ABSTRACT

Objective To conduct data mining of asthma-inducing medications in underage populations based on the U.S.Food and Drug Administration Adverse Event Reporting System(FAERS)database,so as to provide reference for the clinical application of related medications.Methods Adverse drug event(ADE)reports from the first quarter of 2013 to the fourth quarter of 2022 in the FAERS database were collected and screened for reports of asthma adverse events in the this population(under 18 years old),which were categorized into infants,toddlers,children,and adolescents according to different age groups,and were subjected to medication signal mining by using the reporting odds ratio method,the composite standardized method,and the information component method.Results A total of 1 915 reports were obtained after screening,involving 1 042(54.41%)males and 831(43.39%)females;the highest percentage of the reporting population was between 12 and under 18 years old,with a total of 762(39.79%);60.78%of the reports were reported by health professionals;and the results of the clinical referrals showed that serious adverse events occurred in 85.90%of the cases.306 suspected drugs were screened,52 drugs were determined to be valid signals,and 1 044 adverse events were reported,of which 16 drug inserts did not mention the risk of asthma,in order of elosulfatase alpha,canakinumab,tobramycin,vancomycin,ceftriaxone,cetirizine,phenylephrine,imiglucerase,cefuroxime,betamethasone,atropine,tadalafil,riscovastatin,cyclophosphamide,octreotide,and omeprazole.Conclusion The FAERS database was mined for adverse drug event signals and evaluated using the proportional disequilibrium method to identify 16 medicines that may trigger pharmacogenetic asthma and are not documented in the specification,which can be used to provide a good early warning for the clinic.At the same time,focusing on special populations,strengthening the assessment of lung function before medication and monitoring during and after medication,timely interventions were taken to reduce the harm of drug-derived adverse reactions and ensure the safe use of medication.

ABSTRACT

Objective Based on the U.S.Food and Drug Administration Adverse Event Reporting System(FAERS)database,data mining was conducted on hematological adverse events related to antibody drug conjugates(ADC),providing reference for the safe use of ADC drugs in clinical practice.Methods The report data from the third quarter of 2011 to the fourth quarter of 2022 were retrieved from the FAERS database.After data cleaning such as deduplication and name standardization,extract hematological adverse events related to ADC,and use report odds ratio method and the information component method for signal detection.Results A total of 101 610 adverse event reports were extracted,with 8 ADC drugs as the primary suspected drugs,and 5 768 ADC related hematological adverse event reports.Among them,3 423 cases of agranulocytosis were involved,and the signal intensity from strong to weak were sacituzumab govitecan(SG),gemtuzumab ozogamicin(GO),brentuximab vedotin(BV),polatuzumab vedotin(PV),enfortumab vedotin(EV),trastuzumab deruxtecan(TD),inotuzumab ozogamicin(IO)and ado-trastuzumab emtansine(TDM-1).There were 2 327 cases hematopoietic cell deficiency,with signals ranging from strong to weak were IO,SG,BV,EV,PV,TD,TDM-1,and GO.Report with clinical outcome of death of ADC drug related hematological adverse events included BV 179(16.84%),TDM-1 102(13.01%),TD 88(27.08%),GO 12(16.90%),IO 8(11.59%),EV 54(24.32%),PV 22(27.16%),and SG 84(21.05%).Adverse event time analysis showed that the number of events on the first day of TD,IO,and SG medication accounts for ≥ 40%of the total number of cases.The median time of hematological adverse events in TD,GO,IO,EV,PV,and SG was within one treatment course(21 days).Conclusion Attention should be paid to the risk of ADC drug-related hematological adverse event,during the clinical medication process,blood cell count changes should be closely monitored,and any abnormalities should be promptly diagnosed and treated.

