ABSTRACT

Abstract Antimicrobial resistance is a global public health problem. Root canal microbiota associated with apical periodontitis represents a well-known reservoir of antimicrobial resistance genes (ARGs). However, the effect of type 2 diabetes mellitus (T2DM) in this reservoir is unknown. This study aimed to establish if root canal microbiota associated with apical periodontitis in T2DM patients is an augmented reservoir by identifying the prevalence of nine common ARGs and comparing it with the prevalence in nondiabetic patients. Methodology This cross-sectional study included two groups: A T2DM group conformed of 20 patients with at least ten years of living with T2DM and a control group of 30 nondiabetic participants. Premolar or molar teeth with pulp necrosis and apical periodontitis were included. A sample was collected from each root canal before endodontic treatment. DNA was extracted, and ARGs were identified by polymerase chain reaction. Results tetW and tetM genes were the most frequent (93.3 and 91.6%, respectively), while ermA was the least frequent (8.3%) in the total population. The distribution of the ARGs was similar in both groups, but a significant difference (p<0.005) was present in ermB, ermC, cfxA, and tetQ genes, being more frequent in the T2DM group. A total of eighty percent of the T2DM patients presented a minimum of four ARGs, while 76.6% of the control group presented a maximum of three. Conclusions Root canal microbiota associated with apical periodontitis in T2DM patients carries more ARGs. Therefore, this pathological niche could be considered an augmented reservoir.

BACKGROUND Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections in domestic animals have occurred from the beginning of the pandemic to the present time. Therefore, from the perspective of One Health, investigating this topic is of global scientific and public interest. OBJECTIVES The present study aimed to determine the presence of SARS-CoV-2 in domestic animals whose owners had coronavirus disease 2019 (COVID-19). METHODS Nasopharyngeal and faecal samples were collected in Uruguay. Using quantitative polymerase chain reaction (qPCR), we analysed the presence of the SARS-CoV-2 genome. Complete genomes were obtained using ARTIC enrichment and Illumina sequencing. Sera samples were used for virus neutralisation assays. FINDINGS SARS-CoV-2 was detected in an asymptomatic dog and a cat. Viral genomes were identical and belonged to the P.6 Uruguayan SARS-CoV-2 lineage. Only antiserum from the infected cat contained neutralising antibodies against the ancestral SARS-CoV-2 strain and showed cross-reactivity against the Delta but not against the B.A.1 Omicron variant. MAIN CONCLUSIONS Domestic animals and the human SARS-CoV-2 P.6 variant comparison evidence a close relationship and gene flow between them. Different SARS-CoV-2 lineages infect dogs and cats, and no specific variants are adapted to domestic animals. This first record of SARS-CoV-2 in domestic animals from Uruguay supports regular surveillance of animals close to human hosts.

RESUMEN Introducción: El maltrato y el homicidio infantil se destacan dentro del espectro de la violencia intrafamiliar por las consecuencias severas que generan en la salud, el desarrollo socioeconómico de una región y en la calidad de vida de los menores y sus familiares. Objetivo: Caracterizar el maltrato físico, sexual y los homicidios ejercidos hacia los niños y niñas menores de doce años, ocurridos en el contexto familiar en Medellín, Colombia, durante 2010-2015. Metodología: Estudio descriptivo, retrospectivo y transversal. Se realizó un análisis estadístico descriptivo de la ocurrencia de los eventos violentos intrafamiliares en menores de doce años atendidos por el Instituto Nacional de Medicina Legal y Ciencias Forenses en la ciudad de Medellín. Resultados: Se reportaron 3 519 casos de maltrato físico y sexual infligido a menores de doce años dentro del contexto familiar. 56,9 % de los hechos corresponden a maltrato sexual, siendo las mujeres las principales víctimas de este tipo de abuso; por su parte los hombres fueron los más afectados por maltrato físico. Los menores entre los seis y once años son los más maltratados. Se reportaron nueve casos de homicidio, de los cuales siete fueron cometidos contra mujeres y cinco de estos fueron perpetrados por el padrastro de las víctimas. Conclusiones: El maltrato infantil intrafamiliar ha sido un problema persistente en la ciudad de Medellín, y esto lo demuestra el alto número de casos registrados. No obstante, aunque los reportes de homicidio infantil han sido pocos, no se deben subestimar dada la importancia y las grandes consecuencias que esto presenta para la sociedad.


ABSTRACT Introduction: Child abuse and homicide are highlighted in the spectrum of intrafamily violence due to the severe consequences they generate on health, the economic and social development of a region and the quality of life of children and their families. Objective: characterize the physical and sexual abuse, and homicide exercised against children under twelve years old, occurred in the family context in Medellin Colombia, during 2010-2015. Methodology: Descriptive study, retrospective and cross-sectional. It was made a descriptive statistical analysis of the occurrence of intrafamily violent events attended by the National Institute of Legal Medicine and Forensic Sciences in Medellín. Results: 3,519 cases of physical and sexual abuse inflicted on children under twelve years old within the family context were reported. Of these, 56.9% of the events correspond to sexual abuse and women are the main victims of this type of abuse; men are the most affected in physical abuse. Children between six and eleven years old are the most vulnerable. Nine cases of homicide were reported, of which seven were committed against women and five of these were perpetrated by the victims' stepfather. Conclusions: Intrafamily child abuse is a persistent problem in Medellín, and this is demonstrated by the high cases recorded. However, there have been few cases of child homicide, this does not diminish the importance and the number should not be underestimated, because of the enormous consequences they have for society.

