ABSTRACT

Aim To study the hypnotic effect and safety of compound anshen essential oil. Methods Gas chromatograph-mass spectrometer (GC-MS) was used to analyze the main active components of compound anshen essential oil. The mouse model of insomnia was established by intraperitoneal injection of para-chloro-phenyl alanine ( PC PA ) , combined with pentobarbital sodium sleep experiment and EEG characteristic monitoring in rats to study the hypnotic effect and mechanism. The safety of compound anshen essential oil was evaluated by acute toxicity test, skin irritation/allergy test and 90-day repeated administration toxicity test. The clinical effect and safety were evaluated by using the sleep monitoring technology for micro-motion sensitive mattress. Results Four components, including Atractylone (34.61%), (+) -Limonene (17.80%) , Linalool (11.63%), and Ocimene (11.67%) , were detected as the main active components of compound anshen essential oil. Compound anshen essential oil in-halation administration for seven days could effectively reduce the autonomic activity of insomnia mice, shorten the sleep latency (P <0.05) , improve the sleep duration, increase of neurotransmitters such as 5-hydroxy tryptamine (5-HT) and -γ-aminobutyric acid (GABA) in brain of mice with insomnia, and the medium dose group had better hypnotic effect. There was no death or adverse reaction in the safety evaluation test. The sleep balance index of 10 subjects with difficulty in falling a-sleep significantly increased (P <0.05), sleep latency was significantly shortened (P <0.05) , total sleep duration and sleep efficiency were improved, and no ad¬verse reactions were found after using the compound anshen essential oil for two days. Conclusions The compound anshen essential oil developed by the research team is safe and effective in relieving sleep disorders, which may be closely related to the co-regulation of the levels of neurotransmitters such as 5-HT and GABA by the four main active components.

ABSTRACT

@#AIM: To observe the clinical therapeutic efficacy of Ahmed glaucoma valve(AGV)implantation with intravitreal injection of aflibercept or ranibizumab in patients with neovascular glaucoma(NVG). <p>METHODS: A retrospective study. The clinical data of 33 cases(33 eyes)with NVG angle-closure glaucoma who were received intravitreal injection of aflibercept or ranibizumab with AGV implantation from January 2018 to August 2020 in our hospital were involved in this study. There were 18 eyes in the aflibercept group and 15 eyes in the ranibizumab group. All patients were followed-up for 6mo. The best-corrected visual acuity(BCVA), intraocular pressure, neovascularization, and postoperative complications were recorded and evaluated.<p>RESULTS: Before the first intravitreal injection and 1wk after intravitreal injection, the differences of intraocular pressure and BCVA in two groups were not statistically significant(<i>P</i>>0.05); The changes of intraocular pressure and BCVA before the first intravitreal injection and after AGV implant 1wk, 1, 3, 6mo were statistically significant in two groups(<i>P</i><0.01), but there were no statistically significant differences intraocular pressure and BCVA between the two groups at each follow-up time point(<i>P</i>>0.05). There were no significant differences in average number of injections between aflibercept group(1.61±0.98)and ranibizumab group(1.80±0.86)(<i>P</i>>0.05). The differences of neovascular recurrence rate, incidence of early and late complications and surgical success rates were not statistically significant(<i>P</i>>0.05).<p>CONCLUSION: Intravitreal aflibercept or ranibizumab injection combined with AGV implantation is an effective treatment for NVG. The clinical efficacy of aflibercept and ranibizumab were similar.

ABSTRACT

@#AIM: To analyze the epidemiological characteristics of patients hospitalized for ocular trauma during the novel coronavirus(COVID-19)epidemic. <p>METHODS: A retrospective case analysis. Totally 876 cases(905 eyes)of ocular trauma were analyzed in this study, including 545 cases(565 eyes)from January to June 2019 were enrolled ascontrol group, and 331 cases(340 eyes)from January to June 2020 were enrolled in the research group. Information regarding age, sex, location of injury, cause of injury, place of residence, and type of injury were collected and analyzed.<p>RESULTS: The number of hospitalisation for ocular trauma in the research group decreased by 39.3% compared with the control group. The age distribution of patients showed a double peak, at 0-10 years(20.55%, 21.45%)and 41-50 years(17.98%, 19.03%)in the control group and research group, respectively. There was no statistically significant difference in the age distribution between the two groups(<i>P</i>=0.907). Men were predominantly injured in both the control and the research groups, accounting for 80.37% and 83.69%, respectively(<i>P</i>=0.219). The proportion of home injuries in the research group(28.40%)was higher than the control group(11.38%)(<i>P</i><0.001). The proportion of mechanical eye injuries in the research group(98.19%)was higher than the control group(95.60%)(<i>P</i>=0.041), and the proportion of open globe injuries(85.54%)was also higher than the control group(76.58%)(<i>P</i>=0.001). The proportion of patients with no light perception in the research group(18.86%)was higher than that of the control group(9.53%)(<i>P</i><0.001).<p>CONCLUSION: During the COVID-19 epidemic period, the total number of ocular trauma cases was significantly reduced. Mechanical eye injuries are still the main type for hospitalisation of patients with eye injuries. The awareness of eye injury prevention should be strengthened during the period of epidemic.

