ABSTRACT

Mostrar a experiência inicial do serviço com a técnica da angioplastia coronária transluminal. De janeiro de 988 a dezembro de 1989 foram realizadas 305 angioplastias coronárias em 280 pacientes, com idade variando de 33 a 82 anos (49% acima dos 60 anos) sendo 71% homens. Destes, 212 (75%) apresentavam quadro de angina estável e 68 (25%) angina instável ou infarto agudo do miocárdio. Os pacientes foram classificados em 3 grupos: grupo A - pacientes com lesäo maior ou igual a 70% em mais de uma artéria ou mesma artéria, com ou sem oclusäo da outra (múltiplas lesöes ou múltiplos vasos): 36 pacientes; grupo B - pacientes com lesäo maior ou igual a 70% em um vaso com pelo menos uma artéria ocluída: 64 pacientes; grupo C - pacientes com lesäo maior ou igual a 70% isolada de único vaso: 180 pacientes. Dos 280 pacientes tratados, 229 foram tratados com sucesso (75%). Ocorreram 22 complicaçöes (7%) e 9 óbitos (3%). No seguimento tardio (de 1 a 24 meses), 28 pacientes foram recateterizados devido a suspeita clínica de reestenose. Destes, notou-se obstruçäo ou oclusäo em 16, sendo realizada em 10 pacientes, nova dilataçäo, um deles 2 vezes. Os 12 restantes foram tratados clínica ou cirurgicamente. Embora os resultados a curto prazo sejam promissores, o procedimento näo está isento de riscos na fase aguda e é limitado na fase tardia pelo problema da reestenose. Contudo, a lesäo pode ser tratada com nova angioplastia com taxas de sucesso semelhantes ea do procedimentos inicial


Purpose: To show the initial experience of this service with the technique of percutaneous transluminal coronary angioplasty. Patients and Methods: Between januarv 1988 and december 1989, 305 coronary angioplasties were performed in 280 patients with age range of 33 to 82 years (beeing 49% above 60 years old) ana 71% male. Among those, 212 (75%) experienced clinical picture of stable angina and 68 (25% ) of unstable angina or myocardia infarction. Patients were divided in three groups: group Athose with lesions of 70% or worse in more than one vessel or in the same vessel with or without any other vessel total oclusion (multiple lesions or multiple vessels): 36 patients; group Bthose with lesions of 70% or worse in one vessel with at least one ocluded vessel: 64 patients; group Cthose with lesions of 70% or worse isolated in a single vessel: 180 patients. Results: Of the 280 treated patients, 229 were considered succesfull (75%). There has been 22 complications (7%) and 9 deaths (3%). The late follow-up (from 1 to 24 months), displayed 28 new procedures based on clinical suspicion of restenosis. Obstruction or oclusion were present in 16 of the later, with 10 beeing selected to undergo new procedure, one of those twice. The remaining 12 patients received clinical or surgical treatment. Conclusion: Aside from the fact that early and midlerm results indeed are promissing, the procedure is not safe from immedate risks, and limiting late rester nosis. Nevertheless, the lesions might be redilated by the same technique, with sucess rates comparable to the initial one.