ABSTRACT

Debido a que la terapia antirretroviral no logra controlar la activación inmune asociada a la infección por VIH-1, el estudio de moléculas inmunomoduladoras puede proporcionar alternativas para su control. En este sentido, el propósito de este estudio fue evaluar la expresión transcripcional de moléculas asociadas con el metabolismo del colesterol-HDL (C-HDL) y con la respuesta inflamatoria mediada por el inflamasoma NLRP3 en pacientes infectados con VIH-1. En este estudio transversal, se incluyeron 23 pacientes VIH-1 sin tratamiento antirretroviral, con diferentes estadios de progresión, 7 de los cuales son controladores (Carga viral <2000 copias/mL) y 16 progresores (Carga viral >2000 copias/mL), además de 7 controles sanos. En células mononucleares de sangre periférica, se cuantificaron los niveles de la expresión transcripcional de ABCA-1, ABCA-3, Caspasa-5 y TXNIP mediante RT-PCR. Se evaluó la asociación de estos parámetros con variables demográficas y de laboratorio, y se encontró que los individuos VIH-1 progresores mostraron niveles significativamente menores de TXNIP y ABCA-3, sugiriendo que durante la infección por VIH-1 se produce una alteración en la expresión de estas moléculas. Dada la complejidad de las interacciones inmuno-metabólicas durante la infección por VIH-1, se necesitan estudios adicionales para establecer los mecanismos precisos involucrados en estas alteraciones


Because antiretroviral therapy fails to control the immune activation that occurs during HIV-1 infection, the study of immunomodulatory molecules may provide alternative strategies for their control. In this sense, the aim of the research was to evaluate the transcriptional expression of molecules associated with the metabolism of high-density lipoproteins and with the inflammatory response mediated by the NLRP3 inflammasome in patients infected with HIV-1. This is a cross-sectional study, which included 23 HIV-1 patients without antiretroviral treatment, with different stages of progression, 7 of which are controllers (Viral load <2000 copies/mL) and 16 progressors (Viral load >2000 copies/mL), in addition to 7 healthy controls. In peripheral blood mononuclear cells, the levels of transcriptional expression of ABCA-1, ABCA-3, Caspase-5 and TXNIP were quantified by RT-PCR. The association of these parameters with laboratory and demographic variables was evaluated and it was found that HIV-1 progressing individuals showed significantly lower levels of TXNIP and ABCA-3, suggesting that during HIV-1 infection there is an alteration in the expression of these molecules. Given the complexity of the immuno-metabolic interactions during HIV-1 infection, additional studies are needed to establish the precise mechanisms involved in these alterations

BACKGROUND Evolutionary changes in severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) include indels in non-structural, structural, and accessory open reading frames (ORFs) or genes. OBJECTIVES We track indels in accessory ORFs to infer evolutionary gene patterns and epidemiological links between outbreaks. METHODS Genomes from Coronavirus disease 2019 (COVID-19) case-patients were Illumina sequenced using ARTIC_V3. The assembled genomes were analysed to detect substitutions and indels. FINDINGS We reported the emergence and spread of a unique 4-nucleotide deletion in the accessory ORF6, an interesting gene with immune modulation activity. The deletion in ORF6 removes one repeat unit of a two 4-nucleotide repeat, which shows that directly repeated sequences in the SARS-CoV-2 genome are associated with indels, even outside the context of extended repeat regions. The 4-nucleotide deletion produces a frameshifting change that results in a protein with two inserted amino acids, increasing the coding information of this accessory ORF. Epidemiological and genomic data indicate that the deletion variant has a single common ancestor and was initially detected in a health care outbreak and later in other COVID-19 cases, establishing a transmission cluster in the Uruguayan population. MAIN CONCLUSIONS Our findings provide evidence for the origin and spread of deletion variants and emphasise indels' importance in epidemiological studies, including differentiating consecutive outbreaks occurring in the same health facility.

There is no information about the possible impact in denture retention after the use of common denture adhesives (DAs) when poor denture foundations (PDF) are present. Moreover, there is a lack of information about which current formulation provides greater retention and for how long. Twelve models from edentulous patients with different ridge shape and border height were used and complete dentures were manufactured. Four different formulation brands of DAs were tested after 10 minutes and three, six, nine, and 12 hours of DA application using a universal testing machine. The Fittydent® and Fixodent® adhesives had the highest retention at 12 hours. The PDF group increased on average its retention by 400 %. However, the group presented lower retention compared to the good denture foundation group. In conclusion, DAs significantly increased denture retention. The PDF group were the most benefited with the application of DAs. The Fixodent® paste had the highest retention.


