ABSTRACT

【Objective】 To investigate the effects of extracorporeal shock wave lithotripsy (ESWL) on procalcitonin (PCT), renal function and inflammatory index levels in patients with indinavir-induced urinary calculi so as to provide reference for clinical practice. 【Methods】 A total of 77 patients with urinary calculi admitted to the Department of Urology of our hospital from January 2017 to January 2021 were selected for retrospective analysis. They were divided into control group (38 cases, non-indinavir related urinary calculi) and observation group (39 cases, indinavir-related urinary calculi) according to the years of the disease. Before treatment and 7 days after treatment, the serum levels of PCT, CRP, Scr, UA, Kim1, VAS scores, and IgG, IgM, MDA, and SOD levels were compared between the two groups. 【Results】 The expressions of serum PCT and CRP at 7 days after treatment in the two groups were significantly lower than those before treatment, and the improvement degree of the observation group was significantly higher than that in the control group (P0.05), but the intraoperative blood loss and MAP 20 min after induction of anesthesia were significantly better than those in the control group. Seven days after treatment, GSH-Px and SOD were significantly better in the observation group than in the control group and before treatment (P<0.05). 【Conclusion】 ESWL treatment for patients with indinavir-induced urinary calculi can effectively reduce the expressions of serum PCT and CRP, improve renal function indexes, reduce intraoperative blood loss, and reduce the level of inflammatory indexes; the clinical effect is good.

ABSTRACT

Objective:By analyzing factors associated with lower urinary tract symptoms in middle-aged and elderly women aged 55-65 years old, a nomogram model for lower urinary tract symptoms was constructed to assist the clinical development of targeted interventions to reduce the incidence of lower urinary tract symptoms in this population.Methods:In the cross-sectional study, 798 middle-aged and elderly women aged 55-65 years receiving physical examination in the Health Management Center of Third Xiangya Hospital from November 2013 to December 2020 were selected as research participants.Univariate regression analysis was used to compare differences in the basic data related to lower urinary tract symptoms in the population.Multivariate Logistic regression analysis was used to identify related influencing factors, and a nomogram model for lower urinary tract symptoms in the population was established.Results:Among 798 middle-aged and elderly women surveyed, the prevalence of lower urinary tract symptoms was 81.08 %(647 cases), and the rate of urinary tract infections was 21.48 %(139 cases). Variables with statistical significance in univariate regression analysis were included in multivariate Logistic regression analysis.The results showed that body mass index, vaginal delivery, strength of pelvic floor type Ⅱ fiber muscle, pelvic and abdominal coordination and mental health were independent influencing factors for lower urinary tract symptoms in middle-aged and elderly women aged 55-65 years( OR=1.099, 2.681, 0.895, 0.658, 1.057, P=0.010, 0.030, 0.040, 0.010, 0.038). The monogram model based on the five risk prediction indexes produced a consistency index(C-index)of 0.651 and a diagnostic sensitivity and specificity of 66.9% and 58.9%, respectively.The correction curve showed that the predicted results of the model were essentially the same as the actual probability of condition. Conclusions:In this study, the independent influencing factors for lower urinary tract symptoms in middle-aged and elderly women aged 55-65 years were screened by univariate regression and multivariate Logistic regression analysis and a nomogram model for risk prediction of the population was constructed, was proved to have a good ability for accurately and effectively predicting the risk of lower urinary tract symptoms in middle-aged and elderly women aged 55-65 years, and will assist clinicians to screen for high-risk patients, formulate targeted interventions, and reduce the incidence of lower urinary tract symptoms in middle-aged and elderly women aged 55-65 years.

