ABSTRACT

Objective To evaluate regional airway obstruction with balloon catheter in stabilizing the vital signs in experimental animals suffered from acute massive pulmonary embolism. Methods Pulmonary embolism of right lung artery by using auto-blood clots or detachable latex balloons was established in 27 healthy sheep. When the blood oxygen saturation decreased by 25% compared to that before the procedure, the placement of balloon catheter in corresponding right main bronchus was carried out in 18 sheep (study group). Five sheep were used as control group. The blood oxygen saturation in the remaining four sheep did not reach the intervention level. The pulmonary arterial pressure, the peripheral arterial pressure, the central venous pressure, the heart rate, the blood oxygen saturation, the arterial partial pressure of oxygen as well as of carbon dioxide were invasively determined. Results The experimental model of acute massive pulmonary embolism was successfully established in 23 sheep. After the establishment of pulmonary embolism, increased heart rate, tachypnea, a decrease of ≥ 25% in blood oxygen saturation within 30 minutes and a rise in pulmonary arterial pressure were detected. After the placement of balloon catheter in study group, a reduction of the pulmonary arterial pressure and an elevation of the blood oxygen saturation as well as the arterial partial pressure of oxygen rose. were observed. The difference between study group and control group was statistically significant (P < 0.05). Conclusion The result of this animal experiment indicates that obstruction of airway with balloon can be served as a first aid for acute massive pulmonary embolism, which can stabilizes the animal's vital signs and, therefore, can help gain precious time for the follow-up thrombolysis treatment.

ABSTRACT

OBJECTIVETo observe the developing changes of adventitia in restenosis after precutaneous transluminal angioplasty(PTA), and investigate the effect of androgen on restenosis through contrasting the castrated male rat models and its mechanism.

METHODSModels were constructed of castrated male rats and restenosis of the common carotid artery, and specimens were collected at the 3rd, 7th, 14th and 28th day respectively after modeling. Hematoxylin and eosin staining, immunohistochemical staining, and electronic microscopy were performed to observe the condition of restenosis.

RESULTSProliferating cells occurred in adventitia first and phenotype of adventitial cells was changed at the 3rd day after PTA. The adventitial proliferating index was the highest at the 7th day after PTA, and proliferating migration towards intimal was observed. The proliferating cells mostly occurred in the middle layer and neointima at the 14th day after PTA. The areas of adventitia and neointima were larger and the degrees of restenosis were higher in the castrated rats than in the non-castrated ones at different time points. Take the 14 d group, the adventitial area was[(3,566 +/- 337) micron2 vs (2,751 +/- 401) micron2, P = 0.008], the neointimal area[(3,553 +/- 477) micron2 vs (2,757 +/- 435) micron2, P = 0.025], the restenosis rate[(76 +/- 2)% vs (60 +/- 8)%, P = 0.005], and the proliferating index [(29 +/- 2)% vs (13 +/- 1)%, P < 0.001].

CONCLUSIONAdventitial proliferation and migration contribute to restenosis after PTA; Androgen in rats can physiologically relieve restenosis, probably through intervening in the activation of adventitia.

ABSTRACT

OBJECTIVETo evaluate the clinical efficacy of interventional therapy in the treatment of hepatocellular carcinoma (HCC).

METHODSThirty-three patients with HCC were synthetically treated by systematic measures. Among them, percutaneous port-catheter system (PCS) implantation via the femoral artery was performed in 21 cases. Patients with haemorrhage were treated with prothrombin complex concentrate and fibroraas, and patients with hepatic encephalopathy by branched-chain amino acids and arginine.

RESULTSAll indwelling catheters of PCS were patent and no catheter tip dislocations were observed. Of 33 patients with advanced HCC, the mean survival rate was 20.1 months and 12 (36%) patients survived more than 2 years.

CONCLUSIONPatients with advanced HCC were treated by synthetic measures. Survival was prolonged, quality of life was improved significantly, and the effectiveness of interventional therapy was further improved.

ABSTRACT

Objective To study the effect of intra arterial thrombolysis in combination with basic fibroblast growth factor (bFGF), and the protecting mechanism of the bFGF. Methods 50 male Wistar rats were subjected to middle cerebral artery occlusion (MACO) for 4 hours by a single fibrin rich clot. The rats were randomly assigned to the following two expremental groups: 1) treatment with urokinase alone; 2) treatment with urokinase and bFGF. The neurological deficits and the volume of lesions on MRI T2WI were observed at different periods after therapy. The expression of bax were analyzed by immunuohistochemical method at each time point. Results Bax expression increased at 3 hours after reperfusion, and reached the peak level at 12h reperfusion. At 6h after therapy, expression of bax ( P

ABSTRACT

Purpose:To evaluate the feasibility of balloon catheter dilatation and internal stent placement in the treatment of Takayasu's arterial stenosis.Materials and Methods:Three patients with stenosis of common carotid arteries caused by Takayasu' s disease were treated with PTA by balloon catheter and intravaseular stent placement, the lenghs of stenosis were all beyond 8cm.Balloon angioplasty was performed in one patient, the others were treated with Wallstent stents after PTA.Result:Immediately after treatment, angiography and ultrasound showed that the proportion of stenosis was zero and all appeared to achieve in good result. The artery treated with PTA was completely occluded after 1 year follow-up;the other two patients with stents placement were also examined by angiography and ultrasound at 4,5 months and 4 months respectively after the praeedure. For the patient with left carotid artery stenosis, the proximal part of the segment with the intravascular stent showed a restenosis;the other patient was normal. Conclusion:We considered that intravascular stent placement be might to the treatment for Takayacu' s arteritis with long segment stenosis of common carotid artery.

ABSTRACT

Purpose:To understand the effect of ligustrazine on preventing the thrombosis "after percutaneous transluminal angioplasty(PTA).Materials and Methods:Dogs models(n=10)with exper- imental renal arterial stenosis were selected and randomly divided into control and experimental groups with 5 dogs for each. Experimental group dogs were injected ligustrazine(40mg/kg)15 days before PTRA.Peripheral blood samples were collected before recelived drug, before and 4 hours after PTRA in the experiment group;before and 4 hours after PTRA in the control group.Plasma concentrations of thromboxane B2_(TXB_2)and 6-keto-PGIl? were determined by radioimmunoassay.Rates of platelet ag- gregation were also determined by ADP-induced.All dogs were killed 4 hours after PTRA.Renal arteries were removed before killing. The thrombus volume at PTRA sites were measured. Results:Liguslrazine might significantly inhibit platelet aggregation, decrease TXA_2 produced by platelet and increase PGI_2 synthesized in the blood vessel. Therefore, ligustrazine might significantly inhibit thrombosis at the PTRA sites. It Suggests that Ligustrazine could be an effective drug for preventing restenosis of thromboses after PTA.
