ABSTRACT

Intussusception in adults is rare. Gastrointestinal lipomas are rare benign tumors and Intussusception due to a gastrointestinal lipoma constitutes an infrequent clinical entity. The diagnosis of Intussusception was finally raised by a computed tomography (CT) scan of the abdomen.60 year old normotensive non-diabetic male presented to the surgery opd with the chief complaints of intermittent pain abdomen since 5 weeks. Adult bowel intussusception is a rare but challenging condition to diagnose in a timely manner. Due to their intramural location, lipomas can also serve as the leading point for intussusception. A high index of suspicion and appropriate investigations (abdominal ultrasound, CT scan and colonoscopy) can result in prompt diagnosis. Surgical resection remains the treatment of choice and produces an excellent prognosis.

ABSTRACT

Background: To compare the effectiveness of Total contact casts (TCCs), Removable cast walkers (RCWs) and half-shoes to heal diabetic foot lesions. Methods: This proposed study was carried out as a prospective, randomized clinical trial in 100 patients diagnosed diabetic foot with superficial non-infected, non-ischemic diabetic plantar foot ulcers. Out of 100 pts, 10 pts did not use any prosthesis and in 90 pts, one of the three offloading modalities was used: Total contact cast (TCC), Removable cast walker (RCW) and half shoes (HS). Outcomes were assessed at wound healing or at 12 weeks, whichever came first. Primary outcome measures included duration of healing of diabetic wounds and also regarding the percent of wounds healed completely after 12 weeks. Results: The proportions of healing for patients treated with TCC, RCW, and HS were 85.7, 68.7, and 56.2%, respectively. A significantly higher proportion of patients were healed by 12 weeks in the TCC group when compared with the two other modalities. There was also a significant difference in survival distribution (time to healing) between patients treated with a TCC and both an RCW and half-shoe. The mean number of days to complete wound healing is 30 days for TCC, 52 days for RCW and 60 days for HS. Conclusion: The TCC seems to heal a higher proportion of wounds in a shorter amount of time than two other widely used off-loading modalities, the RCW and the half-shoe.

ABSTRACT

Background: Nursing staff plays important role in the health care system; one of the key skills that students should develop during their training is to be prepared for emergency life saving measures like cardiopulmonary resuscitation (CPR) anytime, anywhere. At least the doctors, nursing and paramedical staff are expected to know about it, as they frequently face life threatening situations. Objectives: a) To evaluate the CPR awareness among nursing staff and students. b) To screen the knowledge regarding accurate, effective CPR procedural techniques and various barriers of CPR failure in clinical practice from a student perspective. Methods: A cross-sectional Observational study conducted among nursing staff and students of the Krishna institute of nursing science, Karad. Maharashtra. Results: No one has complete knowledge of BLS. Only 3% of total students have 80% knowledge about BLS.81% of the students has less than 50% of knowledge.Conclusion:It can be concluded that not only the nursing staff and students be trained in Basic Life Support, but also it has to be reinforced from time to time, since the skills of CPR are difficult to teach and once taught difficult to retain. As these are the persons, who are near the patient for 24 hrs easily approachable with in no time till the resident or the CMO attends the patient in the hospital. Also, this should be incorporated into the academic curriculum, as it will become a compulsory topic to be dealt with specially for the nursing students.