ABSTRACT

Objective@#This study aimed to determine the effect of radiotherapy (RT) on the risk of herpes zoster (HZ) in patients with gynecological cancers via a nationwide population-based study. @*Methods@#Based on patient data obtained from the National Health Insurance Research Database, 1928 gynecological cancer patients were identified with 1:1 matching for RT and non-RT cohorts by age, index date, and cancer type. Another cohort consisting of 964 noncancer individuals matched was used as normal control. The incidence of HZ was compared between cancer patients with and without RT. Age, comorbidities, cancer-related surgery and chemotherapy (CT), and cancer type were adjusted as confounders. @*Results@#The risk of HZ in cancer patients was higher than that of non-cancer individuals (14.23 versus 8.34 per 1,000 person-years [PY], the adjusted hazard ratio [aHR]=1.38, p=0.044). In the cancer population, the incidence of HZ for the RT and non-RT cohorts was 20.55 versus 10.23 per 1,000 PY, respectively (aHR=1.68, p=0.009). Age >50 years was an independent factor for developing HZ. The 5-year actuarial incidence for patients receiving neither RT nor CT, RT alone, CT alone, and combined modalities was 5.4%, 6.9%, 3.7%, and 9.9%, respectively (p<0.001). In the RT cohort, the risk rose rapidly in the first year, becoming steady thereafter. @*Conclusion@#This population-based study showed that gynecological cancer patients receiving RT combined with CT had the highest cumulative risk of HZ. Health care professionals should be aware of the potential toxicities.

ABSTRACT

Objective@#To learn the activities of daily living(ADL)of the elderly in Chongqing and its influencing factors,so as to provide basis for developing ADL intervention measures for the elderly. @*Methods@#Multi-stage sampling method was used to select the elderly aged 65 years or over from 2-3 communities each in 11 districts or counties of Chongqing. They were surveyed by ADL scale including physical activities of daily living(PADL)and instrumental activities of daily living(IADL). The influencing factors for ADL disability were analyzed by multivariate logistic regression models.@*Results@#Totally 1 341 valid questionnaires out of 1 400 distributed were recovered,with a response rate of 95.79%. There were 596(44.44%)people with ADL disability,among whom 321(53.86%)cases were mild and 275(46.14%)cases were apparently impaired. There were 385(28.71%)suffered from PADL and 575(42.88%)suffered from IADL. The results of multivariate logistic regression analysis showed that age of 70 years old and over(OR:2.858-7.507,95%CI:2.119-11.816),not living in downtown(OR=2.073-3.709,95%CI:1.434-5.613),having commercial insurance(OR=3.202,95%CI:1.419-7.223)and suffering from at least one chronic disease(OR:2.909-5.629,95%CI:1.562-10.681)were the risk factors for ADL disability in the elderly,while having a diploma or above(OR=0.236,95%CI:0.090-0.621),employing a caregiver(OR=0.466,95%CI:0.260-0.835)and monthly income of 1 000 to less than 4 000 yuan(OR:0.480-0.664,95%CI:0.274-0.923)were the protective factors.@*Conclusions@#The rate of ADL impairment is 44.44% in the elderly aged 65 years or over,and is associated with old age,living away from downtown,participation in commercial insurance and suffering from chronic diseases.

