ABSTRACT

Objective To compare the clinical value of chest CT and Magnetic Resonance Imaging (MRI) in the diagnosis of lymph node metastasis of thoracic esophageal cancer. Methods A retrospective analysis of 90 patients with thoracic esophageal cancer lymph node metastasis diagnosed and treated in our hospital from July 2015 to June 2019. All patients underwent chest CT and MRI scans after admission, and the lesion tissue samples were taken for pathological examination after surgery. Physical examination, with pathological diagnosis results as the gold standard, analyze the sensitivity and specificity of chest CT and MRI. Results The sensitivity, specificity, positive predictive value, and negative predictive value of MRI scan was 88.73%, 94.74%, 98.44%, and 69.23%, respectively, which were higher than 69.01%, 52.63%, 84.48%, and 31.25% of the chest CT scan. In the distribution of lymph node metastasis, the middle of the thorax was the largest, and the proportions of the upper, middle, and lower thoraxes were 26.67%, 60.00%, and 13.33%, respectively. The uppermost mediastinal and paratracheal lymph nodes had the largest metastases in the upper thorax, the paratracheal and subcarinal lymph nodes in the middle thoracic segment had the most metastasis, and the next to the cardia and left gastric artery lymph nodes in the lower thorax had the largest metastases. The accuracy of MRI diagnosis is higher than that of chest CT. Conclusion Both chest CT and MRI can diagnose lymph node metastasis of thoracic esophageal cancer. The diagnostic value of MRI in diagnosing lymph node metastasis of thoracic esophageal cancer is better than chest CT. The diagnostic accuracy of MRI in each part of the lymph node is higher than that of chest CT, which can show the lymph nodes more clearly in transfer situation.

ABSTRACT

@#Pulmonary adenocarcinoma in situ is reclassified as precursor glandular lesions in the fifth edition of WHO classification of thoracic tumours, causing widespread attention and heated debate among domestic thoracic oncologists, radiologists, pathologists and surgeons. We would like to comment on the topic and make a few suggestions on the management of pulmonary nodule during lung cancer screening. We are open to all suggestion and welcome debates.

ABSTRACT

OBJECTIVE@#To investigate the inhibitory effect of genistein on activation of hepatic stellate cells (HSCs) and the role of the autophagy pathway regulated by PPAR-γ in mediating this effect.@*METHODS@#Cultured HSC-T6 cells were exposed to different concentrations of genistein for 48 h, and HSC activation was verified by detecting the expressions of -SMA and 1(I) collagen; autophagy activation in the cells was determined by detecting the expressions of LC3-II and p62 using Western blotting. The autophagy inhibitor 3-MA was used to confirm the role of autophagy in genistein-induced inhibition of HSC activation. A PPAR-γ inhibitor was used to explore the role of PPAR-γ in activating autophagy in the HSCs.@*RESULTS@#Genistein at concentrations of 5 and 50 μmol/L significantly inhibited the expressions of -SMA and 1(I) collagen ( < 0.05), markedly upregulated the expressions of PPAR-γ and the autophagy-related protein LC3-II ( < 0.05) and significantly down-regulated the expression of the ubiqutin-binding protein p62 ( < 0.05) in HSC-T6 cells. The cells pretreated with 3-MA prior to genistein treatment showed significantly increased protein expressions of -SMA and 1(I) collagen compared with the cells treated with genistein only ( < 0.05). Treatment with the PPAR-γ inhibitor obviously lowered the expression of LC3-II and enhanced the expression p62 in genistein-treated HSC-T6 cells, suggesting the activation of the autophagy pathway.@*CONCLUSIONS@#PPAR-γ- regulated autophagy plays an important role in mediating genistein-induced inhibition of HSC activation .

