ABSTRACT

Abstract Introduction: Reoperations in cardiac surgery represent a clinical challenge, particularly because of the higher rate of perioperative morbidity and mortality. Mitral valve reoperation owing to bioprosthesis dysfunction, transcatheter treatment with a prosthesis implantation over the prosthesis has emerged as an alternative, especially for patients with a previous approach. In this study, we analyzed the hydrodynamic behavior of transcatheter prosthesis implantation in conventional mitral bioprostheses through hydrodynamic tests and produced a recommendation for the size of transcatheter valve most adequate for valve-in-valve procedure. Methods: Mitral bioprostheses were attached to a flow duplicator and different combinations of transcatheter prostheses were implanted inside. The equipment simulates the hydrodynamic behavior of the valves submitted in vitro and determines transvalvular pressures and flow parameters. Results: All tests could be performed. Better hydrodynamic performance occurred for transcatheter prostheses 1 mm smaller than bioprostheses, except for the 27-mm bioprostheses. Effective valve areas (cm²) and transvalvular gradients (mmHg) were, respectively: Bioprosthesis × Inovare: 27 × 28 mm: 1.65 and 5.95/29 × 28 mm and 31 × 30 mm: 2.15 and 3.6. Conclusion: The mitral valve-in-valve implantation proved to be feasible in vitro. The use of 27-mm bioprostheses should be judicious, with preference for a 26-mm transcatheter valve. In the 29 and 31-mm bioprostheses, the implantation was very satisfactory, with good effective valve areas and transvalvular gradients, with preference for smaller transcatheter valves.

ABSTRACT

Objective: To evaluate sperm concentration, morphology and motility of Brazilian semen donors from 1992 to 2003, in the city of São Paulo. Methods: Retrospective study analyzing 182 donor semen samples from 1992 to 2003. The first and the second donated sample were analyzed for each donor. Donor average age was 30.8 years. Means with standard errors, medians with minimum and maximum values, and interquartile ranges were calculated for age, sperm concentration, semen volume, oval morphology and motility. The relation between each characteristic of the semen samples and the year of donation, as well as donor age and season of the year were studied by linear and multiple regression analysis. Results: Linear regression analysis showed that the sperm concentration (R2 = 19.1%, R2 = 20.2%, p < 0.0001 respectively) and the oval morphology (R2 = 13%; R2 = 13.5%; p < 0.0001, respectively) decreased significantly, even when the first or the second sperm collection is considered. The ejaculated volume showed slight increase during the period for both samples (R2 = 2.2%, p = 0.048; R-sq = 2.4%. p = 0.038, respectively). All characteristics did not depend on the donors' age or season of the year when the samples were obtained. Conclusions: There was a decrease in spermatic concentration and percentage of oval sperm of semen donors samples from 1992 to 2003, in the city of São Paulo.


Objetivo: Avaliar as características seminais dos doadores de sêmen na cidade de São Paulo, no período de 1992 a 2003. Métodos: Análise retrospectiva das amostras seminais de 182 doadores de um único Banco de Sêmen, na cidade de São Paulo, no período de 1992 to 2003, que tinham a idade média de 30,8 anos. Foram analisadas a primeira e a segunda amostra de cada doador. Médias com desvios padrões, medianas com valores máximos e mínimos e intervalo interquartil foram calculados para idade, volume seminal, concentração, motilidade e morfologia espermática. As relações entre cada característica das amostras seminais e o ano da doação foram estudadas por análise de regressão linear simples. Modelos de regressão linear múltipla foram aplicados para examinar a relação do ano de doação com cada característica seminal, controlando para potencias fatores de confusão, como idade dos doadores e estação do ano em que a coleta foi realizada. Resultados: Análise de regressão linear mostrou que a concentração espermática (R2 = 19,1%, R2 = 20,2%, p < 0,0001, respectivamente) e a morfologia oval dos espermatozoides (R2 = 13%; R2 = 13,5%; p < 0,0001, respectivamente) diminuíram significativamente, na primeira e na segunda coleta seminal. O volume do ejaculado mostrou um discreto, porém significativo, aumento nas duas coletas (R2 = 2,2%, p = 0,048; R- sq = 2,4%, p = 0,038, respectivamente). Todas as alterações não se correlacionaram com a idade do doador nem com a estação do ano em que as coletas de sêmen foram realizadas. Conclusões: Houve diminuição na concentração espermática e na percentagem de espermatozoides nas amostras seminais dos doadores de sêmen, no período de 1992 a 2003, na cidade de São Paulo.

ABSTRACT

OBJECTIVES: To assess the clinical presentation of children with lower urinary tract dysfunction (LUTD) relating to objective examination data. MATERIALS AND METHODS: Forty-four children (36 girls and 8 boys with mean age of 6.8 years) with LUTD were prospectively assessed through a specific questionnaire that analyzed clinical presentation of those patients. These data were then compared to objective data, such as micturition diary and uroflowmetry with electromyography. RESULTS: A urinary tract infection (UTI) antecedent was observed in 31 cases (70.5 percent), and of those, 24 cases of UTI were accompanied by fever. All children presented micturition urgency. Daily urinary incontinence was observed in 33 cases (75 percent) and nocturnal enuresis in 23 (52.3 percent). As for micturition frequency, 15 (34.1 percent) had normal frequency 19 (43.2 percent) presented more than 10 daily micturition episodes and 10 (22.7 percent) thought they urinated less than 5 times a day. In the uroflowmetry and electromyography examination, 14 (31.8 percent) experienced lack of coordination during micturition. Of 10 children with infrequent micturition, 5 confirmed this in their micturition diaries and 2 listed more than 5 micturition episodes per day in the diary. Of 19 patients presenting polaciuria, only 5 confirmed this in their micturition diaries, while 7 had less than 10 micturition episodes per day. CONCLUSION: Most children with LUTD presented a previous UTI, and daily incontinence was verified in around 75 percent of the patients. Complaints of polaciuria or infrequent micturition are not noted completely in the micturition diaries and there is no parameter in the clinical history that offers good sensitivity or specificity for the diagnosis of lack of perineal coordination.