ABSTRACT

Objective To investigate the clinical features of multiple ligament injuries of the knee joint, and evaluate the clinical efficacy of one-stage repair and reconstruction under an arthroscope of multiple ligament injured knee joint. Methods The clinical data were analyzed retrospectively of 22 patients receiving treatment and follow-up for multiple ligament injuries of the knee joint from March 2015 to June 2017 in the Henan Orthopedic Hospital. 13 males and 9 females aged from 24 to 64 years; and left knee injury occurred in 7 cases, right knee in 5 cases, all injuries were close with knee subluxation or dislocation. Imaging, clinical and arthroscopical examination showed anterior and posterior cruciate ligament ruptures in all the patients. The time from injury to operation was from 4 to 16 days, all the patients were treated by single tunnel and single beam reconstruction of anterior and posterior cruciate ligament under an arthroscope, limited incision and reconstruction of PMC (posterior medial complex) and PLC (posterior lateral complex); and the concurrent injuries were treated at that same time. Postoperative X-Ray and MRI were performed to determine the healing of bone tunnel, internal fixation, remodeling of reconstructed tendon and tendon healing. The knee function was assessed on the basis of the range-of-motion of the knee, IKDC score and Lysholm Knee Function score. Results All the patients were followed up from 6 to 14 months with an average of (11.0±1.5) months. The torn inner and outer meniscus and fracture were all healed and the deep venous thrombus was dissolved. Last follow up found varus and valgus stress test was normal or close to normal in 22 patients by Lachman test, anterior and posterior drawer tests were negative, and the tibial shift distance forward or backward was smaller than 5mm. The patients had no subjective symptoms. X-Ray and MRI showed that the tibial femoral tunnel had healed, and the position of internal fixation had no change, and the ligament had been reconstructed and the tendon had healed. In 2 cases, one year later, sensory and motor recovery was observed after common peroneal nerve injury. Range of motion of the knee joint, and Lysholm and IKDC scores were significantly superior at the last following up to at the preoperation (P<0.01); and IKDC comprehensive assessment revealed a normal result (A) in 4 cases, close to normal (B) in 16, abnormal (C) in 2, whereas they are significantly abnormal (D) at admission. Conclusion One-stage arthroscopical repair and reconstruction for multiple ligament injuries of the knee can obviously stabilize the knee joint, improve the knee joint function early, prevent postoperative complications, shorten the hospitalization time, reduce the cost, improve the quality of life, and have the advantages of safety, reliability and less trauma, and the clinical efficacy is satisfactory.

ABSTRACT

Objective To explore the application value of chromosomal microarray analysis(CMA)technolo-gy in children with abnormal development at the endocrine clinic,and to summarize the data of diagnosis and treatment. Methods A retrospective analysis of 15 children with abnormal development was performed at the endocrinology clinic of Guangzhou Women and Childrenˊs Medical Center from January 2015 to December 2017. The whole genome CMA was applied according to the standard operation procedure of CytoScan 750 arrays of Affymetrix,USA. The results were analyzed by chromosome analysis suite( CHAS)software and related bioinformatics methods. Results The report on CMA showed that the genomes of 15 children had the pathogenic copy number variation(CNVs)or variants of uncer-tain significance. The chromosomal abnormalities were consistent with the clinical manifestations of all children. There were deletions in 14 cases and duplications in 3 cases. Among the 15 cases,loss of heterozygosity was found in 2 cases, uniparental disomy in 1 case,trisomy in 2 cases,Turner syndrome in 2 cases,Smith-Magenis syndrome in 1 case,and wolf Hirschhorn syndrome in 1 case. Only 2 of 15 children were diagnosed as chromosomal abnormalities by routine kar-yotype analysis. Conclusions The whole genome high resolution CMA can significantly improve the rate of diagnosis in children with abnormal development at endocrinology clinic,and is worthy of recommendation.

ABSTRACT

Objective@#To compare the effect of two different methods of "3-PR" participatory health education and traditional distribution of publicity materials on health literacy improvement of medical students, to provide a reference for making more effective measure to improve health literacy.@*Methods@#173 Second-grade students in nursing major were selected in a secondary vocational school in Datong City. Two different intervention methods were used to intervene for 8 weeks. The experimental group (92 students) received "3-PR" participatory health education; the traditional health education intervention in the control group(81 students). The "National Residents’ Health Literacy Monitoring Questionnaire 2015" was used to conduct a questionnaire survey on the experimental group and the control group before and after the intervention, and to compare the effects before and after the intervention.@*Results@#The average score of health literacy was (36.04±9.43) points and (36.01±10.17) points before and after intervention in the control group; (35.78±8.91) points and (49.53±13.53) points before and after intervention in the experimental group; No statistical difference between experimental and control group was found before intervention(t=0.18, P=0.86); There was no significant difference in health literacy score before and after intervention for the control group(t=0.03, P=0.98); Health literacy significantly increased in the experimental group after intervention(t=-11.36, P=0.00). Adequate health literacy accounted for 3.70% and 4.94% before and after intervention in the control group and 4.35% and 45.65% in the experimental group before and after intervention. No significant difference was found in adequate health literacy proportion between the two groups before intervention(χ2=0.00, P=1.00). However, the difference showed statistically significant after intervention(χ2=36.58, P=0.00). The change of health literacy score in the control group and the experimental group was (-0.02±7.52) and (12.75±10.77), respectively, accounting for 1.24% decrement and 41.30% increment. The difference between the two groups was statistically significant (t=-8.93, P=0.00). The dimension-and type-specific score of health literacy showed similar pattern.@*Conclusion@#The "3-PR" participatory health education model is more effective than the traditional health education method in improving health literacy, and it could be carried out by multi-disciplinary students.

