ABSTRACT

Purpose@#The purpose of this study was to investigate the effects of home-and-workplace combined exercise on physical function, depression, and work-related disability in patients with ankylosing spondylitis. @*Methods@#This study adopted a non-randomized quasi-experimental design. Fifty-two patients were recruited: home-and-workplace combined exercise (n = 17), home exercise (n = 18), and control group (n = 17). @*Results@#The home-and-workplace combined exercise group showed improvement in spinal mobility and pulmonary function and significantly lower absenteeism and overwork impact than the home-exercise group and control group. The home-and-workplace combined exercise and home exercise groups showed a higher level of activity improvement than the control group. @*Conclusion@#home-and-workplace combined exercise can be recommended to patients with ankylosing spondylitis to enhance their physical function, including spinal mobility and pulmonary function, and reduce work-related disability.

ABSTRACT

Purpose@#The purpose of this study was to investigate the influence and relationship among anxiety, depression, self-esteem, and quality of life, along with factors affecting quality of life, on patients with rheumatoid arthritis. @*Methods@#This study was a cross-sectional descriptive study, and 200 individuals with rheumatoid arthritis who visited a single university hospital as outpatients were recruited through convenience sampling. Using a structured questionnaire, a survey was conducted regarding the general characteristics, anxiety, depression, self-esteem, and quality of life. Collected data were analyzed using t-test, ANOVA, and regression analysis. @*Results@#Results showed that lower levels of anxiety or depression and higher levels of self-esteem were associated with improved quality of life. Depression was identified as the main factor affecting quality of life, followed by anxiety, glucocorticoid treatment, time elapsed after diagnosis, and occupational status. @*Conclusion@#These findings confirmed that depression, anxiety, and glucocorticoid medication are factors influencing quality of life in patients with rheumatoid arthritis, Duration since diagnosis, and occupation should be considered in order to improve the quality of life of these patients with rheumatoid arthritis. The occupation and treatment-related characteristics of the patients must be considered, with particular focus on depression and anxiety.

ABSTRACT

Purpose@#This study was done to determine the correlations among grit, social support, career barriers, and career preparation behavior in mature age nursing students, and identify factors influencing their career preparation behavior. @*Methods@#This cross-sectional, descriptive study included 106 undergraduate nursing students aged 25 years or older at the time of entering three universities in Korea. Grit, social support, career barriers, and career preparation behavior were investigated using structured questionnaires. Data analyses included descriptive statistics, independent t-test, one-way ANOVA, Pearson’s correlation analysis, and stepwise multiple regression analysis and were performed with the SAS program (version 9.4). @*Results@#The factors influencing career preparation behavior of mature age nursing students included: persistence of efforts (β=.35, p<.001) and consistency of interests (β=-.21, p=.011) under the sub-domains of grit; informational support (β=.29, p<.001) under the sub-domains of social support; lack of job information (β=-.36, p<.001) and anxiety about the future(β=.28, p=.008) under the sub-domains of career barriers. These variables accounted for approximately 42% of the variance in career preparation behavior. @*Conclusion@#The findings imply that in order to improve the career preparation behavior and lower the career barriers of mature age nursing students, their individual cognitive and psychological characteristics should be considered while providing diverse job information as part of career guidance.

Purpose@#The aim of this study was to investigate the influence of daily life functions, anxiety, and depression on pain catastrophizing among patients undergoing lumbar spinal surgery. @*Methods@#This study was a cross-sectional descriptive research. The participants were 136 patients undergoing lumbar spinal surgery at one general hospital. They were surveyed using structured questionnaires on daily life functions, anxiety, and depression related to pain catastrophizing. Data were analyzed using t-test, ANOVA, Pearson correlation coefficients, and stepwise multiple regression analysis. @*Results@#The results indicate that pain catastrophizing was negatively correlated with all sub-domains of daily life functions, such as low back pain, low back function, gait ability, social life functions, and mental health, whereas it was positively correlated with anxiety and depression. However, only mental health, a sub-domain of daily life functions after surgery, was found to have a significant effect on pain catastrophizing. @*Conclusion@#This study confirmed that mental health-related daily life functions after surgery were predictors of pain catastrophizing. Therefore, to alleviate pain catastrophizing in patients undergoing lumbar spinal surgery, it is necessary to develop nursing interventions that can assist with mental health-related daily life functions after surgery as well as physical recovery.

