ABSTRACT

ABSTRACT Purpose: To assess Meibomian gland dysfunction using meibography in patients with xeroderma pigmentosum and correlate with ocular surface changes. Methods: This cross-sectional study evaluated patients with xeroderma pigmentosum. All patients underwent a comprehensive and standardized interview. The best-corrected visual acuity of each eye was determined. Detailed ophthalmic examination was conducted, including biomicroscopy examination of the ocular surface, Schirmer test type I, and meibography, and fundus examination was also performed when possible. Meibomian gland dysfunction was assessed by non-contact meibography using Oculus Keratograph® 5M (OCULUS Inc., Arlington, WA, USA). Saliva samples were collected using the Oragene DNA Self-collection kit (DNA Genotek Inc., Ottawa, Canada), and DNA was extracted as recommended by the manufacturer. Factors associated with abnormal meiboscores were assessed using generalized estimating equation models. Results: A total of 42 participants were enrolled, and 27 patients underwent meibography. The meiboscore was abnormal in the upper eyelid in 8 (29.6%) patients and in the lower eyelid in 17 (62.9%). The likelihood of having abnormal meiboscores in the lower eyelid was 16.3 times greater than that in the upper eyelid. In the final multivariate model, age (p=0.001), mutation profile (p=0.006), and presence of ocular surface malignant tumor (OSMT) (p=0.014) remained significant for abnormal meiboscores. For a 1-year increase in age, the likelihood of abnormal meiboscores increased by 12%. Eyes with OSMT were 58.8 times more likely to have abnormal meiboscores than eyes without ocular surface malignant tumor. Conclusion: In the final model, age, xeroderma pigmentosum profile, previous cancer, and clinical alterations on the eyelid correlated with a meiboscore of ≥2. Meibomian gland dysfunction was common in patients with xeroderma pigmentosum, mainly in the lower eyelid. The severity of Meibomian gland dysfunction increases with age and is associated with severe eyelid changes.

Resumo Atravessando as principais teorias e sistemas da psicologia onde o problema da percepção é abordado, não se verifica uma dedicação profunda aos processos por meio dos quais os fenômenos perceptivos são constituídos. Predomina aí o pressuposto da percepção e do objeto percebido como elementos "dados" ontologicamente, ou seja, já constituídos previamente à experiência perceptiva. Na contramão desta abordagem ontológica, buscaremos compreender os processos ontogenéticos através dos quais os fenômenos perceptivos são criados, estruturam-se e modificam-se. Para isso, abordaremos a percepção a partir de uma perspectiva transdisciplinar onde ressoam elementos da biologia, etologia, filosofia e artes. No centro dessa investigação, o corpo emerge como sede dos processos cognitivos e foco das intervenções clínicas, como meio de constituição e transformação da subjetividade.(AU)


Abstract Crossing the main theories and systems of psychology where the problem of perception is discussed, there is not a deep commitment to the processes by which the perceptual phenomena are constituted. There prevails the assumption of perception and perceived object as ontologically "given" elements, i.e., constituted previously to the perceptual experience. Against this ontological approach, we seek to understand the ontogenetic processes through which the perceptual phenomena are created, structured and modified. In order to do so, we will address the perception from a transdisciplinary perspective which resounds elements of biology, ethology, philosophy and arts. In the center of this investigation, the body emerges as source of cognitive processes and focus of clinical interventions as a means of formation and transformation of subjectivity.(AU)