ABSTRACT

Hepatoma is one of the most common malignant tumors of digestive system worldwide, and a main factor leading to cancer-related deaths. Its incidence is increasing year by year, posing a serious threat to human health. Currently, hepatoma is mainly treated by surgical resection, liver transplantation, radiation and drugs, but there are certain adverse reactions and problems of high recurrence rate and low survival rate. Chinese medicine has unique advantages in improving the comprehensive curative effect of hepatoma and reducing adverse reactions. With a variety of active ingredients, Chinese medicine can induce hepatoma cell apoptosis, inhibit the proliferation, migration and reverse multidrug resistance through multiple targets, thus exerting anti-hepatoma effect. It has become an important means for the prevention and treatment of hepatoma as well as a rich resource for anti-hepatoma drug research and development. Wnt/β-catenin signaling pathway, one of the most classical pathways in cancer, is involved in tumor cell proliferation, cell cycle, migration, invasion and tumor angiogenesis. Recently, many studies have reported that the active ingredients of Chinese medicine can play an anti-hepatoma role through this pathway. Therefore, this paper summarized the domestic and foreign literature in recent years, analyzed the relationship between wnt/β-catenin signaling pathway and the specific mechanism of hepatoma occurrence and development, and combed the literature on the effect of flavonoids, terpenoids, alkaloids, polysaccharides and other active ingredients of Chinese medicine on inducing hepatoma cell apoptosis, regulating cell cycle and inhibiting the invasion and metastasis through Wnt/β-catenin signaling pathway. In addition, the paper summarized the research progress of relevant active ingredients of Chinese medicine against hepatoma, to explore their specific mechanism against hepatoma through Wnt/β-catenin signaling pathway, so as to provide theoretical reference for further development of anti-hepatoma drugs.

ABSTRACT

This study aimed to define the most consistent white matter microarchitecture pattern in Parkinson's disease (PD) reflected by fractional anisotropy (FA), addressing clinical profiles and methodology-related heterogeneity. Web-based publication databases were searched to conduct a meta-analysis of whole-brain diffusion tensor imaging studies comparing PD with healthy controls (HC) using the anisotropic effect size-signed differential mapping. A total of 808 patients with PD and 760 HC coming from 27 databases were finally included. Subgroup analyses were conducted considering heterogeneity with respect to medication status, disease stage, analysis methods, and the number of diffusion directions in acquisition. Compared with HC, patients with PD had decreased FA in the left middle cerebellar peduncle, corpus callosum (CC), left inferior fronto-occipital fasciculus, and right inferior longitudinal fasciculus. Most of the main results remained unchanged in subgroup meta-analyses of medicated patients, early stage patients, voxel-based analysis, and acquisition with 30 diffusion directions. The subgroup meta-analysis of medication-free patients showed FA decrease in the right olfactory cortex. The cerebellum and CC, associated with typical motor impairment, showed the most consistent FA decreases in PD. Medication status, analysis approaches, and the number of diffusion directions have an important impact on the findings, needing careful evaluation in future meta-analyses.

ABSTRACT

Objective:To investigate the clinical characteristics, diagnosis, treatment and outcome of patients with human herpesvirus 7 (HHV-7) viral encephalitis after allogeneic hematopoietic stem cell transplantation (allo-HSCT).Methods:The clinical manifestations, laboratory characteristics, diagnosis and treatment process and outcome of 3 patients with HHV-7 viral encephalitis after allo-HSCT in Hebei Yanda Lu Daopei Hospital from 2018 to 2020 were retrospectively analyzed, and the related literature was reviewed.Results:The clinical features of 3 patients with HHV-7 viral encephalitis after allo-HSCT included fever, headache, vomiting, apathy, etc., without specific symptoms or signs. The conventional white blood cell count in the cerebrospinal fluid was normal or slightly higher, mainly lymphocytes, and the cerebrospinal fluid protein was normal or slightly higher. The HHV-7 virus DNA in cerebrospinal fluid was positive, and the treatment with ganciclovir or foscarnet was effective. The prognosis was favorable in two mild cases, but one case with cerebral hemorrhage died eventually.Conclusions:HHV-7 viral encephalitis is a rare infection after allo-HSCT, and it can be easily misdiagnosed due to lack of typical symptoms and indications for routine laboratory tests. The detection of HHV-7 DNA in the cerebrospinal fluid can help confirm the diagnosis. Currently, there is no standard treatment programs, but ganciclovir and foscarnet are effective.