ABSTRACT

Objective@#To investigate the pathologic features of gonadal tissues of disorders of sexual development (DSD) in children.@*Methods@#Fifty-three cases of gonadal developmental disorders were collected from July 2015 to August 2017 at Guangzhou Women and Children′s Medical Center. Clinical manifestations, karyotypes, sex hormone levels, ultrasound imaging, histology and immunophenotype of gonadal tissues were analyzed.@*Results@#The age of patients ranged from 7 months to 17 years with an average of (50.7 ± 47.1) months. Social genders of the patients included 32 males and 21 females. Forty-eight patients had abnormal sex hormone levels. Clinical presentations included: toward female genitalia in 25 cases, male genitalia tendency in 17 cases and ambiguous external genitalia in 11 cases. Hypospadias was seen in 31 cases and short stature was seen in 8 cases. Chromosomal karyotyping of peripheral blood revealed 23 cases of sex chromosome disorders, 22 cases of 46 XY disorders, of which 3 cases were 5α-reductase deficiency and 8 cases of 46 XX disorders. Ultrasound examination showed cryptorchidism in 30 cases, including 16 cases of unilateral, 14 cases of bilateral and 1 case presenting a huge pelvic tumor. A total of 97 gonadal tissues from 53 cases of DSD were examined, including 9 cases of unilateral and 44 cases of bilateral gonads. Microscopically, 55 gonads (56.7%) showed dysplastic testes including 17 unilateral and 19 bilateral gonads. Fourteen were streak gonads (14.4%) including 8 unilateral and 3 bilateral gonadal tissues. Nine streak gonad with epithelial cord-like structures (9.3%) were found, of which 5 were unilateral and 2 were bilateral lesions. Seven gonads were ovotestis (7.2%), unilateral in 5 cases (the other side of the gonads of ovary in 4 cases, 1 case of dysplastic testes) and bilateral in 1 case. Seven gonads showed follicular-rich ovarian tissue (7.2%). One case showed bilateral dysplastic testes with gonadoblastoma and ectopic adrenal cortex. One case of streak gonad showed epithelial cord-like structures and undifferentiated glandular tissue embedded in malignant mixed germ cell tumors (mixed gonadoblastoma, dysgerminoma, mature teratoma and yolk sac tumor). One case had testicular microlithiasis. Uterus and fallopian tube structures were found in 11 cases. Immunohistochemical stains were performed in 15 cases. D2-40, PLAP and CKIT were expressed in germ cells and Calretinin, WT1 and inhibin were positive in Setoli cells. SALL4 and OCT3/4 were positive in 3 cases. Inhibin highlighted interstitial Leydig cells in 2 cases. GPC3 was positive in yolk sac tumor component.@*Conclusions@#Gonadal dysgenesis presents a broad spectrum of gonadal phenotypes with variable degrees of differentiation. The development of bilateral gonadal tissues has certain variability. Chromosomal karyotypes have no correlation with gonadal phenotypes. Accurate histopathologic diagnosis of gonadal dysgenesis plays an important role in the treatment and prognosis of the patient.

ABSTRACT

<p><b>OBJECTIVE</b>To explore clinical effect of arthroscopic cyst removal and wire-guided suture for the treatment of lateral meniscal cyst of knee joint.</p><p><b>METHODS</b>From July 2014 to December 2017, 33 patients with lateral meniscal cyst of knee joint were treated by arthroscopic cysts removal and wire-guided suture, including 13 males and 20 females, aged from 20 to 55 years old with an average age of(36.23 ±2.30) years old, the courses of disease ranged from 3 to 14 months with an average of(4.60±0.83) months; Preoperative MRI examination was clear diagnosed. There were 14 cysts on anterior horn, 18 cysts on meniscal body and 1 cyst on posterior horn;all cysts were solitary, and 3 of them were multilocular. Lysholm score and GLASOW score of knee joint function and clinical efficacy were observed before and after operation at 6 months.</p><p><b>RESULTS</b>All patients were followed up form 6 to 24 months with an average of (7.5±1.2) months. Preoperative symptoms disappeared or significantly alleviated, and all incisions were healed by intention without complication and neurovascular injury. MRI showed meniscal tear areas and cystic defective areas healed, cyst was not recurrenced, healing time ranged form 8 to 12 weeks with an average of (9.6±1.6) weeks, and patients recovered their daily life and exercise. There was significant difference in Lysholm score before operation (61.12±4.35) and after operation at 6 momths(91.32±3.36)(=46.11, <0.01);according to GLASOW assessment, 31 patients with excellent recovery, and 2 good.</p><p><b>CONCLUSIONS</b>Arthroscopic cyst removal and wire-guided suture for the treatment of lateral meniscal cyst of knee joint could reserve meniscus, repair injury of meniscus, recover knee joint function after operation, and is worth popularizing.</p>

