected from the families of pregnant women,the median of salt iodine was 24.8 mg/kg,the qualified iodized salt consumption rate was 95.8% (479/500),and the iodized salt coverage rate was 99.0% (495/500).There was a statistically significant difference in the iodine content of edible salt of pregnant women in diffierent districts (H =26.9,P < 0.05).A total of 500 urine samples were collected from pregnant women,the median of urinary iodine was 138.9 μg/L.There was a statistically significant difference in the levels of urinary iodine content of pregnant women in different districts (H =14.5,P < 0.05).There was no statistically significant difference in urinary iodine of pregnant women during different stages of pregnancy (H =2.6,P > 0.05).Conclusions The iodine intake for pregnant women is in a state of deficiency (< 150 μg/L) in Foshan of Guangdong Province.It is necessary to further strengthen the health education of key populations,and improve the iodine nutritional status of pregnant women.

ABSTRACT

Objective To compare the early clinical efficacy of unicompartmental knee arthroplasty (UKA) and total knee arthroplasty (TKA) for elderly patients with medial compartmental knee osteoarthritis.Methods A total of 35 elderly patients with medial compartmental knee osteoarthritis were selected from September 2014 to March 2018 in our hospital, of whom 16 cases treated by UKA were observation group and 19 cases treated by TKA were control group.ResultsThe operation time, intraoperative blood loss, the rate of perioperative blood transfusion, postoperative drainage volume and hospitalization stays of observation group were less than those of control group, the difference were significant (P<0.05).Before the treatment, there was no significant differences in the KSS score between two groups (P>0.05).After the treatment, KSS scores of two groups had been improved to some extent, and the observation group was better than the control group, the difference was significant (P<0.05).No revision or infection happened in 1 year post-operative follow-up of either group.The observation group (0) were less than the control group (26.32%) in thrombosis rate, the difference was significantly (P<0.05).ConclusionBoth UKA and TKA have certain early clinical efficacy, but UKA had advantages such as shorter operation time, shorter hospitalization, less pain, lower incidence of adverse reaction, which can promote the postoperative function recovery, the early clinical efficacy is more significant.

ABSTRACT

Objective @#To explore the spatial distribution of occupational diseases in Guangdong Province and to provide evidence for the policy development of occupational disease prevention and control. @*Methods @#A database of occupational disease incidence from 2009 to 2016 in Guangdong Province was built. The distribution of occupational diseases in Guangdong Province was displayed based on the geographic information system(GIS), then spatial autocorrelation analysis and trend-surface analysis were carried out to explore the clustering areas and spatial epidemic characteristics of occupational diseases in Guangdong Province.@* Results @#The number of cases with occupational diseases was 5 231 and was increasing year by year from 2009 to 2016 in Guangdong Province. The high-incidence areas were located in Guangzhou,Shenzhen,Foshan and Dongguan. Through global spatial autocorrelation analysis,it was found that there were spatial clustering of occupational diseases in Guangdong Province in each year(P<0.05),and the cumulative incidence was also clustered(Moran's I=0.492,P<0.05). The number of cases in Guangzhou,Shenzhen,Foshan and Dongguan had local spatial autocorrelation,and the local Moran's I values were 10.329,8.614,3.725 and 9.811,respectively(P<0.05). The results of trend surface analysis showed that the overall incidence of occupational disease had a slight increase from west to east,and the Pearl River Delta region was a high-incidence area. @*Conclusion @#The incidence of occupational diseases in Guangdong Province had an obvious spatial clustering,the Pearl River Delta region was a high-incidence area.

ABSTRACT

Objective To evaluate endoscopic ultrasonography (EUS) in diagnosis of gastric submucosal tumors (SMTs), and analyze the characteristics of gastric SMTs under EUS. Methods Clinical data of 614 patients with gastric SMTs, who were evaluated by EUS and underwent endoscopic submucosal dissection (ESD) from September 2008 to December 2016, were retrospectively analyzed. The golden standard for lession origins was the intraoperative diagnosis of ESD, and that for pathological types was the combination of postoperative pathological and immunohistochemical findings. The consistency of diagnosis of EUS was evaluated, and the characteristics of lesions under EUS were analyzed. Results The total consistency in diagnosing lesion origins was 91.25% between EUS and intraoperative results of ESD, and the consistency in diagnosing lesion originated from the muscularis mucosa, submucosa and muscularis propria was 66.67%, 80.85% and 94.50%, respectively. The total consistency in pathological types was 65.99% between EUS and postoperative pathological results, and the consistency of gastrointestinal stromal tumor (GIST), leiomyoma, ectopic pancreas and lipoma was 91.85%, 18.56%, 79.76% and 90.70%, respectively. Conclusion EUS can initially determine the origins and types of gastric SMTs, providing a more accurate basis for endoscopic treatment, but there were some limitations on the diagnosis of leiomyoma and some rare lesions such as hamartoma, inflammatory fibrous polyps, carcinoid, fibroma, etc. Thus, if necessary, the lesion should be removed so as to make a definite diagnosis and prevent malignant change.

