ABSTRACT

OBJECTIVE@#To investigate the fetal and maternal outcomes, risk factors of disease progression and adverse pregnancy outcomes (APOs) in patients with undifferentiated connective tissue disease (UCTD).@*METHODS@#This retrospective study described the outcomes of 106 pregnancies in patients with UCTD. The patients were divided into APOs group (n=53) and non-APOs group (n=53). The APOs were defined as miscarriage, premature birth, pre-eclampsia, premature rupture of membranes (PROM), intrauterine growth restriction (IUGR), postpartum hemorrhage (PPH), and stillbirth, small for gestational age infant (SGA), low birth weight infant (LBW) and birth defects. The differences in clinical manifestations, laboratory data and pregnancy outcomes between the two groups were compared. Logistic regression analysis was performed to analyze the risk factors for APOs and the progression of UCTD to definitive CTD.@*RESULTS@#There were 99 (93.39%) live births, 4 (3.77%) stillbirths and 3 (2.83%) miscarriage, 20 (18.86%) preterm delivery, 6 (5.66%) SGA, 17 (16.03%) LBW, 11 (10.37%) pre-eclampsia, 7 (6.60%) cases IUGR, 19 (17.92%) cases PROM, 10 (9.43%) cases PPH. Compared with the patients without APOs, the patients with APOs had a higher positive rate of anti-SSA antibodies (73.58% vs. 54.71%, P=0.036), higher rate of leukopenia (15.09% vs. 3.77%, P=0.046), lower haemoglobin level [109.00 (99.50, 118.00) g/L vs. 124.00 (111.50, 132.00) g/L, P < 0.001].Multivariate Logistic regression analysis showed that leucopenia (OR=0.82, 95%CI: 0.688-0.994) was an independent risk factors for APOs in UCTD (P=0.042). Within a mean follow-up time of 5.00 (3.00, 7.00) years, the rate of disease progression to a definite CTD was 14.15%, including 8 (7.54%) Sjögren's syndrome, 4 (3.77%) systemic lupus erythematosus (SLE), 4 (3.77%) rheumatoid arthritis and 1 (0.94%) mixed connective tissue disease. Multivariate Cox proportional risk regression analysis showed that Raynaud phenomenon (HR=40.157, 95%CI: 3.172-508.326) was an independent risk factor for progression to SLE.@*CONCLUSION@#Leukopenia is an independent risk factor for the development of APOs in patients with UCTD. Raynaud's phenmon is a risk factor for the progression of SLE. Tight disease monitoring and regular follow-up are the key measures to prevent adverse pregnancy outcomes and predict disease progression in UCTD patients with pregnancy.

ABSTRACT

【Objective:】 To explore the relationship between the moral courage of nurses in Intensive Care Unit (ICU) and the hospital ethical climate, and to provide a theoretical basis for enhancing the moral courage of ICU nurses. 【Methods:】 A total of 468 ICU nurses from 8 tertiary A hospitals in Shaanxi Province were selected as the subjects by convenience sampling method, and the General Information Form, Hospital Ethical Climate Survey, and Nurses’ Moral Courage Scale were used for the questionnaire survey. 【Results:】 A total of 468 questionnaires were distributed and 463 valid questionnaires were returned, with an effective recovery rate of 98.93%. The score of hospital ethical climate for ICU nurses was (100.69±19.76), and the score of moral courage was (83.67±15.48). There was a positive correlation between hospital ethical climate and moral courage (r=0.866, P<0.001). Hierarchical multiple regression analysis showed that hospital ethical climate entered the influencing factor model of ICU nurses’ moral courage, which can independently explain 57% of the variation in ICU nurses’ moral courage. 【Conclusion:】 The moral courage of ICU nurses was at a medium level, and positively correlated with the hospital ethical climate, that is, the better the hospital ethical climate perceived by ICU nurses, the higher their level of moral courage. Nursing managers should focus on deepening the hospital ethical climate, and enhance the level of hospital ethical climate through scientific methods to promote the improvement of ICU nurses’ moral courage.

