ABSTRACT

CONTEXT: The autosomal recessive form of microcephaly-chorioretinopathy syndrome is a rare genetic condition that is considered to be an important differential diagnosis with congenital toxoplasmosis.CASE REPORT: Our patient was a seven-year-old white boy who was initially diagnosed with congenital toxoplasmosis. However, his serological tests for congenital infections, including toxoplasmosis, were negative. He was the first child of young, healthy and consanguineous parents (fourth-degree relatives). The parents had normal head circumferences and intelligence. The patient presented microcephaly and specific abnormalities of the retina, with multiple diffuse oval areas of pigmentation and patches of chorioretinal atrophy associated with diffuse pigmentation of the fundus. Ophthalmological evaluations on the parents were normal. A computed tomography scan of the child's head showed slight dilation of lateral ventricles and basal cisterns without evidence of calcifications. We did not find any lymphedema in his hands and feet. He had postnatal growth retardation, severe mental retardation and cerebral palsy.CONCLUSIONS: The finding of chorioretinal lesions in a child with microcephaly should raise suspicions of the autosomal recessive form of microcephaly-chorioretinopathy syndrome, especially in cases with an atypical pattern of eye fundus and consanguinity. A specific diagnosis is essential for an appropriate clinical evaluation and for genetic counseling for the patients and their families.


CONTEXTO: A forma autossômica recessiva da síndrome de microcefalia-coriorretinopatia é condição genética rara, considerada um importante diagnóstico diferencial com toxoplasmose congênita.RELATO DO CASO: O paciente era um menino branco de sete anos de idade, inicialmente diagnosticado com toxoplasmose congênita. No entanto, suas sorologias para infecções congênitas, incluindo a toxoplasmose, eram negativas. Ele foi o primeiro filho de pais jovens, hígidos e consanguíneos (parentes de quarto grau). Os pais apresentavam perímetro cefálico e inteligência normais. O paciente apresentava microcefalia e anormalidades específicas da retina com áreas ovais de pigmentação múltiplas e difusas, além de manchas de atrofia coriorretiniana associadas à pigmentação difusa do fundo de olho. A avaliação oftalmológica dos pais foi normal. A tomografia computadorizada de crânio da criança mostrou discreta dilatação dos ventrículos laterais e cisternas basais, sem evidência de calcificações. Nós não verificamos a presença de linfedema em suas mãos e pés. Ele possuía retardo do crescimento pós-natal, deficiência mental grave e paralisia cerebral.CONCLUSÃO: O achado de lesões coriorretinianas em uma criança com microcefalia deve aumentar a suspeita da forma autossômica recessiva da síndrome de microcefalia-coriorretinopatia, principalmente em casos com padrão atípico de fundo de olho e consanguinidade. O diagnóstico preciso é essencial para correta avaliação clínica e aconselhamento genético dos pacientes e suas famílias.

CONTEXT AND OBJECTIVE: Oculo-auriculo-vertebral spectrum (OAVS) is considered to be a defect of embryogenesis involving structures originating from the first branchial arches. Our objective was to describe the clinical and cytogenetic findings from a sample of patients with the phenotype of OAVS. DESIGN AND SETTING: Cross-sectional study in a referral hospital in southern Brazil. METHODS: The sample consisted of 23 patients who presented clinical findings in at least two of these four areas: orocraniofacial, ocular, auricular and vertebral. The patients underwent a clinical protocol and cytogenetic evaluation through high-resolution karyotyping, fluorescence in situ hybridization for 5p and 22q11 microdeletions and investigation of chromosomal instability for Fanconi anemia. RESULTS: Cytogenetic abnormalities were observed in three cases (13%) and consisted of: 47,XX,+mar; mos 47,XX,+mar/46,XX; and 46,XX,t(6;10)(q13; q24). We observed cases of OAVS with histories of gestational exposition to fluoxetine, retinoic acid and crack. One of our patients was a discordant monozygotic twin who had shown asymmetrical growth restriction during pregnancy. Our patients with OAVS were characterized by a broad clinical spectrum and some presented atypical findings such as lower-limb reduction defect and a tumor in the right arm, suggestive of hemangioma/lymphangioma. CONCLUSIONS: We found a wide range of clinical characteristics among the patients with OAVS. Different chromosomal abnormalities and gestational expositions were also observed. Thus, our findings highlight the heterogeneity of the etiology of OAVS and the importance of these factors in the clinical and cytogenetic evaluation of these patients. .


CONTEXTO E OBJETIVO: O espectro oculoauriculovertebral (EOAV) é considerado um defeito de embriogênese envolvendo estruturas originadas a partir dos primeiros arcos branquiais. Nosso objetivo foi descrever os achados clínicos e citogenéticos de uma amostra de pacientes com fenótipo de EOAV. TIPO DE ESTUDO E LOCAL: Estudo transversal em um hospital de referência no sul do Brasil. MÉTODOS: A amostra foi composta de 23 pacientes que apresentaram achados clínicos em pelo menos duas das quatro áreas: orocraniofacial, oculares, auriculares e vertebrais. Os pacientes foram submetidos a um protocolo clínico e avaliação citogenética através do cariótipo de alta resolução, hibridização in situ fluorescente para as microdeleções 5p e 22q11 e pesquisa de instabilidade cromossômica para anemia de Fanconi. RESULTADOS: Alterações citogenéticas foram observadas em três casos (13%) e consistiam de: 47,XX,+mar; mos 47,XX,+mar/46,XX e 46,XX,t(6;10)(q13;q24). Observamos casos de EOAV com história de exposição gestacional à fluoxetina, ácido retinoico e crack. Um dos nossos pacientes foi um gêmeo monozigótico discordante que teve restrição de crescimento assimétrica durante a gravidez. Nossos pacientes com EOAV foram caracterizados por um amplo espectro clínico e alguns apresentaram achados clínicos atípicos como um defeito de redução de membro inferior e um tumor do braço direito, sugestivo de hemangioma/linfangioma. CONCLUSÕES: Verificamos grande variedade de características clínicas entre os pacientes com EOAV. Também foram observadas diferentes anomalias cromossômicas e exposições gestacionais. Assim, nossos achados salientam a heterogeneidade da etiologia do EOAV e a importância desses fatores na avaliação clínica e citogenética desses pacientes. .

