ABSTRACT

Imperforate hymen is a rare congenital anomaly. It may present early in infancy due to accumulation of secretion in vagina caused by excessive intrauterine stimulation of cervical mucous glands by maternal estrogen. Most cases present by urinary symptoms caused by hydrometrocolpos as well as bulging of imperforate hymen between the labia. 9 cases have been operated upon in Pediatric Surgical Department by hymenostomy. A new technique was used to drain the hydrometrocolpos and at the same time preserving the hymen intact and not deflorated. Follow up by ultrasound scanning showed that the technique is sufficient for drainage and no recurrence occurred

ABSTRACT

A new hypospadias repair technique is presented for distal cases, 80 patients with distal hypospadias have been operated by this technique during the past 2 years at the pediatric surgical department. It is a modified form of the old Beck technique and applicable only for distal types, glandular, coronal and sub-coronal with or without chrodee and rotation. The urethral meatus could be placed to the tip of the glands penis by mobilization and advancing the urethra without constructing a neo-urethra. This technique can be applied to circumcised patients

ABSTRACT

Gastric duplications are quite rare. They may occur in any part of the stomach, commonly arise from the greater curvature. 110 cases have reported in the English literature. On the other hand 7 of these duplications have been reported in the pyloric region. In this paper we report 3 cases of gastric duplication. In 2 cases the duplication arose from the greater curvature and in one case it arose from the pyloric region causing gastric outlet obstruction. Gastric duplication usually presents with a palpable mass and/ or vomiting. Vomiting from duplication in the pyloric region may simulate congenital hypertrophic pyloric stenosis. Surgical treatment is either by total excision with partial gastrectomy or by excision of the dome of the duplication cyst and stripping of the remaining mucosa from the remaining common wall

ABSTRACT

28 cases of anterior and mid-penile hypospadias were repaired using a modified Mathieu technique, in Pediatric Surgical Unit, Cairo University. Since a watertight urethroplasty was performed without overlying suture lines, catheter drainage and/or urine diversion was considered not necessary and they were not used in any patient. An excellent cosmetic and functional result was achieved with few complications even in boys who had prior unsatisfactory repair. All the children tolerated voiding through the repair post operatively and no one had urinary retention. 2 cases resulted in fistulae [one of them had had a prior failed repair] which needed surgical intervention

ABSTRACT

60 patients to the Pediatric Surgical Unit, Children Hospital, Cairo University with chronic bleeding per rectum. 44 of them were males [37%] while 16 were females [27%]. Bleeding was due to polyposis of different types in 53 cases [88%]. colitis of different forms was responsible for bleeding in 3 cases [5%], while local anal lesions were found in another 3 cases [5%]. In a single case no cause for bleeding was detected [idiopathic]. Patients were managed according to the cause of bleeding. Colonoscopic Polypectomy and biopsies for diagnosis in other lesions have been done in out-patient clinic. Patients were followed up for 3 months and re-endoscopy was carried out when needed. No complication had occurred from flexible fiberoptic colonoscopy or colonoscopic polypectomy. Colonoscopy proved to be a significant diagnostic tool as well as therapeutic aid

ABSTRACT

20 cases of congenital cleft lip have been repaired in the neonatal period [first 4 weeks of life] during the past 1 1/2 years at the pediatric surgical department, children's hospital, Cairo University. Preoperative full assessment for each case has been done and preoperative adhesive strapping was used in cases of bilateral cleft lip to improve the alignment of the protruded prolabium and premaxilla. The techniques of repair used were the modified Tennison-Randle in the unilateral cases and Veau 11 in bilateral cases. The alveolus and anterior plate were closed in most cases prior to lip repair using a Septal mucosal flap and buccal mucoperiosteum. Breast feeding can in fact be safely undertaken immediately following cleft lip repair contrary to the old idea the follow up period in not long enough to conclude a final recommendation, but the preliminary results are encouraging and no particular difficulties were encountered when early repair was performed and the results seemed to be as good as when surgery was delayed
