ABSTRACT

Oral submucous fibrosis [OSF] has always been recognized as a chronic, progressive and disabling disease of the oral mucosa. It is a well-recognized pre malignant condition which in most cases leads to oral squamous cell carcinoma [OSCC]. The objective of this study was to compare the presentation of OSF alone and in patients who have developed OSCC. This cross sectional study was conducted in the Department of Oral Medicine and Oral and Maxillofacial Surgery, Dr Ishrat-ul-Ibad Institute of Oral Health Sciences, Karachi. Out of 106 patients who were diagnosed with OSF, 17 had OSCC as well. The most common presenting complaints amongst patients presenting with OSF along and OSF with OSCC were limited mouth opening and mouth ulcers respectively [p= 0.001]. When compared with patient having OSF alone, patients presenting with OSF and associated OSCC were significantly older and gave history of prolonged use of areca nut and betel quid

ABSTRACT

The aim of the study was to assess temporalis muscle flap as an interpositional material for arthroplasty in patients with temporomandibular joint ankylosis and to identify the most common cause of temporomandibular joint ankylosis. Fifty patients of different age groups, reporting to Oral and Maxillofacial Surgery Department of Armed forces Institute of Dentistry were enrolled in study. Surgical procedure involved condylectomy followed by temporalis muscle flap interpositioning. First postoperative maximal incisal opening [MIO] was recorded on 5th post-op day. Patients were regularly followed up for a period of 18 months and their maximal incisal opening [MIO] was finally recorded. Trauma was found to be the main etiologic factor in TMJ ankylosis[n=46, 92%], followed by infections [n=3]. No case of relapse of limited mouth opening was seen during 18 months followup. Temporalis muscle proved to be a successful interpositional material with low morbidity and complications for the treatment of temporomandibular joint ankylosis

ABSTRACT

Work related musculoskeletal disorders [WMSDs] are one of the most common factors that not only decrease the quality of life but also compromise productivity of health care personnel. This cross sectional study was aimed at looking into the WMSDs affecting physicians, nurses and dentists. A study of two months duration was initiated at Aga Khan University Hospital, Karachi. Subjects included were divided into three groups medical doctors, dental professionals and nurses. A questionnaire was designed for collection of data from the respondents. The data was entered and analyzed using SPSS version 14. Strenuous work position was reported by 38.9% participants and standing work posture 45.6% and only 22.2% reported interchanging their work posture. The most commonly reported WMSDs was lower back pain among all groups. The prevention and reduction of WMSDs among doctors, nurses and dentists should include their education in ergonomics and awareness regarding the importance of work-related risk factors

ABSTRACT

Objective: To compare postoperative sensitivity following composite restoration placed in supra gingival class-V cavities using self etch adhesive and total etch adhesive. Study Design: A randomized clinical trial. Place and Duration of Study: Operative Dentistry Department of Armed Forces Institute of Dentistry, Rawalpindi, from July to December 2009


Methodology: A total of 70 patients having class-V supra gingival carious lesions were divided into two groups. Classes-V cavities not exceeding 3 mm were prepared. One treatment group was treated with self etch adhesive [adhe SE one Ivoclar] and the control group was treated with total-etch adhesive [Eco-Etch Ivoclar] after acid etching with 37% phosphoric acid. Light cured composite [Te-Econom Ivoclar] restoration was placed for both groups and evaluated for postoperative sensitivity immediately after restoration, after 24 hours and after one week. Data was recorded on visual analogue scale


Results: Comparison of sensitivity between the two treatment groups on application cold stimulus after 24 hours of restoration showed significant difference; however, no statistically significant difference was observed at baseline, immediately after restoration and at 1 week follow-up with cold stimulus or compressed air application


Conclusion: Less postoperative sensitivity was observed at postoperative 24 hours assessment in restoration placed using SE adhesives compared to TE adhesives. Thus, the use of SE adhesives may be helpful in reducing postoperative sensitivity during 24 hours after restoration placement

ABSTRACT

To compare oral submucous fibrosis with other white oral lesions for presentation and associated factors. Cross-sectional study. The Departments of Oral Medicine and Oral and Maxillofacial Surgery, Dr. Ishrat-ul-Ibad Institute of Oral Health Sciences [DIKIOHS], Karachi, from May 2008 to May 2009. Patients presenting with oral white lesions were selected by consecutive non-purposive sampling and clinico-demographic data was collected. For patients with oral submucous fibrosis [OSF], additional information like duration of habits, maximal incisal opening [MIO], presence of any other associated lesion were noted. OSF was compared with other white lesions for any association between characteristic of subjects. Chi-square and independent t-tests for determining the statistical significance at p < 0.05. OSF was present in 59.6% [n = 106] of the 178 patients; other white lesions were 40.4% [n = 72]. The mean age of patients with OSF was 34 +/- 12.7 years and 45.81 +/- 16.2 years in patients with other white lesions, [p < 0.0001]. Items containing areca nut were consumed more by patients with OSF, with a significant [p < 0.0001] compared to patients with other white lesions. OSF was the predominant white lesion in patients examined at DIKIOHS. Areca nut was found to be chewed more by patients with OSF and still longer by patients with SCC.

ABSTRACT

The present study was undertaken to compare the efficacy of manual and powered toothbrushes in 70 patients between 19 and 44 years of age who reported with mild gingivitis to AFID. Patients were randomly divided into two groups of 35 each. Group A patients were selected for electronic powered tooth brushes, while group B patients were selected for manual brushes. At baseline, after one and 3 months, plaque was rated using Silness-Loe plaque index. Data were statistically analyzed using SPSS version 12. Both brushes significantly reduced the plaque accumulation, though to different degrees. A lower plaque score [score 0 and 1] was obtained at three months in 88.5% patients using powered brushing and in 77.1% patients using manual brushing. Powered brushing showed lower plaque score in 11.4% more patients than the manual brushing after three months of follow up

ABSTRACT

Lack of knowledge of root form, internal morphology and critical interpretation of radiographs will undoubtedly lead to an error in obturation of root canals. The aim and objective of this study was to clinically and radiographicaly determine the frequency of single and more canals in maxillary second premolars and to compare the clinical and demographic features by taking periapical radiography at two different angles among a sample of local population and also to investigate the gender variations. Patients were recruited from regular pool of patients presenting to Dental department of PIMS, Islamabad, for root canal therapy on maxillary second premolars, during six month study period. Hundred male and female patients were selected according to the inclusion and exclusion criteria. Periapical radiographs through PT were taken at different angulations and visualized. Endodontic files were inserted into the canals through an access opening and a confirmatory radiograph was taken to confirm the numbers of root canals. The preoperative radiographs were correlated with the clinically assessed radiographs. Among the hundred patients [n=57] 57% were males and [n=43] 43% were females. Clinically assessed radiographs showed that 65% had single root canals and 35% had multiple canals. There was no significant difference between characteristics of patients with maxillary second premolars presenting with single canal or multiple canals