Introduction: True knot of the umbilical cord is characterized by a cord loop with increased tension and hence higher risk for obstetric and neonatal outcome intervention.It is a finding observed in 1-2% of births. Aim: Review the etiology, pathophysiology, diagnosis and prognosis of true knot of the umbilical cord. Methodology: We carried out a review searching for scientific articles present in the electronic database PubMed, using the descriptors “true knot“, “umbilical cord”, “Doppler sonography” and “prenatal diagnosis”. Articles published until 2013 were included in the review. Discussion: True knot of the umbilical cord is a relatively common finding at birth. However, its diagnosis in uterus is still quite rare. Several factors have been related to its etiology, including the size of the umbilical cord. Its appearance occurs mostly between 9 and 28 weeks of gestation, in the fetal period of greater mobility. The major part of the true knot of the umbilical cord is not associated to increased risk of obstetric intervention and neonatal outcomes because most are loose. However, true knots can tighten during pregnancy due to fetal movement or during labour, leading to reduced blood flow and consequent fetal asphyxia and death. Thus, the close monitoring of pregnancy in cases of true knot of umbilical cord is required. Prenatal diagnosis of true knot of the umbilical cord can be a challenge. Ultrasound complementary techniques, including color Doppler and Doppler velocimetry can improve the detection rate and help to differentiate true from false knots. And this has important implications for the management and prognosis of pregnancy, and consequences for the fetus and newborn.

OBJECTIVE: To describe gestational, perinatal and family findings of patients with Patau syndrome (PS). METHODS: The study enrolled patients with PS consecutively evaluated during 38 years in a Clinical Genetics Service of a pediatric referral hospital in Southern Brazil. The clinical data and the results of cytogenetic analysis were collected from the medical records. For statistical analysis, the two-tailed Fisher's exact test and the chi-square test with Yates' correction were used, being significant p<0.05. RESULTS: The sample was composed of 27 patients, 63% were male, with a median age of nine days at the first evaluation. Full trisomy of chromosome 13 was the main cytogenetic finding (74%). Only six patients were submitted to obstetric ultrasound and none had prenatal diagnosis of PS. The patients' demographic characteristics, compared to born alive infants in the same Brazilian state showed a higher frequency of: mothers with 35 years old or more (37.5%); multiparous mothers (92.6%); vaginal delivery (77%); preterm birth (34.6%); birth weight <2500g (33.3%), and Apgar scores <7 in the 1st (75%) and in the 5th minute (42.9%). About half of them (53%) died during the first month of life. CONCLUSIONS: The understanding of the PS patients' gestational, perinatal and family findings has important implications, especially on the decision about the actions to be taken in relation to the management of these patients. .


OBJETIVO: Describir los hallazgos gestacionales, perinatales y familiares de pacientes con síndrome de Patau (SP) y compararlos con los de la población de nacidos vivos de la misma provincia, Rio Grande do Sul, presentes en la base de datos del Sistema Único de Salud (DATASUS). MÉTODOS: Esa investigación implicó a pacientes con diagnóstico de SP evaluados consecutivamente durante 38 años en el Servicio de Genética de un hospital pediátrico de referencia en el sur de Brasil. Los datos clínicos y los resultados del análisis citogenético fueron recogidos de los prontuarios médicos. Para el análisis estadístico, se utilizaron la prueba exacto de Fisher bicaudado y la prueba del chi cuadrado con corrección de Yates (p<0,05). RESULTADOS: La muestra fue compuesta por 27 pacientes, el 63% del sexo masculino, con mediana de edad en la primera evaluación de nueve días. La trisomía libre del cromosoma 13 fue el principal hallazgo citogenético (74%). Solamente seis pacientes presentaban relato de ultrasonografía obstétrica y ninguno tuvo diagnóstico pre-natal de SP. Al comparar los datos de esa muestra con los datos de nacidos vivos en la misma provincia, se observó que, para los pacientes con SP, hubo mayor frecuencia de madres con edad >35 años (37,5%); multíparas (92,6%); parto vaginal (77%); prematuridad (34,6%); peso al nacer <2.500g (33,3%) y escore de Apgar <7 en el 1º (75%) y 5º minuto (42,9%). Aproximadamente mitad de los pacientes (53%) murió en el primer mes de vida. CONCLUSIONES: El entendimiento de los hallazgos gestacionales, perinatales y familiares de SP lleva a importantes repercusiones, especialmente sobre la decisión respecto a las conductas a tomar en el manejo de esos pacientes. .

Ao comparar os dados da presente amostra com os dados de nascidos vivos do mesmo estado, observou-se que, para os pacientes com SP, houve maior frequência de mães com idade >35 anos (37,5%); multíparas (92,6%); parto vaginal (77%), prematuridade (34,6%), peso ao nascer <2500g (33,3%) e escore de Apgar <7 no 1º (75%) e 5º minuto (42,9%). Cerca de metade dos pacientes (53%) morreu no primeiro mês de vida. CONCLUSÕES: O entendimento dos achados gestacionais, perinatais e familiares da SP leva a importantes repercussões, especialmente sobre a decisão quanto às condutas a serem tomadas no manejo desses pacientes. .