ABSTRACT

Objective:To investigate the effect of scar uterus on assisted reproduction treatment strategy.Methods:From January 1, 2017 to December 31, 2017, 109 cases of scar uterus group and 63 cases of vaginal delivery history group who underwent in vitro fertilization-embryo transfer (IVF-ET) assisted pregnancy fresh embryo transfer in our hospital were retrospectively analyzed. Results:⑴ There was no significant difference in the total amount of gonadotropin (Gn) and the total days of GN in patients <35 between scar uterus group and the vaginal delivery history group ( P>0.05), but there were significant differences in the number of oocytes, mature oocytes, two pronucleus (2PN) and excellent embryos between the scar uterus group and the vaginal delivery history group ( P<0.05). When the age was ≥35 years old, there was no significant difference in the total number of GN, the total days of GN, the number of oocytes obtained, the number of mature oocytes, the number of 2PN and the number of excellent embryos between the two groups ( P>0.05). ⑵ There were no statistically significant differences in clinical pregnancy rate and embryo implantation rate between the two groups of patients < 35 years old, no matter single embryo transplantation or double embryos transplantation. When transplant two embryos, the clinical pregnancy rate and embryo implantation rate in the scar uterus group were slightly lower than those in the vaginal delivery history group (57.57% vs 71.05% and 39.39% vs 47.37%, respectively), but with no significant difference ( P>0.05). There were no statistically significant differences in the clinical pregnancy rate and embryo implantation rate between single embryo transplantation and double embryo transplantation in patients≥35 years old ( P>0.05). ⑶ There were no statistically differences in biochemical pregnancy rate, early abortion rate, preterm birth rate and average newborn weight between the scar uterus group and the vaginal delivery history group ( P>0.05). Conclusions:Cesarean section may affect the ovarian function. For patients with previous cesarean section, early evaluation of ovarian function is recommended. Single embryo transfer does not reduce the outcome in IVF-ET. It is recommended to perform single embryo transfer for patients with scar uterus to reduce the risk during pregnancy of twin pregnancy.

ABSTRACT

Objective:To investigate the effect of bundles of care on pediatric hypnosis by rectal chloral hydrate.Method:A total of 368 children aged 0 to 3 years old who were treated in Dalian Children′s Hospital from December 2018 to June 2019 and were treated with chloralhydrate enemas were randomly divided into the control group (routine nursing intervention) and observation groups (bundled Intervention) 184 cases each. The chloralhydrate enemas were inserted for sedation, and the observation group added sedation while chloralhydrate enemas for sedation. After admission, record the data of each patient, and do the Ramsay sedation score after sedation. According to the sedation score, the sedation failure rate of chloralhydrate enema was increased, and the proportion of each score in the group was compared.Results:Ramsay sedation scores of the observation group and the control group after enema were 5.42 ± 1.34 and 3.14 ± 1.21, respectively. There was significant difference between the two groups ( t value was 2.08, P<0.05). The failure rates in experimental group and control group were 13.59% (25/184) and 38.59% (71/184), respectively. There was a significant difference between two groups ( χ2 value was 29.82, P<0.05). Conclusion:Bundles of care could decrease the failure rate of rectal chloral hydrate hypnosis significantly, then reduce the suffering of children. It is worth being widely used in clinic.

ABSTRACT

Objective@#To investigate the application of metagenomic next-generation sequencing (mNGS) in detection of the rare or difficult-to-cultivate pathogens.@*Methods@#One patient with acute lymphoblastic leukemia who went through allogeneic hematopoietic stem cell transplantation (allo-HSCT) developed symptoms of infection after transplantation. Conventional microbial culture, polymerase chain reaction (PCR), and mNGS combined with biological information analysis were performed with plasma and cerebrospinal fluid samples, the anti-infective treatment was adjusted according to the test results, and the efficacy was assessed.@*Results@#No suspected pathogens were detected by microbial culture and PCR in the cerebrospinal fluid and plasma samples since the patient developed infection symptoms. However, Legionella pneumophila was analyzed by mNGS in the cerebrospinal fluid specimen on day 23 after allo-HSCT (reads count: 19 655), and it was considered as the principal pathogen after comprehensively evaluating the patient's clinical manifestations and the test results. Then the antimicrobial treatments were adjusted according to the patient's clinical manifestations and laboratory test results, and the number of gene sequences of Legionella pneumophila was monitored by mNGS method. Azithromycin, tigecycline, and other antibiotics effective for Legionella pneumophila were used after detecting this pathogen. A total of 15 mNGS analysis were performed during the 5-month period, and the highest number of Legionella pneumophila sequences monitored in the cerebrospinal fluid was 2 226, the lowest was 253 and eventually turned negative. The clinical symptoms and treatment outcomes were consistent with the mNGS monitoring results.@*Conclusions@#The mNGS technology has significant value in detection of the rare and difficult-to-cultivate pathogens. The mNGS technology provides a valuable supplement to microbial culture and PCR methods.