No existe información acerca del posible impacto en la retención de dentaduras después del uso de adhesivos dentales comunes (DAs) cuando existen rebordes alveolares deficientes (PDF). Más aun, existe una falta de información acerca de cuál formula actual provee mayor retención y por cuanto tiempo. Doce modelos de pacientes edentulos con diferentes formas y alturas en sus rebordes alveolares fueron usados, y dentaduras completas les fueron realizadas. Cuatro diferentes fórmulas y marcas de DAs fueron evaluadas después de 10 minutos, tres, seis, nueve y 12 horas de que se aplicó el DA usando una maquina universal de pruebas. Los adhesivos Fittydent® y Fixodent® presentaron la retención más alta a las 12 horas. El grupo con PDF incrementó su retención hasta en un 400 %. Sin embargo, el grupo presentó menor retención cuando se comparó con el grupo que posee adecuados procesos alveolares. Los DAs incrementaron significativamente la retención de las dentaduras. El grupo PDF fue el más beneficiado con la aplicación de DAs. La pasta Fixodent® provee la más alta retención.

ABSTRACT

Introducción La displasia epifisiaria múltiple (DEM) es una enfermedad poco frecuente y con gran variedad clínica y se caracteriza por deformidades en las articulaciones, dolor, y trastornos de la marcha. La duplicación patelar se asocia con DEM recesiva y consiste en dos segmentos patelares escalonados separados por tejido blando entre ellos. Caso clínico Paciente masculino de 30 años con cuadro clínico de DEM recesiva con duplicación patelar, presenta dolor crónico bilateral de cadera y rodilla, y trastorno de la marcha. Tras el examen físico, se evidenció derrame articular, dificultad para la flexión de las rodillas y un cuerpo libre intra-articular bilateral. Se identificaron dos segmentos patelares, displasia acetabular y de cabeza femoral bilateral con imágenes diagnósticas. El manejo quirúrgico de la duplicación patelar fue resección de los segmentos óseos accesorios, conduciendo a un resultado clínico satisfactorio al año de seguimiento. Discusión Aunque no se realiza el diagnóstico genético de la DEM, nuestro paciente presenta las características fenotípicas y radiológicas de esta entidad. Para la duplicación patelar, se realizó la resección de las patelas accesorias, considerando el alto riesgo de no unión. Sin embargo, existen varios reportes donde unieron los dos segmentos patelares, pero principalmente en niños. Este es el primer reporte publicado sobre el manejo quirúrgico de esta patología en Colombia. La duplicación patelar puede manejarse con éxito mediante la resección de la patela accesoria en adultos. Aunque los hallazgos imagenológicos son muy sugestivos de esta patología, se requiere un adecuado examen físico para evitar un diagnóstico equivoco y tardío.


Background Multiple epiphyseal dysplasia (MED) is a rare disease with a great clinical variation, and is characterised by deformities in the joints, pain, and gait disorders. Duplication of the patella is associated with recessive MED, and consists of two staggered patellar segments separated by soft tissue between them. Clinical case A 30-years-old male patient with a clinical manifestation of recessive MED with duplication of the patella, chronic bilateral hip and knee pain, as well as gait disorder. After the physical examination, joint effusion, difficulty in flexing the knees, and a bilateral intra-articular free body were evident. Two patellar segments, acetabular dysplasia and bilateral femoral head, were identified with diagnostic imaging. The surgical management of duplication of the patella was resection of the accessory bone segments, leading to a satisfactory clinical result at one year of follow-up. Discussion Although the genetic diagnosis of the MED was not made, our patient presented with the phenotypic and radiological characteristics of this disease. For duplication of the patella, the accessory patella resection was performed, considering the high risk of non-union. However, there are several reports where the two patellar segments are joined; but mainly in children. This is the first report published about the surgical management of this pathology in Colombia. Duplication of the patella can be managed successfully by resecting accessory patella in adults. Although the imaging findings are very suggestive of this pathology, an adequate physical examination is required to avoid a false and late diagnosis.

Introducción Las lesiones de la raíz posterior del menisco lateral (RPML) afectan la transferencia de la carga axial de la rodilla, lo cual genera una sobrecarga con el posterior deterioro del cartílago articular. Se han descrito numerosas técnicas quirúrgicas en la literatura para su tratamiento, pero aún no se ha demostrado superioridad de alguna, por ende, existen controversias sobre cual técnica es la más indicada para estas lesiones. El objetivo del estudio es describir la técnica quirúrgica y la evolución clínica de pacientes intervenidos con fijación transtibial de la RPML en una clínica especializada, durante el periodo 2016-2017. Materiales & Métodos Se realizó una descripción de la técnica quirúrgica y una serie de casos retrospectiva. Se incluyeron pacientes con lesiones agudas en la RPML, los cuales fueron intervenidos quirúrgicamente utilizando una nueva variación a técnica transtibial. Para determinar la evolución cínica de los pacientes se realizaron las escalas Lysholm e IKDC, antes y después de la cirugía. Resultados Se intervinieron seis pacientes con lesión aguda de la RPML, cuatro de sexo masculino. El tiempo entre el trauma y la cirugía fue en promedio 2,5 meses. Todos los pacientes presentaron lesión concomitante de ligamento cruzado anterior. Al comparar el estado inicial de los pacientes y el postoperatorio mediante las escalas de Lysholm e IKDC, se encontraron diferencias estadísticamente significativas (p<0,05). Asimismo, no se realizaron reintervenciones durante el seguimiento. Discusión La reparación de la RPML con la nueva variación de la fijación transtibial proporciona una mejoría en la función, el dolor y el nivel de actividad de los pacientes, lo que puede ayudar a retrasar la progresión de la osteoartrosis en la rodilla. Igualmente, este procedimiento se puede realizar de forma segura aún en casos de lesiones ligamentarías concomitantes. Nivel de evidencia: IV


