ABSTRACT

Objective To investigate the effect of Anerdian type III solution on the treatment of acute gouty monoarthritis. Methods Total 108 cases of acute gouty monoarthritis were divided into two groups according to the method of random number table, including 56 cases in the Anerdian group and 53 cases in the Votalin group, respectively. The Anerdian group was applied Anerdian type III solution to compress the swelling joint wet for 30 minutes, while the Votalin group was given Votalin Emulgel for external application in the swollen joint. Then the scores of pain and redness were calculated, and the effects of local treatment were compared directly before and after the treatment, and these variables were also compared after 7 days. Results 24 hours and 72 hours after the treatment, the pain score in the Anerdian group was 3.44 ± 0.89 and 2.43 ± 0.73 (t=2.375, 3.393, P<0.01), while the pain score in the Votalin group was 3.85 ± 0.74 and 3.14 ± 0.68 (t=2.208, 3.120, P<0.01), respectively, which referred to improvement in both groups. And the score of redness in the Anerdian group was 1.57 ± 0.70 and 1.13 ± 0.35 (t=0.589, 0.964, P<0.01), while the score of redness in the Votalin group was 1.95 ± 0.73 and 1.61 ± 0.49 (t=0.151, 0.623, P=0.091, P<0.01) respectively. 24 hours and 72 hours after the treatment, the score of redness in the Anerdian group was smaller than that in the Votalin Group (t=3.379, 4.290, P<0.01). In Anerdian group, local treatment showed fully cured in 28 cases, marked effective in 15 cases, effective in 11 cases, not effective in 2 case, and total effective rate was 96.4% (54/56) after 7 days′treatment, while in Votalin group, local treatment revealed fully cured in 24 cases, marked effective in 11 cases, effective in 8 cases, not effective in 9 case, and total effective rate was 83.0% (44/53). There was statistically significant difference between the two groups in local treatment ( χ2=4.019, P<0.05). Conclusion Anerdian type III solution can alleviate the symptoms of acute gouty unilateral arthritis quickly. It hasbetter curative effect on local treatment of acute gouty monoarthritis than Votalin.

ABSTRACT

Objective To investigate the positive practice environments ( PPE ) of nurses and influencing factors at public hospitals , for reference of building a better PPE .Methods A national cross-sectional survey was performed at 77 public hospitals across seven provinces/metropolises, involving 5374 nurses.PPE included organizational management (internal) and nurses-patient relationship (external). Results The scoring of positive practice environment was 18.51 ±4.69 (total score of 40).The scoring of organizational management and nurses-patient relationship was 9.87 ±3.11 and 8.64 ±2.51 respectively. The scoring of PPE of nurses of general hospital ( GH) was higher than that of traditional Chinese medicine hospital(TCMH) (18.68 ±4.68 versus 18.08 ±4.67, P<0.01).Multivariable analysis showed that , compared with nurses who had not very much pressure about performance assessment , the scores of those who had were declined (βGH =-1.15, 95%CI: -1.55 to -0.76;βTCMH =-1.29, 95%CI: -1.92 to-0.66 ) );compared with nurses who paid less efforts in communicating with their patients , the scoring of those with greater efforts was higher (βGH =2.43, 95%CI:2.00 to 2.86;βTCMH =2.84, 95%CI:2.19 to 3.49).Conclusions PPE of nurses is poor in general in China , and the externally stressful nurse-patient relationship deserves greater attention and efforts than inefficient organization management internally .To improve PPE of nurses , hospitals need to moderate nurses′performance assessment and the nurses need to pay more attention to patient communication .

ABSTRACT

Objective To explore and evaluate the feasibility and accuracy of applying decision tree methods to predict the risk of hospital-acquired pressure ulcers (HAPUs) in intensive care upit (ICU) patients, and provide a theoretical basis for the prevention of HAPUs in clinical practice. Methods A retrospective design was used to collect 468 patients' records, and all of the patients were hospitalized in ICUs including medical intensive care unit and coronary care unit between October 2011 and October 2013 in a 3 A grade hospital in Guangzhou. The CART algorithm was used to construct the decision tree risk prediction model. The area under the receiver operating curve (AUC),sensitivity and specificity were used to evaluate the predictive validity of the decision tree model compared with the Braden Scale. Results The decision tree model had four stratums and eleven nodes.Six classification rules and three styles of high-risk populations were screened out:(1)age >81;(2)age ≤81 combined with fecal incontinence;(3)age ≤81 combined with total Braden score ≤ 13 and diastolic blood pressure < 60mmHg (1 mmHg=0.133 kPa).The AUC of the decision tree model was significantly higher than the Braden Score (Z=2.31,P < 0.05).The sensitivity and specificity of the decision tree model (0.809 and 0.703) were higher than the Braden Score (0.777 and 0.587).Conclusions The decision tree model is an easy and feasible tool to predict the risk of HAPUs in ICU patients, and it can be used to screen high-risk populations.

ABSTRACT

Aim To explore new ways for developing anticancer drugs by the separation of pigment from Fu-sarium species JN158 ( Fusarium sp JN158 ) , the iden-tification of its structure, the screening of anticancer components and the study of its partial mechanism. Methods Pigment separation was done by HPLC, structural analysis by UV, IR, NMR, the screening of anticancer activity by MTT. Western blot was used to analyze the protein expression of CyclinD1, NF-κB, VEGF in tumor cells. Results The results showed that the pigment from Fusarium produced a total of six different peaks, of which peak Ⅵ was the anthocya-nins. Its molecular weight is about 382, molecular for-mula is C17 H18 O10 . According to investigation, this pig-ment was probably a new compound, which could in-hibit the proliferation of MCF-7 cells markedly ( IC50:0.011mmol·L-1 ,P<0.05;the control medicine ube-nimex IC50:10 mmol · L-1 ) in a concentration-de-pendent manner, and had no effect on human umbilical cord intravenous endotheliocyte ( HUVEC ) . The influ-ence on the gene expression of CyclinD1, NF-κB, VEGF in MCF-7 cells varied with the concentration of this compound. The Western blot results showed that VI pigment compound inhibited CyclinD1, NF-κB, VEGF gene expression (P<0.05 or 0. 01),compared with the control group. Conclusion The Ⅵ pigment compound from Fusarium sp JN158 could inhibit MCF-7 proliferation by inhibiting CyclinD1, NF-κB, VEGF gene expression. The compound may be a promising compound against breast cancer.