ABSTRACT

Objective To systematically evaluate the efficacy and safety of oral preparation of ginkgo biloba in treating vascular dementia (VD).Methods A fully literature search was conducted in CNKI,Wanfang,Sinomed,VIP databases,PubMed,Embase,the Cochrane Library and Clinical Trails with the deadline of October,2018 for randomly controlled trials of oral preparation of ginkgo biloba for vascular dementia.After assessing the quality of literatures included,RevMan5.3 software was used to analyze the extracted data.Results A total of 17 studies which including 1 326 patients were involved.The results of meta-analysis showed that:compared with the western medicine group,oral ginkgo biloba preparation combined with western medicine group was much better in the MMSE assessment [MD=2.89,95% CI (2.06,-3.72),P=0.000 1],there was no significant difference between the western medicine group and the pure oral ginkgo biloba group in the ADL assessment [MD=-3.00,95% CI (-4.69,-1.30),P=0.000 5] and overall clinical status assessment [RR=1.29,95% CI (1.20,1.39),P<0.000 01],the efficacy of the pure oral ginkgo biloba group and western medicine group was similar in the ADL assessment [MD=-0.93,95% CI (-3.48,1.63),P=0.48].Conclusions Based on the meta-analysis,oral preparation of ginkgo biloba combinedwithwestemmedicinehas certain advantages compared with conventional western medicine treatment alone,but a large number of large-sample and high-quality trials are requiredtofurther prove and review its effect

ABSTRACT

AIM:To investigate the role of immunoproteasome subunit β2i in deoxycorticosterone acetate (DOCA)/salt-induced vascular inflammation in mice.METHODS:Wild-type and β2i knockout male mice were used.The right kidney was removed and DOCA pellet was subcutaneously implanted in the mice.The mice were then received 1% NaCl as drinking water for 3 weeks.The total RNA and protein were isolated from thoracic aorta 3 weeks later.The aortic tissues were fixed in formalin, embedded in paraffin and sectioned.Western blot, real-time PCR and immunohistochemistry were performed to detect the expression of β2i, macrophage marker Mac-2, NF-κB, and proinflammatory cytokines IL-1β, IL-6 and TNF-α in thoracic aorta.RESULTS:Compared with sham group, DOCA/salt treatment significantly increased the expression of β2i at mRNA and protein levels, increased the infiltration of macrophages and expression of Mac-2, and upregulated the expression of NF-κB and proinflammatory cytokines including IL-1β, IL-6 and TNF-α in wild-type group, whereas theses effects were markedly attenuated in β2i knockout mice.CONCLUSION:Immuneproteasome subunit β2i is involved in DOCA/salt-induced vascular inflammation through activation of NF-κB signaling in the mice.

Besides, we carry out teaching with scientific research mode, scientific evaluation to assess the effects and making analysis to expose the prob-lems, which lay a solid foundation for establishment of the innovative pharmacy experiment in the future.

ABSTRACT

Objective To explore the clinical characteristics of renal transplantation in patients with diabetes mellitus (DM)-induced end-stage nephropathy.Methods The clinical data of 408 cases who underwent renal transplantation in our center from 2009 to 2013 were retrospectively analyzed.The patients were divided into DM group (n =82) and non-DM group (n =326).The postoperative infection,delayed graft function (DGF),adverse events,and the survival rate of patients/kidneys were comparatively analyzed.Results The incidence of postoperative infection,DGF and adverse events was significantly higher in DM group than in non-DM group (23.2% vs.15.6%,P =0.04;17.1% vs.8.6 %,P =0.04;13.4% vs.8.3 %,P =0.03).No significant difference was found in the 1-,2-,and 3-year survival rate of patients and kidneys between the two groups after operation (P> 0.05).Conclusion The incidence of postoperative infection,DGF and adverse events is higher in DM patients.The DM does not affect the survival rate of patients/kidneys through appropriate treatment.It is important to prevent complications in DM patients after renal transplantation.