Purpose@#The purpose of this study was to compare the health‐related quality of life of premature menopausal women with that of normal postmenopausal women and to identify factors related to health-related quality of life in these two groups. @*Methods@#A secondary analysis was conducted of a sample of 5,910 postmenopausal women drawn from women aged 19–79 years, using raw data from the Korea National Health and Nutrition Survey between 2014 and 2017. Health-related quality of life was compared between premature menopausal women and normal postmenopausal women using the t‐test and chi‐square test, and factors related to health‐related quality of life were analyzed by multivariate logistic regression using SAS version 9.4. @*Results@#There was no significant difference in health‐related quality of life between premature menopausal women and normal postmenopausal women. Subjective health status and symptoms of depression were identified as factors related to quality of life in women with premature menopause, while engaging in aerobic physical activity, subjective health status, stress, and symptoms of depression were found to be related to quality of life among normal postmenopausal women. @*Conclusion@#It is necessary to support self‐care measures for both premature and normal menopausal women to adapt to changes caused by menopause, and multidimensional strategies should be developed considering postmenopausal women’s subjective health status and symptoms of depression to improve their quality of life.

ABSTRACT

PURPOSE: This study was conducted to identify subjective attitudes towards terminal patients in nursing students who had clinical practice. The types of subjective attitude were classified by applying Q methodology. According to those types, basic reference data for the development of educational programs were provided.METHODS: Thirty-four final Q samples were selected, and Q classification with a nine-point scale was performed with P samples of 43 nursing students. A key factor analysis was conducted with the collected data using the PC QUANAL program.RESULTS: Nursing students' attitudes towards terminal patients were grouped into three types. The total variable was 49.96%. Students with Type 1 (“wish for life-sustaining medical treatment”) thought that terminal patients accurately understood their medical condition and wanted to prolong their lives. Others with Type 2 (“need for service and support”) believed that a multidisciplinary nursing system needs to be established to help terminal patients prepare for death. Students with Type 3 (‘awareness and acceptance of death’) thought that terminal patients wanted to die with dignity at a hospice unit.CONCLUSION: This study analyzed various types of attitude towards terminal patients, as perceived by nursing students with clinical training experience. Development of educational programs for each attitude type analyzed in this study could contribute to systematic training programs for nursing students caring for terminal patients.

ABSTRACT

PURPOSE: This study aimed to investigate the severity of lower urinary tract symptoms (LUTS) and to identify factors that influenced LUTS in advanced cancer patients with chemotherapy-induced peripheral neuropathy (CIPN). METHODS: This cross-sectional study included a total of 158 advanced cancer patients with CIPN. A structured questionnaire including the International Prostate Symptom Score and the Functional Assessment of Cancer Therapy/Gynecology Oncology Group/Neurotoxicity scale was used. Data were analyzed using the Pearson correlation coefficient and multiple regression analysis. RESULTS: Nocturia was the most prevalent LUTS. A positive relationship was found between CIPN symptoms and LUTS. The duration of cancer diagnosis and the severity of CIPN were key factors that influenced LUTS. CONCLUSIONS: The severity of CIPN symptoms was the most important predictor of LUTS. Nurses’ care for advanced cancer patients should incorporate a comprehensive health assessment, which includes a history of treatment and physical neuropathic symptoms, for any patient complaining of CIPN symptoms.

ABSTRACT

PURPOSE: This study identifies degree of awareness and legal knowledge about overall abuse and violence, and investigates attitudes towards domestic violence in emergency nurses. METHODS: The data collection period was between August and December 2012 for 131 nurses who worked in the emergency center of five hospitals in South Korea. This study surveyed emergency nurses about the recognition of abuse and violence, the legal knowledge of abuse and violence, and the attitudes towards domestic violence. RESULTS: This study showed that approximately 60.0% of participants experienced incidents of suspected abuse or violence, but the reporting rates were low. Of all the participants, 70.2% knew that they must report the discovery of abuse or violent incidents, but 45.0% did not know that if someone who had duty to report but did not report that he/she had a legal responsibility. Most emergency nurses agreed that "even if the domestic violence is severe it should not be suggested to victims that they run away, leaving children at home", and "perpetrators are patients who need treatment." CONCLUSIONS: Appropriate awareness about abuse and violence, and systematic education are required for emergency nurses so that they can provide appropriate interventions.

