RESUMEN Introducción: La enfermedad por SARS-CoV-2 se convirtió en un importante problema de salud pública. El 5 % de los pacientes requirió intubación endotraqueal por insuficien cia respiratoria aguda hipoxémica, y se generó un aumento de consulta por estenosis traqueal. Se realizó el trabajo con el objetivo de expresar los resultados de la cirugía de resección traqueal en pacientes con estenosis post COVID 19. Material y métodos: Estudio prospectivo, observacional, analítico. Se incluyeron 11 pacientes. La evaluación prequirúrgica y postquirúrgica fue la misma en todos los pacientes. Se valoró en el postquirúrgico la ventilación y la fonación hasta los 30 días. Se utilizó la clasificación de Clavien y Dindo para calificar a las complicaciones post-quirúrgicas, con seguimiento hasta los 60 días postquirúrgicos. Análisis estadístico: se aplicó prueba de Wilcoxon para comparar los resultados. Resultados: El 27.2 % de los pacientes tuvieron complicaciones postquirúrgicas. Fue estadísticamente significativa la comparación de la ventilación entre el prequirúrgico y el postquirúrgico (p<0.05) con mejoría en el postquirúrgico. Al comparar la fibrobron coscopía prequirúrgica con la postquirúrgica (diámetro de la luz traqueal) también el resultado fue estadísticamente significativo (p <0.05). Conclusión: Los resultados obtenidos son similares a los expresados en la literatura. La cirugía de resección traqueal es un procedimiento seguro y efectivo y debe ser considerada como tratamiento de primera línea para la estenosis traqueal.


ABSTRACT Introduction: The disease caused by SARS-CoV-2 has become a significant public health issue. 5 % of the patients required endotracheal intubation due to acute hypox emic respiratory failure, leading to an increased number of consultations for tracheal stenosis. This work was done with the aim of presenting the results of tracheal resection in patients with post-COVID-19 stenosis. Material and methods: Analytical, prospective, observational study. 11 patients were included. The preoperative and postoperative evaluation was the same for all patients. Post-surgical ventilation and phonation were assessed up to 30 days. The Clavien and Dindo classification was used to grade post-surgical complications, with follow-up extended up to 60 post-surgical days. Statistical analysis: the Wilcoxon test was used to compare the results. Results: 27.2 % of the patients had postoperative complications. The comparison of pre- and postoperative ventilation (p < 0.05) was statistically significant, with improve ment in the postoperative period. When comparing pre- and postoperative fiberoptic bronchoscopy (tracheal lumen diameter), the result was also statistically significant (p < 0.05). Conclusion: The results obtained are similar to those expressed in the literature. Tra cheal resection is a safe and effective procedure and should be considered as first-line treatment for tracheal stenosis.

Objetivo: Os ortodontistas usam alicates ortodônticos continuamente, e essas ferramentas têm um forte potencial para infecções nosocomiais. Este estudo teve como objetivo comparar a eficiência de três métodos de desinfecção de alicates ortodônticos. Material e Métodos: As pontas ativas de 26 alicates ortodônticos (cortadores distais e alicates Weingart) foram contaminadas com microrganismos, vírus e esporos S. aureus, E. coli e C. albicans. Os métodos de controle microbiano foram desinfecção com álcool 70%, esterilização com esferas de vidro (250 °C calor seco) e irradiação com luz ultravioleta (250 nm UV-C) por 30 e 60 segundos. O número de unidades formadoras de colônias (UFC) e unidades formadoras de placas (UFP) foi quantificado e comparado para cada microrganismo após incubação em placas de cultura. Resultados: Todas as pontas do alicate dos grupos que receberam luz ultravioleta ou foram submetidos à esterilização com esferas de vidro apresentaram número significativamente menor de esporos, bactérias e fungos do que suas respectivas amostras controle (p<0,001). A desinfecção física com luz UV-C pode representar uma alternativa confiável em comparação com outros métodos químicos e físicos devido ao aumento de microrganismos resistentes a produtos químicos e à emissão de subprodutos nocivos após o tratamento químico. Conclusão: Os métodos de controle microbiano testados foram eficazes na desinfecção de alicates ortodônticos, tornando a luz ultravioleta-C uma alternativa promissora para eliminar os microrganismos dos alicates (AU)


Objective: Orthodontists use orthodontic pliers continuously, and these tools have a strong potential for nosocomial infections. This study aimed to compare the efficiency of three methods for disinfecting orthodontic pliers. Material and Methods: The active tips of 26 orthodontic pliers (distal end cutters and Weingart pliers) were contaminated with S. aureus, E. coli, and C. albicans microorganisms, viruses, and spores. The microbial control methods were 70% alcohol disinfection, glass bead sterilization (250 °C dry heat), and ultraviolet light irradiation (250 nm UV-C) for 30 and 60 seconds. The number of colony-forming units (CFU) and plaque-forming units (PFU) was quantified and compared for each microorganism after incubation in culture plates. Results: All tips of the pliers in the groups that received ultraviolet light or were subjected to glass bead sterilization showed a significantly lower number of spores, bacteria, and fungi than their respective control samples (p<0.001). Physical disinfection with UV-C light may represent a reliable alternative compared to other chemical and physical methods due to the increase in microorganisms resistant to chemical products and the emission of harmful by-products after chemical treatment. Conclusion: The tested microbial control methods were effective in the disinfection of orthodontic pliers, making ultraviolet-C light a promising alternative to eliminate microorganisms from pliers (AU)