Objective@#To explore the occupational disease spatial distribution characteristics in Guangzhou and Foshan city in 2006-2013 with Geographic Information System and to provide evidence for making control strategy.@*Methods@#The data on occupational disease diagnosis in Guangzhou and Foshan city from 2006 through 2013 were collected and linked to the digital map at administrative county level with Arc GIS12.0 software for spatial analysis.@*Results@#The maps of occupational disease and Moran’s spatial autocor-relation analysis showed that the spatial aggregation existed in Shunde and Nanhai region with Moran’s index 1.727, -0.003. Local Moran’s I spatial autocorrelation analysis pointed out the "positive high incidence re-gion" and the "negative high incidence region" during 2006~2013. Trend analysis showed that the diagnosis case increased slightly then declined from west to east, increase obviously from north to south, declined from? southwest to northeast, high in the middle and low on both sides in northwest-southeast direction.@*Conclusions@#The occupational disease is obviously geographical distribution in Guangzhou and Foshan city. The corresponding prevention measures should be made according to the geographical distribution.

ABSTRACT

Objective To study the effects of PM2.5 on reproductive hormone levels and pregnancy outcome in female rats.Methods Thirty healthy 4-week old female Sprague-Dawley rats were randomly divided into the control group (normal saline), low dose of PM2.5(1.5 mg/kg) group and high dose of PM2.5 (37.5 mg/kg) group.After the blood samples were collected, the animals were exposed to PM2.5for 10 days, and then the rats were mated.On the 19th day of pregnancy, the rats were sacrificed for pregnancy outcome observation and blood samples were collected for hormone test.The blood hormone levels were detected using an ELISA kit.Results The live fetus rates in the control, low dose PM2.5 and high dose PM2.5 groups were 90.77%, 59.49% and 60.27%, respectively (P0.05), the level of FSH in the high dose group was significantly decreased (P<0.05).Conclusions PM2.5may affect pregnancy outcome through influencing the hormone levels.

ABSTRACT

AIM To study the chemical constituents from Metaplexis japonica (Thunb.) Makino.METHODS The dichloromethane fraction of 95% ethanol extract from M.japonica was isolated and purified by silica and preparative HPLC column,then the structures of obtained compounds were identified by physicochemical properties and spectral data.RESULTS Nine compounds were isolated and identified as dibutyl phthalate (1),(Z)-2-ethylidene-3-methylsuccinic acid (2),dehydrololiolide (3),medioresinol (4),isovanillin (5),vomifoliol (6),β-sitosterol (7),daucosterol (8),pregnenolone (9).CONCLUSION Compounds2-4 are isolated from genus Metaplexis for the first time,compounds 5-8 are first isolated from this plant.

Methods Totally 92 cancer-related depression patients were randomly divided into two groups, with 46 patients in each group. All patients already finished the treatment such as radiotherapy and chemotherapy, and were in stable stage. The Fluoxetine Hydrochloride Dispersible Tablets was given to the patients in the control group, 20 mg each time,once a day,orally;Shugan Hewei Formula was given to those in the treatment group,a dose a day,twice a day, orally. The treatment for both groups lasted for 6 weeks. Before and after treatment, HAMD scores and KPS scores were observed; the change of cerebral neurotransmitters levels in the brain of patients were detected; IL-6 and TNF-α in serum were tested by ELISA. Results HAMD scores in the two groups decreased markedly after treatment (P<0.05). HAMD scores in treatment group was less than that in control group after treatment (P<0.05). KPS scores increased in treatment group after treatment, and were better than the control group (P<0.05). The contents of NE, 5-HT increased, IL-6 and TNF-αdecreased in both groups after treatment (P<0.05). The contents of NE and 5-HT in the treatment group were higher than the control group after treatment, while the contents of IL-6 and TNF-α were lower than the control group (P<0.05, P<0.01). No adverse reactions and side effects were observed in the treatment group. 5 cases of mild nausea and 6 cases of abnormal liver function were found in the control group. Conclusion Shugan Hewei Formula can increase the contents of NE and 5-HT in the brain of cancer-related depression patients, decreased cytokines levels, and improve patients' depressive state.