ABSTRACT

Objective: To observe the clinical efficacy in lowering blood lipids with ultrasound therapy device and Xuezhikang. Methods:Selected 80 cases as an object of study in Heilongjiang Provincial Hospital Integrative Medicine treated from January 2013 to July 2014, the observation group 40 cases, used ultrasound therapy device and Xuezhikang, control group 40 cases, only single use of Xuezhikang overall treatment, compared the two groups of patients before and after treatment and lipid levels in the blood. Results:The total efficiency of compared two groups of patients, a statistically significant (x2=13.536, P<0.05);compare TG group before and after treatment in the observation group, TC, HDL-C, LDL-C, were statistically significant(t=3.265, t=4.021, t=-2.871, t=3.665;P<0.05). In the control group before and after treatment group TG, TC, LDL-C were statistically significant (t=2.18, t=2.994, t=2.276;P<0.05). After treatment, the comparison between the two groups TG, TC, HDL-C, LDL-C, were statistically significant (t=-2.998, t=-3.046, t=2.887, t=3.028;P<0.05). Conclusion:Combined use of ultrasound therapy device and Xuezhikang lipid-lowering therapy, can significantly lower blood lipid levels in patients, and the effect is significant, worthy of clinical application.

ABSTRACT

Objective To evaluate the feasibility and safety of transumbilical laparoendoscopic sin-gle-site dismembered pyeloplasty ( U-LESS-P ) for the treatment of ureteropelvic junction obstruction (UPJO). Methods Between Mar.2011 and Mar.2012, U-LESS-P was performed in 8 consecutive pa-tients with UPJO by one experienced laparoscopic surgeon .The patients included 5 males and 3 females, with an average age of 28 (16-45) years.Of the 8 patients, six presented with flank pain , and two were a-symptomatic and discovered incidentally by health check .Of the 8 patients, seven had UPJO on the left side and one on the right side .The diagnosis was established by renal ultrasonography , diuretic renal scan , intra-venous urography (IVU) or/and computed tomography urography (CTU).Renal ultrasonography, IVU and ( or) CTU showed hydronephrosis and UPJO in the affected side , while diuretic renal scan demonstrated re-nal function deteriorated .No patients had undergone abdominal surgery previously .A 2-2.5 cm umbilical in-cision was made for single-port access .The procedures were performed using 30°5 mm or 10 mm laparoscope with a combination of conventional and bent laparoscopic instruments . Results All procedures were com-pleted successfully .None was converted to open surgery or traditional laparoscopic surgery .The mean opera-tive time was 153 (117-190) min, and the average estimated blood loss about 20 (10-40) ml.The mean time to resume oral diet was 1.5 days.The drainage remained 2-7 days.The mean hospital stay was 6 (4-8) days.With the follow-up of 3-6 months, symptom-free was investigated in all 8 cases.Ultrasonography , diuretic renal scan and IVU showed decreased or disappeared hydronephrosis .No operative complication , such as anastomotic stoma stenosis , was founded . Conclusions U-LESS-P is a safe and effective proce-dure for the treatment of UPJO , with the advantages of decreased operative morbidity , postoperative rapid re-covery and improved cosmetic result .

Objective To explore a promising system for tumor CD 44 receptor-targeted imaging and to investigate their physic-chemical properties and targeting effect on CD 44 abundant cancer cells in vitro.Methods The superparamagnetic iron oxide ( SPIO) nanoparticles were prepared by a coprecipitation in alkaline media starting from a mixed of the ferrous and ferric solution.And then the surface of the SPIO nanoparticles were modified with APTMS by a reaction with the hydroxyl groups.Finally, the hyaluronan-modified SPIO ( SPIO-HA) nanoparticles were prepared.Control and experimental groups were established after adding SPIO or SPIO-HA as agents respectively.Transmission electron microscopy ( TEM) and particle size analyzer were used to measure these nanoparticle sizes and the hydrodynamic diameters.Thermogravimetric analysis ( TGA) was carried out to evaluate the HA-content on the surface of SPIO-HA.The MRI T2 ralaxivities (1/T2 ) of the two groups at different Fe concentrations (0.09, 0.18, 0.27, 0.36, 0.45 mmol/L ) were measured on a 3.0T MR system.HepG2 cells and HL7702 cells were used for assessment of cells viability by methyl thiazolyl tetrazolium ( MTT ) assay.Prussian blue staining , immunoassay fluorescence image and flow cytometry were carried out to determine the targeted cellular uptake of SPIO-HA nanoparticles.MRI were performed to show the MR T 2 value changes after incubating with HepG2 cancer cells by using T 2 WI sequences at a clinical 3.0 T MR system.One-way analysis of variance was performed to determine significant changes in MR T 2 values of blank control , SPIO-HA and SPIO groups.Results The SPIO-HA and SPIO NPs were fairly homogeneous with an average core size of 18.2 and 22.4 nm, hydrodynamic diameter of 91.1 and 103.2 nm, Zeta potential of (-45.00 ±0.86) mV and (-18.50 ±0.73) mV, and magnetic relaxivity of 0.212 ×106 M-1 · s-1 and 0.191 ×106 M-1 · s-1.Based on the TGA data , HA accounted for 24%weight of each SPIO-HA.The internalization of the SPIO-HA was confirmed by prussian blue staining , while the cells showed no obvious blue stains with SPIO , incubation of SPIO-HA with tumor cells led to blue color inside the cells.After that, we examined cancer cell binding of FITC-SPIO-HA by immunoassay fluorescence image and flow cytometry.The green fluorescence resulting from FITC-SPIO-HA was observed inside the cells in both the cytoplasm and the plasmalemma.Tumor cells treated with SPIO-HA exhibited higher fluorescence signals with 7.97-fold enhancement observed for HepG 2 cells over control particles.In vitro MR, mean T2 values of blank control , SPIO and SPIO-HA groups were ( 115.20 ±0.36 ), ( 115.07 ±0.81 ) and ( 21.67 ±0.21 ) ms, respectively.There was significant difference among those three groups (F=31 703.339,P<0.01), MR T2 values of HepG2 cells treated with the SPIO-HA NPs were lower than blank and SPIO group.In comparison, SPIO did not generate any MRI signal changes compared with blank group.Conclusion The tumor CD44 receptor-targeted MR molecular probe SPIO-HA had a good physic-chemical property and well targeted HepG2 cells.