ABSTRACT

Objective To investigate the bacteria distribution and drug resistance of lower respairory tract infection of re‐spairtory wards .Methods Sputum from patients with low respiratory tract infection was collected ,K‐B methods and minimun in‐hibitory concentration were used to make distribution and antibiotic resisitance .Results Five hundred and twenty‐nine strains were isolated ,of which gram negative organisms accounted for 416(78 .8% ) ,and gram positive organisms accounted for 88(16 .6% ) ,and fungi accounted for 25(4 .7% ) .In whtich ,the pseudomoas aeruginosa(25 .0% )and acinetobacter baumannii(13 .0% )accounted for the first and second one .Pseudomoas aeruginosa showed varying degrees of resistance to most commomly used antibacterials ,but there were still some alternatives .Acinetobacter baumannii had higher resistance to most commomly used antibacterial ,except for cefoperazone sulbactam .Cefoperazone sulbactam showed high susceptibility to most gram negative organisms and gram positive or‐ganisms .Conclusion In the respiratory wards ,gram negative organisms were predominant in low respiratory tract infection .Pseud‐omoas aeruginosa and acinetobacter baumannii play a main role .The later is resistant to most commonly used antibacterials and the former showes high resisitance except for a few antibacterials .

ABSTRACT

Objective To investigate the iodine nutritional level of 5 special groups (newly married women,pregnant women, lactating women, babies and infants, and students) in Ningxia, and to provide the basis for formulating prevention and control strategies. Methods Clustering and random sampling method were used. In 2008 and 2009, in the 22 counties investigated, in every county with 9 townships or more, nine townships were randomly selected according to their sub-area positions of east, west, south, north and center; four villages were randomly selected in each chosen township, four people with special needs and 2 infants were randomly selected for urine samples collection in each chosen village. In every county with 9 or less townships, one township was randomly selected respectively in east, west, south, north and center sub-areas; four villages were randomly sampled in each chosen township, eight people with special needs and 3 infants' urine samples were randomly collected in each chosen village. In the 22 counties, one township was randomly selected respectively in east, west,south, north and center sub-areas, one village elementary school was randomly sampled in each chosen township,twenty students aged 8 - 10 were randomly selected to collect their urine samples in each school. The iodine concentration was determined by arsenic-cerium contact method. Results A total of 6894 copied of urine samples from newly married women, pregnant women, lactating women, babies and infants, and students were examined, the urinary iodine medians were 209.3, 187.4, 184.0, 216.5, 216.3 μg/L, respectively. From low to high in the order was lactating women, pregnant women, newly married women, students, babies and infants. The level of urinary iodine of babies and infants, pregnant women and lactating women were appropriate, the one of newly married women and students were higher than appropriate. The proportion of less than 100 μg/L of urinary iodine of the 5 kinds special groups were 11.1% (53/475), 35.4% (308/871), 35.4% (659/1863), 19.1% (283/1483), 8.4%(185/2202), respectively, while the urinary iodine of the pregnant women and lactating women were relatively high. The urinary iodine medians of the 5 special groups were also very different among counties. Conclusions The urinary iodine of the 5 special groups in Ningxia presents obvious differences between populations and regions.Current iodized salt is sufficient to ensure iodine nutrition needs for the 5 special groups. But married women and students have higher levels of iodine nutrition, indicating that the salt iodine concentration of Ningxia residents have cut space, full consideration of the 5 special groups and regional differences should be taken.