Background injuries of posterior lateral meniscus root (PLMR) affect the transfer of the axial load of the knee. Several surgical techniques have been described for it treatment, but still none has demonstrated superiority. Consequently, there are controversies about which technique is most indicated for these injuries. The aim is to describe the a surgical technique and the clinical follow up of patients operated with transtibial fixation of PLMR in a specialized clinic, during the 2016-2017. Methods Retrospective case series and description of the surgical technique. We included patients with acute injuries in the PLMR, who were operated using a new variation to the transtibial technique. For the clinical follow-up, the IKDC and Lysholm scores were performed before and after surgery. Results Six patients with acute lesion in the PLMR were intervened, four were male. The time between trauma and surgery was on average 2.5 months. All the patients presented a concomitant lesion of the anterior cruciate ligament. When comparing the initial state of the patients and the postoperative period, statistically significant differences were found (p <0.05). Likewise, reinterventions were not performed during follow-up. Discussion The repair of PLMR with the new variation of the transtibial fixation provides an improvement in the function, the pain and the level of activity of the patients, which can help to delay the progression of osteoarthrosis in the knee. Likewise, this technique can be performed safely even in cases of concomitant ligament injuries. Level of clinical evidence: Level IV

Abstract On a farm with permanent history of fasciolasis a study was performed aimed to know the efficacy of triclabendazole (TCBZ) and then to contrast with that of nitroxynil. Thirty-nine cattle naturally infected with Fasciola hepatica were randomly allocated into 4 experimental groups: Group 1 (control) was left untreated. Group 2 was treated with of 12 mg/kg body weight (bw) of TCBZ by oral route. Group 3 treated with 24 mg/kg bw TCBZ orally. Group 4 was treated with 10 mg/kg bw of nitroxynil subcutaneously. The anthelmintic efficacy was calculated as the percentage of reduction in faecal egg count (FEC) at 14 and 28 d post-treatment. Results indicated that there were no significant differences in the percentage of FEC reduction between control group and the groups treated with 12 or 24 mg/kg of TCBZ. On the contrary, the treatment with nitroxinyl significantly reduced the FEC and decreased the percentage of positive animals. In conclusion, Fasciola hepatica is reported for first time as resistant to TCBZ in Chile, which highlights the need of rotating drugs and assessing the efficacy of the administered drug in order to avoid the selection of resistant worms.


Resumo Em uma fazenda com histórico de fasciolose permanente, foi realizado um estudo com o objetivo de conhecer a eficácia do triclabendazol (TCBZ) e depois contrastar com o do nitroxinil. Trinta e nove bovinos naturalmente infectados com Fasciola hepatica foram distribuídos aleatoriamente em 4 grupos experimentais: Grupo 1 (controle), sem tratamento. O grupo 2 foi tratado com 12 mg/kg de peso vivo (PV) do TCBZ por via oral (VO). Grupo 3 tratado com 24 mg/kg de PV TCBZ por VO. O grupo 4 foi tratado com 10 mg /kg de PV Nitroxinil via subcutânea. A eficácia anti-helmíntica foi calculada comparando a percentagem de redução na contagem de ovos fecais (FEC) 14 e 28 dias pós tratamento. Não houve diferença significativa na porcentagem de redução FEC entre o grupo controle e os grupos tratados com 12 ou 24 mg/kg de TCBZ. Entretanto, o tratamento com nitroxinil reduziu significativamente o FEC e diminuiu a porcentagem de animais positivos. Em conclusão, a Fasciola hepatica é relatada pela primeira vez como resistente ao TCBZ no Chile, o que destaca a necessidade de realizar uma rotação em relação aos medicamentos anti-helmínticos e avaliar a eficácia do mesmo, a fim de evitar a seleção de vermes resistentes.

ABSTRACT

Se presenta caso de paciente masculino de 67 años de edad, fototipo III, con antecedentes patológicos personales de lepra lepromatosa en tratamiento, que acude a consulta especializada de Dermatología en el Hospital Guillermo Fernández Baquero (La Habana, Cuba) por lesiones ulcerosas diseminadas. Se realizan complementarios y se concluye caso como fenómeno de Lucio. Resaltando la infrecuencia de este eritema necrosante y la severidad de los estados reactivos de la lepra pues son procesos distintos pero destructores de los tejidos, supuestamente dirigidos por el sistema inmunológico que aumentan en gran medida la morbimortalidad de esta enfermedad


A case of a 67 year old male patient, phototype III, with a personal pathological history of lepromatous leprosy in treatment is presented at a specialized dermatology clinic at Guillermo Fernández Vaquero hospital for disseminated ulcerative lesions. Complementary are performed and case is concluded as a phenomenon of Lucio. Highlighting the infrequency of this necrotizingerythema and the severity of the reactive states of leprosy are different but destructive processes of the tissues, supposedlydirected by the immune system that greatly increase the morbidity and mortality of this disease.