ABSTRACT

PURPOSE: The objective of this study was to investigate the relationship between fatigue, sleep disturbance, and gestational stress in women during late stage of pregnancy. METHODS: This study was conducted with 113 healthy pregnant women at gestational age greater than 27 weeks who were registered at community health centers and received prenatal care. A structured questionnaire regarding fatigue, sleep disturbance, and gestational stress was used. The data was analyzed using a t-test, an ANOVA, and Pearson correlation coefficients. RESULTS: The subjects with unplanned pregnancies and irregular exercise patterns showed a higher level of fatigue than those with planned pregnancies and regular exercise patterns. Pregnant women with caffeine intake manifested higher levels of gestational stress and sleep disturbance than those without. The levels of sleep disturbance and gestational stress increased as the fatigue levels increased. The fatigue levels increased with increased levels of sleep disturbance. CONCLUSION: Planned pregnancy, regular exercise patterns, and caffeine intake were related with fatigue, sleep disturbance, and gestational stress in women during late stages of pregnancy. Fatigue, sleep disturbance, and gestational stress had close associations to each other. In the future, such results should guide development of nursing intervention programs for women in late stages of pregnancy.

ABSTRACT

PURPOSE: This is a retrospective study that investigated cancer patients' complaints of moderate or severe pain to analyze pain characteristics, pain relief interventions and their effects. METHODS: This is a retrospective study that investigated cancer patients' complaints of moderate or severe pain to analyze pain characteristics, pain relief interventions and their effects. RESULTS: The most frequent region of pain was the abdomen. The most frequent factor that exacerbated pain was movement. The most frequent pain alleviating factor was administration of analgesics. The most frequent pain type was breakthrough pain, and the most frequent non-pharmaceutical intervention for pain control was heat therapy. Among all, analgesics were routinely prescribed for 52.2% of the participants. Morphine sulfate was the most frequently used analgesic while Gabapentin was the most frequently used non-narcotic analgesic. At the time of discharge, 82.5% of the participants marked their pain intensity as 3 points or lower. CONCLUSION: For cancer patients complaining of moderate or severe pain, it is important to actively control pain from the beginning of admission. Thus, it is necessary to educate not only cancer patients using narcotic analgesic for pain control and their families but nurses about the effects and side-effects of drugs. Moreover, patients and their families need to learn how to assess and record pain at home to collect data that can be referred for future treatment.

ABSTRACT

PURPOSE: This cross-sectional study aimed at determining the relationship between patient-reported quality of life (QOL) and nurse-led bedside evaluations of chemotherapy-induced peripheral neuropathy symptoms. METHODS: One hundred ninety-five patients treated at the oncology clinic at our institution were assessed using Functional Assessment of Cancer Therapy/Gynecologic Oncology Group-Neurotoxicity and nurse-led bedside examinations. The relationship between self-reported QOL and bedside examinations was evaluated using Spearman rank correlations. RESULTS: Scores of upper and lower extremity muscle strength based on the bedside examinations showed a weak negative correlation with the emotional well-being subscale of Functional Assessment of Cancer Therapy-General. Further, weak negative relationships were present between QOL and the following nurse-reported parameters: vibration perception in the hand, upper extremity muscle strength, touch and vibration perception in the feet, and tendon reflexes. CONCLUSION: Collectively, our results indicate that nurse-led bedside evaluation is a noninvasive and useful method for detecting neurotoxicity and evaluating the patient's QOL both during and after treatment.

