ABSTRACT

Las Prótesis totales convencionales aún cuando bien confeccionadas, pueden presentar deficiencias en la retención, estabilidad y soporte, además de la incomodidad y acumulo de alimentos. Las sobredentaduras sobre implantes fueron consideradas una alternativa para solucionar problemas y experiencias negativas con las prótesis convencionales, así también pueden ser una alternativa a los tratamientos complejos (quirúrgico/protésico) con prótesis fijas soportadas por implantes. Buscando reducir el período de cicatrización para la confección de las prótesis, las investigaciones mostraron altos índices de éxito para los tratamientos con sobredentaduras mandibulares inmediatas sobre implantes, que fueron utilizadas en la rehabilitación de pacientes edéntulos. Este trabajo tiene como objetivo abordar, por medio de una revisión de literatura, la viabilidad de la utilización de sobredentaduras mandibulares implanto retenidas. Los implantes de carga inmediata que retienen una sobredentadura mandibular que ha mostrado ser un método reproducible, previsible y de confianza para restablecer una eficiente función masticatoria a los pacientes totalmente edéntulos


Conventional complete denture even though well confectioned, could present deficiencies in the retention, stability and support, in addition uncomfortable and retention of food below it. The overdentures have been considered an alternative to resolve problems and negative experiences with the conventional dentures, thus also can be an alternative to the complex treatments (surgical/prosthetic) with implant-supported prosthesis. In the attempt to reduce the healing time for the confection of the dentures showed higher rates of success to the implants when immediate mandibular overdentures were used in the rehabilitation of edentulous patients. The aim of this paper was to discuss by means of a Literature review, the viability of the use of mandibular implant-supported overdentures. The immediate mandibular overdentures have been showed like a reproducible method, predictable and confidence to provide an efficient function to the total edentulous patients

ABSTRACT

El propósito de esta revisión fue explorar el concepto de Plataforma modificada en Implantología y su desempeño clínico. La definición de plataforma modificada será considerada junto con una revisión de literatura pertinente, con la finalidad de proveer directrices basadas en evidencias científicas. Fueron utilizadas las siguientes bases de datos: Pubmed, Biblioteca Cochrane e ISI. Los criterios de inclusión fueron: artículos clínicos, de laboratorio y de revisión sobre el tema, en lengua inglesa. Fueron seleccionados 40 artículos, utilizando los descriptores "plataforma switching dental implants", "platform shifting and dental implants", "expanded platform dental implant". De acuerdo con esta revisión, los implantes plataforma modificada presentan una tasa de remodelación ósea ( media < 1mm) más favorable que los implantes convencionales. Este hecho parece ser resultante de un conjunto de factores biológicos y mecánicos. El concepto de plataforma modificada tiene importante relevancia en el área estética


The purpose of this revision was to explore the concept of Platform Switching in implantology and its acting in the clinical practice. The concept of platform switching will be considered together with a revision of pertinent literature, with the purpose of providing guidelines based on scientific evidences. The following databases were used: Pubmed, Library Cochrane and ISI in the last 17 years. The inclusion approaches were: clinical articles, laboratorials and of revision on the topic, in English language. 41 articles were selected, using the describers "platform switching dental implants", "platform shifting and dental implants", "expanded platform dental implant". In accordance with this revision, implant them platform switching they present a rate of more favorable bony remodeling that you implant them conventional. This fact seems to be resulting of a group of biological factors and mechanics. The concept of platform switching has important relevance in the aesthetic area

ABSTRACT

The aim of this study was to use photoelastic models to analyze the distribution of stress caused by the incidence of loads on a mandibular distal extension removable partial denture, both on the abutment teeth and on differently shaped residual ridges: distal ascending, descending-ascending, horizontal and distal descending. The best type of retainer and location of the rest on the last abutment tooth were determined for the different types of ridge. Four models were made from photoelastic resin (PL-1 for the teeth and PL-2 for the alveolar ridge), one for each kind of ridge. For each model, 4 removable partial dentures (RPD) were made (16 RPD altogether): T-bar retainer and distal rest, T-bar retainer and mesial rest, circumferential retainer and distal rest, and circumferential retainer and mesial rest. The models were placed on a circular polariscope and a 100 N axial load (point load) was applied to premolars and molars of the RPD. The formation of photoelastic bands was photographed for qualitative analysis. Results showed that the horizontal ridge had better distribution of stress, while the distal descending ridge had greater concentration of stress. The circumferential retainer had greater areas of stress for all types of ridges except the horizontal ridge, where there was no influence related to retainer type. The distribution of stress was similar among the different types of ridges when the rest was mesial or distal to the last abutment tooth, except for the distal descending ridge, where there was greater concentration of stress when the rest was located distally to the last abutment tooth. Thus, it may be concluded that (1) the situation was least favorable for the distal descending ridge and most favorable for the horizontal ridge, (2) the T-bar retainer had more favorable stress distribution, except when the ridge was horizontal, in which case there was no influence in relation to the type of retainer, (3) the location of the rest showed similar behavior in all except the distal descending ridge.


