ABSTRACT

Both anti-glomerular basement membrane (GBM) disease and the anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) are common causes of pulmonary-renal syndrome. Organizing pneumonia (OP), a special pattern of interstitial lung disease, is extremely rare either in AAV or anti-GBM disease. We report an old woman presented with OP on a background of co-presentation with both ANCA and anti-GBM antibodies.

ABSTRACT

Objectives@#This paper aimed to investigate the prevalence of diabetes mellitus (DM) and explore the associated risk factors in a very elderly southwest Chinese population.@*Methods@#From September 2015 to June 2016, a cross-sectional survey was conducted to obtain a representative sample of 1,326 participants over 80 years old living in Chengdu. The presence of DM was based on fasting plasma glucose (FPG) and 2-h plasma glucose (2-hPG) levels during an oral glucose tolerance test (OGTT). A logistic regression model was used to calculate the odds ratios ( s) and 95% confidence intervals ( s) of the potential associated factors.@*Results@#The participants' mean age was 83.5 ± 3.1 years. The overall prevalence of DM was 27.4%. The prevalence was higher in males (30.2%) than females (24.7%) ( = 0.02). The prevalence of DM increased with body mass index (BMI) and decreased with aging. The multivariate analysis suggested that male sex ( = 1.433; 95% , 1.116-1.843), hypertension ( = 1.439; 95% , 1.079-1.936), overweight or obesity ( = 1.371; 95% , 1.023-1.834), high heart rate (≥ 75 beats/min; = 1.362; 95% , 1.063-1.746), and abdominal obesity ( = 1.615; 95% , 1.216-2.149) were all significantly positively correlated with DM. However, age was negatively correlated with DM ( = 0.952; 95% , 0.916-0.989).@*Conclusions@#The prevalence of DM and newly diagnosed DM in a very elderly southwest Chinese population was high. OGTT screening should be performed regularly in people aged ≥ 80 years to ensure timely diagnosis of DM.

ABSTRACT

OBJECTIVE@#To determine the environmental contamination degree of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in corona virus disease 2019 (COVID-19) wards, to offer gui-dance for the infection control and to improve safety practices for medical staff, by sampling and detecting SARS-CoV-2 nucleic acid from the air of hospital wards, the high-frequency contact surfaces in the contaminated area and the surfaces of medical staff's protective equipment in a COVID-19 designated hospital in Wuhan, China.@*METHODS@#From March 11 to March 19, 2020, we collected air samples from the clean area, the buffer room and the contaminated area respectively in the COVID-19 wards using a portable bioaerosol concentrator WA-15. And sterile premoistened swabs were used to sample the high-frequency contacted surfaces in the contaminated area and the surfaces of medical staff's protective equipment including outermost gloves, tracheotomy operator's positive pressure respiratory protective hood and isolation clothing. The SARS-CoV-2 nucleic acid of the samples were detected by real-time fluorescence quantitative PCR. During the isolation medical observation period, those medical staff who worked in the COVID-19 wards were detected for SARS-CoV-2 nucleic acid with oropharyngeal swabs, IgM and IgG antibody in the sera, and chest CT scans to confirm the infection status of COVID-19.@*RESULTS@#No SARS-CoV-2 nucleic acid was detected in the tested samples, including the 90 air samples from the COVID-19 wards including clean area, buffer room and contaminated area, the 38 high-frequency contact surfaces samples of the contaminated area and 16 surface samples of medical staff's protective equipment including outermost gloves and isolation clothing. Moreover, detection of SARS-CoV-2 nucleic acid by oropharyngeal swabs and IgM, IgG antibodies in the sera of all the health-care workers who participated in the treatment for COVID-19 were all negative. Besides, no chest CT scan images of medical staff exhibited COVID-19 lung presentations.@*CONCLUSION@#Good ventilation conditions, strict disinfection of environmental facilities in hospital wards, guidance for correct habits in patients, and strict hand hygiene during medical staff are important to reduce the formation of viral aerosols, cut down the aerosol load, and avoid cross-infection in isolation wards. In the face of infectious diseases that were not fully mastered but ma-naged as class A, it is safe for medical personnel to be equipped at a high level.

