ABSTRACT
As faringolaringectomias com esvaziamento cervical são um desafio para o cirurgião de cabeça e pescoço, envolvendo alta complexidade de execução técnica devido às alterações anatômicas, condições clínicas e estádio TNM, predispondo complicações no pós-operatório recente. Com o objetivo de diminuir o risco de complicações, utilizou-se a cola de fibrina na sutura faríngea e no campo operatório após o esvaziamento em um paciente submetido à faringolaringectomia total com esvaziamento cervical por carcinoma espinocelular de recesso piriforme. O uso da cola de fibrina no intra-operatório para a proteção de suturas na faringolaringectomia com esvaziamento cervical parece prevenir as complicações associadas ao método, porém, estudos prospectivos devem ser realizados para essa comprovação.
The pharyngolaryngectomies remain a challenge to the head and neck surgeon, with difficulties to operation due anatomic alterations, patient condition and TNM stage whose favor the inset of complications in the recent post operative. With the objective of decreasing the general complication risk we used the fibrin glue to protect the pharynx suture and the operative of neck dissection during the reconstruction after the pharyngolaryngectomy with bilateral neck dissection for hypopharynx carcinoma. The use of fibrin glue during the operative time for suture protection and also in the neck dissection seems to prevent the associated complications, although prospective randomized trials have to be done to assure the correct use.