ABSTRACT

Objective To evaluate the effects of the comprehensive psychological intervention on anxiety and depression of the patients with breast cancer during chemotherapy ,and to explore the influence of depression and anxiety before and after chemother-apy on the cellular immune function .Methods A total of 81 married female patients with breast cancer were surveyed and assessed with self-designed questionnaire ,the Herth Hope Scale ,the Hamilton Depression Scale(HAMD) and the Hamilton Anxiety Scale (HAMA) .64 cases of breast cancer with depression and anxiety according with the included standards (HAMD scores ≥8 and HA-MA scores ≥7) were randomly divided into the intervention group(n=32) and the control group(n=32) .The levels of T lympho-cyte subsets(CD3+ ,CD4+ ,CD8+ ,CD4+ /CD8+ ) and the natural killing(NK) cells in peripheral venous blood were determined by the flow cytometry(FCM) .Results The Herth scores after chemotherapy in the intervention group were increased by 37 .19% than that of the control group ,while the HAMD scores were decreased by 36 .01% compared with the control group ,the differences be-tween them showing statistical significance (P<0 .01) .In the comparison between before and after chemotherapy ,the Herth scores in the intervention group were increased by 12 .57 ± 4 .57 ,while the HAMD and HAMA scores were declined by 9 .32 ± 1 .86 and 4 .35 ± 1 .15 respectively ,the differences showed the statistical significance (P< 0 .01) .Compared with the control group ,CD3+ , CD4+ ,CD4+ /CD8+ ratio and NK cells before and after chemotherapy in the intervention group were increased by 2 .10 ± 1 .63 , 1 .51 ± 1 .02 ,0 .05 ± 0 .02 and 0 .78 ± 0 .52 correspondingly ,the differences among them had statistical significance(P<0 .001) .The HAMD scores were negatively correlated with the CD4+ /CD8+ ratio(r= -0 .554) .Conclusion The cellular immune hypofunction in the patients with breast cancer is probably related with anxiety and degree of depression .The comprehensive psychological inter-vention model can improve the anxiety and depression mood during chemotherapy .