ABSTRACT

Abstract Orthodontic-surgical treatment with the "Surgery First Approach" provides immediate facial aesthetic improvements and significantly reduces the patient's orthodontic treatment time, avoiding the transient worsening of the facial profile due to dental decompensation that occurs in surgical cases. Thus, this clinical case describes the retreatment of a 22-year-old female leukoderma patient, whose main complaint was related to the proclination of upper and lower incisors. The patient used a mio-relaxing plate for 30 days, which evidenced the skeletal mandibular deficiency and the ½ bilateral Class II malocclusion. Orthognathic surgery first approach associated with the extraction of the 4 premolars was chosen considering the patient's aesthetic demand. The use of a mio-relaxing plate in the diagnostic stage was essential for the real diagnosis of mandibular deficiency and the technique employed made it possible to conclude the treatment avoiding aesthetic commitment, with excellent results. Resumo O tratamento ortodôntico-cirúrgico por meio do benefício antecipado proporciona melhorias estéticas faciais imediatas e reduz de maneira significativa o tempo de tratamento ortodôntico do paciente, evitando a piora transitória do perfil facial devido à descompensação dentária que ocorre em casos cirúrgicos. Assim, este caso clínico descreve o retratamento de uma paciente com 22 anos de idade, leucoderma, sexo feminino, que apresentava queixa principal relacionada à inclinação vestibular dos dentes anteriores. Após uso de placa miorrelaxante por 30 dias, verificou-se a presença de Classe II esquelética com deficiência mandibular e ½ Classe II dentária bilateral. Considerando a demanda estética da paciente, optou-se pela abordagem ortodôntico-cirúrgica com Benefício Antecipado associada à extração de 4 pré-molares para correção da inclinação dentária anterior. O uso da placa miorrelaxante foi fundamental para o diagnóstico real da deficiência mandibular e a técnica empregada possibilitou concluir o tratamento evitando o comprometimento estético pré-cirúrgico, com obtenção de excelentes resultados. ABSTRACT

Objective: To evaluate the fracture resistance (RF) of Class II Glass-ionomer Cement (GIC) ART restorations with and without proximal retentions. Material and Methods: 20 freshly extracted human molars were used. Forty (40) standard Mesial-Occlusal (MO) and DistalOcclusal (DO) preparations (20 for each material) were performed with a 245 bur. The unprepared surfaces of the teeth were protected with nail polish and the specimens submerged in 0.5Mol EDTA solution, pH 7.4 for 8h under stirring. The preparations were finished with dentine spoons and 50% received proximal retention with # 3 excavators. 20 cavities were restored with Chemfil Rock (10 with retention and 10 without retention) and 20 cavities were restored with Equia Fil (10 with retention and 10 with no retention) and were stored in an oven at 37ºC and 100% relative humidity for 24h and submitted to axial compression loading in Test Machine - EMIC at a rate of 0.5 mm / minute, until restoration fracture occurred. The values were analyzed by two-way ANOVA (p<0.05). Results: ChemFil Rock presented 300.84 (69.20) (without retention) and 361.70 (81.08) (with retention) and Equia Fil showed 314.60 (69.97) (without retention) and 366.67 (103.38) (with retention). Data obtained with retention were statistically superior to those obtained with non-retained ART restorations (p=0.014). No statistical differences were detected between materials (p=0.761). Conclusion: Retentive grooves improved fracture resistance of Class II GIC ART restorations. (AU)


Objetivo: avaliar a resistência à fratura (FR) de restaurações de ART de Classe II de Cimento de ionômero de vidro (CIV) com e sem retenções proximais. Material e Métodos: Foram utilizados 20 molares humanos recém-extraídos. 40 cavidades padronizadas no sentido Mesial-Oclusal (MO) e Oclusal-Distal (OD) (20 para cada material) foram realizadas com uma broca 245. Os preparos cavitários foram submersos em solução 0,5 mol Mol EDTA, pH 7,4 por 8h sob agitação e foram finalizados com colheres de dentina, nos quais 50% receberam retenções proximais com escavadores #3. 20 cavidades foram restauradas com Chemfil Rock (10 com e 10 sem retenção) e 20 cavidades foram restauradas com Equia Fil (10 com e 10 sem retenção) e armazenadas em estufa a 37ºC e 100% de umidade relativa por 24h e submetidos a carga axial de compressão na máquina de ensaios EMIC a uma taxa de 0,5mm/min, até que a fratura de restauração ocorresse. Os valores foram analisados por ANOVA twoway (p<0,05). Resultados: ChemFil Rock apresentou 300.84 (69.20) (sem retenção) e 361.70 (81.08) (com retenção) e o Equia Fil apresentou 314.60 (69.97) (sem retenção) e 366.67 (103.38) (com retenção). Os dados obtidos com retenção foram estatisticamente superiores àqueles sem retenção (p=0.014). Não houve diferença estatística entre os materiais (p=0.761). Conclusão: Os sulcos retentivos melhoraram a resistência à fratura de restaurações de ART de Classe II de Cimento de ionômero de vidro (CIV).(AU)