ABSTRACT

PURPOSE: This study has attempted to comparatively analyze the parenting stress that parents of a child with leukemia and their information needs at the end of treatment after complete remission (CR) of leukemia. METHODS: For data collection, a structured questionnaire survey on parenting stress and information needs was conducted using parents of children with leukemia visiting the Blood & Marrow Transplantation Center at C University Hospital in Seoul from August to December 2011. A total of 100 parents were examined. RESULTS: The parents of children under treatment after CR had greater parenting stress than parents of children whose treatment was finished. The parents of children whose treatment was finished had greater information needs than the parents of children whose treatment was ongoing. The differences had no statistical significance. In both groups, information needs about patients' growth development and school adjustment was high. CONCLUSION: This study confirms that it is necessary to add information about daily life management, child's growth development, sociopsychological adaptation, school adjustment, and support of family, to the current symptom care-centered education program.

ABSTRACT

PURPOSE: The purpose of this study was to describe and explore the experience process of life of patients with Crohn's disease. METHODS: Using a grounded theory methodology, 24 interviews were performed with 7 men and 5 women, 17-47 years of age, suffering from Crohn's disease. RESULTS: 'Tuning of two conflicting lives' was identified as the core category, and 5 subcategories were identified and they were integrated into the core category. The identified outcomes were 'Living a withdrawn life', 'coping flexibly', 'drifting with tangled emotions' and 'maintaining long-term remission'. CONCLUSION: When caring for these patients, it is important to identify needs, allow patients to express what they want at that moment and support them in maintaining a daily life that can be perceived as normal.

ABSTRACT

PURPOSE: This study was examined to identify the relationship between mood state, interaction anxiety, and quality of life of cancer patients with chemotherapy-induced alopecia. METHODS: The participants were 115 cancer patients. Data were collected from August to December 2012, and analyzed using descriptive statistics, Pearson's correlation coefficients, t-test, ANOVA and Duncan's multiple test. RESULTS: The mean scores of mood state (119.17+/-35.94), interaction anxiety (45.57+/-11.73), and global health status (38.08+/-22.45), functional scales (45.41+/-20.98), symptom scales (57.96+/-18.28) in quality of life were moderate or below. Negative correlations were found between mood state and quality of life, interaction anxiety and quality of life, but positive correlation were found between mood state and interaction anxiety. CONCLUSION: These findings indicate that mood state and interaction anxiety are important factors to improve quality of life of cancer patients with chemotherapy-induced alopecia, And it is meaningful in that this study prepared basic data for development of nursing intervention programs for effective management of chemotherapy-induced alopecia.

ABSTRACT

PURPOSE: The aim of this study was to understand factors that influence post-traumatic stress (PTS) in Korean forensic science investigators. METHODS: A total of 111 forensic science investigators were recruited in Korea. PTS was measured using the tool modified by Choi (2001) from the original developed by Foa, Riggs, Dancu, and Rothbaum (1993) based on DSM-IV. Factors influencing PTS included demographic and job-related characteristics, emotional intelligence, and death anxiety. RESULTS: PTS scores were positively correlated with personality type, fatigue from work, and death anxiety. PTS scores were negatively correlated with length of career as a forensic science investigator and emotional intelligence. The factors that had the greatest influence on PTS were death anxiety, years spent as a forensic science investigator, personality type, emotional intelligence, fatigue, and homicide experience. The explanatory power of these six factors was 44.0%. CONCLUSION: Therefore, it is necessary to regularly evaluate the mental health of those who are vulnerable to PTS. Based on these results, various interventions could be implemented for promoting overall health of the forensic science investigators.

ABSTRACT

PURPOSE: The aim of this study was to investigate nursing students' awareness of biomedical ethics and attitudes toward death of terminal patients. METHODS: A structured questionnaire was developed to examine nursing students' biomedical ethics. Their attitudes toward terminal patients' death were measured by using the Collett-Lester Fear of Death Scale. Surveys were conducted with 660 nursing students enrolled at a three-year college located in Daejeon, Korea. Data were analyzed using descriptive statistics, Wilcoxon rank sum test and Kruskall Waills test. RESULTS: Students who have experienced biomedical ethics conflicts, agreed to prohibition of cardiopulmonary resuscitation (CPR) and have no religion exhibited more negative attitudes toward death compared to students without the above characteristics. Of the participants, 81.2% answered that life sustaining treatment for terminal patients should be discontinued and 76.4% replied that CPR on terminal patients should be prohibited. The majority of the correspondents stated that the two measures above are necessary "for patients' peaceful and dignified death". CONCLUSION: Study results indicate the need to establish a firm biomedical ethics value to help nursing students form a positive attitude toward death. It also seems necessary to offer students related training before going into clinical practice, if possible. The training program should be developed by considering students' religion, school year, experience with biomedical ethics conflicts and opinion about CPR on terminal patients. The program should also include an opportunity for students to experience terminal patient care in advance via simulation practice on standardized patients.