ABSTRACT

Objective To investigate the relationship between the fasting blood glucose level and the severity of coronary artery disease among the Kazakh population in Xinjiang. Methods A total of 411 Kazakh patients with no known history of diabetes mellitus were enrolled in this study for highly suspected of coronary artery disease(CAD) and subesquently underwent coronary artery angiography. Coronary artery disease was diagnosed in 280 patients after coronary artery argiography. The severity of the disease was expressed in terms of Gensini score. All subjects were classified into 4 groups according to the level of fasting plasma glucose (FPG)< 5.6 mmol / L, 5.6-6.1 mmol / L, 6.1-7.0 mmol / L and ≥7.0 mmol / L. All the biochemical parameters, clinical data and coronary artery angiographic data were compared, and the possible risk factors related to the severity of coronary artery disease were studied. Results Multivariate logistic regression analysis showed that FPG was significantly associated with coronary artery disease(OR 1.53, 95% CI 1.25-1.87,P<0.001).The results showed that the prevalence of angiographic CAD and the Gensini score increased with the increasing FPG levels among the four groups.The FPG level was significantly correlated with the Gensini score (P<0.001). Conclusions FPG is significantly associated with coronary artery disease and its severity in the Kazakh population in Xinjiang.

OBJECTIVE: To study the influence and mechanism of bioactive ingredients of Ligusticum chuanxiong Hort. on the transport of gastrodin based on cell culture model in vitro. METHODS: Cell toxicity of gastrodin, ligustilide, senkyunolide I and senkyunolide A were detected by MTT assay. The transport mechanism of gastrodin and the influence of ligustilide, senkyunolide I and senkyunolide A on the transport of gastrodin were studied in MDCK-MDR1 monolayer cells. The changed expressions of P-gp caused by ligustilide, senkyunolide I and senkyunolide A were analyzed by Western blotting. RESULTS: Gastrodin showed relatively poor absorption in MDCK-MDR1 cells for its apparent permeability coefficients were less than 1×10-6 cm·s-1. P-gp inhibitor made the Papp(B→A)/Papp(A→B) of gastrodin reduced from 1.29 to 0.79, which indicated that the transport of gastrodin was influenced by P-gp. In the presence of ligustilide(30 μg·mL-1) or senkyunolide I(120 μg·mL-1), the Papp(A→B) of gastrodin in MDCK-MDR1 were significantly increased(P<0.01). In the presence of senkyunolide A(120 μg·mL-1), the Papp(A→B) of gastrodin in MDCK-MDR1 were markedly increased(P<0.05). High, medium and low dose of ligustilide, senkyunolide A and senkyunolide I could significantly inhibit the expression of P-gp protein. CONCLUSION: The results indicate that the transport mechanism of gastrodin might be passive diffusion as the dominating process with the active transportation mediated mechanism involved. Ligustilide, senkyunolide A and senkyunolide I increase the transport of gastrodin attribute to down-regulate P-gp expression.

ABSTRACT

Purpose To study the clinicopathological features of congenital solitary intestinal fibromatosis (SIF).Methods Clinical,radiological and pathological features of 2 cases of SIF were analyzed along with a literature review.Results Two cases of SIF with one girl and one boy.Both presented with vomiting and abdominal distention at the birth day.A plain abdominal radiograph revealed small bowl obstruction and pneumoperitoneum.Jejunum atresia and perforation were detected at surgery.Microscopically,spindle tumor cells diffusely infiltrated in the whole intestinal wall,tumor cells were monomorphic with bland looking and rare mitosis was reviewed.Immunohistochemically,vimentin was positive while muscular markers as SMA,desmin and Myogenin were all negative.And S-100,CD117,CD34,ALK and β-catenin were not expressed.ETV6 split gene was not detected in one of the patients by FISH.Two patients of SIF had no tumor recurrence with follow-up till now.Conclusion Congenital SIF is a rare and unique intestinal tumor in neonate,and it should be differentiated from congenital fibrosarcoma and myofibroma.SIF appears a very good prognosis after segmental resection.