ABSTRACT

OBJECTIVE@#To assess the efficacy and safety of Guanxin Danshen Dripping Pills (GXDS) in the treatment of depression or anxiety in patients with coronary heart disease (CHD) after percutaneous coronary intervention (PCI).@*METHODS@#From September 2017 to June 2019, 200 CHD patients after PCI with depression and anxiety were included and randomly divided into GXDS (100 cases) and placebo control groups (100 cases) by block randomization and a random number table. Patients in the GXDS and control groups were given GXDS and placebo, respectively, 0.4 g each time, 3 times daily for 12 weeks. The primary outcomes were scores of Patient Health Questionnaire-9 (PHQ-9), Generalized Anxiety Scale (GAD-7) and the Seattle Angina Pectoris Scale (SAQ). The secondary outcomes included 12 Health Survey Summary Form (SF-12) scores and the first onset time and incidence of major adverse cardiovascular events (MACEs). Other indices including blood pressure, blood lipids, microcirculation and inflammatory-related indices, etc. were monitored at baseline, week 4, and week 12.@*RESULTS@#In the full analysis set (200 cases), after treatment, the PHQ-9 and GAD-7 scores in the GXDS group were considerably lower than those in the control group (P<0.05). Compared with the baseline, the total PHQ-9 scores of the experimental and control groups decreased by 3.97 and 1.18, respectively. The corrected mean difference between the two groups was -2.78 (95% CI: -3.47, -2.10; P<0.001). The total GAD-7 score in the GXDS group decreased by 3.48% compared with the baseline level, while that of the placebo group decreased by 1.13%. The corrected mean difference between the two groups was -2.35 (95% CI: -2.95, -1.76; P<0.001). The degree of improvement in SAQ score, SF-12 score, endothelin and high-sensitive C-reactive protein levels in the GXDS group were substantially superior than those in the placebo group, and the differences between the two groups were statistically significant (P<0.05). Similar results were obtained in the per protocol population analysis of 177 patients. Three cases of MACES were reported in this study (1 in the GXDS group and 2 in the placebo group), and no serious adverse events occurred.@*CONCLUSIONS@#GXDS can significantly alleviate depression and anxiety, relieve symptoms of angina, and improve quality of life in patients with CHD after PCI. ABSTRACT

With the increasing aging of our population, the incidence of drug-related problems among elderly patients in the community is gradually increasing. Drug-related problems can lead to an increased incidence of adverse health outcomes such as falls and frailty in elderly patients in the community. Medication review is one of the effective means of addressing drug-related problems. The medication review models in Europe and the United States have been developed for decades to address drug-related problems; medication review model in the United Kingdom focuses on the management of drug therapy for elderly patients in aged care facilities, while the medication review models in the United States and Australia have a broader scope(covering all elderly patients in the community), and the frequency of medication reviews in the United States and Australia are clearer and more standardized. Compared with the above countries, China’s medication review model has a late start. Our country can learn from the advanced experience of the United Kingdom, the United States, Australia and other countries, actively explore and build a drug review model suitable for China’s national conditions, so as to improve the health level of elderly patients in the community and save social public medical resources.

ABSTRACT

OBJECTIVE To explore the pharmaceutical care model for elderly patients with drug-related problems (DRPs) based on the comprehensive medication management review (CMMR), and to observe the effect of pharmaceutical care under the model. METHODS The pharmaceutical care new model for elderly patients was established by concerning CMMR guidelines in Australia. In other words, clinical pharmacists receive medical care through referral by health practitioners and active consultation by patients or their caregivers. Visits were made by a combination of face-to-face and telephone interviews. Follow-up was carried out in the form of outpatient follow-up, telephone follow-up, appointment, and home guidance to know about the treatment of DRPs by healthcare practitioners or patients.Under this model, the occurrence status and classification of DRPs in elderly patients were studied through cross-sectional investigation. The effect of pharmaceutical care was evaluated from the aspects of hospitalization rate, adverse drug reaction (ADR) incidence,medication compliance, the number of medications, blood lipid level, etc., through self-before-after comparison. RESULTS In this study, a patient-centered, evaluation-intervention-reevaluation closed-loop pharmaceutical care model was formed. Among 317 study subjects, the average number of DRPs was 1.03 (0-7) cases. Compared before and after the intervention, the number of drug types increased from 2.00(0.00,3.00) to 2.00(1.00,3.00), but the level of low-density lipoprotein cholesterin in patients decreased from 3.48 (2.58, 4.29) mmol/L to 3.11 (2.29,3.81) mmol/L (P<0.05). There was no statistical significance in hospitalization rate, ADR incidence within or medication compliance. CONCLUSIONS CMMR-based pharmaceutical care model can effectively identify and manage the patients’ DRPs and reduce the level of blood lipid.

