ABSTRACT

Introduction: Forensic Odontology plays a role in discerning a deceased individual in any disaster condition. In highly necrotised bodies, ABO blood group antigens can be found from teeth. Aim: We studied ABO blood grouping from dentin and pulp in freshly extracted teeth and also from the teeth stored in sea water. Materials and Methods: A total of 60 samples were selected & divided into 3 groups with 20 samples each. Group I analyzed within period of a week without any storage medium, group II, III were analyzed after 1 and 2 month of storage in sea water. Results: Statistical analysis was done using chi square test. By Absorption –Elution, pulp in group I, II, III, gave 90%,75%,75% of positivity. In dentin, Group I, II, III showed 55%, 45%, 20% of positivity. By Absorption -Inhibition method pulp in group I, II, III showed 45%, 20%, 0% of positivity. In dentin, group I, II, III showed 20%,5%,5% of positivity. Conclusion: This study concluded that pulp is most reliable than dentin even in sea water storage and absorption elution is most effective method in blood group identification in teeth than absorption inhibition method. ==================================== Introduction: Oral cancer (OC) is associated with various risk factors and high mortality rates, and contributes significantly to the worldwide cancer burden. Objectives: To assess and evaluate patients’ current knowledge, awareness, and behavior regarding OC risk in a cancer trust hospital. Materials and Methods: The study involved 600 patients who attended cancer trust hospital, East Godavari district, from September 2021 to October 2021. A self- administered questionnaire of 20-questions was given to each patient that included socio-demographic and disease-specific information and their answers evaluated. Results: The data was examined using descriptive statistics, and the connection between the variables, education, family income, and other factors was assessed using a chi-square test (with a 5% significance threshold). The results were analysed with reference to their implications for interventions aimed at patient’s awareness for oral cancer symptoms. Conclusion: According to the findings of this study, people lacked information and awareness about identified risk factors for oral cancer. Knowledge of maintaining a healthy lifestyle that eliminates the consumption of established oral cancer risk factors was low. At the community and individual levels, health education linked to primary prevention of oral cancer must be improved.

ABSTRACT

Background: The peculiarities of the prostate are that in old age when most of organ regresses in size it enlarges and causes trouble. By virtue of its position, it guards outlet of urinary bladder. It must be admitted that even today the extent nature of etiology, pathogenesis and diagnosis are all-debatable. Therefore, the present study was aimed at elucidating the age and mode of Benign Prostatic Hyperplasia among the patients in and around Hubli, Karnataka. Methods: The study was conducted after the institutional ethical committee approval and written informed consent from all the patients. 21 cases of BPH were included in the present study. The signs and symptoms of mode of presentation were recorded. The urine was examined for the presence of albumin, pus cells, epithelial cells and RBCs. Results: The maximum incidence of the disease was in the age group of 60-69 years. Majority of the patients had difficulty in micturition followed by increased frequency of micturition both during day and night Patient with retention usually had distension of bladder. These patients complained of dull aching pain in the suprapubic area and in the groin region. Burning micturition (14.28%), dribbling of urine (4.76%), urgency (4.76%), and haematuria (4.76%) were present. Routine urine examination showed albumin (28.56%), pus cells (23.80%), epithelial cells (19.04%) and haematuria (4.76%). Conclusion: Incidence of BPH at Hubli region of Karnataka is 0.3001% of total hospital admission. The maximum age incidence of BPH is in the age group of 60-69 years. Majority of the patients had difficulty in micturition increased frequency of micturition both during day and night a large number of cases show association with presence of urinary tract infection.

ABSTRACT

In this paper, we formulate a mathematical model for the evaluation of parameters responsible for the retardation and eventual acceleration of bone regeneration on the contralateral side of the mandible of experimental animals, following the discovery of a new mechanism of bone regeneration called the osteobstruction mechanism (a negative mechanism of bone regeneration as opposed to the well established and extensively documented positive mechanisms such as osteogenesis, osteoinduction and osteoconduction). This osteobstructive mechanism was demonstrated by episodes of overtaking and re-overtaking on single photon emission computed tomography (SPECT) following evaluation of osteoblastic activities in a sequential animal experiment to validate both the Ogunsalu sandwich technique (a double guided tissue technique; D-GTR) and the interceed membrane technique (a single guided tissue regeneration technique; S-GTR) utilizing SPECT, histological and histomorphometric evaluation. This work is now given special attention in terms of mathematical analysis because of limited experimental observations since experiments cannot be observed infinitely. Mathematical modelling is as such essential to generalize the results of this osteobstructive mechanism in bone regeneration. We utilize the Fisher's equation to describe bone cell mobilization during bone regeneration by two different techniques: the Ogunsalu sandwich bone regeneration technique (D-GTR) and the S-GTR.