Abstract Introduction Sporotrichosis is a common disease in tropical regions, caused by the fungus Sporothrix schenckii, affecting mainly rural workers and in direct contact with animals. Although treatment by indiscriminate use of oral antifungal drugs gives rise resistant isolates, leading to therapeutic failures and no remission of the disease. To evaluate the effectiveness of red low-power laser photobiomodulation in inactivation of S. schenckii infection induced in rodents. Methods Subcutaneously inoculation (2x103 S. schenckii/ml, 0.2 ml suspension) in the left footpad, in 27 mice divided into: control (n = 6, inoculated, without irradiation): early stage (not inoculated) = 1th biopsy; intermediate (9 weeks of evolution) = 2nd biopsy; final (21 weeks of evolution) = 3th biopsy. Treated (n = 21, inoculated and irradiated): early (13 weeks of evolution, 4 weeks after first irradiation) = 4th biopsy, intermediate (17 weeks of evolution, 8 weeks after first irradiation) = 5th biopsy, final (21 weeks of evolution, 12 weeks after first irradiation) = 6th biopsy. Serial irradiation with biopsies occurred every 30 days during each month, for three months. At the end of testing, the mice were euthanized, and histological analyzes of biopsies were performed. Results Each laser treatment session showed an inactivation of S. schenckii in treated animals compared to controls, with a regression of pseudoepitheliomatous hyperplasia, chronic inflammation, neutrophils, granulomas, giant mononuclear cells and steroid corpuscles. Conclusion The laser photobiomodulation was effective on S. schenckii inactivation, appearing to be an interesting therapeutic option in infections caused by this organism.

ABSTRACT

ABSTRACT Fournier's Gangrene (FG) is an infectious disease caused by several synergic microbes, with high morbidity and mortality rates; therefore, the search for new less invasive and mutilating treatments, with faster recovery, has been proposed. Surgical intervention, the use of several systemic and topic antibiotics, and hyperbaric oxygen therapy are currently the best approach for the treatment of these patients. The use of Photodynamic Inactivation (PDI) aims to lower morbidity and mortality, by reducing bacterial microbiota and speeding wound healing. In the present study, viable bacteria were separated in four groups: Group L-/F- (no irradiation with red laser and absence of methylene blue photosensitizer), Group L-/F+ (no irradiation with red laser and presence of methylene blue), Group L+/F- (irradiation with red laser and absence of methylene blue) and L+/F+ (irradiation with red laser associated to methylene blue). In all groups, exposure time to treatment was 5, 10 and 15 minutes. The concentration of methylene blue photosensitizer was 0.1mg/L, and the dose of red laser (660nm wave length) was 176.9mW/cm2. Following irradiation, the reduction of number of bacteria was evaluated, and the results were expressed in colony forming units (CFU) and as exponential reduction. As the main results, in the L+/F+ group, there were no Clostridium perfringens and Staphylococcus aureus CFUs and there was a reduction of Escherichia coli that was not observed in the other groups.

ABSTRACT

RESUMO Objetivo Verificar a associação entre zumbido e disfunção temporomandibular em idosos. Métodos Estudo transversal realizado com a inclusão de idosos com vida independente. A disfunção temporomandibular foi avaliada por exame odontológico e o zumbido foi verificado pela história médica. A análise estatística foi realizada utilizando o teste Qui-quadrado, o risco relativo e a regressão logística. Resultados O zumbido foi observado em 82,9% dos indivíduos com disfunção temporomandibular e, através desta análise, observou-se que a disfunção temporomandibular é um fator de risco para o zumbido. Conclusão Houve associação entre zumbido e disfunção temporomandibular na população idosa. Ressalta-se a importância de identificar fatores de risco para o zumbido, que podem ser modificados por meio de intervenções específicas, uma vez que esta prática é essencial na prevenção de episódios futuros, bem como na gestão do processo de tratamento de pacientes idosos, em geral.


ABSTRACT Purpose To verify the association between tinnitus and temporomandibular dysfunction in the elderly. Methods A cross sectional study was conducted with the inclusion of elderly individuals with independent living. Temporomandibular dysfunction was assessed by odontological evaluation and tinnitus was verified by medical history. Statistical analysis was performed using the chi-square test, relative risk and logistic regression. Results Tinnitus was observed in 82.9% of individuals with temporomandibular dysfunction and through this analysis is shown that temporomandibular dysfunction as a risk factor for tinnitus. Conclusion The results showed that there was association between tinnitus and temporomandibular dysfunction in the elderly population and emphasizes the importance of identifying risk factors for tinnitus that can be modified through specific interventions, since it is essential in the prevention of future episodes, as well as managing the process of treatment of elderly patients in general.

ABSTRACT

Esta pesquisa apresenta um estudo de caso desenvolvido pelo serviço de psicologia de um Instituto de Hemodiálise e objetiva realizar avaliação psicológica e acolhimento da família de uma adolescente (14 anos) com doença renal crônica, em tratamento hemodialítico. A adolescente apresenta queixas frequentes de dor difusa, baixo rendimento escolar e carência afetiva durante sessões dialíticas. Instrumentos: Entrevistas semidirigidas com a família e a adolescente, observação da hora lúdica, Escala de Stress para Adolescentes, técnica projetiva House-Tree-Penson (H-T-P), orientação familiar, visita domiciliar e discussão com a equipe multiprofissional. Pôde-se concluir que o tratamento hemodialítico causa alterações significativas na vida de adolescentes, relacionadas a aspectos psicossociais, físicos e emocionais. Promover saúde do doente renal crônico em desenvolvimento requer acolhimento da família, da equipe multiprofissional e da escola. Neste estudo, constatou-se negligência familiar e expuseram-se as novas configurações da parentalidade. Apontou-se a necessária implementação de políticas públicas de saúde aos doentes crônicos e educação especial, com atividades domiciliares para atender satisfatoriamente aos indivíduos que usam serviços de alta especialidade, promovendo sua inserção social e diminuindo a fragmentação no seu desenvolvimento.