ABSTRACT

Objective: To explore the epidemiological characteristic of a COVID-19 outbreak caused by 2019-nCoV Omicron variant BF.7 and other provinces imported in Shenzhen and analyze transmission chains and characteristics. Methods: Field epidemiological survey was conducted to identify the transmission chain, analyze the generation relationship among the cases. The 2019-nCoV nucleic acid positive samples were used for gene sequencing. Results: From 8 to 23 October, 2022, a total of 196 cases of COVID-19 were reported in Shenzhen, all the cases had epidemiological links. In the cases, 100 were men and 96 were women, with a median of age, M (Q1, Q3) was 33(25, 46) years. The outbreak was caused by traverlers initial cases infected with 2019-nCoV who returned to Shenzhen after traveling outside of Guangdong Province.There were four transmission chains, including the transmission in place of residence and neighbourhood, affecting 8 persons, transmission in social activity in the evening on 7 October, affecting 65 persons, transmission in work place on 8 October, affecting 48 persons, and transmission in a building near the work place, affecting 74 persons. The median of the incubation period of the infection, M (Q1, Q3) was 1.44 (1.11, 2.17) days. The incubation period of indoor exposure less than that of the outdoor exposure, M (Q1, Q3) was 1.38 (1.06, 1.84) and 1.95 (1.22, 2.99) days, respcetively (Wald χ2=10.27, P=0.001). With the increase of case generation, the number and probability of gene mutation increased. In the same transmission chain, the proportion of having 1-3 mutation sites was high in the cases in the first generation. Conclusions: The transmission chains were clear in this epidemic. The incubation period of Omicron variant BF.7 infection was shorter, the transmission speed was faster, and the gene mutation rate was higher. It is necessary to conduct prompt response and strict disease control when epidemic occurs.

ABSTRACT

This paper aims to investigate the protective effect and mechanism of Astragalus membranaceus and Angelica sinensis before and after compatibility against triptolide(TP)-induced hepatotoxicity. The experiment was divided into a blank group, model group, Astragalus membranaceus group, Angelica sinensis group, and compatibility groups with Astragalus membranaceus/Angelica sinensis ratio of 1∶1, 2∶1, and 5∶1. TP-induced hepatotoxicity model was established, and corresponding drug intervention was carried out. The levels of alanine transaminase(ALT), aspartate transaminase(AST), and alkaline phosphatase(ALP) in serum were detected. Pathological injuries of livers were detected by hematoxylin-eosin(HE) staining. The levels of malondialdehyde(MDA), superoxide dismutase(SOD), glutathione peroxidase(GSH-Px), and reduced glutathione(GSH) in the liver were measured. Wes-tern blot method was used to detect the expression of nuclear factor erythroid 2-related factor 2(Nrf2), Kelch-like ECH-associated protein 1(Keap1), peroxisome proliferator-activated receptor gamma, coactivator-1 alpha(PGC-1α), heme oxygenase-1(HO-1), and NAD(P)H quinone dehydrogenase 1(NQO1) in livers. Immunofluorescence was used to detect the expression of Nrf2 and PGC-1α in livers. The results indicated that Astragalus membranaceus/Angelica sinensis ratio of 2∶1 and 5∶1 could significantly reduce the levels of serum AST, ALT, and ALP, improve the pathological damage of liver tissue, increase the levels of GSH and GSH-Px, and reduce the content of MDA in liver tissue. Astragalus membranaceus/Angelica sinensis ratio of 1∶1 and 2∶1 could significantly improve the level of SOD. Astragalus membranaceus and Angelica sinensis before and after compatibility significantly increased the protein expression of HO-1 and NQO1, improved the protein expression of Nrf2 and PGC-1α, and decreased the protein expression of Keap1 in liver tissue. The above results confirmed that the compatibility of Astragalus membranaceus and Angelica sinensis had antioxidant effects by re-gulating Keap1/Nrf2/PGC-1α, and the Astragalus membranaceus/Angelica sinensis ratio of 2∶1 and 5∶1 had stronger antioxidant effect and significantly reduced TP-induced hepatoto-xicity.

