ABSTRACT

Purpose: To compare the masticatory performance and satisfaction levels of subjects with normal dentition and subjects with implant-supported overdentures with two types of attachments (ball and bar-clip retention systems). Methods: The sample comprised 24 subjects, 12 edentulous patients and 12 dentate subjects. The denture wearers received two dental implants and new maxillary and mandibular dentures. Three months after the first surgical phase, the mandibular dentures were connected to the implants by a gold ball attachment. Two months later new mandibular overdentures were made with a bar-clip attachment. Satisfaction levels (modified OHIP-EDENT and verbal rating scale) and masticatory performance (chewing for 40 masticatory cycles) were measured with the old dentures, with the unattached new dentures, and with the new dentures with ball and bar-clip attachments. Data were analyzed by Friedman and chi-square tests. Results: Both masticatory performance and satisfaction levels significantly improved after implant treatment. No significant differences were observed between the overdentures with ball and bar attachments. However, the masticatory performance after treatment was still significantly lower than the performance of the healthy subjects. Conclusion: The rehabilitation of edentulous patients with overdenture with ball or bar-clip attachment improves both satisfaction and masticatory performance, but the outcomes levels are not equal as those found for dentate subjects.


Objetivo: Comparar o nível de satisfação e a performance mastigatória dos indivíduos com dentição normal e pacientes reabilitados com uma sobredentadura suportada por implantes com dois tipos de conectores diferentes, sistema de retenção bola e barra. Metodologia: Vinte e quatro indivíduos participaram do estudo, sendo 12 pacientes desdentados e 12 indivíduos dentados totais. Os desdentados receberam dois implantes e novas dentaduras, superiores e inferiores. Três meses depois da primeira fase cirúrgica, a dentadura inferior foi conectada aos implantes através do sistema bola. Dois meses depois, uma nova sobredentadura inferior foi realizada, porém desta vez conectado através do sistema barra-clipe. O nível de satisfação (OHIP-EDENT adaptado e escala verbal de medição) e a performance mastigatória (mastigação por 40 ciclos mastigatórios) foram medidos em vários momentos do tratamento, com as dentaduras antigas, novas não conectadas por implantes, e depois de ter conectado a sobredentadura pelo sistema bola e barra-clipe. O teste de Friedman e o teste qui-quadrado foram aplicados para analisar os resultados.Resultados: O nível de satisfação e a performance mastigatória melhoraram significativamente após a reabilitação com os implantes. Nenhuma diferença foi observada entre as sobredentaduras com sistema bola e barra-clipe. Entretanto, a performance mastigatória ainda permaneceu significativamente menor que a performance dos indivíduos dentados. Conclusão: A reabilitação de pacientes edêntulos reabilitados com sobredentadura com bola ou barra-clipe melhora tanto o nível de satisfação quanto a performance mastigatória, porém ainda abaixo do nível dos indivíduos com dentição normal.

ABSTRACT

Objetivo: A reabilitação oral do desdentado total com prótese total implantossuportada visa, além de restaurar a estética, restaurar a performance mastigatória. O objetivo deste estudo foi comparar a performance mastigatória entre usuários de próteses totais convencionais superiores e inferiores implantossuportadas (n = 12) e indivíduos com dentição natural completa (n = 12). Material e método: A performance mastigatória foi avaliada pela mastigação do alimento-teste Optocal Plus (versão brasileira) por 40 ciclos mastigatórios. O material mastigado foi submetido a um sistema de tamises e o peso das partículas em cada peneira foi determinado pela aplicação da equação de Rosim-Rammler, obtendo-se o tamanho mediano das partículas (X50). Os testes foram realizados dois meses depois da primeira fase cirúrgica, quando foram colocadas novas próteses totais convencionais superior e inferior (t1) e, depois de cinco meses, quando foi realizada a segunda fase cirúrgica para colocação das conexões e instalação da prótese total inferior implantossuportada (t2). Os indivíduos do grupo controle realizaram o teste uma única vez. O teste t para amostras pareadas e independentes foi aplicado para comparação intra e interindivíduos, respectivamente. Resultados: O tamanho mediano das partículas foi signifcativamente maior para os pacientes portadores de próteses totais, tanto em t1 como em t2 em relação ao Grupo controle, denotando melhor performance mastigatória para os dentados. Conclusões: apesar de os indivíduos dentados apresentarem melhor performance mastigatória do que os pacientes reabilitados com as próteses superior convencional e inferior implantossuportada, esta pode ser considerada capaz de melhorar a performance mastigatória quando comparada a próteses totais superior e inferior convencionais.