ABSTRACT

Fibrosis is a pathological feature of most chronic inflammatory diseases, which is pathologically characterized by activation of fibroblasts and deposition of extracellular matrix components. Fibrosis affects nearly every tissue in the body. If highly progressive, the fibrotic process eventually leads to organ malfunction and even death. Due to the complexity of the pathological mechanism of fibrosis, there is still a lack of effective therapeutic drugs. Salviae Miltiorrhizae Radix et Rhizoma, dry roots and rhizomes of Salvia miltiorrhiza, is a kind of traditional Chinese medicines for activating blood and removing stasis. Modern pharmacological studies have shown that the tanshinones and salvianolic acids in Salviae Miltiorrhizae Radix et Rhizoma have a wide range of anti-fibrosis effects, such as inhibiting the production of collagen fibers, promoting the degradation of fibrin, and inhibiting cell proliferation. In this review, pharmacological actions and mechanisms of active ingredients of Salviae Miltiorrhizae Radix et Rhizoma were summarized to provide references for the further development and utilization of Salviae Miltiorrhizae Radix et Rhizoma.

ABSTRACT

Hypophosphatemia is a common metabolism disease in humans. Fibroblast growth factor 23 (FGF23) inhibits phosphate reabsorption by targeting on the renal tubules. FGF23C-tail contains 73 amino acids from C-terminus of FGF23, serves as an inhibitor of FGF23, and can increase phosphate reabsorption. Therefore, FGF23C-tail is an important drug for hypophosphatemia. In this paper, we constructed a fusion protein of FGF23C-tail with HSA, and investigated the expression of the fusion protein in the Pichia pastoris system. The recombinant gene was constructed by fusion PCR. A high-yield strain was selected by G418 resistance and fermentation yield, and the expression yield was 43.7 mg·L-1 in flask. In 5 L fermenters, the highest expression yield could reach 265.6 mg·L-1. FGF23C-tail-HSA could be used as an inhibitor for FGF23, and could significantly increase blood phosphorus levels in rats. The procedures for care and use of animals were approved by the Ethics Committee of YiChun University. This paper provided a basis research for further studying physiological activity of FGF23C-tail-HSA.

ABSTRACT

OBJECTIVE@#Renal fibrosis is the most common manifestation of chronic kidney disease (CKD). Noting that existing treatments of renal fibrosis only slow disease progression but do not cure it, there is an urgent need to identify novel therapies. Hydrogen sulfide (H2S) is a newly discovered endogenous small gas signaling molecule exerting a wide range of biologic actions in our body. This review illustrates recent experimental findings on the mechanisms underlying the therapeutic effects of H2S against renal fibrosis and highlights its potential in future clinical application.@*DATA SOURCES@#Literature was collected from PubMed until February 2019, using the search terms including "Hydrogen sulfide," "Chronic kidney disease," "Renal interstitial fibrosis," "Kidney disease," "Inflammation factor," "Oxidative stress," "Epithelial-to-mesenchymal transition," "H2S donor," "Hypertensive kidney dysfunction," "Myofibroblasts," "Vascular remodeling," "transforming growth factor (TGF)-beta/Smads signaling," and "Sulfate potassium channels."@*STUDY SELECTION@#Literature was mainly derived from English articles or articles that could be obtained with English abstracts. Article type was not limited. References were also identified from the bibliographies of identified articles and the authors' files.@*RESULTS@#The experimental data confirmed that H2S is widely involved in various renal pathologies by suppressing inflammation and oxidative stress, inhibiting the activation of fibrosis-related cells and their cytokine expression, ameliorating vascular remodeling and high blood pressure, stimulating tubular cell regeneration, as well as reducing apoptosis, autophagy, and hypertrophy. Therefore, H2S represents an alternative or additional therapeutic approach for renal fibrosis.@*CONCLUSIONS@#We postulate that H2S may delay the occurrence and progress of renal fibrosis, thus protecting renal function. Further experiments are required to explore the precise role of H2S in renal fibrosis and its application in clinical treatment.

