ABSTRACT

Objective@#To investigate the molecular genetic and clinicopathologic characteristics, immunophenotypes, diagnostic and differential diagnostic features of myxoid angiomatoid fibrous histiocytoma (MAFH).@*Methods@#Three cases of MAFH were collected from the archives of Zhejiang Provincial People′s Hospital between January 2015 to August 2018. The clinical and radiologic features, histomorphology, immunohistochemistry, molecular genetics and prognosis were analyzed.@*Results@#Patients consisted of 2 women and 1 man aged 37 years, 46 years, and 57 years, respectively. The clinical manifestations of 3 patients were presented as a painless, slowly-enlarged mass with a duration ranging of 2 weeks, 2 months and 50 years. These tumors were located at the deep somatic soft tissue of extremities or limbs (right hip, left forearm, left wrist, respectively) and 2 were preoperatively considered as ganglion cyst or giant cell tumor of tendon sheath by imaging examinations. The diameter of circumscribed mass lesion was ranged from 3.0 to 7.5 cm, which exhibited a gray white to tan and gelatinous cut surface. Extensive hemorrhage and cystic changes were observed in 2 cases. Under low magnification, all tumors showed a dense fibrous pseudo-capsule with a peritumoral lymphoplasmacytic cuff and a multi-nodular growth pattern. Blood-filled cystic spaces were observed in 2 tumors. The myxoid stroma occupied 60.0%, 80.0% and 90.0% area of the entire tumor, respectively. Within the myxoid areas, tumor cells were oval to stellate and arranged in cord-like, microcystic and reticular growth patterns. Transitions of myxoid tumor components to more solid areas with typical histology of angiomatoid fibrous histiocytoma (AFH) were observed at least focally in all the three cases. The tumor cells exhibited minimal atypia and scarce mitoses (1 to 2/50 HPF) without necrosis, and prominently focal intracytoplasmic vacuoles were identified in one case. The results of immunohistochemistry staining showed that, 2/3 cases focally expressed desmin, 2/3 focally expressed epithelial membrane antigen (EMA), and 1/3 focally expressed CD99. The positive index of Ki67 was approximately 1% to 5%. Fluorescence in situ hybridization analysis showed that EWSR1 gene rearrangement occurred in all of the three cases. During the period of follow-up, one case showed local recurrence at 15 months, one case showed postoperative recurrence at 24 months, and the recurrent tumor slowly grew for 120 months until the second resection, without recurrence at the following 2 months. The left case showed a disease-free survival at 32 months.@*Conclusions@#MAFH is a rare subtype of AFH with a low-grade behavior and may lead to diagnostic confusions. Carefully searching for the typical AFH histomorphology and combining with EWSR1 gene rearrangement test can help to distinguish MAFH from other mimickers.

ABSTRACT

Objective To establish a refined model of trunk composed of different lumbar segments, lumped thoracic spine and pelvis, analyze the kinematic differences between patients with lumbar disc herniation (LDH) and healthy people during three daily activities, and to compare the refined trunk model with the simplified trunk model adopting the whole lumbar segment, and discuss the necessity of using the refined trunk model for kinematic analysis of LDH patients. Methods Motion capture system NDI was used to collect kinematic parameters of each segment from 15 healthy people and 7 male LDH patients during level walking, trunk flexion and contralateral pickup, then the kinematic differences between patients and healthy people by the two models were compared respectively. Results During level walking, the rotation of the thoracic segment and pelvis for LDH patients increased, while no significant change was found in motion angle of the whole lumbar segment, and the rotation angle of L4-5 segment significantly reduced. During trunk flexion, the flexion angles of all lumbar segments for LDH patients were reduced by varying degrees, and the flexion angle of L3-4 segment was significantly different from that of healthy people. During contralateral pickup, the performance on the sagittal plane was similar to that during flexion. However, the lateral bending angles of L3-4 segment and L4-5 segment for LDH patients were significantly lower than those for healthy people. ConclusionsLDH patients mainly restrict the motion of injured lumbar segments in daily activities. During some motions, only refined model can discover the abnormal motion of injured lumbar segments. Therefore, it is necessary to subdivide the lumbar spine into 5 independent segments for analyzing the kinematic characteristics of LDH patients.

ABSTRACT

Objective To observe the effects of neuromuscular stimulation (NMES) intensity on post-stroke dysphagia. Methods Thirty patients with dysphagia resulting from a stroke were randomly divided into a control group,an NMES group, and an intensive NMES group, with 10 patients in each. The 3 groups all received conven-tional swallowing training.Patients in the NMES group also received NMES once a day and those in the intensive NMES group received it twice a day.Kubota's water drinking test and videofluoroscopy were used to assess the subjects' swallowing function before treatment and after 2 and 4 weeks of treatment. Results After 2 weeks, the average water drinking test score was significantly better in the intensive NMES group than in the control group. The fluoroscopic results of all three groups had improved significantly, but the results in the NMES group and the intensive NMES group were significantly better than those of the control group. The intensive NMES group also scored significantly higher than the NMES group. Conclusion NMES can improve the swallowing reflex and the contraction of the suprahyoid muscles. It can strengthen the effect of re-learning swallowing. Two NMES sessions a day are better than one.

ABSTRACT

Contents of laboratory diagnosis is complex.Traditional teaching method cannot arouse students' interest in study and the teaching effect is poor. Teachers should choose different teaching emphasis for students in different specialty, increase and decrease theory teaching contents according to the demands of students' future actual work, introduce case analysis, train students' capability of clinical logical thinking, enhance students' perceptual knowledge of theory through the training of experiment skills and using PBL teaching method to make students truly participate in teaching course so as to arouse their enthusiasm,These reforming measures will benefit the improvement of teaching quality.