Objectivo: Establecer la asociación clínica, patológica y microbiológica de Helicobacter pylori en biopsias gástricas en una población del departamento de Caldas (Colombia). Materiales y método: Se incluyeron 72 pacientes que fueron remitidos para endoscopia digestiva a la clínica San Marcel de la ciudad de Manizales durante el segundo semestre de 2015; a quienes se les tomaron dos biopsias del antro y dos del cuerpo del estómago para estudio histopatológico y cultivo microbiológico. Los datos fueron sometidos a procedimientos estadísticos descriptivos y relacionales. Resultados: se encontró una proporción de H. pylori del 47,2% por análisis histológico y del 26,4% por cultivo microbiológico, con predominio del sexo femenino. Siendo la epigastralgia el principal motivo de consulta asociado a la infección por H. pylori. Conclusión: se encontró asociación entre motivo de consulta, sexo femenino, cultivo microbiológico y presencia histológica de H. pylori. La edad se asoció al diagnóstico endoscópico establecido. El cultivo presentó frente al hallazgo histopatológico una especificidad elevada (84,2%) y una baja sensibilidad (38,2%).


Objective: To establish the clinical, pathological and microbiological association of Helicobacter pylori in gastric biopsies in a population of the department of Caldas (Colombia). Materials and methods: We included 72 patients, who were referred for digestive endoscopy to the San Marcel clinic in the city of Manizales during the second half of 2015; two biopsies of the antrum and two of the body of the stomach were taken for histopathological study and microbiological culture. Data were submitted to descriptive and relational statistical procedures. Results: H. pylori was found in 47.2% by histological analysis and 26.4% by microbiological culture, with predominance in the female sex. Epigastralgia is the main reason for consultation associated with H. pylori infection. Conclusion: we found an association between the reason for consultation, female sex, microbiological culture and histological presence of H. pylori. Age was associated with established endoscopic diagnosis. The culture presented a high specificity (84.2%) and a low sensitivity (38.2%), compared to the histopathological finding.

ABSTRACT

Organic response to infection is characterized by a systemic reaction known as acute phase response (APR). In order to know the effect of the administration of Escherichia coli lipopolysaccharide (LPS) on physiological, hematological and biochemical variables, 10 sheep weighing 45 ± 5 kg were divided in two groups: Experimental group treated with 3 doses of 1 µg.kg-1 LPS and control group treated with saline solution (SS) at the same frequency as experimental group. Body temperature (BT°), heart rate (HR) and respiratory rate (RR) were monitored. Blood samples for hemogram and enzyme activity for aspartate amino transferase (AST) and gamma glutamyl transferase (GGT) were collected between 1 and 24 h post-LPS. LPS-treated sheep presented mean values of BT (41.2 ± 0.4°C), HR (132 ± 12.3 beats/min) and RR (107.2 ± 25 cycles/min) higher than those observed in control sheep (39.8 ± 0.2°C, 88.8 ± 8.7 beats/min and 53.6 ± 17.1 cycles/min respectively). Between 4 and 8 hours post-injection (hpi) of LPS the leukocyte count was associated with lymphopenia, followed by leukocytosis at 24 hours. No changes were observed in the activity of AST and GGT enzymes. The results characterize APR induced by LPS in sheep, representing a useful model to study cardiovascular, hematological and biochemical responses to infection. (AU)


A resposta orgânica à infecção é caracterizada por uma reação sistêmica conhecida como resposta de fase aguda (RFA). Para conhecer o efeito da administração de lipopolissacárideo (LPS) de Escherichia coli sobre as variáveis fisiológicas, hematológicas e bioquímicas, 10 carneiros pesando 45 ± 5 kg foram divididos em dois grupos: o grupo experimental tratado com três doses de 1 µg.kg -1 de LPS e grupo controle tratado com solução salina (SS) na mesma frequência que o grupo experimental. Temperatura corporal (T°C), frequência cardíaca (FC) e frequência respiratória (FR) foram monitoradas. As amostras de sangue foram tomadas para hemograma e atividade da enzima aspartato aminotransferase (AST) e gama- -glutamiltransferase (GGT) entre uma e 24 h pós-LPS. Ovelhas tratadas com LPS apresentaram valores médios de T°C (41,2 ± 0,4°C), FC (132 ± 12,3 batimentos/min) e FR (107,2 ± 25 ciclos/min) acima dos observados em ovelhas do tratamento controle (39,8 ± 0,2°C, 88,8 ± 8,7 batimentos/min e 53,6 ± 17,1 ciclos/min, respectivamente). Entre 4 e 8 horas após a injeção de LPS, a contagem de leucocitos foi asociado com linfopenia, seguida de leucocitose as 24 horas. Nenhuma mudança na atividade das enzimas AST e GGT foi observada. Os resultados caracterizam uma resposta de fase aguda induzida por LPS em ovelhas, o que representa um modelo útil para estudar os sistemas cardiovascular, hematológico e bioquímico em resposta à infecção.(AU)