ABSTRACT

The objective of this study was to determine the impact of a pharmaceutical care (PC) program in a sample of public outpatients with metabolic syndrome (MS) who were being treated in Brazil's health system; the patients were randomized into PC or standard care. The pharmacotherapy follow-up (PF) was performed in a total of 120 patients with type 2 diabetes for 6 months. Adherence to treatment (measured with the Morisky test), negative outcomes associated with medication (NOM) and anthropometric and biochemical parameters were measured before and after PF. The Framingham scoring method was used to estimate changes in 10-year coronary heart disease risk scores in all patients. Ninety-six of 120 patients had characteristics of MS and were randomized into two groups (G): the control group (CG: 36) and the intervention group (IG: 38). Among the MS patients, 100% were taking a glucose-lowering drug; many were also taking anti-hypertensive drugs (CG: 72%; IG: 73%), and some patients were also taking hypolipemic drugs (CG: 12.0%; IG: 14.7%). Only 20.7% of the IG patients were considered adherent to their prescribed drugs. In the CG, an increase of coronary heart disease (CHD) risk (22±2 to 26±3; p<0.05) was observed, while in the IG, there was a reduction in CHD risk (22±2 to 14±2%; p<0.01). The PC program administered to patients with MS monitored through the primary healthcare services of the Brazilian public health system improved patient health, resulting in clinical improvements and a decrease in cardiovascular risk in IG patients over a period of ten years.


O objetivo deste estudo foi o de determinar o impacto de um Programa de atenção Farmacêutica (AF) em uma amostra de pacientes ambulatoriais de Sistema Público de Saúde do Brasil portadores de Síndrome Metabólica, randomizados em AF ou atenção à saúde usual. Realizou-se o seguimento farmacoterapêutico com 120 pacientes com diabetes tipo 2 durante seis meses. Avaliou-se o nível de aderência ao tratamento (teste Morisky), resultados clínicos negativos associados a medicamentos (RNM), parâmetros bioquímicos e antropométricos, antes e após o seguimento. O método de Framingham foi usado para calcular as variações no risco de doenças coronarianas em 10 anos em todos os pacientes. Dos 120 pacientes, 96 tiveram características de SM e foram então randomizados em dois grupos (G): Controle (GC: 36) e Intervenção (GI: 38). Entre os pacientes com SM, 100% faziam uso de medicamentos para diminuir a glicose, anti-hipertensivos (GC: 72%; GI: 73%) e hipoglicemiantes (GC: 12.0%; GI: 14.7%). Apenas 20,7% do GI foram considerados aderentes aos fármacos prescritos. No GC foi observado aumento do risco de Doença Arterial Coronariana (DAC) (22±2 para 26±3; p<0,05), enquanto no GI foi observado redução (22±2 para 14±2%; p<0,01). O Programa de AF para pacientes com SM monitorados na atenção primária do Sistema de Saúde Pública brasileiro melhora o funcionamento do serviço resultando na melhoria clínica dos pacientes com redução do risco de doença cardiovascular em um período de dez anos.

ABSTRACT

A transmissão vertical do vírus da hepatite B representa a principal via de disseminação do VHB nas regiões de alta prevalência, sendo a prevenção dessa forma de transmissão importante para a redução dos altos índices de cronicidade característicos da evolução da doença quando da infecção durante o período gestacional. Sobretudo, as medidas de prevenção são de fácil execução e consideradas altamente eficazes, apresentando uma ótima relação custo-benefício frente às complicações agudas e crônicas da hepatite B. O objetivo dessa revisão é abordar os aspectos clínicos, epidemiológicos e obstétricos envolvidos no curso da hepatite B, bem como, os aspectos pertinentes à prevenção da transmissão vertical da patologia.

ABSTRACT

Tomando como ponto de partida o conceito de repetição - tal como ele é desenvolvido na filosofia de Gilles Deleuze - o presente artigo pretende trabalhar a noção de hábito visando à realização de uma crítica à abordagem desta noção feita pela psicologia. Mostra também como a constituição do hábito é inseparável de uma contemplação passiva que se encarrega da fundação do tempo do vivo, demonstrando como é possível pensar a emergência da subjetividade a partir de uma nova inflexão. Visa, portanto, compreender a subjetividade procurando esclarecer a sua emergência à luz de uma perspectiva ontológica e estética, para além da Psicologia.


Taking as a start point the concept of repetition - as it's been developed by Gilles Deleuze's philosophy - the article treats the habit notion and, in particular, a critique to the way psychology deals with such notion. In addition, presents how the habit's constitution is inseparable from a passive contemplation, which is responsible for the foundation of the alive time, still showing how possible it is to think about the subjectivity emergence in terms of a new inflection. Therefore, based on an ontology and esthetic perspective, it goals an understanding of subjectivity, as well as its emergence beyond psychology.