En este trabajo se formula un modelo matemático para la evaluación de los parámetros responsables del retardo y la posterior aceleración de la regeneración del hueso en el lado contralateral de la mandíbula de animales experimentales, tras el descubrimiento de un nuevo mecanismo de regeneración del hueso, denominado mecanismo de osteobstrucción de la regeneración del hueso (un mecanismo negativo de regeneración del hueso, el cual contrasta con mecanismos positivos bien establecidos y ampliamente documentados, tales como la osteogénesis, la osteoinducción y la osteoconducción). Este mecanismo osteobstructivo fue demostrado mediante una serie de fenómenos consecutivos en la tomografía computarizada por Emisión de Fotones Individuales (SPECT), tras la evaluación actividades osteoblásticas en experimentos secuenciales con animales, para validar tanto la técnica de sandwich de Ogunsalu (técnica de regeneración de tejidos guiada doble; RTG-D) como la técnica de membrana con interceed (técnica de regeneración de tejidos guiada simple; RTG-S) utilizando SPECT así como evaluaciones histológicas e histomorfométricas. Este trabajo recibe aquí especial atención en términos de análisis matemático, ya que en los experimentos las observaciones no puede realizarse infinitamente. Los modelos matemáticos son pues esenciales para generalizar los resultados de este mecanismo del osteobstructivo de la regeneración ósea. Se utiliza la ecuación de Fisher para describir la movilización de las células óseas durante la regeneración del hueso por dos técnicas diferentes: la técnica de Ogunsalu para la regeneración ósea por "sandwich" (D-GTR) y la técnica S-GTR.

ABSTRACT

Extreme degree of cystic, haemorrhagic and necrotic changes in a thymoma is rare. A 22-year-old male presented with cough, grade 2 dyspnoea, and occasional chest pain for the past six months. Radiological investigations revealed a large cystic lesion in the anterior mediastinum. A benign cystic tumour was suspected. Surgical resection of the tumour was done. Grossly, the almost entirely cystic and haemorrhagic dumb-bell shaped encapsulated tumour showed a subcapsular residual nodule. Histopathological examination was suggestive diagnosis of benign thymoma (World Health Organization [WHO] Type A, medullary type) associated with the rare features of cells with dendritic processes containing melanin pigment seen singly scattered throughout the tumour.

BACKGROUND & OBJECTIVE: Infection due to Mycobacterium bovis typically occurs in cattle and animals transmit infection to each other. The choice of appropriate clinical specimen is very important for isolation of M. bovis and M. tuberculosis from cattle. The present study reports the isolation of M. tuberculosis and M. bovis from different types of specimens from cattle suspected to be suffering from tuberculosis in certain organized cattle farms in north India. METHODS: A total of 768 specimens (heparinized or EDTA containing blood (162), fine needle aspirates from prescapular lymph gland (PSLG,160), milk (154), pharyngeal swab (PhS, 98), rectal pinch (RP, 97) and faecal sample (97) from 161 cattle of organized cattle farms in north India suspected to be suffering from tuberculosis were analyzed. After decontamination by modified Petroff's method isolation of M.tuberculosis complex was done on Lowenstein-Jensen medium (with and without pyruvate). The culture isolates were identified as M. tuberculosis and M. bovis on the basis of biochemical tests. RESULTS: A total of 54 M. tuberculosis complex isolates were obtained, of them 40 were identified as M.bovis and 14 as M. tuberculosis. M.bovis were isolated from 12 of 38 animals in group A (Tuberculin +ve with signs of tuberculosis), 7 of 37 animals in group B (Tuberculin +ve and apparently healthy), 9 of 21 group C animals in (Tuberculin -ve with clinical signs of tuberculosis), 4 of 26 animals in group D (Tuberculin -ve and apparently healthy), 4 of 27 group E animals (having non-mycobacterial infection) and 4 of 12 animals in group F (having clinical signs such as debilitated condition, cough, decreasing milk production, etc). Maximum number of M. bovis (19/40, 47.5%) and M. tuberculosis (5/14, 35.7%) isolates were grown from prescapular lymph gland biopsy (PSLG) followed by blood from which 9/40 (22.5%) M. bovis and 4/14 (28.5%) M. tuberculosis were isolated. M. bovis [6/40(15%)] and M. tuberculosis [4/14(28.5%)] were also isolated from milk. Only 3/40 (7.5%) isolates of M.bovis could be isolated from 97 rectal pinch followed by 98 pharyngeal swab 2/40 (5%) and 97 fecal samples 1/40 (2.5%) while 1/14 (7.1%) M.tuberculosis isolates were obtained from pharyngeal swab. INTERPRETATION & CONCLUSION: Among the samples analyzed, PSLG was found to be most suitable specimen for isolation of M. tuberculosis complex from cattle and is thus of diagnostic importance. M. bovis in milk indicates the need to investigate the transmission to human in such settings. Isolation of M. bovis and/or M. tuberculosis from apparently healthy cattle indicates sub-clinical infection in the herd. Further, isolation of a significant number of M. tuberculosis from cattle suggests possible human-to-cattle transmission which need to be confirmed by prospective studies including tools like DNA fingerprinting.