ABSTRACT

Objective To determine urinary iodine level among babies and infants aged 0-30 months in Ningxia, and to provide scientific evidence for strengthening iodine supplement or decreasing salt iodine level in special population. Methods One thousand four hundred and eighty-three babies and infants were selected randomly from 664 administrative villages of 22 counties (city or district) by two-stage sampling method. Urinary iodine was tested with the arsenic cerium catalysis spectrophotometric method and related influencing factors were analyzed. Results Urinary iodine median was 216.5 μg/L of the whole autonomous region, and the value that was lower than 100 μg/L, accounted for 19.1%(283/1483), 100 - 300 μg/L accounted for 49.3%(731/1483), higher than 300 μg/L, accounted for 31.6% (469/1483). Urinary iodine median was in 130.6 - 328.4 μg/L of all counties, which was higher than 100 μg/L. The urinary iodine median of men(223.2 μg/L) was slightly higher than that(210.2 μg/L) of female, no significant difference was observed(Z = - 1.76, P > 0.05). Urinary iodine level changed little when child was younger than one year old(Z = - 0.624, P > 0.05). Then the value dropped gradually after one year old(χ2 = 13.59, P < 0.05), decreased with age by month, and the proportion of the value smaller than 100 μg/L was increased gradually. Urinary iodine level(257.5 μg/L) of child whose mother had taken iodine oil pills was higher than that (221.2 μg/L) of child whose mother had significant difference was observed(Z = - 2.54, P < 0.05). The urinary iodine level (239.1 μg/L) of child who received breast feeding was higher than that (204.2 μg/L) of child without breast feeding among one year old and younger infants and babies, significant difference was observed (Z = - 2.74, P < 0.05). Conclusions Current iodine level in iodized salt is probably higher than suitable in Ningxia, and the value could be decreased. It is unnecessary to strengthen iodine supplement procedure in special population other than people in Xiji county.

ABSTRACT

<p><b>OBJECTIVE</b>To carry out the citation analysis of publications in Chinese Journal of Preventive Medicine (CJPM) among the preventive medicine authors and analyze the impact of this magazine in preventive medicine domain.</p><p><b>METHODS</b>Using Chinese scientific periodical literature evaluation and statistical analysis system (V1.0), the citation status of all CJPM publications in 2000-2005 was analyzed, the analysis covered 21 columns, including the review, editorial and original article, the data were collected up to November, 2007.</p><p><b>RESULTS</b>From 2000 to 2005, CJPM had more than 30 columns and carried 1196 articles and 92. 89% (1111/1196) articles were from 21 main columns. During 2003 to 2005, the impact factors of CJPM were 0. 897, 1.011 and 0. 891 respectively. Among 21 main columns, the citation frequency of six columns including original article, editorial, review, courses, discussion and case report were higher than 80%. In five columns (original article, editorial, report, review and academic trends), the average citation frequency of individual articles was more than 4 times. The citation frequency of 20 authors was higher than 20 times and these authors were from medical schools, teaching hospitals, centers of diseases control and the research institutes. The individual citation frequency of 17 articles was more than 20 times and the individual citation frequency of three articles was more than 50 times. 34.9% of the citations of the 2000-2005 CJPM articles were from the top 20 journals, and the self-citation rate was 4. 85%.</p><p><b>CONCLUSION</b>The publications in Chinese Journal of Preventive Medicine are most frequently cited, which indicated that those publications have high quality, this journal has a great effect in preventive medicine field of China.</p>

ABSTRACT

<p><b>BACKGROUND</b>Somafostatin receptors (SSTRs) have been suggested to involve in mediating the effect of somatostatin on hepatic stellate cells (HSCs) in an activation-dependent way. We, therefore, try to investigate the relationship between expression of SSTRs and activation of rat HSCs.</p><p><b>METHODS</b>HSCs were isolated from rats by in situ perfusion and single-step density gradient centrifugation. SSTR1-5 mRNA levels in the differentiated first passage HSCs were detected by means of a reverse transcription polymerase chain reaction. On the other hand, hepatic fibrosis was induced in adult male Sprague-Dawley rats by carbon tetrachloride intoxication, and the expression of SSTR1-5 in normal as well as fibrotic livers was measured by immunohistochemical staining.</p><p><b>RESULTS</b>SSTR mRNA and SSTR could not be found in freshly isolated rat HSCs or normal rat liver. However, SSTR1-3 mRNA appeared as HSCs became wholly activated, and could also be identified on the membrane of activated HSCs in the perisinusoid space, fibrous septa, etc.</p><p><b>CONCLUSION</b>The expression of SSTR1-3 in the rat HSC is closely related to its activation. This may reflect one of the main negative regulation mechanisms in the course of HSC activation.</p>