Aim: The oral rehabilitation of a total edentulous by an implant-supported overdenture aims to re-establish the esthetic and masticatory performance. The aim of this study was to compare the masticatory performance in users of maxillary complete denture and mandibular implant-supported overdenture (n = 12) with dentulous individuals (n = 12). Materials and methods: The masticatory performance was measured by the mastication of a chewable test material, Optocal Plus (Brazilian version) during 40 cycles. The material test was submitted to a system of sieves and the particle weights in each sieve were determined for calculating the median particle size (X50) using the Rosim-Rammler equation. The tests were performed two months after the frst chirurgical phase when the new maxillary and mandibular complete dentures were installed (t1), and fIve months after, when the second chirurgical phase was performed for adaptation of the connections and installation of the mandibular implant-supported overdenture (t2). The t test for paired and unpaired samples were applied for intra- and inter-group comparison, respectively. Results: The median particle size was signifcantly larger for the patients with complete dentures in both evaluation (t1 and t2), meaning better masticatory performance for the dentulous individuals. Conclusions: Despite the dentulous individuals have presented a better masticatory performance than the patients rehabilitated with the maxillary complete denture and mandibular implant-supported overdenture (p = 0.001), the last one can be considered capable to improve masticatory performance in relation to maxillary and mandibular complete dentures.

ABSTRACT

The present article aimed to present a standardized protocol for the production of a chewable test material that has been used in masticatory efficiency and performance studies. This chewable material has advantages in respect to its physical properties when compared to other artificial and natural test foods. It is constituted by mixing condensation silicon (58.3 percent by weight), common plaster (10.2 percent by weight), alginate (12.5 percent by weight), solid vaseline (11.5 percent by weight), tooth paste (7.5 percent by weight), and catalyst paste (20.8 mg/g), adding also three drops of mint essence. The mixed material is then inserted into an acrylic mould with perforations of 12 mm in diameter and 5 mm in height to produce rounded tablets with those measures after polymerization. It was named "Optocal - Brazilian version". A volume of 3 cm³ is indicated for a chewing test, which corresponds to 12 tablets using the present methodology. The present protocol can make the production of this chewable material easier, helping in its standardization for studies on masticatory function.

ABSTRACT

A prótese total sobre implantes também chamada de Protocolo de Branemark ou Prótesede Toronto é uma das alternativas para pacientes totalmente edêntulos com rebordo ósseo quenão proporciona retenção e nem estabilidade para uma prótese total convencional. A prótese aparafusada sobre implantes recupera e proporciona uma melhora na mastigação, deglutição, nacapacidade fonética e conseqüentemente, na qualidade de vida dos pacientes. O tratamento convencional de um protocolo é realizado em duas etapas, uma cirúrgica e uma protética, comum intervalo de quatro e seis meses respectivamente para mandíbula e maxila. Sobre os implantes são aparafusados pilares e sobre estes a prótese. Esta é confeccionada a partir de uma barrametálica, na qual os dentes acrílicos são fixados. Este artigo descreve a manutenção de um paciente reabilitado com uma prótese do tipo Protocolo de Branemark sobre quatro implantes após dois anos da instalação da prótese.


A total conventional prosthesis implant retained called Branemark Protocolo or Toronto’sprosthesis is one of the alternatives of rehabilitation for edentulous patient with a residual bonethat do not provides neither retention and stability for a conventional prosthesis. An implantretained screw prosthesis recovers and provides an improvement of quality in mastication, swallowing, phonetic and consequently in the quality of life. The traditional treatment of afixed retained denture is carried through in two stages, surgical and a prosthetic one, with aninterval of four and six months respectively for lower and upper jaw. The abutments are screwon the dental implants and over those, as well as the alloy infrastructure with the acrylic teeth are screw above them. This paper describes the longitudinal maintenance of 2 years of onepatient that was rehabilitated with a full fixed acrylic prosthesis with four dental implants.