ABSTRACT

SUMMARY OBJECTIVE: Disease-related skeletal muscle loss is highly prevalent among patients with Crohn's disease. Low skeletal muscle mass lead to disability and interventions to prevent skeletal mass loss as an effective strategy to prevent disability. The aim of this article was to identify the factor associated with skeletal muscle loss of Crohn's disease and seek for management target for the prevention of sarcopenia-related disability. METHODS: Patients with Crohn's disease were divided into low and normal skeletal muscle mass groups based on L3 skeletal muscle index using abdominal CT scans. The clinical and laboratory parameters and colonoscopy were compared between the two groups. Univariate and multivariate regression logistic models were built to identify the prognostic markers of Crohn's disease-associated muscle loss. RESULTS: A total of 191 Crohn's disease patients were enrolled in this study, of whom 116 (60.73%) were detected to have low L3 skeletal muscle index, including 71 (68.26%) males. The multivariate logistic regression analysis showed that age (OR: 1.031, 95%CI: 1.006-1.057), female gender (OR: 2.939, 95%CI: 1.386-6.233), disease duration (OR: 0.988, 95%CI: 0.980-0.996), endoscopic disease activity (simple endoscopic score for Crohn's disease) (OR: 0.923, 95%CI: 0.855-0.996), serum albumin (OR: 1.079, 95%CI: 1.009-1.154), and serum creatinine (OR: 1.037, 95%CI: 1.011-1.063) were associated with L3 skeletal muscle index among Crohn's disease patients. CONCLUSION: The gender, age, and duration of disease were uncontrollable factors associated with muscle loss of Crohn's disease. The treatment target of mucosal healing and improved nutritional status may be beneficial for maintaining muscle mass among Crohn's disease patients.

ABSTRACT

ObjectiveTo study the in vitro anti-hepatocarcinoma HepG2 cell mechanism of Jaranol. MethodThe methyl thiazolyl tetrazolium (MTT) assay was employed to examine the inhibition of Jaranol (0, 5, 10, 25, 50, 100, 150, 300 μmol·L-1) on HepG2 cell proliferation at different time (24 , 48 , 72 h), annexin V-fluorescein isothiocyante/propidium iodide (Annexin V-FITC/PI) kit to detect the effect of Jaranol (0, 3, 15, 75 μmol·L-1) on HepG2 cell apoptosis, and Western blot to determine the influence of Jaranol on the expression of B-cell lymphoma 2 (Bcl-2) and Bcl-2-associated X protein (Bax) in HepG2 cells. Transcriptome sequencing was performed to analyze the differential expression of genes and changes of related signaling pathways after the treatment of HepG2 cells with Jaranol (15 μmol·L-1). Real-time PCR was carried out to verify the relative mRNA content of differential genes [TEK, platelet-derived growth factor receptor α (PDGFRA), spleen tyrosine kinase (SYK), phosphatidylinositol-4,5-bisphosphate 3-kinase catalytic subunit gamma (PIK3CG), Janus kinase 3 (JAK3), membrane-associated guanylate kinase inverted 2 (MAGI2)]. ResultCompared with the blank group, Jaranol decreased HepG2 proliferation (P<0.05, P<0.01), increased apoptosis rate of HepG2 cells (P<0.05, P<0.01), raised Bax expression (P<0.05, P<0.01), and reduced Bcl-2 expression (P<0.05, P<0.01). Transcriptome sequencing yielded 59 000 regulated genes, 125 of which showed significantly different expression, with 47 up-regulated and 74 down-regulated. Kyoto Encyclopedia of Genes and Genomes (KEGG) analysis showed that the differential genes related to apoptosis in the phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt) signaling pathway changed significantly after drug addition. The mRNA expression of TEK, PDGFRA, SYK, PIK3CG, JAK3, and MAGI2 decreased in Jaranol (15 μmol·L-1) group compared with that in the control group (P<0.05). ConclusionIn vitro cytological experiment verified that Jaranol inhibited the proliferation of HepG2 cells and promoted the apoptosis, possibly by influencing the expression of some differential genes in the PI3K/Akt signaling pathway. The result lays an experimental basis for the follow-up study of the anti-tumor effect of Jaranol, and the further development and utilization of flavonoids.