La respuesta orgánica a la infección se caracteriza por una reacción sistémica conocida como respuesta de fase aguda (RFA). Para conocer el efecto de la administración de lipopolisacárido (LPS) de Escherichia coli sobre las variables fisiológicas, hematológicas y bioquímicas, 10 ovejas con un peso de 45 ± 5 kg se dividieron en dos grupos: grupo experimental tratado con 3 dosis de 1 µg.kg-1de LPS y grupo control tratado con solución salina (SS) en la misma frecuencia que el grupo experimental. Introducción: la luxación de la patela es una patología relativamente frecuente sin embargo, su manejo es controversial. En los últimos años la tendencia a realizar reconstrucciones que asemejen más la anatomía nativa, ha generado un renovado interés por las reconstrucciones del ligamento patelofemoral medial (LPFM). De las estructuras que permiten mantener la patela en adecuada posición, el LPFM es la estructura más importante y su lesión es reconocida como esencial para que se origine una luxación de patela. Objetivo: La reconstrucción del LPFM con la técnica de doble haz fijada anatómicamente y verificada por artroscopia pretende acercarse a los parámetros actuales de reconstrucción. Este estudio busca describir los resultados del tratamiento quirúrgico con esta técnica en pacientes con luxación de patela tratados entre mayo de 2010 y mayo de 2016 mediante la aplicación de escalas funcionales establecidas en la literatura, para identificar la mejoría de los síntomas y la no recidiva un año posterior a finalizar el período de rehabilitación. Resultados: Se intervinieron 16 rodillas en 15 pacientes (una de las pacientes con compromiso bilateral), de las cuales el 81,2% fueron de sexo femenino. El tiempo entre la primera luxación y la intervención quirúrgica fue en promedio 11,81 meses. Al año se encontraron diferencias estadísticamente significativas para las escalas funcionales Lisholm, Tegner, y Kujala y no se realizaron reintervenciones durante el seguimiento. Conclusión: La reconstrucción del LPFM, con doble fijación en patela y fijación asistida por artroscopia, es una variación a la técnica abierta tradicional que, teniendo en cuenta parámetros anatómicos, se puede realizar de forma segura. La evolución clínica de los pacientes es satisfactoria teniendo en cuenta que las escalas funcionales medidas en estos pacientes mejoraron estadísticamente significativo con respecto a los puntajes iniciales.


Introduction: dislocation of the patella is a relatively frequent pathology, however, its handling is controversial. In the recent years the tendency to perform reconstructions that more closely resemble native anatomy has generated a renewed interest in reconstructions of the medial patellofemoral ligament (LPFM).1,2 Of the structures that maintain the patella in a suitable position, LPFM is the most important one and its lesion is recognized as essential for the origin of a patella dislocation.3 Objective: The reconstruction of the LPFM with the anatomically fixed and double-beam technique, which was verified by arthroscopy, aims to approach the current reconstruction parameters. This study looks at the results of the surgical treatment with this technique in patients with patellar dislocation treated between May 2010 and May 2016, through the application of functional scales established in the literature to identify the improvement of symptoms and non-recurrence, one year after the end of the rehabilitation period. Results: 16 knees were involved in 15 patients (one of the patients with bilateral involvement), of whom 81.2% were female. The time between the first dislocation and the surgery had an average of 11.81 months. There were found statistically significant differences for the Lisholm, Tegner, and Kujala functional scales and there were no interventions performed during the year of follow-up. Conclusion: The reconstruction of LPFM, with double fixation in patella and fixation assisted by arthroscopy is a variation to the traditional open technique, taking into account anatomical parameters can be performed safely. The clinical evolution of the patients is satisfactory considering that the functional scales measured in these patients improved compared to their initial scores.

ABSTRACT

El enfoque de la discapacidad constituye actualmente una perspectiva que pretende hacerse cargo de la incorporación de la dimensión social en la comprensión de la salud mental, y su hegemonía en Chile es visible en el diseño de las legislaciones y políticas públicas sobre salud mental durante los últimos 20 años. Sin embargo, tanto este enfoque de la salud mental como las respectivas legislaciones y políticas desarrolladas, presentan importantes dificultades para hacerse cargo de la situación de justicia y ciudadanía de este grupo de personas. Estas dificultades del enfoque se hacen particularmente evidentes cuando se requiere garantizar el ejercicio de una ciudadanía activa, ya que los aspectos relativos a la ciudadanía han demostrado tener un efecto terapéutico sobre las deficiencias o trastornos mentales. La urgente necesidad de una ley de salud mental para Chile demanda el desafío de diseñar una legislación que se haga cargo de las particularidades de la discapacidad mental, pero además la posibilidad histórica de incluir efectivamente la perspectiva de las personas con discapacidad mental y sus familias en la discusión de una nueva ley.


Nowadays, the disability approach is a perspective that proposes a way to deal with the incorporation of social dimension in mental health understanding. Its hegemony in Chile turns visible considering the design of mental health laws and public policies over the past 20 years. However, this approach to mental health as well as the legislations and policies developed and related to it, hardly take over the situation of justice and citizenship of this group of people. These difficulties are particularly relevant when the exercise of active citizenship is required, since citizenship has been proved to show a therapeutic effect on the deficiencies or mental disorders. The urgent need of a mental health law for Chile demands the challenge of designing a law which recognizes the particularities of mental disability, but it also offers the historical possibility to effectively include the perspective of people with mental disabilities and their families in the discussion for a new law.