ABSTRACT

The objective of this study was to evaluate parity as a risk factor for autoimmune thyroid disease ( AITD) by comparing features of thyroid autoantibodies of pregnant women with different parities. This prospective cohort study included 477 women of first trimester of pregnancy in Peking University International Hospital from October 2016 to April 2017. Subjects were categorized as never parturition group ( n=287) or previously parturition group(n=190). All pregnant women in this study voluntarily signed informed consents. Thyroid peroxidase antibody (TPOAb), thyroglobulin antibody (TgAb), thyrotropin (TSH), and free T4 were measured in the first trimester of pregnancy( from 7 to 12 weeks) . Meanwhile, height, weight, blood pressure, gestational weeks and pregnancy history were recorded. Data were statistically analyzed. The positive rates of thyroid autoantibodies including TPOAb and TGAb in previously parturition group were higher than those in no parturition group(10.0%vs 5.2%, P<0.05). The previously parturition group tended to be older and had lower TSH levels than the never pregnant group( P<0.05) . TSH 2.5 mIU/L and 4.0 mIU/L were used as upper reference limits respectively. The rate of abnormality of thyroid function was higher in the never parturition group than those in previously parturition ones ( 33. 33 by 29. 5%, 21. 2% by 10. 8%) . In both previously parturition and never parturition groups, the commonest diseases of abnormal thyroidal function were SCH and subclinical thyrotoxicosis, and there was significant difference in disease of abnormal thyroidal function abnormal(χ2=15.704, P<0.05 and χ2=14.560, P<0.05). Parity was associated with AITD after adjustment for age, BMI, and gestational week [OR=2.346(1.075-5.121)]. Parity may affect AITD. Pregnant women should be monitored for thyroid function and thyroid autoantibodies even if they did not suffer from thyroid disease in the first pregnancy.

ABSTRACT

<p><b>BACKGROUND</b>The E-26 transformation-specific related gene (ERG) is frequently expressed in cytogenetically normal acute myeloid leukemia (CN-AML). Herein, we performed a meta-analysis to investigate the relationship between the prognostic significance of ERG expression and CN-AML.</p><p><b>METHODS</b>A systematic review of PubMed database and other search engines were used to identify the studies between January 2005 and November 2016. A total of 667 CN-AML patients were collected from seven published studies. Of the 667 patients underwent intensive chemotherapy, 429 had low expression of ERG and 238 had high expression of ERG. Summary odds ratio (OR) and the 95% confidence interval (CI) for the ERG expression and CN-AML were calculated using fixed- or random-effects models. Heterogeneity was assessed using Chi-squared-based Q- statistic test and I2 statistics. All statistical analyses were performed using R.3.3.1 software packages (R Foundation for Statistical Computing, Vienna, Austria) and RevMan5.3 (Cochrane Collaboration, Copenhagen, Denmark).</p><p><b>RESULTS</b>Overall, patients with high ERG expression had a worse relapse (OR = 2.5127, 95% CI: 1.5177-4.1601, P = 0.0003) and lower complete remission (OR = 0. 3495, 95% CI: 0.2418-0.5051, P< 0.0001). With regard to the known molecular markers, both internal tandem duplications of the fms-related tyrosine kinase 3 gene (OR = 3.8634, 95% CI: 1.8285-8.1626, P = 0.004) and brain and acute leukemia, cytoplasmic (OR = 3.1538, 95% CI: 2.0537-4.8432, P< 0.0001) were associated with the ERG expression. In addition, the results showed a statistical significance between French-American-British (FAB) classification subtype (minimally differentiated AML and AML without maturation, OR = 4.7902, 95% CI: 2.7772-8.2624, P< 0.0001; acute monocytic leukemia, OR = 0.2324, 95% CI: 0.0899-0.6006, P = 0.0026) and ERG expression.</p><p><b>CONCLUSION</b>High ERG expression might be used as a strong adverse prognostic factor in CN-AML.</p>

The metabolism of thyroid hormone changes during pregnancy. Subclinical hypothyroidism during pregnancy may adversely affect the pregnancy outcomes and neuropsychological development of child, controversially. The diagnostic criteria of subclinical hypothyroidism during pregnancy need to be specific. It is important to figure out when should pregnancy with subclinical hypothyroidism be treated and what is the specific goal of the treatment.