<p><b>OBJECTIVE</b>To explore the clinical efficacy of double-plate fixation for the treatment of old tibial plateau fractures with Schatzker type IV through anterior midline and posteromedial approaches.</p><p><b>METHODS</b>From July 2013 to July 2015, 15 patients with old tibial plateau fractures were treated with internal fixation using locking reconstructive plate for the posteromedial fragment and anatomical locking plate for anteromedial fragment through antero midline and posteromedial approaches. There were 9 males and 6 females, with an average age of 49.2 years old (ranged, 21 to 61 years old). Eight patients had injured in the left side and 7 in the right side. According to Schatzker classification, all patients were type IV. The mean interval from injury to operation was 26.5 days (ranged, 21 to 65 days). The main clinical symptoms before operation were knee joint swelling, pain, deformity and limitation of motion. The X-ray and CT confirmed the fracture type. The indexes such as tibial plateau tibial shaft angle (TPA), femoral tibial angle (FTA) and posterior slope angle (PSA) were compared between immediate postoperation and final follow-up using postoperative X-ray film. The knee functions were evaluated using the HSS (Hospital for Special Surgery) knee score system.</p><p><b>RESULTS</b>Two patients had incision complications which healed by correct treatment, 1 patient had traumatic arthritis. All patients were followed up for mean 16.6 months (ranged, 13 to 24 months). No infections, deep venous thrombosis, implant loosening and breakage, fragment displacement, plateau surface collapse and bone nonunion found. The bone union time ranged from 3 to 8 months (mean 6.07 months) after operation. The average immediate postoperative value of TPA, FTA and PSA were(86.81±1.67)°, (168.00±3.29)° and(10.20±1.47)° respectively; and(86.47±1.67)°, (168.53±3.03)° and (10.54±1.21)° respectively at the final follow-up evaluation, showing no statistical differences(>0.05). According to the HSS score system, 26.33±3.86 in pain, 20.00±1.79 in function, 16.00±1.55 in range of motion, 8.67±0.94 in muscle strength, 8.53±1.67 in flexion deformity, 9.33±0.94 in joint stability, and the total mean score was 88.86±8.92. The outcomes were excellent in 10 cases, good in 4, and fair in 1.</p><p><b>CONCLUSIONS</b>Double-plate fixation via combined anterior midline and posteromedial approaches is an ideal surgical method for old tibial plateau fractures with Schatzker IV type, showing satisfactory exposure, reliable reduction and fixation, and benefiting for early functional exercise. The short-term clinical results was satisfactory.</p>

ABSTRACT

Objective To observe the therapeutic effect of bone cement injectable pedicle screw system combined with intervertebral fusion for lumbar spondylolysis and osteoporosis. Methods The clinical data were analyzed retrospectively of 21 patients with lumbar spondylolysis and osteoporosis who received treatment of bone cement injectable pedicle screw system and interv ertebral fusion from Aug. 2013 to Nov. 2015. The 21 patients (9 males and 12 females) aged from 60 to 80 years (mean 64 years old); 6 of them presented degenerative spondylolysis, 15 with isthmic spondylolisthesis; 2 cases had I degree slippage, 13 had II degree slippage, 6 had III degree slippage, and all the cases were unisegmental slippage including 9 cases in L4 and 12 cases in L5. Bone mineral density of lumbar vertebrae (L2-L5) was measured with dual-energy X-ray absorptiometry, and T values conforming to the diagnostic criteria of osteoporosis were less than or equal to -2.5; All patients were operated with whole lamina resection for decompression, bone cement injectable pedicle screws system implantation, propped open reduction and fixation intervertebral fusion. The clinical outcomes were determined by the radiographic evaluation including intervertebral height, height of intervertebral foramen, slip distance, slip rate and slip angle, and Oswestry disability index (ODI) on preoperative, 3 months after operation and the end of the time, and the interbody fusion were followed up. Results Cerebrospinal fluid leakage of incision was observed in two cases after operation, compression and dressing to incision, Trendelenburg position, dehydration and other treatments were taken, and the stitches of incisions were taken out on schedule. Slips in the 21 patients were reset to different extent, and lumbar physiological curvatures were recovered. The intervertebral height and height of intervertebral foramen were obviously higher 3 months after operation than that before operation (P0.05). Based on the ODI score, the excellent/fine rate was 90.6% and 92.5% for 3 months after operation and the last follow-up, respectively. During the period of following up, no loss of the correction degrees, no loosening of the screws, and 19 cases were noted bony fusion with the fusion success rate of 90.5%. Conclusion The treatment effect of bone cement injectable pedicle screw system and intervertebral fusion for lumbar spondylolysis and osteoporosis is satisfied.