ABSTRACT

Objective To compare the differences between Hertel exophthalmometry and CT on exophthalmos measurement,and explore its practical application value in forensic medicine.Methods Fifty-six normal individuals without eye injuries or diseases were selected as the control group.The absolute values of exophthalmos were measured in the standardized CT image workstation.The difference of binocular exophthalmos was compared in normal group.Forty-seven cases with unilateral orbital fracture were divided into injured eye group and normal eye group according to the presence of binocular orbital fractures.The differences of the absolute values of eophthalmos from the same eye and the relative values of eophthalmos between two eyes for same person measured by Hertel exophthalmometry and CT were analyzed.Results There was no statistical difference of exophthalmos between the normal eyes measured by CT method.In the normal eye group,the absolute value of exophthalmos measured by CT method was (16.66±5.41) rm,which was (16.16±4.45)mm when measured by the Hertel exophthalmometry method.There was no statistical difference between two groups (P>0.05).In the injured eye group,there was statistical difference between the absolute values of exophthalmos measured by two measurement methods (P<0.05).There was no statistical difference of the relative values of exophthalmos between the injured eyes and the normal eyes by two measurement methods (P>0.05).Conclusion CT method has a good consistency with Hertel exophthalmometry,which can be applied into the practice of medicolegal expertise.

ABSTRACT

<p><b>OBJECTIVE</b>To compare clinical efficacy of double titanium plate and clavicular hook plate for the treatment of Neer II distal clavicular fracture.</p><p><b>METHODS</b>From April 2013 to June 2015, 42 patients with Neer II distal clavicle fractures were non-randomly selected, including 25 males and 17 females. All patients were fresh closed fractures. Twenty patients were treated with double titanium plate with tight rope(group A), including 11 males and 9 females aged from 16 to 49 years old with an average age of(33.8±10.7) years;the time from injury to operation ranged from 2 to 5 days with an average of (3.5±1.8) days. Twenty-two patients were treated with clavicle hook plate(group B), including 14 males and 8 females aged from 27 to 53 years old with an average age of (37.7±9.9) years;the time from injury to operation ranged from 2 to 5 days with an average of (3.1±2.0) days. Operative time, blood loss, hospital stays and postoperative complications were observed and compared; fracture healing were compared among preoperative, postoperative at 1, 3 and 6 months; VAS and ASES scoring were used to evaluate shoulder function.</p><p><b>RESULTS</b>All patients were followed up from 6 to 12 months with an average of 7.2 months. All incisions were healed at stage I. Blood loss in group A were less than that of group B(<0.05); while there were no significant difference in hospital stays and operative time between two groups. No shoulder pain occurred in group A, 4 cases occurred shoulder pain in group B, and had significant difference. Fracture healing time ranged from 3 to 6 months with an average of 4.3 months. VAS score and ASES score in group A were higher than that of group B in pain, function and total score.</p><p><b>CONCLUSIONS</b>Compared with clavicle hook plate, double titanium plate has advantages of shorter incision wound, less bleeding and simply operation, less complications, and could avoid pain for removing internal fixation. Over reduction of acromioclavicular joint during operation does not affect its superior curative effect. It is better choice for the treatment of Neer II distal clavicle fracture.</p>