Introducción: la crisis del método clínico es una preocupación constante para los profesionales de la salud porque esta comprende aspectos muy sensibles de la práctica médica y la preparación para su empleo se produce tempranamente en la formación de los estudiantes. Objetivo: determinar el desempeño en la aplicación del método clínico de los estudiantes de tercer año de la carrera de Medicina en la provincia Holguín. Métodos: se realizó un estudio descriptivo con los estudiantes del tercer año de la carrera de Medicina de la Universidad de Ciencias Médicas de Holguín en el curso escolar 2009-2010. La muestra estuvo integrada por 439 estudiantes, el 66,6% de la población estudiada. Se consideraron los resultados obtenidos por los estudiantes en los exámenes prácticos de la asignatura de Medicina Interna. Resultados: el 95% de los alumnos presentaron dificultades en el interrogatorio del paciente, el 92% en el examen físico y el 93,6% no pudieron realizar el diagnóstico correcto. En cuanto a los criterios de los profesores sobre la interrelación interrogatorio-examen físico, análisis complementarios, diagnóstico y conducta, el 46,7% lo consideró regular y el 23,3%, malo. Conclusiones: Los estudiantes del tercer año de la carrera de Medicina carecieron del dominio y aplicaron de manera deficiente el método clínico durante los exámenes prácticos de Medicina Interna.


Introduction: the crisis of the clinical method is a constant concern for all the health professionals as it turns around many sensitive aspects of the medical practice and takes place at an early stage in their formation as doctors. Objective: to determine the role of clinical method application by third-year students of Medicine Career of Holguín province. Methods: A descriptive study in the third-year medical students from the University of Medical Sciences in Holguín was conducted during the school year 2009-2010. The sample comprised 439 students representing 66.6% of the studied population. The results obtained in the practical examinations of Internal Medicine Subject were considered. Results: Ninety-five percent of the students had difficulties in interviewing the patients; 92% had problems in the physical examination and 93.6% had troubles in making the accurate diagnosis. As for the professors' assessment of the interrelation between interview-physical examinations, routine tests, diagnosis and treatment: 46.7% considered it fair and 23.3%, bad. Conclusions: the mastery and the application of the clinical method during the practical examinations of Internal Medicine are deficient, since the students were unable, through the interview and the physical exam, to make a correct diagnosis or develop a logical reasoning of diagnosis-treatment.

ABSTRACT

Objective: to evaluate voluntary intake of four forage species by collared peccary (Pecari tajacu) under confinement. Methods: four peccaries underwent a consumption test to assess their preference among four forages (Leucaena leucocephala, Guazuma ulmifolia, Brosimum alicastrum and Pennisetum purpureum). The experiment was divided into two phases. The first consisted of an adaptation period of 4 days during which they were offered 1 kg of each forage in different feeders for 4 hours, then removing and weighing the remaining forage to determine consumption. The voluntary intake test was assessed in the second phase using a Latin square design (4x4). Forage intake was analyzed using the Statgraphics 5.1 software. Results: highly significant differences (p <0.01) between consumption of each forage were observed, being G. ulmifolia the most consumed (170.18 g dry matter), followed by L. leucocephala, and B. alicastrum (132.19 and 98.37 g dry matter, respectively). The least consumed was P. purpureum (21.65 g dry matter). Conclusions: considering its high consumption and moisture content, G. ulmifolia could be successfully used as cut fodder for peccaries under confinement.


Objetivo: evaluar el consumo voluntario de cuatro especies de plantas forrajeras por el pecarí de collar bajo confinamiento (Pecari tajacu). Métodos: cuatro pecaríes fueron sometidos a una prueba de consumo en cautiverio para evaluar su preferencia entre cuatro sustratos forrajeros (Leucaena leucocephala, Guazuma ulmifolia, Brosimum alicastrum y Pennisetum purpureum). El experimento se dividió en dos fases. La primera consistió en un periodo de adaptación de 4 días, durante los cuales se les ofreció 1 kg de cada forraje en diferentes comederos durante 4 horas, retirando luego el forraje y pesando el remanente para determinar su consumo. La prueba de consumo voluntario se ejecutó en la segunda fase mediante un diseño de cuadrado latino (4x4). El consumo de forraje se analizó mediante el software Statgraphics 5.1. Resultados: se encontraron diferencias altamente significativas (p<0.01) entre los consumos de cada forraje, siendo G. ulmifolia el más consumido (170.179 g materia seca), seguido de L. leucocephala, y B. alicastrum (132.19 y 98.37 g de materia seca, respectivamente), siendo P. purpureum el menos consumido (21.65g materia seca). Conclusiones: por su alto consumo y gran contenido de humedad, G. ulmifolia podría utilizarse exitosamente como forraje cortado para pecaríes bajo confinamiento.

A prova de consumo voluntário executou-se na segunda fase mediante um desenho de quadrado latino (4x4). O consumo de forragem analisou-se mediante o software Statgraphics 5.1. Resultados: Foram encontradas diferenças altamente significativas (P <0.01) entre o consumo de cada forragem, sendo G. ulmifolia o mais consumido (170.179 g de matéria seca), seguido por L. leucocephala e B. alicastrum (132.19 e 98.37 g de matéria seca, respectivamente), e P. purpureum o menos consumido (matéria seca 21.65g). Conclusões: Devido ao consumo elevado e ao alto teor de umidade, G. ulmifolia poderia ser utilizado com sucesso como forragem cortado para catetos em confinamento.

Este artículo presenta los fundamentos de una plataforma web multidispositivo que favorece la colaboración y formación nutricional de los diferentes agentes involucrados en los procesos de cuidado y educación de los niños (padres, educadores, especialistas), haciendo uso de los últimos avances producidos en el campo de las tecnologías semánticas para mejorar la precisión y adecuación de las recomendaciones realizadas automáticamente por el sistema.