ABSTRACT

Objective:To establish an HPLC method for the determination of palmatine hydrochloride and berberine hydrochloride in Baishi Shuitiao Powder simultaneously. Methods:A reversed phase ion pair HPLC method was adopted. An Agilent Zorbax SB-C18 (150 mm × 4. 6 mm,5 μm)column was used, the mobile phase consisted of acetonitrile-0. 1% phosphoric acid (50 :50) (0. 2 g do-decyl sodium sulfate added to 100 ml solution), the flow rate was 1. 0 ml·min-1, the detection wavelength was at 345 nm, the injec-tion volume was 10 μl, the column temperature was at room temperature. Results: A good linear correlation was observed within the range of 1. 11-22. 16μg·ml-1 for palmatine hydrochloride (r=0. 9996) and 2. 11-42. 24μg·ml-1 for berberine hydrochloride (r=0. 9998). The average recovery was 98. 94% with RSD of 2. 43% for palmatine hydrochloride and 101. 3% with RSD of 2. 05% for berberine hydrochloride (n=6). Conclusion:The method is rapid, simple and accurate, and can be used for the content determina-tion of palmatine hydrochloride and berberine hydrochloride in Baishi Shuitiao Powder.

ABSTRACT

Objective To investigate the incidence of white matter lesions(WML) in acute ischemic stroke ,and to analyze its the risk factors .Methods Totally 248 cases of acute ischemic stroke patients were selected ,patients′general information were in‐vestigated ,the WML severity with Fazekas rating scale were evaluated and patients were divided into severe group and no severe group according to the results ,the differences of general information between two groups were compared ,the severe WML was set as dependent variable ,the risk factors were ananlzed by multiariable Logistic regression analysis .Results There were 106 cases se‐vere WML patients among 248 cases acute ischemic stroke ,the incidence was 42 .74% ;single factor analysis found that the age in severe group was significantly higher than non severe group ,the incidence of hypertension ,diabetes in severe group was significantly higher than non severe group ,the difference was statistical significance (P<0 .05);multiariable Logistic regression analysis showed that age (OR=4 .116 ,95% CI:1 .816-6 .454 ,P=0 .000) ,hypertension (OR=1 .462 ,95% CI:0 .842-1 .946 ,P=0 .026) and dia‐betes (OR=1 .157 ,95% CI:0 .698-1 .673 ,P=0 .038)were the independent risk factors of severe WML in acute ischemic stroke . Conclusion The incidence of severe WML in acute ischemic stroke patients is high ,we need to targeted prevention measures on ol‐der age ,hypertension ,diabetes mellitus patients to reduce severe WM L occurred .

ABSTRACT

Objective To search MRI features of patients with HIV-negative cryptococcus meningoencephalitis (CM)and evaluate the value in judging the prognosis of CM.Methods The findings of cranial MRI and prognosis were retrospectively analyzed in 19 cases with India-ink capsule staining-proved HIV-negative CM in our department.The prognosis was evaluated by Glasgow Outcome Scale:score 1=death,score 2=persistent vegetative state,score 3=severe disability,score 4=moderate disability,score 5=good recovery. The patients with score 1-3 were classified as the group of poor prognosis,while those with score 4,5 as good prognosis.Results Abnormal manifestations on cranial MRI were observed in 84.2%(1 6/1 9)cases,meninges enhanced in 84.2% (1 6/1 9),brain edema in 31.6%(6/1 9),brain parenchyma lesion in 47.4% (9/1 9 ),vasculitis in 5.2% (1/1 9 ).The mean duration of follow-up was 3.5 years,The prognosis of 5/9(44.4%)cases with brain parenchyma lesion confirmed by cranial MR was poor.Conclusion The MRI manifestation of HIV-negative CM is diverse,and brain parenchyma lesion confirmed by cranial MRI may associated with poor prognosis.

Objective To investigate the relationship of HSP27 expression with treatment response ,remission rate ,prognosis in multiple myeloma(MM) patients .Methods Eighty‐six patients with MM first diagnosis and 27 relapse cases were collected re‐spectively .Control was defined as non‐hematological malignance .The MM patients divided into first diagnosis ,relapse conventional chemotherapy group and bortezomib group .HSP27 expression and protein level were measured on pre‐and post 1 ,2 and 3 treatment circles .Results The tendency of HSP expression and protein level:relapse‐MM patients> first‐diagnosis MM patients> control . HSP27 expression and level in Ⅲ phase was higher than those in Ⅱ phase for first‐diagnosis MM patients .Bortizomib‐containing chemotherapy protocol showed more effectively decrease on HSP27 than chemotherapy treatment only .At post‐1 treatment circle , HSP271 and delayed the relapse time .Conclusion BMMNC‐HSP27 expression and protein level is associated with staging ,treatment response and prognosis in MM patients .

