ABSTRACT

The problem about whether or not retirees should pay for basic medical insurance is not only a poli-cy problem,but also one of the focus of theoretical controversy in China. The risk of fund shortage for medical insur-ance or the limited income of the retired workers is the practical issue to be considered when assessing whether the current medical insurance premium policy should be changed or not,but do not constitute a decisive factor in evalua-ting whether it is or not to affordable for the retired workers. The nature of the system and the inherent rights and obli-gations are the fundamental reasons for the decision. Because of the obligations for medical insurance payment,the a-bility to pay and how to undertake are all belong to the problems at different levels,it should be discussed separately. In view of the internal mechanism for medical insurance system and the change of the external environment of social economy,retired workers bearing the obligation of payment is a necessary requirement. In addition,while implemen-ting the principle of universal payment obligation,for the employees who are economically challenged and unable to pay their insurance premiums,can solutions such as lighten burden,through paying duty exemptions,individual gov-ernment subsidies and other ways.

ABSTRACT

Objective To develop a cellular shear stress loading system with an adjustable stress mode and relevant parameters, and subsequently verify the effectiveness and feasibility of this system. Methods The hardware of the system was developed by using a peristaltic pump and self-designed multi-channel flow chamber, and the mode control program of shear stress based on LabVIEW was designed to control the device via RS485 interfacing and Modbus protocol. Additionally, the relationship between the shear stress and system parameters was calibrated, and finite element analysis was also conducted. Finally, the feasibility of the system was confirmed via the in vitro cell experiment. Results The mode and magnitude of shear stress of the system could be controlled via either the peristaltic pump or computer, and the cellular long-term effect was also able to be detected. The calibration results of the system indicated that the level of shear stress exhibited significantly linear positive correlation with the revolution of the peristaltic pump (P<0.001). Finite element analysis demonstrated that the level of shear stress on the slide was uniformly distributed and the result of simulation was accordant with calibration. Cytoskeleton staining suggested that cellular morphology of MLO-Y4 cells was changed, and microfilament increased and arrayed along fluid flow direction. Conclusion The system is stable and reliable enough to provide different loading modes and magnitude of cellular shear stress to offer a convictive platform of the research for different cellular stress signal transduction mecha-nisms.

ABSTRACT

<p><b>OBJECTIVE</b>To examine the expression of cysteinyl leukotriene receptor-1 (CysLTR-1) and cysteinyl leukotriene receptor-2 (CysLTR-2) in the adenoid tissues from children with adenoid hypertrophy (AH) and to explore the role of leukotrienes in the pathogenesis of AH.</p><p><b>METHODS</b>Sixty children with AH who were treated by adenoidectomy and/or tonsillectomy were classified into two groups: simple AH and AH plus allergic rhinitis (n=30 each). Twenty children who underwent tonsillectomy due to recurrent purulent tonsillitis were selected as the control group. The expression of CysLTR-1 and CysLTR-2 in the excised tonsil and/or adenoid tissues was determined by immunofluorescence histochemical labeling and integrated optical density measurement.</p><p><b>RESULTS</b>The expression of CysLTR-1 and CysLTR-2 in the adenoid and tonsil tissues increased significantly in both the simple AH group and AH plus allergic rhinitis group compared with the control group (P<0.01). The expression of CysLTR-1 and CysLTR-2 in the AH plus allergic rhinitis group increased more significantly compared with the simple AH group (P<0.01).</p><p><b>CONCLUSIONS</b>CysLTR-1 and CysLTR-2 are highly expressed in the adenoid tissues from children with AH, suggesting that leukotrienes are involved in the pathogenesis of AH.</p>

ABSTRACT

Objective The dynamic accumulation trend of secondary metabolite in cultivated Spatholobus suberectus Dunn at different growth time was studied to provide scientific basis for confirming its reasonable harvesting time. Methods Ethanol-soluble extractives were measured by the method recorded in Chinese Pharmacopoeia ( A) Appendix XA ( 2010 edition). The content of total flavonoids was tested by ultraviolet spectrophotometer. The contents of protocatechuic acid, catechin, and epicatechin in Spatholobus suberectus Dunn were determined by high performance liquid chromatography ( HPLC). Results The contents of ethanol-soluble extractives, total flavonoids and the major components such as protocatechui cacid, catechin and epicatechin in Spatholobus suberectus Dunn showed increasing trend with the prolongation of growth time. The changes of contents of alcohol-soluble extract, total flavonoids and epicatechin in samples collecting at different months in one year showed a parabolic trend, and the contents reached the peak values from July to September. Catechin content kept increasing continually. The content of ethanol -soluble extractives of Spatholobus suberectus Dunn aged 4-6 years met the requirements of the current edition of Chinese Pharmacopoeia when harvested from July to September. Conclusion It is preliminarily recommended that Spatholobus suberectus Dunn cultivated for more than 7 years is suitable for harvesting, but the optimal harvest period and the harvest month still need to be further studied.