This study assesses the psychological well-being from a 14 years old teenager suffering from a chronic kidney disease. He depends on dialysis and presents frequent complaints of diffuse pain, low school performance, and affective shortcomings during hemodialysis. The research was done at a psychology department of a Hemodialysis Institute. The study aimed to evaluate the adolescent's psychological condition and the reception of her family. They were used as instruments: a semi-directed interview with the family and the adolescent, observation of play time, Stress Scale for Adolescents, House-Tree-Person projective technique (H-T-P), home visit, discussion with the clinical team and family orientation. Results showed that the hemodialysis treatment brought significant changes to the adolescent, concerning psychosocial, physical and emotional aspects. They also showed family negligence, need of reviewing the parental configurations and public health policies' regarding chronic kidney disease patients, together with special education including home activities. We can conclude chronic kidney disease patients need high-quality health service and special attention, so they can achieve social acceptance and reduce problems in their development. May be reduced chronic kidney disease teenagers' health requires reception by family, school and professional team who can provide these cares.


Esta investigación presenta un estudio de caso clínico desarrollado por el servicio de psicología de un Instituto de Hemodiálisis y objetiva realizar evaluación psicológica y acogida de la familia de una adolescente (14 años) con enfermedad renal crónica en tratamiento hemodialítico. La adolescente presenta quejas frecuentes de dolor difusa, bajo rendimiento escolar y carencia afectiva durante las sesiones dialíticas. Instrumentos: Entrevistas semi-dirigidas con la familia y la adolescente, observación de la hora lúdica, Escala de síntomas de estrés para adolescentes, técnica proyectiva House-Tree-Penson (H-T-P), orientación familiar, visita domiciliar y discusión clínica con el equipo multiprofesional. Se puede concluir que el tratamiento hemodialítico causa cambios significativos en la vida de adolescentes, relacionados con aspectos psicosociales, físicos y emocionales. La promoción de la salud del paciente renal crónico en desarrollo requiere acogida de la familia, del equipo multiprofesional y de la escuela. En este estudio, se constató negligencia familiar y se colocaron en pantalla las nuevas configuraciones de la parentalidad. Se señala la necesaria implementación de políticas públicas de salud a los enfermos crónicos y educación especial, con actividades domiciliares para atender satisfactoriamente a los individuos que usan servicios de alta especialidad, promoviendo su inserción social y disminuyendo la fragmentación en su desarrollo.

ABSTRACT

O objetivo deste estudo retrospectivo implica em comparar a presença de velamento, o espessamento de mucosa e a presença de septo no seio maxilar em radiografias panorâmicas e tomografias computadorizadas de feixe cônico (TCFC). A amostra foi composta por 100 pacientes do arquivo, que apresentavam as radiografias panorâmicas e as TCFC antes do início do tratamento ortodôntico, sendo 57 mulheres e 43 homens, com média de idade de 19,08 anos. Foram incluídas nesta pesquisa apenas os exames que apresentavam boa qualidade e bom posicionamento do paciente. A avaliação do velamento, espessamento de mucosa e presença de septo no seio maxilar foi realizada por um único examinador calibrado, de forma independente, nas radiografias e nas TCFC. A concordância entre os exames foi avaliada por meio do teste Kappa. Os resultados demonstraram boa concordância entre as radiografias panorâmicas e TCFC (Kappa entre 0,62 a 0,72), com -90, -88 e 89% de convergência entre os métodos, para velamento do seio maxilar; espessamento de mucosa e presença de septo no seio maxilar, respectivamente. Pode-se concluir que foi encontrada concordância satisfatória na avaliação do velamento, espessamento de mucosa e presença de septo no seio maxilar entre as radiografias panorâmica e TCFC. Dessa forma, as características do seio maxilar foram identificadas, de maneira semelhante, na radiografia panorâmica e na TCFC. (AU).


The objective of this retrospective study was to compare the presence of filled maxillary sinus, mucosal thickening and the presence of septum in the maxillary sinus on panoramic radiographs and concomitant computed tomography (CBCT) scans. The sample consisted of 100 patients from the archive who presented the panoramic radiographs and the CBCT before the beginning of the orthodontic treatment, being 57 women and 43 men, with a mean age of 19.08 years. Only the exams that presented good quality and good positioning of the patient were included in this study. The evaluation of the presence of filled maxillary sinus, mucosal thickening and the presence of septum in the maxillary sinus was performed by a single examiner, regardless of who was calibrated on panoramic radiograph and CBCT. The agreement among the exams was assessed by means of Kappa test. The results showed good agreement between panoramic radiograph and CBCT (Kappa between 0.62 and 0.72), with -90, -88 and 89% convergence between the methods, for the presence of filled maxillary sinus, mucosal thickening and presence of septum in the maxillary sinus, respectively. It can be concluded that satisfactory agreement was found in the evaluation of the the presence of filled maxillary sinus, mucosal thickening and presence of septum in the maxillary sinus between the panoramic and CBCT radiographs. Thus, the characteristics of the maxillary sinus were similarly identified in panoramic radiography and CBCT. (AU).