ABSTRACT

PURPOSE: The purpose of this study was to examine risk factors for pathological gambling of horse race participants. METHODS: The participants, 508 horse race gamblers, completed the DSM-IV criteria of pathological gambling, Alcohol Use Disorders Identification Test (AUDIT-K) and Symptom Checklist-47-Revision (SCL-47-R). Data were analyzed using t-test, chi2-test, Fisher's exact test, and logistic regression analyses. Behaviors related to horse racing, alcohol abuse, and mental health were analyzed between problem or pathological gamblers compared to recreational gamblers. RESULTS: The prevalence rates of recreational, problem, and pathological gambling were 36.6%, 39.4%, and 24.0%, respectively. Frequency of gambling (> or =4/day), frequency of racetrack visiting (> or =3/month), accompaniment (alone), and mental health (SCL-47-R scores) were all associated with increased risks of problem and pathological gambling. Expenditure on betting (> or =200,000 won/day) and alcohol abuse (AUDIT-K 8-20 scores) group members had higher levels of gambling pathology than recreational gamblers. CONCLUSION: Problem and pathological gambling are highly associated with alcohol abuse and mental health disorders, suggesting that clinicians should carefully evaluate this population.

ABSTRACT

PURPOSE: The purpose of this study was to identify the stress and the coping strategies in breast cancer patients and their spouses. METHODS: The stress level was measured by the Stress Questionnaire of Andersson & Albertsson (2000). The coping strategies were measured by the modified Lazarus & Folkman's Ways of Coping Questionnaire. The data were collected by a survey sampling 49 couples from one hospital in Seoul. The data were analyzed by t-test, ANOVA and paired t-test. RESULTS: There was no significant differences between the stress level of breast cancer patients and their spouses. The problem-focused coping of breast cancer patients was significant higher than their spouses. The cancer patients and their spouses used problem-focused coping mode more than emotion-focused coping mode. In the problem-focused coping mode, breast cancer patients used two coping strategies - 'seeking information' and 'cognitive reconstruction' - significantly more than their spouses. In emotion-focused coping mode, the breast cancer patients used one coping strategy, 'emotional expression', significantly more than the their spouses. CONCLUSION: Further study needs to attempt to develop nursing interventions that could improve positive coping strategies.

ABSTRACT

PURPOSE: This study was conducted to determine the risk factors among patients with depression with Crohn's disease. METHODS: Data were collected by questionnaire from 276 patients who were diagnosed with Crohn's disease at a tertiary hospital located in Seoul. Measurements included patients' demographic characteristics, clinical characteristics, depression level, and health-related quality of life. Data were analyzed using t-test, chi2-test, Wilcoxon rank sum test, and logistic regression analyses. RESULTS: The incidence rate of depression (BDI-II> or =14scores) was 31.9% (n=88). Univariate analysis revealed that being a woman, school graduation status, economic status (low), BMI( or =3 years), CDAI (> or =150 scores), frequency of hospital admission (> or =2), extra-intestinal manifestation (arthralgia, stomatitis), administration of 5-aminosalicylic acid, and disease related quality of life (SIBDQ<50 scores) were associated with depression. Multivariate analysis revealed that economic status (low), school graduation status, and quality of life (SIBDQ<50 scores) were more likely to report high level of depression. CONCLUSION: Future research should consider managing depression as an essential component of comprehensive care for patients with Crohn's disease. In addition, further research is needed to develop strategies to better improve quality of life among patients with Crohn's disease who are depressed.