Objective To explore the relationship of adult attachment,life orientation style and job burnout among nurses.Methods Relationship Questionnaire (RQ) and Experience of close relationships (ECR) inventory were used to measure 566 nurses' adult attachment.Life orientation test (LOT-R) and Maslach's Burnout Inventory-General Scale (MBI-GS) were administrated to measure subjects'optimism,pessimism,and job burnout.Results ①Distribution of adult attachment style in nurses were 36% for secure,29.3% for dismissing,18.4%for preoccupied and 6.7% for fearful.②Attachment avoidance were positive correlated with pessimism (r =0.139) and negative correlated with optimism (r =-0.212) significantly; and there was significant positive correlation between attachment avoidance and ineffectiveness (r=0.122) also.Attachment anxiety was positively related to pessimism (r=0.151),and three indexes of job burnout (r=0.238,0.281,0.143 respectively).Meanwhile,there were significant negative correlations among optimism and job burnout indexes (r =-0.12,-0.193,-0.230).③There were significant differences among four attachment groups on optimism and pessimism.For job burnout,the four groups had significant differences only on depersonalization,instead of ineffectiveness and emotion exhaustion.④ Regression analysis suggested that attachment anxious was a valid positive predictor of job burnout,while optimism was a negative predictor.Conclusion Both adult attachment and life orientation style are contributed to job burnout among nurses.

ObjectiveTo investigate the application of the ratio of early diastolic peak velocity of mitral valve(E) and early diastolic peak velocity of mitral annular(Ea) on evaluating left ventricuiar diastolic function in patients with chronic heart failure.MethodsLeft atrial diameter(LAD),left ventricular end-diastolic diameter (LVEDd),interventricular septal thickness (IVST),left ventricular ejection fraction (LVEF),E and late diastolic peak speed of mitral valve (A) of 30 patients with diastolic heart failure(DHF,DHF group),30 patients with systolic heart failure(SHF,SHF group) and 30 healthy persons (control group) were detected by conventional echocardiography.E/A ratio and E-wave deceleration time (DT) were calculated.Early diastolic peak velocity of longitudinal septal and left ventricular lateral part of mitral annulus (Esep,Elat) were measured by tissue Doppler imaging (TDI).Ea and E/Ea ratio were calculated as well as the relevant analysis was made.Results ( 1 ) LAD and IVST in DHF group were significantly higher than those in control group [(44.90±8.31) mm vs.(30.87±3.48) mm,(9.43±2.18) nn vs.(8.17 ± 1.12)mm,P<0.05].LVEDd in SHF group was obviously higher than that in control group and DHF group [(66.30 ± 10.21) mm vs.(44.77 ± 4.17) mm,(48.60 ± 8.85) mm,P<0.05],and LVEF and DT were significantly lower than those in control group and DHF group(P < 0.05 ).(2) Esep,Elat and Ea in DHF and SHF group were significantly lower than those in control group( P < 0.01 ),and E/Esep,E/Elat and E/Ea ratios were obviously higher than those in control group (P < 0.01 ).( 3 ) The level of B type natriuretic peptide (BNP) in DHF and SHF group was significantly higher than that in control group [(477.73 ± 722.76),(978.60 ± 825.04) ng/L vs.(29.23 ± 17.84) ng/L,P < 0.05 ].The level of BNP in SHF group was significantly higher than that in DHF group (P<0.05 ).(4) Multivariate linear regression analysis showed that NYHA classification and BNP were correlated with E/Ea ratio (P < 0.05).(5) The area under curve (AUC) of E/Ea in diagnosing chronic heart failure was 0.984 (95% CI:0.000-1.000).AUC of E/Ea in identifying DHF and SHF was 0.857(95% CI:0.773-0.940).The sensitivity and specificity of E/Ea were much higher than those of E/A.Conclusions Chronic left ventricular systolic insufficiency is commonly combined with different-degree diastolic dysfunction.The evaluation of left ventricular diastolic function can be more simple and accurate combined with pulse wave Doppler and TDI.

ABSTRACT

Ethanol fermentation from Jerusalem artichoke tubers by recombinant Saccharomyces cerevisiae strains expressing the inulinase gene (inu) from Kluyveromyces marxianus was investigated. The inu native and pgk promoters were used to drive the expression of the inu gene, and the inulinase was expressed as an extracellular enzyme. All positive clones (confirmed by PCR) were able to express inulinase as measured by enzyme activity in the culture supernatant, among which two clones HI6/6 and HPI6/3 were selected, and their inulinase activity and ethanol fermentation performance were compared with their wild type. The inulinase activities of 86 and 23.8 U/mL were achieved, which were 4.6-fold and 1.5-fold higher than that of the wild type. Furthermore, ethanol fermentation was carried out with the recombinants and medium containing 200 g/L raw Jerusalem artichoke meal, and ethanol concentrations of 55 g/L and 52 g/L were obtained, with ethanol yields of 0.495 and 0.453, respectively, equivalent to 96.9% and 88.6% of the theoretical value.