ObjectiveTo study the effect of flower removal on the content of three alkaloids in different parts of Fritillaria thunbergii from different regions and at different growth stages. MethodThe content of peiminine, peimine, and peimisine in the bulb, root, stem, and leaf of F. thunbergii after flower removal and with flower un-removed at different growth stages and in different regions were determined simultaneously by ultra-performance liquid chromatography-evaporative light scattering detection (UPLC-ELSD) method. The UPLC was conducted on ACQUITY UPLC BEH C18 column (2.1 mm × 150 mm, 1.7 μm) with the mobile phase of 0.02% triethylamine aqueous solution (A) and methanol (B)elution gradient(0-2 min, 45%A; 2-5 min, 45%-25%A; 5-7 min, 25%A; 7-17 min, 25%-10%A; 17-20 min, 10%A), flow velocity of 0.20 mL·min-1, column temperature 35 °C, sample room temperature of 20 °C, and injection volume of 3 µL. The ELSD was carried out at drift tube temperature 45 °C and with the sprayer parameter of 40%. ResultThe flower removal significantly increased the yield of F. thunbergii. At the budding stage, the alkaloid content in the bulb of F. thunbergii from Ningbo in Zhejiang, Pan'an in Zhejiang, and Nantong in Jiangsu after flower removal were significantly higher than that of flowering un-removal treatment, while it showed no significant difference between the flower removal and un-removal treatments for the samples from Fengjie in Chongqing. At the flowering stage, the alkaloid content in the bulb of F. thunbergii from Nantong in Jiangsu after flower removal was significantly higher than that of flower un-removal treatment, while it showed an opposite trend for the samples from Pan'an in Zhejiang and Fengjie in Chongqing and had no significant difference between the two treatments for the samples from Ningbo in Zhejiang. At the bulb expansion stage, the alkaloid content in the bulb of F. thunbergii from Ningbo in Zhejiang and Pan’an in Zhejiang after flower removal were significantly higher than that of flower un-removal treatment, which was opposite for the samples from Nantong in Jiangsu and had no significant difference between the treatments for the samples from Fengjie in Chongqing. At the harvest stage, except for the samples from Pan'an in Zhejiang, the samples from the rest 3 regions showed decreased alkaloid content in the bulb after flower removal compared with that of flower un-removal treatment. The alkaloid content in the leaf was higher than that in the bulb of F. thunbergii at all growth stages and from different origins. ConclusionFlower removal can increase the yield of F. thunbergii. The alkaloid content in the bulb of F. thunbergii with flower removed was higher than that with flower un-removed at the budding stage, while this trend was reversed at the harvest stage. Both the yield and the alkaloid content of F. thunbergii from Pan'an in Zhejiang were increased by flower removal. The above-ground part of F. thunbergii has a potential development value.

Multiple medications can increase the risk of adverse drug reactions and adverse events in the elderly.Simplification of multiple medications is a systematic process of weighing the benefits and harms of drug use under the supervision of medical personnel, combining the patient's medical condition, current clinical status and personal preferences and tapering or stopping certain drugs while ensuring efficacy.Simplification of multiple prescription drugs can reduce falls, shorten the lengths of hospitalization, and reduce emergency visits for patients.This article gives a brief overview of simplification of multiple prescription drugs for the elderly and introduces the current available tools to do so.

Aim To investigate the protective effect of sinomenine(SIN)against dibutyltin(DBT)induced injury in HL02 cells and explore the potential mechanism.Methods HL02 cells were cultured and divided into control,model and SIN-treated groups.Cell proliferation was detected by MTT method.Cell morphology was observed.Cell apoptosis was detected by Acridine orange/ethidium bromide(AO/EB)fluorescent staining and Annexin V-FITC/PI double staining.Meanwhile,intracellular reactive oxygen species(ROS)concentration was detected by DCFH-DA staining.Mitochondrial membrane potential(MMP)was tested by JC-1 dye.Moreover,the mRNA expression of apoptosis-related proteins was detected by qRT-PCR,and the protein expression of Bcl-2,Bax,caspase-9,cleaved-caspase-3 were measured via Western blot.Results The pretreatment with SIN increased the cell viability and decreased morphological changes induced by DBT in a dose-dependent manner.Meanwhile,cell apoptotic rates and intracellular ROS decreased,and the loss of MMP was partially restored.Compared to DBT-treated group,SIN treatment could increase the mRNA levels of Bcl-2,Bcl-xL and decrease the mRNA levels of Bax,Bad,cytochrome-c,Apaf-1,caspase-9 and caspase-3.Furthermore,SIN could significantly up-regulated the DBT-induced decrease in Bcl-2/Bax ratio,and down-regulated the DBT-induced over-expressions of caspase-9 and cleaved-caspase-3.Conclusions SIN could protect HL02 cells against DBT-induced cell injury,which is related to the inhibition of ROS-mediated mitochondrial apoptosis.