ABSTRACT

Exercise-induced fatigue is a comprehensive response to a variety of physiological and biochemical changes in the body, and can affect people’s quality of life to different extents. If no timely recovery after occurrence of fatigue, accumulated gradually, it can lead to “burnout”, a “overtraining syndrome”, “chronic fatigue syndrome”, etc., which will cause endocrine disturbance, immune suppression, even physical illness. Exercise-induced fatigue becomes an important factor endangering human health. In recent years, many experts and scholars at home and abroad are committed to the research of exercise-induced fatigue, and have put forward a variety of hypothesis to explain the cause of exercise-induced fatigue. They expect to find out the methods for preventing and eliminating exercise-induced fatigue. This article discusses mainly the pathogenesis, model building, elimination/ relief, etc. of exercise-induced fatigue to point out the research achievements of exercise-induced fatigue and its existing problems.

ABSTRACT

Objective To observe the treatment effect of internal fixation with absorbable suture anchors for calcaneal apophysitis (Sever's disease). Methods The clinical data of 34 Sever's disease patients having received internal fixation with absorbable suture anchors from January 2010 to August 2012 were analyzed retrospectively. Of the 34 patients, 28 were male and 6 were female, aged from 11 to 30 years old (mean 19 years old); 20 with unilateral lesions, 14 with bilateral lesions; 32 suffered inflammatory edema, and 2 with epiphyseal avulsion. All of the patients were diagnosed as suffering from Sever's disease by X-ray combined with clinical symptoms. The sclerotic bone was chiseled off, the attachment site of Achilles tendon was fixed with auxiliary sutures of anchors and then fixed to the bone surface. Different rehabilitation programs were adapted in different periods after operation. The operation time, the site of anchorage and perioperative complications were analyzed, and the therapeutic effects were evaluated by Arner-Lindholm standard. Results All patients were followed up for 2-24 months (mean 9 months). The average operation time for one side was 40.5 minutes. No iatrogenic injury to nerve, vascular or tendon occurred during surgical procedure. No wound infection or foreign-body reaction was found after operation. The therapeutic effects evaluated by Arner-Lindholm standard were excellent in 29 cases, good in 4 and bad in 1 case. Conclusion Absorbable suture anchoring fixation is a reasonable and effective method for the treatment of Sever's disease, and it allows early exercise training for recovery of ankle function. The postoperative functional recovery is found to be satisfactory.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate clinical and pathological features of lung lesions in children.</p><p><b>METHODS</b>Clinical manifestations, radiologic imaging, histopathological features and immunohistochemical results were analyzed in 215 cases of lung lesions in children.</p><p><b>RESULTS</b>A total of 215 cases of lung lesions in children aged 0 day to 13 years (average age of 27.2 months and the median age of 18.0 months) were selected, including 137 male and 78 female patients with a male to female ratio of 1.76:1.00. The incidence of congenital lung disease was higher in patients of less than 1 year old than those of over 1 year old age, and the difference of the two groups was statistically significant (P = 0.004). 142 cases had acquired lung diseases, and 73 cases had congenital bronchopulmonary dysplasia. Lung abscess was the most common lesion seen in 86 cases (40.0%), including 1 case of fungal abscess. Congenital pulmonary airway malformation (CPAM) was the second most common, seen in 44 patients (20.5%), including 20 cases of type 1, 18 cases of type 2 and 6 cases of type 4 CPAM. Pulmonary sequestration was found in 25 cases (11.6%) including 14 cases of intralobar type and 11 cases of extralobar type. Two cases of extralobar pulmonary sequestration showed simultaneous CPAM2 type 2 lesion. Other lesions included tuberculosis (13 cases, 6.0%), emphysema (12 cases, 5.6%), interstitial pneumonia (7 cases, 3.2%), pulmonary hemorrhage (6 cases, 2.8%), bronchogenic cyst (4 cases, 1.9%), bronchiolitis obliterans (2 cases, 0.9%), idiopathic pulmonary hemosiderin deposition disease (2 cases, 0.9%) and 1 cases of lung non-specific changes. 13 cases of neoplastic lesions (6.0%) were found, of which 11 cases were primary tumors (5.1%), including inflammatory myofibroblastic tumor in 5 patients (2.3%), pleuropulmonary blastoma in 5 cases (1 case of type I, 2 type II and 2 type III) and 1 case of mucoepidermoid carcinoma (0.5%) and 2 cases of metastatic tumors (hepatoblastoma and Wilm's tumor, 0.9%).</p><p><b>CONCLUSIONS</b>Infectious diseases are the most common lung diseases in children. Congenital bronchopulmonary dysplasia is the most common in children of less than 1 year old. Malignant lesions are rare.</p>