ABSTRACT

Soluble fibre has been shown to augment the cholesterol-lowering effects of low-fat diets in individuals with mild to moderate hypercholesterolaemia. Combination therapy with a statin poses advantages in certain settings and may allow use of lower doses of multiple drugs rather than maximum doses of a single drug. The primary objective of the study was to compare the efficacy of combination of isapgol and atorvastatin versus atorvastatin alone, in the same dose, in reduction of low-density lipoprotein cholesterol (LDL-C), total-cholesterol levels in hypercholesterolaemic patients after 12 weeks of therapy. In a 12-week study, 100 subjects from both sexes and of > 20 years having hyperlipidaemia, with LDL-C level > 130 mg/dl and total cholesterol > 220 mg/dl were included, and were randomised to receive either a combination of isapgol powder (Naturolax) 5.6 g twice daily and atorvastatin 10 mg once daily or atorvastatin 10 mg once daily alone orally. Serum levels of total cholesterol, LDL-C, high-density lipoprotein cholesterol (HDL-C), and triglyceride were assessed at 8 and 12 weeks. Ninety-seven patients completed the study. At the end of the 8th week, both the groups had a significant reduction in mean LDL-C (20.5% in isapgol + atorvastatin group and 16.0% among atorvastatin alone group) as compared to baseline. But between the groups, however, the difference was not significant. At the end of the 12th week fall in LDL-C at 31.4% for isapgol + atorvastatin was significantly greater than 22.8% among the atorvastatin group (p < 0.05). Serum total cholesterol, HDL-C and triglyceride were significantly lowered within the groups at 8th and 12th weeks but between groups, the difference was not significant. Comparison of adverse events profile in both the groups shows that more number of patients from atorvastatin alone group (n = 14, 28%) had adverse reactions than the number of patients from the combination group (n = 4, 8%; p < 005).

ABSTRACT

Diapers have been used for care of babies since decades to prevent soiling and for social convenience. However, the use of diaper poses a risk of developing diaper dermatitis. Recent innovations in diaper technology have led to development of superabsobent disposable diapers, emollient delivering diapers and breathable diapers. These newer types of diapers reduce the incidence of diaper dermatitis. The non-biodegradable material used in superabsorbent diapers is, however, a matter of serious concern because of its toxic effects and environmental pollution. The different type of diapers, recent advances in them and their environmental concerns are discussed in this article.

ABSTRACT

Diaper dermatitis, also know as nappy rash, is an inflammation of the skin covered by nappy. It probably results due to an interaction of multiple factors like increased wetness, elevated pH due to urine, fecal enzymes and microorganisms under the nappy. It manifests as an erythematous rash occurring on the convex surfaces of skin under the nappy. Rashes resembling nappy dermatitis can also be caused by some diseases which may have serious systemic manifestations. Therefore it is essential to differentiate and treat them. The principle of treatment of diaper dermatitis is to keep the skin in the nappy area as dry as possible with frequent nappy change. The superabsorbent disposable diapers are known to reduce the incidence of diaper dermatitis. Barrier creams to protect the infant's skin and mild topical corticosteroids to reduce the inflammation are mainstays of therapy. The incidence and severity can be reduced by keeping the skin dry under the nappy and protected from irritants and infections.