O objetivo da pesquisa foi analisar, atraves de modelos fotoelasticos, a distribuicao das tensoes formadas pela incidencia de cargas sobre a protese parcial removivel de extremidade livre mandibular, nos dentes suporte e no rebordo residual nas formas: ascendente distal, descendente-ascendente, horizontal e descendente distal. Nos diferentes tipos de rebordo foi verificado o melhor tipo de grampo e a localizacao do apoio no ultimo dente suporte da extremidade livre. Foram construidos 4 modelos de resina fotoelastica (PL-1 para os dentes e PL-2 para o rebordo alveolar), um modelo para cada tipo de rebordo. Para cada modelo foram confeccionados 4 proteses parciais removiveis (PPR)(total de 16 PPR): com grampo em barra T e apoio na face distal, com grampo em barra T e conector na mesial, grampo circunferencial e apoio na face distal e grampo circunferencial e apoyo na face mesial. Os modelos foram posicionados em um polariscopio circular e aplicado carga pontuais e axiais de 100 N nos pre-molares e molares das proteses. As franjas formadas foram fotografadas para analise. Os resultados foram analisados de uma forma qualitativa e podemos observar que o rebordo horizontal apresentou uma melhor distribuicao das tensoes, enquanto o rebordo descendente distal apresentou as maiores concentracoes de tensoes. O grampo circunferencial apresentou as maiores formacoes de tensoes em todos os tipos de rebordo, exceto no rebordo horizontal, onde nao houve influencia quanto ao tipo de grampo. Nao houve diferenca significante entre os tipos de rebordo ao posicionar o apoio na mesial ou distal do ultimo dente suporte, com excecao feita ao rebordo descendente distal, onde se acentuou a concentracao de tensoes ao posicionar o apoio na distal do ultimo dente. Desse modo, podemos concluir que (1) o rebordo mais desfavoravel foi o descendente distal e o mais favoravel o horizontal, (2) o grampo em barra T apresentou distribuicao de tensoes mais favoraveis, com excecao do rebordo horizontal, onde nao houve influencia quanto ao tipo de grampo, (3) e que a localizacao do apoio apresentou comportamentos semelhantes, a nao ser no rebordo descendente distal.

ABSTRACT

El valor estético de una restauración cerámica es parcialmente influenciado por múltiples factores. Muchos de estos factores son conocidos por el odontólogo, sin embargo, existen otros , tales como: grosor de la cerámica, color del agente cementante, color de la estructura dental subyacente, limitaciones de las guías de colores disponibles, composición de los materiales cerámicos, etc. que pueden interferir en la correcta selección del color, siendo un problema clínico muy común. El propósito de este trabajo fue hacer una revisión bibliográfica sobre la influencia de los factores físicos y sensoriales y como estos pueden afectar en la selección del color de las restauraciones protésicas con el fin de evitarlos o disminuirlos.


The esthetic value of dental ceramic restorations is influenced by several factors. Many factors is know by the dentist , however, others factors like: ceramic thickness, luting agent, shade guides, color of dental substrate, the limited range of available ceramic shades, varying compositions of ceramic materials, is a common clinical problem. The aim of this study was made a literature review of the influence of physical and sensorial factors on the shade selection in ceramics restorations for avoid or minimize these factors.

ABSTRACT

Actualmente el uso de prótesis sobre implantes se incrementó gradualmente desde el descubrimiento de la oseointegración. La indicación inicial de los implantes fue para pacientes edéntulos totales extendiéndose después para pacientes parcialmente edéntulos. Las alternativas de retención de las piezas protésicas van desde las prótesis atornilladas hasta las prótesis cementadas o por preparación de los pilares de la forma convencional. Las prótesis cementadas sobre implantes se utilizan frecuentemente en los casos donde la inclinación de los implantes imposibilita la confección de prótesis atornilladas, debido a la salida de los tornillos de fijación por vestibular o entre los espacios interdentales comprometiendo la estética, y la resistencia cuando están localizados en las cúspides oclusales de los dientes posteriores. En ese sentido, la confección de "copings" en los pilares preparados, puede tornar el tratamiento en un proceso más rápido y seguro. No obstante, pueden surgir problemas en el momento de la instalación, debido a que el intermediario puede asumir diversas posiciones en relación a la posición del implante dificultando la colocación de la prótesis definitiva, principalmente en situaciones clínicas con más de dos implantes. De este modo, el objetivo de este trabajo fue mostrar una técnica del uso de una guía para posicionamiento de los pilares protésicos en prótesis fija cementada sobre implantes, por medio de la descripción de un caso clínico


Nowadays, the use of implant-retained prostheses has been increasing progressively since the advent of the osseointegration. The basic indication was in total edentulous patients and it was also enlarged for partially edentulous patients. Alternatives of retention could be from screw-retained implant prostheses, cement-retained implant prostheses or conventional preparations. Cement-retained implant prostheses are used frequently when the implant inclination don't allow the use of screw-retained prostheses because the screw emerge in vestibular surface or the interdental space resulting in an unaesthetic appearance and when the screw emerge on the top of the posterior cuspid teeth, diminishing the resistance. The confection of the copings on the abutments, turn the treatment faster and secure. Although, some complications can appear in the placement, because the abutment can take different positions in relation to the implant modifying the set of the prosthesis, mainly in clinical situations with more than two implants support. Thus, the aim of this study was demonstrated the use of guide to positioning of cement-retained implant prostheses, by means of a clinical case