Objective To analyze the parametric errors during the use of Sysmex CA-1500 automated coagulation analyzer and their treatment measures .Methods Blood samples with test results showing “*”and without figure results were collected .Sysmex CA-1500 automated coagulation analyzer was employed to retest prothrombin time (PT ) ,activated partial thromboplastin time (APTT) and fibrinogen(FIB) .Results FIB ,PT and APTT were found abnormal in 226 cases(95 .77% ) ,5 cases(2 .12% ) and 5 cases(2 .12% ) ,respectively ,with FIB the mainly .ERR008 ,ERR004 ,ERR016 ,ERR002 ,ERR032 and ERR100 were found abnormal in 227 cases(76 .95% ) ,64 cases(21 .69% ) ,one case(0 .34% ) ,one case(0 .34% ) ,one case(0 .34% ) and one case(0 .34% ) ,respec-tively ,with ERR008 the mainly .Difference of no-error results between the initial and repeat test of PT and APTT showed no statis-tical significance(P>0 .05) ,while those of FIB demonstrated statistical significance (P<0 .01) .Conclusion Samples with paramet-ric errors during the use of Sysmex CA-1500 automated coagulation analyzer should be retested to promote the quality of blood co-agulation detection .

ABSTRACT

Objective To investigate the application of radiofrequency surgery technology in the treatment of proliferating dermatosis of face.Methods A total of 572 patients treated with radio fre quency technology from September 2008 to March 2010 in Shanxi Provincial People's Hospital,were analyzed retrospectively.Results In all 572 eases were cured,and the cure rate was 92.13 %.All of the operations by radio frequency achieved effectively ideal results.Conclusions This allows less bleeding during surgery,and precise cut,and less pigmentation.Radiofrequency technology is applica bie to highly refined facial surgery.It is worthy to be popularized in clinical practice.

ABSTRACT

Objective: To observe the clinical effect of oral nutrition supplementation for maxillofacial tumor patients before operation. Methods: 60 patients who suffered from maxillofacial neoplasm were divided into two groups. In observation group, on basis of routine diet, oral nutrition supplements (Fortisip) had been added for 5~7 days before operation. In control group, only routine diet had been supplied. After operation, the two groups were all supplied with tube feeding. The blood routine test, plasma total protein, albumin, pre albumin and lymphocyte count were measured in all patients. Results: Every biochemical item reduced significantly after operation and no statistical significance was found between groups. But the absolute decreasing values had significant difference between the groups. Conclusions: Oral nutrition supplements (Fortisip) contains balanced nutrients and tastes well. It is safe and effective to use a nutritional supplement, being helpful to prevent malnutrition and to improve the immune function in perioperative period of patients with maxillofacial neoplasm.

Objective: To try to find out the morbidity of elderly chronic pulmonary disease's malnutrition by means of MNA and analyze the consistency between traditionalindex and MNA score. Methods: According to the score of MNA,we divided the 143 elderly pulmonary disease patients into three groups: A group(malnutrition); B group(malnutrition risk);C group(well nourished).The records included anthropometry,biochemical markers,dietary intake as well as the occurrence rate of pneumonia and respiratory failure. Results: There were marked differences on anthropometry,biochemical markers,dietary intake and the occurrence rate of pneumonia and respiratory failure among three groups. And the dietary intake of all three groups did not reach the standard of normal people. Conclusion: MNA is a good method that can be used to evaluate the nutritional state in elderly pulmonary disease patients.

ABSTRACT

Blood glucose level responses to different diets were examined in 12 healthy man. The test breakfasts consist of different proportion of protein and carbohydrate. Three kinds of test breakfast were fed respectively for each person. Blood glucose were measured before and 1, 2 and 4 hr after breakfast. The results indicated that high-CHO diet maintained higher level of blood glucose than other diets in 1-2 hr after breakfast. However the blood glucose level began dropping from 2 hr and dropped below fasting level at 4 hr after breakfast. The high-protein diet resulted in decreasing of blood glucose level in 1-2 hr after breakfast. The blood glucose level then increased continuously until 4 hr after breakfast.The results in this experiment showed that adjustment of the proportion of carbohydrate and protein in diets could maintain the blood glucose to a higher and stable level.