ABSTRACT

Butenafine is a new antifungal agent similar to allyl amine antifungals. A randomized controlled trial was conducted in 75 patients to compare its efficacy with clotrimazole in tinea cruris and corporis that was diagnosed on clinical features and demonstration of hyphae in a potassium hydroxide (KOH) preparation. Twenty patients treated with butenafine once daily for 2 weeks and 20 treated with clotrimazole twice daily for 4 weeks were analysed. At the end of treatment, 2 weeks and 4 weeks later, the KOH preparation was negative in 90.9%, 95.5% and 90.9% of patients respectively in the butenafine group and 100%, 96.4% and 92.85% respectively in the clotrimazole group. There was a reduction of 81.5% in the sign and symptom score at 4 weeks following treatment in the butenafine group and 85.93% in the clotrimazole group. There was no statistically significant difference between the two groups. Adverse effects were mild in both groups and did not require discontinuation of therapy except one patient treated with clotrimazole who developed dermatitis at the site of application. Butenafine appears to be as effective as clotrimazole in the treatment of tinea cruris and corporis while requiring a single daily application for a shorter of 2 weeks.

ABSTRACT

OBJECTIVE: The study was conducted to evaluate usefulness of single strand conformation polymorphism (SSCP) over DNA sequencing in the diagnosis of rifampicin (Rif)-resistant tuberculosis. METHODS: Forty seven isolates of Mycobacterium tuberculosis (MTB) Rif-resistant and 25 Rif-sensitive were obtained from Vancouver, Mexico city and New Delhi and were analyzed by polymerase chain reaction (PCR) amplification of rpoB gene and the mutations were identified by DNA sequencing and SSCP. RESULTS: The mutations observed by DNA sequencing in 47 RIF-resistant isolates showed that the most common mutation among Vancouver isolates was in codon 526, Hist-->Arg and in Mexico isolates was in codon 531, Ser-Leu and New Delhi isolates was in codon 516, Asp-->Val. Using fluorescence based PCR-SSCP, it was possible to distinguish Rif-resistant isolates from Rif-sensitive isolates. CONCLUSION: DNA sequencing is a highly accurate method for the detection of mutations associated with drug resistance in tuberculosis but is more expensive and requires special equipment and personnel. SSCP is a simple, accurate method and suitable for analysis of large number of samples and the results are available in less than 72 hours.

ABSTRACT

Interferon- (IFN-γ ) has been considered to be a critical protective immunomodulatory component against Mycobacterium tuberculosis (M. tb.) infection. In this study T-cell proliferation and IFN-γ production upon stimulation with M. tb. were assessed in patients of pulmonary tuberculosis and healthy contacts. The studies were based on lymphocyte transformation test and detection of intracellular IFN-γ production by CD4 + ve T-cells by flowcytometry. Patients showed lower levels of proliferation, the stimulation index being in the range of 2·17 1·1 (mean + SD) compared to the contacts (SI = 4·59± 1·6) (P < 0·01). The kinetics of intracellular induction of IFN-γ on M. tb. stimulation showed a proportional increase in the CD4 + ve T-cell population. The increase was maximal between 96-120 h of culture. In healthy contacts the number of IFN-γ expressing CD + ve T-cells increased to 2·5 to 41 × 104 cells/ml in M. tb. stimulated cultures compared to control cultures (0·1 —15 × 104). In contrast patients showed no/marginal increase in CD4 + ve T-cell population expressing intracellular IFN- γ Thus the lack of induction of IFN in CD4 + ve T-cells in patients could be a critical shortcoming in their ability to combat tubercle bacilli infection.

ABSTRACT

A stool survey was carried out in some of the villages of Dadraul and Bhawal Khera PHC's of district Shahjahanpur (Uttar Pradesh). Out of 381 individuals examined 111 (29.2 per cent) were found positive for one or the other intestinal parasite. Ascaris lumbricoides superseded all the parasites by showing a positivity of 17.85 percent. Other parasites found were Hookworm, Hymenolepis nana, Tapeworm, Trichuris trichiura, Enterobius vermicularis, Entamoeba histolytica, E. coli and Giardia lamblia. Parasitic load was slightly higher in females (33.59 per cent) than males (28.18 per cent). The highest positivity was encountered in the age groups between 6 to 14 years. This high prevalence of intestinal parasites may be due to the lack of awareness about personal cleanliness and hygiene and illiteracy among rural women. Majority of them had helminthic infections. It is concluded that in rural areas of district Shahjahanpur intestinal helminthic infections are more prevalent that protozoan infections.

ABSTRACT

Axenically grown E. histolytica possess significant acid phosphatase activity. The Km of the enzyme was found to be 7.1 x 10(-3) and was maximally active at pH 4.5. Acid phosphatase activity was significantly inhibited by Cu2+, cysteine and was activated by tartrate and fluoride. It was found that E. histolytica acid phosphatase differs in some properties as compared to the enzyme reported from other sources.