Objective@#To analyze the clinical outcomes of repeated superovulation induction in patients with adenomyosis or moderate to severe pelvic endometriosis after failure in previous IVF-ET cycles with the ultra-long protocol.@*METHODS@#We retrospectively analyzed the clinical data about 37 patients with adenomyosis or moderate to severe pelvic endometriosis in our center from 2009 to 2013, who underwent repeated IVF-ET after failure in the previous cycles with the ultra-long protocol, namely by injection of 2-6 ampoules of 3.75 mg gonadotropin-releasing hormone agonist (GnRH-a). All the patients met the following requirements: hCG-negative at 14 days after transfer, within 3-7 days after menstruation, and properly down-regulated serum follicle stimulating hormone (FSH) (<10 mIU/ml), luteinizing hormone (LH) (<10 mIU/ml), estradiol (E2) (<30 pg/ml), follicle diameter (<10 mm) and endometrial thickness, and received GnRH (Gonal-F, Serono) for ovulation induction. We compared the clinical and laboratory data and pregnancy outcomes between the first and repeated cycles before and after ovulation induction.@*RESULTS@#The repeated cycles, as compared with previous ones, showed significant increases in the antral follicle count (AFC) on the first day of stimulation (7.55 ± 1.86 vs 6.45 ± 2.5, P<0.05), number of follicles =≥14 mm in diameter on the hCG trigger day (7.81 ± 3.6 vs 5.56 ± 3.68, P<0.05), level of E2 ([2 362.15 ± 1 210.49] vs [1 749.22 ± 1 139.44] pg/ml, P<0.05), and numbers of oocytes retrieved (7.51 ± 3.23 vs 4.78 ± 3.41, P<0.05) and embryos transferred (2.00 ± 0.33 vs 1.50 ± 0.67, P<0.05), exhibited a remarkably reduction in the dose of GnRH ([1 791.65 ± 1 889.41] vs [3 439.56 ± 1 836.53] IU, P<0.05), and achieved a clinical pregnancy rate of 62.16%.@*CONCLUSIONS@#With proper reduction of the FSH, LH and E2 levels and follicle diameter, repeated superovulation induction for IVF-ET can improve the ovarian response and pregnancy outcomes of the patients with adenomyosis or moderate to severe pelvic endometriosis after failure in the previous IVF-ET cycles with the ultra-long protocol.

ABSTRACT

Presenilin (PS) is a transmembrane protein identified in familial early-onset Alzheimer disease, and it is mainly expressed in cell membranes and organelle membranes. As an important catalytic core of the γ-secretase multimeric enzyme complex, PS has been implicated in regulating various proteins. Recent researches have shown that mutations in PS are identified in the familial dilated cardiomyopathy, and the PS gene plays an important role in cardiac formation and regulation of calcium homeostasis in myocardial cells. In this review, we summarized the function of PS in heart and the mechanisms underlying the effects of PS on calcium homeostasis, such as amyloid β protein (Aβ), 1,4,5-inositol trisphosphate receptors, ryanodine receptors and PS as endoplasmic reticulum (ER) Ca2+ leak channels, hoping to provide a theoretical basis for the therapy of dilated cardiomyopathy.

ABSTRACT

Objective: To prepare a series of polyethylene (PEG)-modified mesoporous silica nanoparticles (MSNs-PEG) used for danshensu delivery carrier. Methods: By the co-hydrolysis method with silica coupling agent, the content of the azide groups was controlled into MSNs. The structures of MSNs-PEG were characterized by FTIR, XRD, and TEM analyses. The results showed that PEG chains have been grafted on the surface of MSNs. The safety of MSNs-PEG carrier was preliminarily evaluated by MTT, the release rule of MSNs-PEG was investigated by in vitro release experiment. Results: PEG can be effectively and controlled grafted onto MSNs. The MSNs-PEG have the good stability in aqueous solution. The loading rate of MSNs-PEG was higher in the experimental results of danshensu. The drug loading and entrapment efficiency were 6.8 % and 22.8 %. The graft of PEG could change the release of the drug, which could effectively prolong the time of drug release. And with the increase of the amount of PEG (mass fraction), the release time of danshensu could be prolonged effectively. Conclusion: The click chemistry method is easy to control the PEG graft content, and effectively controls the release rate of danshensu.