ABSTRACT

<p><b>BACKGROUND</b>Colorectal serrated polyp is considered as histologically heterogeneous lesions with malignant potential in western countries. However, few Asian studies have investigated the comprehensive clinical features of serrated polyps in symptomatic populations. The aim of the study was to evaluate the features of colorectal serrated polyps in a Chinese symptomatic population.</p><p><b>METHODS</b>Data from all consecutive symptomatic patients were documented from a large colonoscopy database and were analyzed. Chi-square test or Fisher's exact test and logistic regression analysis were used for the data processing.</p><p><b>RESULTS</b>A total of 9191 (31.7%) patients were detected with at least one colorectal polyp. The prevalence of serrated polyps was 0.53% (153/28,981). The proportions of hyperplastic polyp (HP), sessile serrated adenoma/polyp (SSA/P), and traditional serrated adenoma (TSA) of all serrated polyps were 41.2%, 7.2%, and 51.6%, respectively, which showed a lower proportion of HP and SSA/P and a higher proportion of TSA. Serrated polyps appeared more in males and elder patients while there was no significant difference in the subtype distribution in gender and age. The proportions of large and proximal serrated polyps were 13.7% (21/153) and 46.4% (71/153), respectively. In total, 98.9% (89/90) serrated adenomas were found with dysplasia. Moreover, 14 patients with serrated polyps were found with synchronous advanced colorectal neoplasia, and large serrated polyps (LSPs) (odds ratio: 3.446, 95% confidence interval: 1.010-11.750, P < 0.05), especially large HPs, might have an association with synchronous advanced neoplasia (AN).</p><p><b>CONCLUSIONS</b>The overall detection rate of colorectal serrated polyps in Chinese symptomatic patient population was low, and distribution pattern of three subtypes is different from previous reports. Moreover, LSPs, especially large HPs, might be associated with an increased risk of synchronous AN.</p>

ABSTRACT

<p><b>BACKGROUND</b>The long-term outcomes of patients with autoimmune hepatitis (AIH) given the immunosuppressive treatment are considered to be preferable. However, little is known about the response of AIH patients with cirrhosis to immunosuppressive treatment. We assessed the effects of immunosuppressive therapy in Chinese AIH patients with cirrhosis from a tertiary hospital.</p><p><b>METHODS</b>Patients with a clinical diagnosis of AIH January 2000 and December 2015 were retrospectively reviewed. Two-hundred and fourteen patients who were followed up and satisfied the simplified AIH criteria were included in the study. Among these patients, 87 presented with cirrhosis when initially diagnosed for AIH. Immunosuppressive treatments were employed in 57 AIH patients who did not present with cirrhosis and 39 patients who presented with cirrhosis. Initial responses to immunosuppressive treatment of patients with and without cirrhosis were analyzed. Independent risk factors were assessed for predicting the prognosis of patients. The t-test and Cox regression statistical analysis were used.</p><p><b>RESULTS</b>In total, 96 AIH patients including 39 with cirrhosis and 57 without cirrhosis underwent immunosuppressive therapy. The overall complete remission after initial immunosuppressive treatment was achieved in 81/96 patients (84.4%), whereas 9/96 (9.4%) achieved incomplete response, and 6/96 (6.3%) occurred treatment failure. Compared to noncirrhotic patients, patients who presented with cirrhosis responded to treatment to a comparable extent regarding complete response (noncirrhosis 50/57 [87.7%] vs. cirrhosis 31/39 [79.5%], P = 0.275), incomplete remission (noncirrhosis 4/57 [7.0%] vs. cirrhosis 5/39 [12.8%], P = 0.338), and treatment failure (noncirrhosis 3/57 [5.3%] vs. cirrhosis 3/39 [7.7%], P = 0.629). Importantly, the remission rate was comparable (54/57 [94.7%] and 36/39 [92.3%], P = 0.629) for noncirrhotic and cirrhotic patients after immunosuppressive therapy. Compared to patients who maintained remission (n = 19) after drug withdrawal, patients who experienced relapse (n = 17) had significantly higher levels of serum immunoglobulin G at entry (15.0 ± 6.5 g/L vs. 22.3 ± 5.8 g/L, t = 2.814, P = 0.004). Moreover, cirrhosis at presentation significantly increased the risk of disease exacerbation (hazard ratio [HR]: 4.603; P = 0.002). The treatment of immunosuppressant (HR: 0.058; P = 0.005) and the level of aspartate aminotransferase at presentation (HR: 1.002; P = 0.017) also increased the risk of disease progression.</p><p><b>CONCLUSIONS</b>The efficacy of initial immunosuppressive treatment in AIH patients with cirrhosis is comparable to that in those without cirrhosis. Cirrhotic patients not treated by immunosuppressants have poor long-term outcomes.</p>

ABSTRACT

@#Objective To simulate and evaluate the upper and lower limbs posture on rehabilitation devices training with the Computer Aided Tri-Dimensional Interface Application (CATIA) man-machine analysis module. Methods The human body sizes were determined as the 95th percentile Chinese men. Based on the ergonomics analysis of CATIA, three-dimensional human body was constructed in the upper and lower limbs rehabilitation equipment. Virtual human relationship was established. The attitude of the upper and lower limbs in rehabilitation equipment was simulated and evaluated. Results The human limbs model moved in the normal range and the scores were more than 90 which meant comfortable. The forces of various parts of the body was under the stress limit, as well as the force moment. The arm tension was 3-5 points with the rapid upper limb assessment, which meant the rehabilitation equipment was acceptable. Conclusion Rehabilitation robot may be optimized through the model of the human upper extremity stretch domain analysis, body posture assessment, rapid upper limb assessment and biomechanical analysis under CATIA.