ABSTRACT

O objetivo deste artigo foi fazer uma revisão de literatura sobre a osseointegração periimplantar em pacientes fumantes, no qual foram apresentados dados da literatura publicados entre o período de 1991 e 2006. Além das diversas associações entre o fumo e o processo de osseointegração, foram, também, encontrados estudos que relacionam a superfície do implante com a osseointegração em pacientes fumantes. Entre os autores investigados, amaioria concorda que o fumo representa um maior risco a taxas de insucesso na osseointegração de implantes detitânio, porém apenas de maneira conjunta a outros determinantes tais como pobre quantidade e qualidade óssea,doenças sistêmicas, limitações anatômicas, grau de fixação inicial do implante, higiene bucal, oclusão, entre ou-tros. Devido a isto, o fumo não pode ser considerado por si só responsável pela falha na osseointegração em pacientes fumantes.


The purpose of this research was to make a literature review about peri-implant osseointegration in patients who smoke, in which literature data were presented during the period of 1991 to 2006. Besidesthe associations between smoking and the osseointegration process, the literature presents information that also relates the surface of implants and osseointegration around titanium implants. Among the research authors, the majority agrees that smoking represents a higher rise of failure on the osseointegrationprocess around titanium implants, but only if it considered together with other factors, such as poorbone quantity and quality (type IV),systemic diseases, like diabetes and osteoporosis, anatomic limitations, initial stability degree, oral hygiene, occlusion, among others. For these reasons, smoking can´t beconsidered, itself, the responsible for the failures of osseointegration in patients who smoke.

ABSTRACT

The difficulty of totally edentulous individual in performing the basic oral functions in a satisfactory and pleasant way directs the investigations for improvement of the rehabilitation techniques. This study aimed to perform an intra-patient analysis of the satisfaction level and masticatory capacity during the phases of the rehabilitation treatment with osseointegrated implants. Twelve edentulous patients (mean age: 61 years) participated. Satisfaction level and masticatory capacity were analyzed based on specific questionnaires for edentulous patients and by a verbal rating scale from 0 to 10, at three moments: with the old dentures, with conventional new ones after they had been received two osseointegrated implants in the mandible (after 60 days of use). After 5 months of the first surgical phase, gold cap ball attachments were adapted and the last evaluation was performed with the upper denture and the lower overdenture, after 30 days of use. The Friedman test was applied. The rehabilitation with implants produced a significant improvement in the satisfaction level (95.83%) and the masticatory capacity (94.07%). The new full dentures produced a better satisfaction and masticatory capacity than the old dentures. Besides, lower overdenture supported by two osseointegrated implants and a new upper conventional denture produced a higher scores than the conventional new dentures.

ABSTRACT

The aim was to correlate the chewing performance and maximum bite force in children, considering the morphologic characteristics of occlusion and body mass index (BMI). Thirty-six children participated (19 girls and 17 boys, mean age 9.06 years). The occlusal characteristics were evaluated by intra-oral examination. Body weight (kg) and height (m) were assessed using a balance with anthropometrical scale, and BMI determined (kg/m2). For chewing performance the children chewed a round silicone tablet (5 mm thick and 15 mm in diameter, 1.2 g ±0.005 g) with 20 strokes. The particle sizes were analyzed by a software. The bite force was determined with a pressurized rubber tube connected to a sensor element. There was no difference between genders in all variables. Bite force correlated negatively with particles sizes (p<0.001), meaning that high bite forces implicated in better chewing performance, and was weakly correlated with BMI (r=0.410, p<0.05). Children with different molar and canine relationship did not show differences among the variables. It was concluded that chewing performance depended on the maximum bite force, but other factors like the number and area of occlusal contacts, the amount of lateral excursion during mastication and dentition phase also may play a role. The occlusal characteristics did not influence the chewing measurements, whereas body size had a relationship with bite force.

ABSTRACT

Em alguns casos a prótese parcial removível unilateral convencional oferece riscos de aspiração ou deglutição ao paciente, os quais podem ser minimizados com a utilização de sistemas de encaixe. Neste trabalho a retenção do aparelho protético por meio de um sistema de encaixe com travamento (MK1) é relatada em dois casos clínicos. Essa associação de prótese parcial remivível e prótese fixa é uma alternativa para o paciente que deseja uma prótese que lhe dê segurança similar à de uma reabilitação fixa, com as vantagens de higienização de uma prótese removível.

O presente estudo relata um caso clínico onde foram utilizados pilares protéticos tipo UCLA personalizados para otimizar função e estética numa reabilitação protética.