ABSTRACT

Objective To analyze the current situation in studies of translational medicine in Guangdong Province. Methods The number of centers for translational medicine,academic conferences,scientific fund-supported projects, and academic papers was calculated,respectively. Results Guangdong Province established 24 centers for transla-tional medicine,held 22 academic conferences,completed 42 scientific projects,and published 79 academic papers in domestic and foreign journals, including 12 papers concerned about traditional Chinese Medcine. Conclusion Guangdong Province have made some achievements in translational medicine that present certain development rules. It is suggested that in the future,the government should pay more attention to the development of translational medi-cine research. In order to promote the development of translational medicine research in Guangdong province, the government should try to combine the development of translational medicine research and the development of provin-cial key laboratory and key disciplines in Guangdong province.

ABSTRACT

<p><b>OBJECTIVE</b>To retrospectively assess the feasibility and safety of percutaneous minimally invasive pedicle screw fixation for cervical fracture using intraoperative three-dimensional fluoroscopy-based navigation.</p><p><b>METHODS</b>Thirty patients admitted from April 2012 to May 2014 in Beijing Jishuitan Hospital with cervical fracture underwent pedicle screw fixation using intraoperative three-dimensional fluoroscopy-based navigation, with 8 patients using minimally invasive technique (CAOS-MIS group), and the other 22 patients using conventional open approach (CAOS-open group). Operative time, blood loss and postoperative neurovascular complications were recorded. Screw positions were studied by postoperative CT scan. All patients were followed up for at least 6 months. Neck visual analogue score (VAS) and American Spinal Injury Association (ASIA) classification were evaluated preoperatively and at 6-month follow-up. Independent-sample t test and Chi-Square test were used for statistical analysis.</p><p><b>RESULTS</b>Operation time was (139 ± 18) minutes and blood loss was (73 ± 40) ml in CAOS-MIS group and correspondingly (154 ± 42) minutes and (296 ± 171) ml in CAOS-open group. The blood loss in CAOS-MIS group was significantly lower than that in CAOS-open group (t = 5.695, P < 0.01). No screw-related injury to nerve or vertebral artery was observed. Thirty-four screws were placed in CAOS-MIS group with 28 screws (82.4%) classified as Grade I, meanwhile in CAOS-open group 108 screws were placed with 96 screws (88.9%) classified as Grade I. There was no statistical difference between the two groups (χ² = 0.998, P > 0.01). VAS score showed no statistical difference preoperatively (t = 0.334, P > 0.01), however statistical difference existed at 6 months follow-up (t = 4.111, P < 0.01) with (0.4 ± 0.5) in CAOS-MIS group and (1.5 ± 0.7) in CAOS-open group. There were 1 patient from class B to improve to D, 1 case from D to E in CAOS-MIS group, and 1 patient from class A to improve to B, 1 case from B to D, 2 cases from C to D, 3 cases from D to E in CAOS-open group 6 months after surgery. Cervical X-ray demonstrated fractures healed well in all cases at 6 months follow-up.</p><p><b>CONCLUSION</b>It is feasible and safe for percutaneous minimally invasive pedicle screw fixation for cervical fracture using intraoperative three-dimensional fluoroscopy-based navigation, which can also decreases the incidence of postoperative neck pain.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the relationship of the incidence of polynuclear zygotes with clinical pregnancy after short coincubation of gametes in in vitro fertilization (IVF).</p><p><b>METHODS</b>We retrospectively analyzed 3 862 cases of short gamete coincubation IVF, which were divided into six groups according to the percentage of polynuclear zygotes: 0,1-10%, 11-20%, 21-30%, 31-50%, 41-50%, and > or = 51%. We compared the rates of clinical pregnancy, implantation and abortion among the six groups.</p><p><b>RESULTS</b>No statistically significant differences were found in the patients'age, dose of gonadotropin, peak E2, number of follicles at the hCG trigger, and number of oocytes retrieved among the six groups. The 1-10% group showed higher rates of pregnancy and implantation, while the > or = 51% group exhibited lower rates of pregnancy and implantation but a higher rate of abortion than the other groups, none with significant differences (P > 0.05).</p><p><b>CONCLUSION</b>The incidence of polynuclear zygotes after short coincubation of gametes in IVF cannot serve as a prognostic indicator of the outcome of clinical pregnancy.</p>