Sporotrichosis is a disease that affects the lymph vessels, skin and some internal organs. Most cases are presented as asubacute chronic mycosis caused by the Sporothrix schenckii fungus; fairly common in tropical regions. The aim of thisstudy was to evaluate the susceptibility of Sporothrix schenckii yeast cells to the effects of photodynamic inactivation.For this, the viable cells were separated into four groups: irradiated with photosensitizer group (L+F+); irradiated withoutphotosensitizer group (L+F-), without irradiation and with photosensitizer group (L-F+); and without irradiation andwithout photosensitizer group (L-F-). The methylene blue photosensitizer concentration used was 0.1 mg/mL, and theAluminum Gallium Arsenide laser dose was 26.3 J/cm2. Then, counting of colony forming units (CFUs) was performedin each group. The main result was that the irradiated group with photosensitizer (L+F+) was the one that showed nogrowth of CFUs. Thus, it was concluded that Sporothrix schenckii can be inactivated by use of photodynamic therapy


A esporotricose é uma doença que afeta os vasos linfáticos, pele e alguns órgãos internos. A maioria dos casos seapresenta como uma micose subaguda à crônica, provocada por fungos do complexo Sporothrix schenckii, bastantecomuns em regiões tropicais. O objetivo deste estudo foi avaliar a suscetibilidade aos efeitos da inativação fotodinâmicaem células leveduriformes de fungos do complexo Sporothrix schenckii. Para tal, as células viáveis foram separadas emquatro grupos, sendo estes: grupo irradiado com fotossensibilizador (L+F+); grupo irradiado sem fotossensilizador (L+F-),grupo não irradiado com fotossensibilizador (L-F+); e grupo não irradiado sem fotossensibilizador (L-F-). A concentraçãodo fotossensibilizador azul de metileno utilizada foi de 0,1 mg/mL, e a dosagem do laser de Arseneto de Gálio Alumíniofoi de 26,3 J/cm2. Em seguida, foi realizada a contagem das unidades formadoras de colônias (UFCs) em cada grupo.Como principal resultado, verificou-se que o grupo irradiado com fotossensibilizador (L+F+) foi o único que nãoapresentou crescimento de UFCs. Dessa forma, concluiu-se que fungos do complexo Sporothrix schenckii podem serinativados com o uso da terapia fotodinâmica

Objective: To evaluate the factors related to the time of exclusive breastfeeding (EBF) in infants from Pará de Minas and Três Corações (Minas Gerais) and Brumado (Bahia), Brazil. Material and Methods: All mothers attended in public and private hospitals of these cities between June and December 2012 (n=156) were invited to answer an interview related to EBF. Data were collected: (1) at maternity ward; (2) at 4 and (3) 6 months of the infant's life. In (2) and (3), data were collected by phone. Data were recorded in the SPSS software, and descriptive analyses and association of exposure and outcome variables related to the practice of EBF were performed. The chi-square test was used, considering p <0.05. Results: The total of women was in (1) 156, (2) 104 and (3) 123, and the EBF rate was 50.0%, 39.0% and 4.8%, respectively. In (1) mothers with lower schooling level have opted for EBF more frequently. In (1), the use of bottle did not interfere in EBF; on the other hand, in (2) and (3), the use of bottle has decreased the frequency of EBF (p < 0.05). Conclusion: The ideal frequency of EBF recommended by WHO to the six months of the infant's life is not followed in the evaluated cities. In addition, EBF can be influenced by the use of bottle and maternal schooling level.

Introdução: Os aparelhos de protração mandibular fixos, como o Forsus Fatigue Resistant Device, constituem uma alternativa para o tratamento da má oclusão de Classe II, sobretudo nos casos de pacientes pouco colaboradores. Contudo, assim como outros aparelhos de protração mandibular, o Forsus apresenta tendência à vestibularização dos dentes anteroinferiores, o que o contraindica para pacientes que apresentem essa característica prévia. Com o intuito de controlar esse processo, dispositivos de ancoragem esquelética podem ser associados aos aparelhos de protração mandibular fixos como método de ancoragem. Objetivo: Apresentar a aplicação conjunta do aparelho Forsus com reforço de ancoragem por meio de mini-implantes para o controle da vestibularização de incisivos inferiores. Método: Foram instalados 2 mini-implantes (Neodent 1,6 x 10 mm) na região mesial ao primeiro molar e, em seguida, esses dispositivos foram conectados ao fio ortodôntico e associados ao aparelho Forsus. Para avaliação da posição dos incisivos inferiores com a utilização do conjunto Forsus/mini-implante foram realizadas medidas cefalométricas (IMPA, 1.NB, 1-NB, ANB, trespasses vertical e horizontal) em 4 momentos: inicial (T0), antes da instalação do conjunto (T1), final de tratamento (T2) e 1 ano após remoção (T3), por meio do programa Dolphin 11.5. Resultados: Ao avaliar os dados cefalométricos obtidos pela diferença entre T2 e T1, observou-se aumento do IMPA (3,7°) e do 1.NB (0,7°), além da diminuição dos trespasses horizontal (3,2 mm) e vertical (2,5 mm), do 1-NB (1,5 mm) e do ANB (2,9°). Verificou-se ainda, que um ano após o término do tratamento (T3), os valores relacionados à inclinação dos incisivos inferiores foram mantidos. Conclusão: O conjunto Forsus/mini-implante permitiu o controle da inclinação dentária anteroinferior por meio do reforço de ancoragem oferecido pelos dispositivos de ancoragem esquelética temporários. Estudos clínicos prospectivos randomizados são necessários para demonstrar de forma mais abrangente os efeitos desta proposta terapêutica. (AU)


