ABSTRACT

The route of transmission of leprosy is still unclear, Recently some authors proposed ge possibility that the bacilli discharged from nasal mucosa might be the source of infection in leprosy. This study was undertaken to evaluate the histopathologic findings of nasal mucosa in 24 fresh untreated patients with leprosy using hematoxyline and eosin stain, and Ziehl-Neelsen stain. Among 24 cases. under clinical observation, 10 cases(41,7%) complain of nasal stuffiness, 8 cases (33. 3%) has congestion, 1 case(4. 2%) was crusted and 1 case(4.2%) was ulcerated. 2 Bacterial index of nasal mucosal smear showed that 16 cases were positive, - 15 cases were 1+, 7 cases were 2+, 2 cases 3+ and 1 case was 4+ Linder H.E. stain, the histopathologic findings of (he inferior turbinate of nasal mucosa showed that 22 cases were cystic dilatation of mucous glands, 1) cases were granuloma with foamy cells cells and 14 cases were epidermal atrophy. TJnder Ziehl-Neelsen stain bacterial index of nasal mucosal biopsy showed that 21 cases were positive 3 cases were 3+, 7 cases were 4+6 cases were 5+ and 5 cases were 6+. As above results, the histopathologic findings of nasal mucosa were similar to the skin lesions of leprosy.

ABSTRACT

Inflammatory linear verrucose epidermal nevus(ILVEN) is characterized by an early age of onset, predominance in females, pruritus, unresponsiveness to treatment, and a distinctive inflammatory and psoriasiform histologic appearance. We present two cases of typical ILVEN. One of them was a 16-year-old male, who had had a mild pruritic linear verrucose patch involving the right arm which grew slowly for about 8 years. The other case was a 14-year-old girl whose lesion appeared at the age of 13. This lesion was localized on the right posterior thigh and showed linear distribution and verrucose appearance. Histopathological findings of two cases showed hyperkeratosis with foci of parakeratosis, moderate to marked acanthosis, elongation of the rete ridges, and occasional slight spongiosis, and chronic inflammatory infiltration in the dermis.

ABSTRACT

In the past, irritant or radiation therapy have been used to treat discoid lupus erythematosus, therefore malignant and proriferative change in the lesion of discoid lupus erythematosus were not uncommon. In recent year, however, because the preferred treatment of discoid lups erythematosus consisted of steroids and antimalarials, the association with squamous cell carcinoma has been extremely rare. We present a case of giant squamous cell carcinoma arising in the discoid lupus erythematosus in 39 year-old women. She had had a 20 year history of facial discoid lupus erythematoseus. In September of 1976, a thumb tip-sized tumor mass found on the right cheek and the maas grew into the adult palm size rapidly. The tumor show ulceration, serosanguinous discharge, easy bleeding, and foul oder. Histopathologically, the specimen obtained from plaques of the left cheek showed hyperkeratosis with plugging, atrophy of the stratum malpighii, a patchy, chiefly lymphoid cell infiltrate with a tendency to arrangement about the cutaneous appendages, hydropic degeneration of the basal cells, edema and vasodilatation in the dermis. The specimen obtained from tumor mass of the right cheek showed many nests consisted of well differentiated squamous cells, keratin pearls.

ABSTRACT

We present a case of generalized congenital ichthyosiform dermatosis associated with neurosensory hearing loss, neovascularization of corneas, partial alopecia and abnormalities of nails and teeth on a 26 years old female patient. This case is strictly similar to the previously reported cases. The combination of these abnormalities was stuggested to be a new syndrome by some authors. This case is the first reported case in Korea, although in the literature we could find several cascs with similar patterns of malformations.