ABSTRACT

Objective To evaluate the viscoelastic properties of porcine acellular dermal matrix (PADM) by comparison and analysis on physical parameters of the skin. Methods Full-thickness skin defects were performed on the back of white rabbits as wound models, and randomly divided these wounds into 3 groups according to different methods of skin grafting: PADM group (autogenous skin and PADM were grafted to the surface of the wound), TS group (autogenous skin was grafted in situ), NS group (normal skin as the control). Experiments on the stress-strain relationship of the implanted skin in the three groups were conducted. Results The curves of stress relaxation, creep, and stress-strain relationship showed that under a given stress, PADM group had the lowest strain, NS group had the highest strain, while the strain of TS group was in between the PADM and NS group. Conclusions The skin viscoelasticity mechanical model is a four-parameter solid model. When the skin wounds are grafted by using PADM which may have poor elasticity, its recovery capacity after deformation is also poor, and this is in agreement with the clinical results.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the advantages and disadvantages of ultracision-harmonic scalpel assisted tonsillectomy by compared with conventional tonsillectomy.</p><p><b>METHODS</b>Eighty-eight patients were randomly divided into ultrasonic scalpel group (group A, 42 cases) and control group (group B, 46 cases). The tonsillectomy in group A was performed with ultracision-harmonic scalpel, and in group B, the tonsillectomy was performed by routine method. The surgical time (complete removal of tonsils), blood loss, and postoperative sore throat situation were recorded.</p><p><b>RESULTS</b>Surgical time in group A [(14.7 ± 4.0) min] was shorter than that in group B [(28.9 ± 7.6) min], t = -10.691, P < 0.05. Blood loss in group A [(3.1 ± 1.1) ml] was less than that in group B [(19.0 ± 5.2) ml], t = -19.544, P < 0.05. Postoperative sore throat was less painful in group A than that in group B in 10 hours after surgery, but much painful than group B 3 days after surgery and most patients lasted longer. There were statistical differences (P < 0.05). The average peel off time for the tunica albuginea was 8 days after the operation by using traditional method, by compared with the ultrasonic scalpel method, average time was 11 days, the difference showed statistical significance (t = 5.115, P < 0.05).</p><p><b>CONCLUSIONS</b>Compared with traditional tonsillectomy, ultracision-harmonic scalpel tonsillectomy had shorter operative time, less blood loss and so on, but the sore throat symptoms persisted longer. In addition, the tunica albuginea peeled off later, so avoidance of secondary bleeding caused by improper diet was mandatory.</p>

ABSTRACT

Objective To evaluate the usefulness of 18F-FLT PET/CT imaging in detecting and staging nasopharyngeal carcinoma (NPC) patients.Methods Thirteen patients with NPC underwent wholebody 18F-FLT PET/CT imaging,one of which underwent whole-body 18F-FDG PET/CT imaging one day earlier.SUVmax and SUVmean from 18F-FLT and18F-FDG imaging were obtained using circular ROI in primary and metastasis lesions and were compared with the results of histopathology.The staging results by 18 F-FLT PET was compared with those by CT.Results The SUVmax and SUVmean obtained from 18F-FLT imaging in 22nasopharyngeal sites in 13 patients were 6.04 ±3.61 and 5.09 ±2.89,and the SUVmax and SUVmean in 26 lymphadenopathy were 5.56 ± 3.11 and 4.65 ± 2.79.18 F-FDG SUVmax were higher than 18 F-FLT SUVmax in one primary lesion ( 8.32 vs 4.38 ) and two lymph nodes (3.30 ± 0.07 vs 1.48 ± 0.06) in the patient who underwent the two imagings.Compared with CT staging results,the TNM stage in 3 patients had been changed based on 18 F-FLT PET/CT imaging.Conclusions High radioactivity of primary and second lesions can be detected on 18F-FLT imaging in patients with NPC and 18F-FLT PET/CT imaging may be useful in staging for NPC.