Introduction: Fixed mandibular protraction appliances, such as the Forsus Fatigue Resistant Device, are an alternative for Class II maloclusion treatment, especially in cases of non-cooperative patients. However, similarly to other mandibular protraction appliances, the Forsus device shows a tendency to vestibularization of the anterior inferior teeth that contraindicates it for patients who have this previous characteristic. In order to control this process, skeletal anchoring devices can be associated with fixed mandibular protraction appliances as an anchoring method. Objective: To introduce the joint application of Forsus appliance with anchorage reinforcement through mini-implants to control the vestibularization of lower incisors. Method: Two mini-implants (Neodent 1.6 x 10 mm) were installed in the mesial region of the first molar, and then these devices were connected to the orthodontic wire and associated with the Forsus appliance. In order to assess the position of the lower incisors using the Forsus/mini-implant assemblage, cephalometric measures (IMPA, 1NB, 1-NB, ANB, vertical and horizontal trespasses) were performed at 4 moments: initial (T0), before the installation of the assemblage (T1), end of treatment (T2), and 1 year after removal (T3), using Dolphin 11.5 program. Results: When evaluating cephalometric data obtained by the difference between T2 and T1, an increase in IMPA (3,7 °) and 1NB (0,7 °) was observed, in addition to a decrease in horizontal (3,2 mm) and vertical trespasses (2,5 mm), in 1-NB (1,5 mm) and of ANB (2,9°). Moreover, it was observed that one year after the end of the treatment (T3), the values related to the inclination of the lower incisors were maintained. Conclusion: The Forsus/ mini-implant assemblage allowed the control of the anterior-inferior tooth inclination by reinforcing the anchorage offered by the temporary skeletal anchorage devices used. Randomized prospective clinical trials are needed to demonstrate more comprehensively the effects of this therapeutic proposition.(AU)

ABSTRACT Purpose To investigate the influence of the presence of Temporomandibular Disorders (TMD) on postural balance in elderly individuals. Methods The study sample consisted of 150 elderly: 103 women (67.7±5.0 years) and 47 men (69.3±5.5 years). Evaluation of the presence and severity of TMD included an anamnesis questionnaire, an evaluation of the temporomandibular joint (TMJ), and a muscular examination, which allowed the division of the elderly into 2 groups: G1 (experimental, n=95), with TMD; G2 (control, n=55), without TMD. Postural balance was assessed by means of the one-leg stance test (OLST) on a force platform (BIOMEC400), thus permitting the following measurements: center of foot pressure (COP); mean sway velocity (MV) and frequency (MF) of COP in both the anteroposterior (VAP and FAP) and medial/lateral (VML and FML) directions. The statistical analysis of data was performed using independent t-test, Variance Analysis, and Chi Square test (α=5%). Results Presence of TMD was observed in 63.3% of the individuals (Group 2), with different degrees of severity (mild: 42.7%, moderate: 14.7%, severe: 6%). Significantly higher TMD was observed for women (73.8%) compared with men (40.4%) (p=0.0002). No significant difference was found in between the groups for all balance parameters, e.g., presence and severity of TMD, presence of pain to palpation of TMJ and of masticatory and cervical muscles. Conclusion The presence and severity of TMD, in addition to the presence of pain to palpation of TMJ and masticatory and cervical muscles did not alter the variables related to postural balance in this population.

Este trabalho teve como finalidade relatar o caso clínico de uma paciente de três anos, gênero feminino, cuja queixa principal era a estética dos dentes anteriores superiores,devido ao tamanho desproporcional de um elemento dentário, que se apresentava também com coloração alterada. Ao exame clínico e radiográfico diagnosticou-se uma anomalia denominada fusão dentária, união de dois dentes adjacentes, na região anteros superior. O tratamento consistiu na secção dos dois dentes e exodontia do supranumerário, seguida do tratamento restaurador do incisivo central direito que apresentava lesão de cárie. Concluiu-se que, por meio de adequado diagnóstico e planejamento, o tratamento proposto foi efetivo na reabilitação da função e da estética,assim como na prevenção de complicações clínicas e ortodônticas que poderiam advir da anomalia dentária descrita, caso não tratada. A autoestima da criança foi restabelecida.


Dental anomalies can cause serious disorders in the development of teeth if not diagnose dearly. This study aimed to report a case of a three-year-old patient, female, who together with her responsible, whose main complaint was the aesthetics of the upper front teeth due to disproportionatesize and altered staining. During the clinical and radiographic examinations, ananomaly called teeth merger, which is the union of two adjacent teeth, was diagnosed in the upper anterior region. The treatment included the sectioning of the two teeth and extraction of the super numerary, followed by restorative treatment of the right central incisor that had caries lesions. It was concluded that, through proper diagnosis and planning, the proposed treatment was effective in the rehabilitation of function and aesthetics, as well as in the prevention of clinical and orthodontic complications that could result from the dental anomaly described, ifuntreated. The child’s self-esteem was re-established.

