ABSTRACT

Chikungunya means “that which bends up,” indicating severe incapacitating arthritis or polyarthralgia. It is a viral infection caused by the chikungunya virus (CHIK V) belonging to the family Togaviridae and is transmitted by the mosquitoes Aedes aegypti and Aedes albopictus. It is a self-limiting condition with a high fever of up to 40°C (104°F), debilitating arthritis/polyarthralgia, and cutaneous involvement. CHIK V virus affects the joints causing tenosynovitis and enthesopathy presenting as arthralgia. Leprosy is also associated with the loss of sensation, arthritis/arthralgias including involvement of several joints and tenosynovitis specially during reactions. Apart from leprosy, numerous other conditions can present with loss of sensation, which must be ruled out while considering leprosy. We report two patients with chikungunya who presented with a loss of sensation like leprosy

ABSTRACT

Background: Vaginal hysterectomy is preferable due to faster recovery, decreased morbidity and absence of an abdominal incision. The aim was to compare the risks and complications of laparoscopy assisted vaginal hysterectomy and total abdominal hysterectomy in terms of intra-operative and post-operative complications.Methods: A retrospective observational study was conducted in the Gynaecology ward at Vinakaya Mission’s Kirupananda Variyar Medical College and Hospitals, Salem, Tamil Nadu, India. The data for the past 1-year record was taken for analysis. A total of 80 subjects were included in the study and were divided into two groups with 40 patients under TAH (total abdominal hysterectomy) group and 40 under LAVH (Laproscopic assisted vaginal hysterectomy) group. The primary outcome of the present analysis was incidence of perioperative complications like blood loss and the secondary outcomes were operating time, blood loss, urinary tract injury, rate of conversion to laparotomy, postoperative pain, and length of postoperative stay.Results: The mean intra-operative blood loss was measured among both the groups and it was found to be very high among TAH group (201 ml) compared to LAVH group (149.8 ml) and the difference was found to be statistically significant (p <0.05). Similarly, the duration of operative procedure was found to be less in LAVH group (57.9 mins) compared to TAH group (72.6 mins) and the difference was found to be statistically significant (p <0.05). Post-operative wound infection (14 vs 0) was found to be more among the patients in TAH group than that of the LAVH group and the difference was found to be statistically significant (p <0.05).Conclusions: LAVH is a safe and effective surgical treatment for benign gynaecological diseases and should be offered whenever possible, taking into account the low rate of complications and cost-effectiveness.

ABSTRACT

Dental treatment still remains as one of the most anxious visits despite awareness between dentists and patients in building trusting relationships. The fear of dental treatment determines the frequency of treatment availed with long-term implications in oral health maintenance. Aim: This study aims to estimate prevalence, extent, and factors influencing dental anxiety in a sample of the adult population visiting a private dental hospital in Chennai, India. Materials and Methods: The study sample included 300 consecutive outpatients visiting a private dental hospital in Chennai. Any patient aged 18 and above was considered for the study. A structured custom-made questionnaire composed of ten questions was designed to assess the anxiety levels of dental patients. The level of the patient's anxiety was assessed taking into account various factors that could influence their mental state. This included their age, gender, educational qualification, number/frequency of their dental visits, reason for their visit, past dental experiences, and nature of the dental procedure planned. Enquiries about the related time of anxiety manifestations as well as their preferred anxiety reducing protocols were done as part of the study. Statistics: The data analysis was performed using SPSS Version 11.5. Analysis initially was performed with Chi-square test for frequency and reason of visit, with the independent samples t-test used for assessing mean score differences along with Pearson's correlation coefficient. Results: The study revealed that dental anxiety has a wide prevalence rate encompassing both genders. Distinctive predominance among females (65.2%) and professionals (66.9%) exhibiting a greater degree of anxiety was identified. Pain and extraction have been cited as duress and anxiety inciting procedures (72.6%). Conclusion: This survey has revealed that age, gender, level of education, and procedure along with visit frequency has a direct effect on the patient's state of mind and anxiety.

ABSTRACT

Classification for cleft lip and palate is imp ortant fo r both clinical research and epidemiological investigation. Classification of the cleft lip and cleft palate plays an important role in diagnosis and planning the treatment. It involves the embryological processes, the frontonasal and the right and left processes of the maxilla. The incisive foramen is a basic anatomic landmark for classification of cleft l ip and palate. Davis and Ritchie’s classification was a fundamental classification, which is followed by symbolic representation of Kernahan and their modifications. Newer approaches have also used mathematical expressions to provide a complete description of the deformity including those which can be used for computerized data analysis. This article is a review of the past and the most recent classifications, a bird's eye view on how improvements/advancements in the field have led to a better understanding and representation of the various types of cleft deformities.
