ABSTRACT

Objective: To determine the outcome of patients receiving radioactive iodine therapy for toxic nodular goiter coming to Aga Khan University Hospital Karachi


Methods: A total of 89 patients who visited the outpatient department of Aga Khan University Hospital from January 2010 to August 2017 were recruited for the study. Toxic nodular goiter was diagnosed on the basis of having hot nodule on thyroid scan with low TSH and high FT4/T4. Other demographic and laboratory data were also recorded


Results: Eighty nine patients with toxic nodular goiter received a dose range from 10 to 30mCi RAI. Six months after RAI, 36.2 percent became hypothyroid, 38.5 percent became euthyroid while 25.3 percent remained hyperthyroid. Thyroid outcome at 3 months were correlating with 6 months results


Conclusion: Radioactive iodine therapy is a safe and effective way of treating toxic nodular goiter which usually results in cure of hyperthyroidism in majority of patients

ABSTRACT

Objective: to study the clinical behavior of dengue infection in cancer patients


Material and Methods: we reviewed medical records of cancer patients who were diagnosed with dengue infection in year 2011 as per discharge notes. Patients fulfilling revised dengue WHO/TDR classification with positive dengue lgM serologist were finally chosen for analysis


Results: from initially screened 63 patients, 43 fulfilled revised dengue WHO/TDR classification criteria, 31 [of these 43] with positive dengue lgM were finally analyzed. There were 16 males and 15 females, mean age was 39.0 [23.0] years. 23 patients were = 18 years of age. 81 % patients reported within first three days of illness. Major cancer bulk was from solid organ cancer group [n=21] followed by hematological group [n=10]. Presenting features were fever [100%] followed by aches [58.1 %], hemorrhagic manifestations [35.5%], vomiting [29%] and diarrhoea [25.8%]. Twelve [38. 7%] patients developed severe dengue with one death making 3.2% crude mortality rate


Conclusion: the spectrum of dengue infection severity in cancer patients seems to be different from general population. Clinically dengue was more severe with solid organ cancers as compared to hematologic cancers possibly highlighting the role of cellular mediated immunity. Other risk factors identified were relatively elder age and more co-morbid conditions

ABSTRACT

Abstract: The resting ankle- brachial index [ABI] is a non-invasive method to assess the patency of lower extremity arterial system and to screen for the presence of peripheral occlusive arterial disease. Diagnostic efficacy of ABI to predict atherosclerosis is well documented and reduction in index [< 0.9] is indicative of generalized atherosclerosis. Present study was planned to evaluate the role of ABI as a possible non- invasive marker for coronary atherosclerosis


Objective: To evaluate the relationship between ABI and the extent of coronary artery disease estimated by angiography. Study design: Cross Sectional Study. Settings: Departments of Medicine and Cardiology, Mayo Hospital, Lahore. Duration: Six months, from July 2006 to January 2007


Subjects: One hundred and fifty patients referred for coronary angiography were selected. ABI was calculated on each patient before undergoing angiography. A single cardiologist using a computer based quantitative coronary angiography made angiography interpretations


Results: The mean age of patients was 51.23+/-10.23 years. There were 147 [98%] patients with normal ABI [>0.9], 2[1.3%] with mild reduction in ABI [0.7- 0.89], and 1[0.7%] with moderate ABI [0.4- 0.69]. In patients with normal ABI 34 [22.7%] patients had normal coronary angiogram, 43 [28.7%] patients had single vessel disease, 33 [22%] patients had two vessels disease, 35 [23.3%] patients had three vessels disease, and 2 [1.3%] patients had four vessel disease. In patients with mild ABI [0.7- 0.89] there was 1[0.7%] patient who had single vessel disease, and 1 [0.7%] patient with four vessel disease. In the moderate ABI category [0.4 - 0.69] there was 1 [0.7%] patient with two vessel disease


Conclusion: Ninety eight percent of the studied population with otherwise symptomatic coronary artery disease had normal ABI. On the other hand 77% of patients with normal ABI had abnormalities on coronary angiogram. ABI lacks the sensitivity to screen atheromatous CAD and cannot be recommended as such