ABSTRACT Objective This in situ/ex vivo study investigated the effect of CO2 laser irradiation and acidulated phosphate fluoride gel (APF) application, separately and in combination, on enamel resistance to erosion. Material and Methods During 2 experimental 5-day crossover phases, 8 volunteers wore intraoral appliances containing bovine enamel blocks which were submitted to four groups: 1st phase - control, untreated and CO2 laser irradiation, 2nd phase - fluoride application and fluoride application before CO2 laser irradiation. Laser irradiation was performed at 10.6 µm wavelength, 5 µs pulse duration and 50 Hz frequency, with average power input and output of 2.3 W and 2.0 W, respectively (28.6 J/cm2). APF gel (1.23%F, pH 3.5) was applied on enamel surface with a microbrush and left on for 4 minutes. Then, the enamel blocks were fixed at the intraoral appliance level. The erosion was performed extraorally 4 times daily for 5 min in 150 mL of cola drink. Enamel loss was measured profilometrically after treatment and after the in situ phase. The data were tested using one-way Repeated Measures Anova and Tukey's test (p<0.05). Results CO2 laser alone (2.00±0.39 µm) did not show any significantly preventive effect against enamel erosion when compared with the control group (2.41±1.20 µm). Fluoride treated enamel, associated (1.50±0.30 µm) or not (1.47±0.63 µm) with laser irradiation, significantly differed from the control. Conclusion The APF application decreased enamel wear; however, CO2 laser irradiation did not enhance fluoride ability to reduce enamel wear.

The aim of this in vitro study was to evaluate the effects of dentalbleaching with ozone (O3) on color change and enamelmicrohardness. Enamel blocks (3 x 3 x 3mm) were randomlydistributed for treatments (n=10). Color change (ΔE) and Knoopmicrohardness of the enamel blocks were evaluated before andafter the following treatments: C – deionized water (control); HP– 37.5% hydrogen peroxide (Pola Office+/ SDI); PLA – placebogel; O3– ozone; and O2– oxygen. Four 8-minute applicationswere used for HP and PLA, and one 19-minute application for O3and O2.One-way ANOVA revealed that ΔE was not significantlyinfluenced by the treatment (p = 0.112). For the treatments withHP, PLA, O3 and O2, ΔE was greater than 3.3. The paired t testshowed significant decrease in microhardness after treatments (p< 0.001) but no significant difference between treatments(ANOVA; p = 0.313). Dental bleaching treatments with O3, HP,O2and PLA induced enamel color changes that may be clinicallydiscernible, although enamel microhardeness decreased.


O objetivo deste estudo in vitro foi avaliar os efeitos doclareamento dental com ozônio (O3) quanto à alteração de core microdureza do esmalte. Blocos de esmalte (3 x 3 x 3mm)foram aleatoriamente distribuídos entre os tratamentos(n=10). Alteração de cor (ΔE) e microdureza Knoop foramavaliados antes e após cada um dos seguintes tratamentos: C– água deionizada (controle); PH – peróxido de hidrogênio a37,5% (Pola Office+/ SDI); PLA – gel placebo; O3– ozônio;O2– oxigênio. Quatro aplicações de PH e PLA foramrealizadas por 8 minutos cada e uma aplicação de O3e O2foram realizados por 19 minutos em cada bloco de esmalte.ANOVA a um critério mostrou que os valores de ΔE não foramsignificativamente influenciados pelo tratamento (p = 0,112).Para os tratamentos com PH, PLA, O3 e O2, o ΔE foi maior que3,3. O teste t pareado mostrou diminução significativa dosvalores de microdureza no final do tratamento quandocomparado com o tempo baseline (p < 0,001), mas não houvediferença significativa entre os tratamentos (ANOVA; p =0,313). O tratamento com O3, PH, O2e PLA levou a alteraçãode cor do esmalte clinicamente perceptível, embora tenha sidoobservada diminuição da microdureza do esmalte com arealização dos tratamentos.

Abstract The aim of this study was to investigate condyle concentricity and morphology, and their association with Class I and II malocclusions (Angle). The sample consisted of 49 individuals of both genders, between 11 and 35 years old, divided into two groups, G1: 26 patients with Class I malocclusion, and G2: 23 patients with Class II malocclusion, selected for orthodontic treatment. Evaluation of the condyle morphology and position was performed by the same previously calibrated examiner using cone-beam computed tomography (CBCT) images of the subjects. The CBCT scans were analyzed by means of a 3D program (Dolphin 11.5, Dolphin Imaging & Management Solutions, Chatsworth, CA, USA), with a 25% level of sensitivity. The images obtained from the coronal slices were employed for the condyle morphology analysis, which classified the condyle form as rounded, as flat or convex, and as triangular or angled. The sagittal slices were used to classify further the condyles as concentric and displaced anteriorly or posteriorly. A clinical examination was also performed, including TMJ and muscle palpation. The kappa test was used to evaluate investigator calibration; the Chi-square and paired t-tests were used for analysis. The convex and anteriorly positioned condyles were found most frequently, regardless of the type of malocclusion. No association was observed between the groups regarding condylar characteristics.

