ABSTRACT

Objective To investigate the effect pre‐injection of dexmedetomidine(Dex) on hemodynamics during intratracheal intubation period of general anesthesia with propofol or etomidate induction .Methods Eighty ASA Ⅰ - Ⅱ patients with elective surgery were randomly assigned to 4 groups:propofol group(P) ,P combined with Dex group(PD) ,etomidate group(E) and E com‐bined with Dex group(ED) ,20cases in each group .SBP ,DBP ,BIS and HR in 4 groups were recorded at the time points of before an‐esthesia after entering the operation room (T0 ) ,at l min after beginning for giving Dex(T1 ) ,10 min of infusing Dex(T2 ) ,3 min af‐ter anesthetic induction (T3 ) ,immediate intubation(T4 ) ,at l min after intubation(T5 ) ,3 min after intubation(T6 ) and 5 min after intubation(T7 ) .The SpO2 values and Ramesay scores were recorded at T 0 and T2 .Results HR at T2 and T3 in the group PD and Ed was decreased compared with those at T0 ,and lower than those in the group P and E(P<0 .01);DBP and SBP at T3 in the group ,PD and E were decreased compared with at T 0 (P<0 .05) ,while SBP and DBP in the group ED were higher than those in the group P and PD (P<0 .05) ,SBP and DBP in the group P were lower than those in the other 3 groups(P<0 .05) ;SBP and DBP at T4 in the group E were significantly higher than those in other 3 groups (P<0 .01);the blood pressure at T5 in the group E was significantly higher than that at T0 and in other 3 groups(P<0 .05);SBP and DBP at T6 and T7 in the group P and PD was de‐creased to the preoperativel levels (P<0 .05) .The SpO2 value at T2 in the group PD was lower than that at T0 (P< 0 .05);the Ramsay score at T2 in the group PD and ED was higher than that in the group P and E(P<0 .05) .The BIS value at T2 in the group PD and ED was lower than that at T0 ,which at T3 -T7 in the four groups was lower than that at T0 (P<0 .05) .Conclusion Pre‐injection of Dex can maintain the hemodynamic stability during endotracheal inbubation period by etomidate induction and also re‐duce the cardiovascular reaction caused by endotracheal intubation .

ABSTRACT

Objective The hsa-miR-106b-25 gene cluster is involved in various biological processes of carcinoma .This study aims at a prediction and function analysis of the hsa-miR-106b-25 gene cluster, miR-106b, miR-93, and miR-25, so as to provide some evidence for further studies on the functions of the three miRNAs and the mechanisms of their interaction . Methods We obtained the sequences of miR-106b, miR-93, and miR-25 from the miRBase, predicted their target genes with TargetScan , PicTar, and miRanda, and used 3 or more experimentally verified target genes from the miRTarbase as the gene set for further bioinformatic analysis .We predic-ted the biological processes of the target genes by GeneOntology analysis and enriched KEGG ( Kyoto Encyclopedia of Genes and Genomes) by pathway analysis, produced protein-protein interaction ( PPI) networks with STRING . Results The target genes of the miR-106b-25 gene cluster were significantly enriched in such biological processes as the regulation of macromolecule metabolism , regulation of metabolic process , and cell cycle process , while the KEGG pathway mainly in glioma, melanoma, prostate cancer , and gallbladder carcino-ma.The proteins encoded by the targeted genes of showed complicated interactions , and those encoded by the KAT2B, PTEN, TP53, CDH1, MDM2, E2F1, RB1, and SMAD7 plaid a core role in the interac-tion network. Conc lusoi n MiRNAs of the miR-106b-25 gene cluster regulate the downstream target proteins involved in tumorigenesis by participating in the cell cycle and cancer signaling pathway .

ABSTRACT

Objective To evaluate bone cement longer handle artificial femoral head replacement treatment the clinical effect of the treatment of femoral fractures between rotor-instability.Methods There were 98 patients with fracture of instability-between the femur rotor ,women 60 cases,38 cases,male,aged 75 to 94 years old,average 83.5 years.Fractured by Tronzo-Evans type III 56 cases,type IV 42 cases.All use of bone cement longer handle arti-ficial femoral head replacement therapy ,to observe clinical therapeutic effect .Results All cases of postoperative X-ray showed both fracture reduction and fixation well between the rotor and prosthesis position is good .96 cases of post-operative for 6 to 24 months followed-up,the average 15 months.Harris score was 85.4%.Conclusion Bone cement longer handle artificial femoral head replacement has quick postoperative functional recovery ,weight-bearing activities earlier,which can avoid the advantages of long-term complications in bed ,instability is treatment of older femoral frac-tures between rotor reasonable operation method .

ABSTRACT

Objective To explore the clinical effects of three nail and plate systems in treating femoral intertrochanteric fracture.Methods The clinical data of 75 patients with femoral interochanteric fiactures treated by internal fixation were analyzed.According to different treatment methods,75 patients were divided into three groups:DHS group(treated with dynamic hip screw) 26 cases,PFN group(treated with proximal femoral nail) 15 cases,LCP group (treated with anatomical proximal femoral locking plate) 34 cases.Fracture union time,complications and hip function scores according to Harris's scoring were recorded.Results All patients were followed up for 18 to 36 months.Four patients appeared coax vara in DHS group and one case appeared coax vara in PFN group,three case appeared coax vara in anatomical proximal femoral locking plate (LCP.) There was no significant difference in fracture union time and the excellent and good rate of hip function scores among the three groups (F =2.25,χ2 =0.032,all P > 0.05).Conclusion The effects of three nail and plate systems in treating femoral intertrochanteric fracture is similar,but they have their own characteristics and indications.And the reasonable treatment should be selected according to different fracture types.

ABSTRACT

Objective To investigate the clinical effects of bridge type bone plate series designed by ourselves. Methods A sort of bridge type bone plate series were designed according to mechanics and structure of bridges. They were used in the internal fixation of 12 shinbone fractures and 41 fractures of middle and lower parts of thighbone. The treatment results were followed up and statistics was done to evaluate fracture healing at 8, 12, 16, 20, and 24 weeks respectively. Results Of the 53 fractures, 48 achieved clinical union and 27 solid union within 12 weeks, 51 reached clinical union and 47 solid union within 16 weeks, 53 reached clinical union and 52 solid union within 20 weeks, and all reached solid union within 24 weeks. Conclusion The self designed bridge type bone plate series are a nice internal fixation device for clinical treatment of fractures.