ABSTRACT

Objective To explore the significance of MYCN gene amplification in children with neuroblastic tumors(NT).Methods The clinicopathological data of 154 cases with NT were reviewed,including general data, classification of pathology,clinical stage and prognosis.MYCN gene amplification was detected by fluorescence in situ hybridization(FISH) and its relationship between pathological characteristics and prognostic significance was analyzed.Results There was 154 cases of NT aged 1 day to 11 years,with a mean age of 26.1 months,and the median age of 20.5 months.Male and female ratio was 1.48 : 1.00.According to International Neuroblastoma Staging System (INSS) ,20 cases were of stage Ⅰ (13.0%) ,23 cases of stage Ⅱ (14.9%) ,43 cases of stage Ⅲ (27.9%) ,64 cases of stage Ⅳ(41.6%) and 4 cases of Ⅳs (2.6%).There were 72 cases(46.8%) with favorable histology,and 82 cases(53.2%) with unfavorable histology.MYCN amplification was found in 20 cases (13.0%) and the signal ratio of MYCN and chromosome 2 (CEP2) was 4.08-43.29.One hundred and thirty-four cases of MYCN non-amplification included MYCN gain in 91 cases(68.0%) ,MYCN negative in 43 cases(32.0%).MYCN expression showed the significant differences in ages, neuroblastoma type, international neuroblastoma pathology classification (INPC), mitosis karyorrhexis index (MKI), and clinical stages (all P < 0.05).No significant difference was found in gender(P > 0.05).Of 20 MYCN amplification cases,4 cases (20.0%) survived and 16 cases (80.0%) died,and the overall survival rate was 20.0% (4/20 cases) ,with survival time was (17.10 ± 2.24) months;of 134 MYCN non-amplification cases,96 cases (71.6%) survived and 38 cases (28.4%) died, with survival time of (28.71 ± 1.28)months.Survival analysis showed the cases with MYCN amplification had worse prognosis (x2 =19.596, P < 0.05).Conclusions Patients with MYCN amplification had poorer prognosis and lower incidence of MYCN amplification of pediatric NT was found in China.

ABSTRACT

Purpose To investigate the histopathological features of cystic lung diseases ( CLD) , and to discuss the timing of clinical interventions. Methods HE and immunohistochemical staining were performed and reviewed in 125 cases of CLD. Results 125 ca-ses of CLD aged from birth to 11 years and 6 month, with an average age of 23. 0 months, median age 15 months, of which 60 cases were less than 1 year (48. 0%). 75 cases were male and 50 cases female, with male to female ratio of 1. 5 ∶ 1. Grossly, 50 cases showed single or multiple cysts with the size 0. 5 ~8. 0 cm in diameter, which did not communicate with bronchial cavity. 18 cases showed honeycomb cysts with the diameter of 0. 1~2. 0 cm. 26 cases were solid lesions without visible cysts. 21 cases were observed lung abscess with thick and rough wall and pus inside. 7 cases of emphysema showed microcysts with crepitation. 2 cases were identi-fied cystic and solid masses, with fish-fresh like cut surface. Histopathologically, 94 cases (75. 2%) were related to congenital bron-chopulmonary dysplasia in 125 cases of CLD, in which there were 59 patients (47. 2%) of congenial pulmonary airway malformation (CPAM), including 29 cases of type 1 (49. 2%), 18 cases of type 2 (30. 5%), and 12 cases of type 4 (20. 3%), there were 26 ca-ses (20. 8%) of pulmonary sequestration, including 15 cases of intralobar type (57. 7%) and 11 of extralobar cases (42. 3%), 5 ca-ses were complicated with CPAM type 2, 8 cases were bronchial cyst (6. 4%) and 1 case of enteric cyst (0. 8%). Acquired lesions were detected in 31 cases (24. 8%), including 21 cases of infected lung abscess, 1 case of fungal abscess. 7 cases of emphysema, and 3 cases of pleuralpulmonary blastoma (typeⅠ1 case and typeⅡ2 cases). Conclusion Pediatric CLD is characterized as com-plexed categories. The prognosis depends on correct pathological diagnosis, combined with imaging evaluation and appropriate timing of surgery.