Introduction Raman spectroscopy may become a tool for the analysis of glucose and triglycerides in human serum in real time. This study aimed to detect spectral differences in lipid and glucose components of human serum, thus evaluating the feasibility of Raman spectroscopy for diagnostic purposes. Methods A total of 44 samples of blood serum were collected from volunteers and submitted for clinical blood biochemical analysis. The concentrations of glucose, cholesterol, triglycerides, and low-density and high-density lipoproteins (LDL and HDL) were obtained using standard biochemical assays. Serum samples were placed in Eppendorf tubes (200 µL), kept cooled (5 °C) and analyzed with near-infrared Raman spectroscopy (830 nm, 250 mW, 50 s accumulation). The mean spectra of serum with normal or altered concentrations of each parameter were compared to determine which Raman bands were related to the differences between these two groups. Results Differences in peak intensities of altered sera compared to normal ones depended on the parameter under analysis: for glucose, peaks were related to glucose; for lipid compounds the main changes occurred in the peaks related to cholesterol, lipids (mainly triolein) and proteins. Principal Components Analysis discriminated altered glucose, cholesterol and triglycerides from the normal serum based on the differences in the concentration of these compounds. Conclusion Differences in the peak intensities of selected Raman bands could be seen in normal and altered blood serum samples, and may be employed as a means of diagnosis in clinical analysis.

OBJECTIVE: The aim of the present study was to compare dentoalveolar changes in mandibular arch, regarding transversal measures and buccal bone thickness, in patients undergoing the initial phase of orthodontic treatment with self-ligating or conventional bracket systems. METHODS: A sample of 25 patients requiring orthodontic treatment was assessed based on the bracket type. Group 1 comprised 13 patients bonded with 0.022-in self-ligating brackets (SLB). Group 2 included 12 patients bonded with 0.022-in conventional brackets (CLB). Cone-beam computed tomography (CBCT) scans and a 3D program (Dolphin) assessed changes in transversal width of buccal bone (TWBB) and buccal bone thickness (BBT) before (T1) and 7 months after treatment onset (T2). Measurements on dental casts were performed using a digital caliper. Differences between and within groups were analyzed by Student's t-test; Pearson correlation coefficient was also calculated. RESULTS: Significant mandibular expansion was observed for both groups; however, no significant differences were found between groups. There was significant decrease in mandibular buccal bone thickness and transversal width of buccal bone in both groups. There was no significant correlation between buccal bone thickness and dental arch expansion. CONCLUSIONS: There were no significant differences between self-ligating brackets and conventional brackets systems regarding mandibular arch expansion and changes in buccal bone thickness or transversal width of buccal bone. .


OBJETIVO: o objetivo do presente estudo foi comparar as alterações dentoalveolares transversais e a espessura óssea da arcada inferior em pacientes submetidos ao tratamento ortodôntico utilizando sistemas de braquetes autoligáveis ou convencionais. MÉTODOS: uma amostra de 25 pacientes requerendo tratamento ortodôntico foi recrutada com base no tipo de braquete. No Grupo 1, 13 pacientes foram tratados com braquetes autoligáveis (SLB, slot 0,022"); o Grupo 2 incluiu 12 pacientes, nos quais foram colados braquetes convencionais (CLB, slot 0,022"). Utilizou-se tomografia computadorizada de feixe cônico e um programa 3D (Dolphin) para avaliar as alterações pré-tratamento (T1) e 7 meses após o início desse (T2). As medições em modelos de gesso foram realizadas com o auxílio de um paquímetro digital. As diferenças intergrupos, bem como intragrupo, foram analisadas por meio de teste t de Student. Além disso, o coeficiente de correlação de Pearson foi utilizado. RESULTADOS: alterações dentoalveolares significativas foram observadas em ambos os grupos. Entretanto, não houve diferenças significativas entre os grupos. Houve uma diminuição da espessura óssea na região posterior e das medidas transversais em ambos os grupos. Não houve uma correlação significativa entre a espessura óssea posterior e a expansão da arcada dentária, em nenhum dos dois sistemas de braquetes utilizados. CONCLUSÕES: comparando-se o uso dos aparelhos autoligáveis e convencionais, concluiu-se que não houve diferenças dentoalveolares significativas quanto à expansão da arcada inferior e quanto à espessura óssea posterior. .

This cross-sectional study aimed at assessing the cephalometric characteristics in the elderly, taking into account differences between genders, age ranges, ethnic groups and dental aspects. The sample consisted of 250 elderly subjects of both genders (163 female, mean age: 68 yr.; 87 male: 70.4 yr.). Conventional lateral cephalograms for cephalometric analysis were scanned and analyzed by Dolphin Imaging software 11.5. The statistical treatment of the data evaluated the influence of gender and age range variables (independent ttest), as well as ethnic group, facial profile, and dental characteristics (one-way ANOVA), on cephalometric measurements. A confidence interval of 95% and level of significance of 5% were considered for all the tests performed. The results revealed: 1) The cephalometric measurements evaluated showed significantly lower values for the female gender; 2) a significant decrease in the cephalometric values was observed in relation to the growth pattern, with the advancement of age; 3) significant cephalometric differences were observed between the ethnic groups and the facial profiles; 3) dentulous patients had greater absolute values for all the components evaluated, followed by the individuals with partial dental losses and by those who were edentulous. It may be concluded that the cephalometric alterations observed in this study are inherent to facial maturity, and that they represent specific characteristics regarding each of the variables evaluated. These modifications must be taken into account when planning the treatment for younger patients, to minimize the modifications arising from the natural aging process.