ABSTRACT

Objective To investigate therapeutic effect,safety and post transplant immunological reconstitution for patients of lymphoma after autologous bone marrow transplantation and occurrence of Rheumatoid Arthritis.Methods The clinical situation,laboratory index and immunological reconstitution was observed for patients of lymphoma after inductive treatment,BCEA of pretreatment,autologous bone marrow transfusion,G-CSF and radiotherapy to lymph node.Results Recovery of hemopoiesis was observed after 28 days.Disappearance of joint swelling and negative ESR,CRP,IgMRF were observed after 2 months.The patients started to work after 3 months. No recurrence of lymphoma and Rheumatoid Arthritis has been observed until today.Conclusion The autologous bone marrow transplantation was safe and effective for the treatment to patient of lymphoma with rheumatoid arthritis,which was a new method for the treatment to lymphoma and other autoallergic diseases.

ABSTRACT

Objective To analyze the effect of bortezomib together with dexamethsone on the treatment of refractory multiple myeloma.Methods Bortezomib at 1.3 mg/m2 iv drop 1 d,4 d,8 d,11 d.Dexamethasonc 20 mg/d,1~4 d,8~11 d,17~20 d.21 days are used as a course.For bi-course dexamethasone was given by 20 mg/d,at 1~4 d.Results After 2~6 periods,3 cases achieved completely recovery(CR),4 cases are near to completely recovery(CR),3 cases got partially recovery(PR).Conclusion It is a good choice to treat refractory multiple myeloma by Bortezomib together with dexamethagone.

Objective Clinical research was made to observe the relationship between idiopathic thrombocytopenic purpura(ITP)and Helicobacter pylori(Hp)and to evaluate the effect of Hp eradication in the treatment of refractory ITP.Methods 13C urea breath test and Hp antibody of serum were made in 48 previously untreated patients with ITP from Department of Hematology of the First Affiliated Hospital of China Medical University during November 2004 to May 2005 years and normal controls consisted of 52 heathy peoples whose gastroscopy was normal.Omeprazole,clarithromycin and amoxicillin were used for Hp eradication to refractory ITP with Hp infection.13C urea breath test,Hp antibody of serum and the level platelet and PAIgG was detected after therapy 4~8 weeks.Results Of 48 cases with ITP,33(68.18%)had Hp infection and 24 of 52(46.12%)normal population had Hp infection,the infection rate of Hp in ITP patients being statistically higher than the control group(P

ABSTRACT

In order to investigate the expression of erythropoietin receptor(EPOR) in leukemic cell line and clarify the mechanism of proliferative signal transduction mediated by EPOR in the leukemic cell line KOCL-33, biotinylated EPO and flow cytometry were used for detection of EPOR expression, (3)H-TdR incoporation for cell proliferation, immunoprecipitation and Western blot analysis were used to investigate the tyrosine phosphorylation of signal transduction proteins as the causation of enhanced proliferation of KOCL-33 cells. There was EPOR expression and the cell proliferation could be stimulated by EPO in KOCL-33 cells. Dimerization and tyrosine phosphorylation of EPOR were found after 1 minute incubated with EPO. Coimmunoprecipitation of SHC and PLC-gamma and enhanced tyrosine phosphorylation following to the stimulation of EPO were found. Our results suggest that dimerization of EPOR and tyrosine phosphorylation were appeared under the stimulation of EPO, and SHC and PLC-gamma play a cooperative effect in EPOR mediated signal tranduction.

ABSTRACT

Objective To determine the content of protocatechuic acid in Spatholobus suberectus Dunn.Methods HPLC method was developed for the determination of protocatechuic acid in Spatholobus suberectus Dunn.The separation was performed on C18 column with acetonitrile-water-orthophosphoric acid(8 ∶92 ∶0.1) as a mobile phase with the flow rate being 1.0 mL/min.The detection wavelength was set at 260 nm.Results A satisfactory separation between protocatechuic acid and impurity was obtained.The calibration curve was linear in the range of 0.020~2.008 ug,r=0.999 9.The average recovery was 98 %,and RSD was 0.1 %(n=5).Conclusion The method is accurate and can be used for the detemrmination of protocatechuic acid in Spatholobus suberectus Dunn.