A good diet is one of the pillars for ensuring the proper physical and intellectual development of children. In a society in which schooling is initiated at increasingly early ages, the coordination between school and family acquires a vital role in assuring proper child nourishment and care. This article presents the foundations of a multi-device web platform that fosters the collaboration and nutritional training of the different actors (parents, educators, specialists) involved in the processes of care and education of children. The platform uses the latest advances in the field of semantic technologies to improve the accuracy of the recommendations made automatically by the system.

Se realizó un estudio descriptivo, y de corte transversal, con el propósito de investigar la presencia del hongo Phymatotrichopsis omnívora como agente etiológico , del alerta epidemiológica denunciada en un medio de comunicación regional, la cual le atribuyo a este hongo el deterioro de la cosecha del durazno, conocida por los pobladores como secason y algunas lesiones de piel en pacientes adultos y pediátricos de los caseríos, Gabante, área urbana y el Cedral, del Municipio Tovar, Estado Aragua; Venezuela. Se examinaron 144 pacientes con lesiones de piel sospechosos de micosis superficial que acudieron a la consulta de los centros de salud de la zona. Se realizó examen de piel y faneras, examen directo; cultivos para hongos y biopsia de piel los cuales fueron procesados en el laboratorio del Hospital Central de Maracay y en el Servicio de Anatomía Patológica del Instituto Autónomo de Biomedicina, Caracas. Los datos básicos se registraron en ficha epidemiológica; se procedió al análisis estadístico; encontrándose entre las 10 primeras patologías dermatológicas: Pitiriasis alba (18,75%), Queratosis Folicular (11,1%), Quiste Millium (10,4%), Acné (6,25%),Prurigo Infantil (6,25%), Picaduras de Insectos (5,5%),Dermatitis Seborreica (4,9%), Micosis Superficial (4,16%), Urticaria (4,16%), Melasma (3.47%), otras (2.7%). De origen micótico encontramos, dos con Pitiriasis Versicolor cuyo agente etiológico fue Malassezia furfur; dos Onicomicosis, uno Tiña Corporal y uno con Tiña Inguinal; aislándose en los cultivos Trichophytum rubrum. En conclusión: de 144 personas examinadas ninguna presentó lesiones atribuidas al hongo Phymatotrichopsis omnívora.


It was made a cross-sectional descriptive, with the purpose of investigating the presence of the mushroom Phymatotrichopsis omnivore as an etiological agent, of an epidemic alert reported on a regional communications medium; which attributed to this fungus deterioration peach crop, known to residents as 'secasón' and some skin lesions in adult and paediatrics patients in the hamlets, Gabante, urban and Cedral, Tovar Municipality, Aragua; Venezuela. We examined 144 patients with skin lesions suspected of superficial fungal infections and they were seen at the health centers in the area. Examination was made of skin and appendages, direct examination, fungal cultures and skin biopsies which were processed in the laboratory of the Central Hospital of Maracay and the Pathology Department of the Autonomous Institute of Biomedicine, Caracas. Baseline data were recorded in epidemiological record, we proceeded to statistical analysis to be among the top 10 dermatological diseases: Pityriasis Alba (18.75%), follicular keratosis (11.1%), cyst Millium (10.4%) Acne (6.25%), Child Prurigo (6.25%), insect bites (5.5%), seborrheic dermatitis (4.9%) Superficial Mycoses (4.16%), urticaria (4.16 %), melasma (3.47%), other (2.7%). Of fungal origin found, two with Pityriasis Versicolor whose etiologic agent was Malassezia furfur; two onychomycosis, one Corporal and one Tinea jock itch, isolated from cultures Trichophytum rubrum. In conclusion: of 144 people tested had no injuries attributed to the fungus Phymatotrichopsis omnivorous.

ABSTRACT

Esta guía tiene el propósito de reunir en un único documento conceptos que pudieran ser utilizados por los diferentes actores involucrados en el saneamiento básico de las zonas rurales. La guía fue preparada respondiendo al pedido de las autoridades del país, tanto del nivel nacional como local, que quieren hacer la diferencia en atender a las zonas rurales del país. Tales zonas, de acuerdo a la Encuesta Nacional Materno-Infantil 2007-2009 son las más vulnerables y tienen aún los mayores rezagos en el acceso a agua y saneamiento básico. Consideramos, por tanto, que este instrumento apoya el proceso de disminución de equidades, aportando directamente a los problemas de grupos de mayor vulnerabilidad y en rezago frente al resto de la población guatemalteca.

Rhodnius pallescens is the main vector of Trypanosoma cruzi in Panama and one of the most relevant secondary vectors in Colombia. Despite the importance of this species, there is limited knowledge about the genetic variability along its geographical distribution. In order to evaluate the degree of karyotype variability we analyzed the meiotic behavior and banding pattern of the chromosomes of 112 males of R. pallescens coming from different regions of Colombia and Panama. Using the C-banding technique we identified two chromosomal patterns or cytotypes characterized by differences in the amount, size and distribution of constitutive heterochromatic regions in the chromosome complement (2n = 20 autosomes plus XY in males). The individuals can be easily classified in each cytotype by the analysis of the chromosomes during first meiotic prophase. The frequencies of the cytotypes are variable according to the geographic origin of the populations. This chromosomal divergence together with morphological data supports the existence of three genetically different populations of R. pallescens and provides new information to understand the distribution dynamics of this species.