ABSTRACT

For comparison of clinical classification of leprosy to histopathologic classification, a detailed histopathologic study, using hematoxylin and eosin stain and Ziehl-Neelsen stain, was done on 72 fresh uncomplicated cases of leprosy. The clinical classification was done using the criteria of Ridley and Jopling (1966), and the microscopic features were classified according to Ridley's(1974) definition. Clinical classification revealed that 8 of total 72 patients had tuberculoid(TT), 9 had borderline tuberculoid (BT), 5 had borderline(BB), 10 had borderline lepromatous.(BL), and 31 had lepromatous leprosy(LL). Nine patients were claasified as indeterminate(I) group. Histopathologic classification showed that 3 cases presented tuberculoid(TT), 10 presented borderline tuberculoid(BT), 4 presented borderline(BB), 9 presented borderline lepromatous(BL), 20 presented subpolar leprornatous(LLs), and 10 presented polar lepromatous(LLp) histopathologic characteristics, Sixteen cases were classified as indeterminate(I) leprosy by histopathologic findings. On comparison of clinical classification to histopathologic classification, the two were in consonance with each other in 50 cases(69.4%) and the disparity between them was noticed in 22 cases(30.6%). Among the 22 cases which showed disparity, there was a shift of one step either towards the tuberculoid or lepromatous end of the spectrum in 15 cases, and ihe remaining 7 cases were classified as indeterminate group beaause of nonspecific histopathologic changes.

ABSTRACT

Nevus Iipomatosus cutaneous superficialis (NLCS) is a very rare skin disease which usually is present at birth. The lesion consists of grouped papules and nodules which on histological examination show the presence of ectopic fatty tissue in the corium. The authors observed a 17year old female with NLCS. She noticed soft papules and nodules on the coccygeal area since 8 years old. We confirmed this case by clinically and histopathological findings which showed the strands or islands of mature fatty tissue within papillay, mid and lower dermis.

ABSTRACT

We studied germination activities of fungus conidiospores in active growth phase using the hanging slide culture method, then investigated the inhibitory effects of two antifungal agents(Tolnaftate and Clotrimazole) on germination activities. The fungal strains used in this study were Trichophyton (T.) rubrum, T. mentagrophytes and Candida (C.) albicans. Each strain was cultured on the Sabouraud's glueose agar plate and examined daily for detection of active growth phase of conidiospores (blastospores in C. albicans). After the conidiospores reached to active growth phase the activated spores were inoculated in one drop of Sabouraud's glucose broth on the cover glass and the hanging slide cultures were performed. The mean hermination rates of T. rubrum and T. nentagrophytes were 87.5% and 78% respectively in 72 hours. In C.albicans the average germination rate was 85.7% after cultivation for 24 hours, this germination rate was much faster than that of T.rubrum and T.mentagrophytes. For detection of miniman inhibitory concentration (MIC) of the antifungal agents to germination activities the antifungal agents were diluted with Sabouraud's glucose broth to various concentration and hanging slide culture was performed with the media containing the antifungal remedies. The minimal concentration of antifungal agents in which the conidiospores could completely not be germinated was denoted to minimal inhibitory concentration. The MIC of Tolnaftate to T.rubrum and T.mentagrophytes were 0.0.1ug/ml. The MIC of Cloirimazole to T. rubrum and T.mentagrophytes were 0.1ug/ml and to C. albicans was 1ug/ml. This results were similar to the previous reports by other investigators. This is a new method that could be applicated for evaluation of antifungal activities of antifungal agents.

ABSTRACT

Demacre', a new topical steroid cream containing 0.025% beclomethasone dipropionate, was used in several kinds of dermatoses. Total 35 patients including ll cases of contact dermatitis, 5 of atopic dermatitis, 3 of hand eczema, 3 of seborrheic dermatitis, 2 of neurodermatitis, 2 of nummular eczema, 2 of autosensitization dermatitis, 1 of chronic eczema, 3 of psoriasis, 1 of insect bite, 1 of miliaria rubra, and 1 of intertrigo were treated by Demacre'. Among these patients, we observed excellent effects in 4 patients (11%), good effects in 22 patients (63%), and fair effects in 3 patients (9%). Namely, among these 35 patients, 26 patients (74%) revealed excellent to good therapeutic effects on their skin conditions with Demacre'. No untoward side effects were noted in all subjects treated with Demacre.

ABSTRACT

We presented a case of acute febrile neutrophilic dermatosis (Sweet's syndrome) in 57 years-old female. This case is the 31 st case in the literature and the first case in Korea. Erythematous plaque was presented on the face with tendency of recurrences. Histologically a dense inflammatory cell infiltration composed predominantly of polymorphonuclear neutrophiles, lmphocytes and histiocytes was seen in dermis. Positive laboratory findings were polymorphonuclear leukocytosis and elevated ESR. The symptoms were well centrolled with orally administered preclnisolone.