Methods109 hospitalized patients with coronary heart disease were randomly divided into a experimental of 56 patients and a control group of 53 patients according to length of stay. The control group was treated with the conventional therapy; the experimental group was imposed five-emotion restrictive psychological intervention on the basis of conventional therapy. The investigation was done using self-rating anxiety scale(SAS) and self-rating depression scale(SDS) on 1 day after admission and ld before discharge respectively. SAS and SDS scores were surveyed. At the same time Holter monitoring and normal ECG test were compared. Results Compared with control group, the experimental group 1 d before discharge of the SAS and SDS scores were (41.9±7.4) and (38.6±8.4), and the difference was statistically significant (P<0.01) . The total effective rate of Holter monitoring and normal ECG was 64.15% and 92.85% in the control and experimental group, and the difference was significant (P<0.01) . ConclusionThe pathological seven emotions have a certain affection on the incidence of coronary heart disease, treatment and prognosis of the course. Five-emotion restrictive psychological intervention can adjust timely psychological state of patients, eliminate negative emotions, and significantly help the incidence, treatment and prognosis of the disease.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effectiveness of ICSI in overcoming the high incidence of tripronucleates zygotes resulting from insemination in a previous IVF cycle.</p><p><b>METHODS</b>We retrospectively analyzed the matched-pair cycles in 37 patients with a > 35 % incidence of tripronucleate zygotes in an IVF cycle, with ICSI used in the subsequent cycle, evaluated the incidences of diploid (2PN) and triploid (3PN) zygotesand the number of normal embryos obtained, and compared the rates of clinical pregnancy and embryo implantation between the IVF and ICSI groups.</p><p><b>RESULTS</b>The mean age of the ICSI group was significantly older than that of the IVF group, while the ampules of gonadotropin and peak E2 showed no remarkable difference between the two. The numbers of follicles at hCG trigger, retrieved oocytes and mature oocytes were markedly lower in the former than in the latter. The percentage of 2PN was significantly higher while that of 3PN significantly lower after ICSI than after IVF (74.24% vs 34.42%; 11.57% vs 51.04%, P < 0.01), and more normal diploid embryos were obtained with ICSI (3.83 +/- 2.08 vs 2.52 +/- 1.71, P < 0.01). Four singletons were achieved in 31 IVF embryo transfer cycles, in comparison with 11 singletons and 3 twins in 36 ICSI embryo transfer cycles. The ICSI group showed significantly higher rates of clinical pregnancy and embryo implantation than the IVF group (38.89% vs 12.90%; 28.33% vs 7.41%, P<0.01).</p><p><b>CONCLUSION</b>For women with a high incidence o triploidy in a previous IVF cycle, ICSI can effectively increase the number of normal diploid zygotes.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the safety of early rescue intracytoplasmic sperm injection (ICSI) for complete failure of in vitro fertilization (IVF).</p><p><b>METHODS</b>We conducted early rescue ICSI for 105 conventional IVF cycles and follow-up evaluation of the clinical outcomes.</p><p><b>RESULTS</b>A total of 64 neonates were born, and no significant differences were found in the sex ratio, birth weight and birth defects.</p><p><b>CONCLUSION</b>Early rescue ICSI can significantly improve the clinical outcome in patients with complete IVF failure, but does not increase the clinical risk.</p>