Methods Based on the current situation of traditional Chinese medicine EMR system functional evaluation and the evaluation methods of EMR System Function Application Level Classification Evaluation Methods and Standards (Trial)formerly issued by the Ministry of Health to research traditional Chinese medicine EMR system application evaluation methods. Results Evaluation methods of traditional Chinese medicine EMR system should be the establishment form 3 part:establishing evaluation index system and evaluation standard, establishing qualitative and quantitative evaluation, and establishing evaluation scores system. Conclusion Traditional Chinese medicine EMR system application evaluation method provides a scientific basis for designing traditional Chinese medicine EMR system evaluation index system.

ABSTRACT

<p><b>BACKGROUND</b>Exhaled nitric oxide (NO) is a noninvasive biomarker of airway inflammation in pulmonary diseases. Hydrogen sulfide (H2S), as the third member of the gasotransmitter family, is involved in the pathophysiological process in lung diseases. H2S also exists in exhaled breath and can be sampled non-invasively. The study investigated the level of exhaled H2S in patients with chronic obstructive pulmonary disease (COPD) and its correlation with exhaled NO.</p><p><b>METHODS</b>Levels of exhaled NO and H2S, lung function, and cell differential counts in induced sputum were studied in 19 patients with acute exacerbation of COPD (AECOPD), 19 patients with stable COPD and seven healthy smoke controls.</p><p><b>RESULTS</b>Exhaled H2S levels were similar in patients with AECOPD (10.0 parts per billion (ppb), 8.0-13.0 ppb), stable COPD (10.0 ppb, 9.0-12.0 ppb), and healthy controls (9.0 ppb, 8.0-16.0 ppb) (P > 0.05). Exhaled NO levels were similar in patients with AECOPD (155.0 ppb, 129.0-190.0 ppb), stable COPD (154.0 ppb, 133.0-175.0 ppb) and healthy controls (165.0 ppb, 112.0-188.0 ppb) (P > 0.05). Exhaled H2S levels correlated positively with exhaled NO in all healthy controls and patients with COPD (r=0.467, P < 0.01). No significant correlation was found between the exhaled H2S level and percentage of predicted FEV1 (P > 0.05) and proportion of different cell types in induced sputum (P > 0.05).</p><p><b>CONCLUSIONS</b>There is a correlation between exhaled H2S and exhaled NO. The role of exhaled H2S in airway inflammation in COPD still needs further investigation.</p>

ABSTRACT

<p><b>BACKGROUND</b>The role of gastro-protecting agents on symptomatic chronic gastritis is unclear. This multicenter, open, randomized trial was designed to compare the comprehensive effects of gefarnate with sucralfate on erosive gastritis with dyspeptic symptoms.</p><p><b>METHODS</b>Totally 253 dyspepsia patients confirmed with erosive gastritis were enrolled from six centers in China. They randomly received either daily 300 mg gefarnate or 3 g sucralfate for six weeks. The primary endpoint was the effective rate of both treatments on endoscopic erosion at week six.</p><p><b>RESULTS</b>Gefarnate showed an effective rate of 72% and 67% on endoscopic score and dyspeptic symptom release, which is statistically higher than sucralfate (40.1% and 39.3%, P < 0.001, intension-to-treat). For histological improvement, gefarnate showed both effective in decreasing mucosal chronic inflammation (57.7% vs. 24.8%, P < 0.001, intension-to-treat) and active inflammation (36.4% vs. 23.1%, P < 0.05, intension-to-treat) than the control. A significant increase of prostaglandins and decrease of myeloperoxidase in mucosa were observed in gefarnate group. Severity of erosion is non-relevant to symptoms but Helicobacter pylori (H. pylori) status does affect the outcome of therapy.</p><p><b>CONCLUSIONS</b>Gefarnate demonstrates an effective outcome on the mucosal inflammation in patients with chronic erosive gastritis. Endoscopic and inflammation score should be the major indexes used in gastritis-related trials.</p>