ABSTRACT

Objective:To study the effect of physical therapy and heel sound feedback on lower limbs motor function, mobility and activities of daily living (ADL) for stroke patients based on International Classification of Functioning, Disability and Health (ICF) core set. Methods:From April, 2018 to May, 2020, 113 stroke patients with motor dysfunction were divided into ischemia group (n = 67) and hemorrhagia group (n = 46) according to the cause of stroke. They received physical therapy for lower limbs and heel sound feedback for eight weeks, and assessed with ICF core set for stroke-gait, Fugl-Meyer Assessment-Lower Extremities (FMA-LE), Timed 'Up and Go' Test (TUGT), and modified Barthel index (MBI) before and after intervention. Results:The main effect of time was significant for qualifiers of ICF core set for stroke-gait, the scores of FMA-LE and MBI, and TUGT time (F > 100.59, P < 0.001), and it improved time by time as Post Hoc test. The main effect of groups was not significant (F < 2.29, P > 0.05), nor as Post Hoc test. The interactive effect between time and groups was significant for TUGT time (F = 6.45, P < 0.01), perhaps improved more in the hemorrhagia group, however, the interactive effect was not significant for the others. Conclusion:Physical therapy and heel sound feedback can improve motor function of lower limb, mobility and ADL for stroke patients.

ABSTRACT

Objective:To analyze the main factors affecting the <italic>Ziziphus jujuba</italic> distribution and expand the understanding of its distribution and the corresponding influencing factors by comparing the distribution sites of <italic>Z. jujuba</italic> predicted by models with those recorded in the literature. Method:More than 200 distribution sites of <italic>Z. jujuba</italic> accompanied by 55 environmental factors were obtained from literature and specimen review. The environmental factors that affect the distribution of <italic>Z. jujuba</italic> were explored by maximum entropy (MaxEnt) model, and the potential distribution areas of <italic>Z. jujuba</italic> in China were analyzed by ArcGIS, followed by the verification of the main environmental factors using receiver-operating characteristic (ROC) curve and Jackknife method. Result:The area under the curve (AUC) values for the test data and training data were both greater than 0.9, which perfectly satisfied the standard, indicating that the research results were accurate and reliable. Conclusion:The annual average temperature, the average temperature in May, the average temperature in the warmest season, vegetation type, soil type, average temperature in June, average temperature in September, and average temperature in August are proved to be the main environmental factors affecting the distribution of <italic>Z. jujuba</italic>, which can be found almost all over China, except for Heilongjiang and Tibet. <italic>Z. jujuba</italic> is most suitable to be planted in southeastern Sichuan, Chongqing, southern Gansu, Ningxia, most areas of central Shaanxi, eastern and southwestern Shanxi, Henan, eastern and northern Hubei, northern and eastern Anhui, Shandong, Hebei, Beijing, Tianjin, western Liaoning, and Zhejiang. As revealed by literature review, the most suitable growing areas of <italic>Z. jujuba</italic> are southeastern Sichuan, central Shaanxi, southwestern Shanxi, western and northern Henan, Shandong, and southwestern and eastern Hebei.

ABSTRACT

Objective:To observe the effect of modified Bazhentang on the nutritional status and immune function of patients with Qi and blood deficiency syndrome in neoadjuvant chemotherapy (NAC) for gastric cancer. Method:One hundred and ten patients were randomly divided into observation group and control group with 55 cases each. Both groups accepted FOLFOX6 protocol. Patients in control group took Jianpi Shengxue tablets orally, 3 tablets/time, 3 times/day. Patients in observation group received modified Bazhentang, 1 dose/day. The course of treatment was six weeks in both groups. Before and after treatment, scores were graded according to patient generated-subjective global assessment (PG-SGA), Qi and blood deficiency syndrome, and the Revised Piper Fatigue Scale (PFS-R). Levels of serum total protein (TB), albumin (ALB), prealbumin (PAB), CD4<sup>+</sup>, CD8<sup>+</sup>, helper T lymphocyte 17 (Th17), regulatory T cell (Treg), immunoglobulin G (IgG), IgM, and IgA were detected before and after therapy. Body mass index (BMI) and fat free mass index (FFMI) were measured before and after treatment. Weight loss was recorded, and the acute or subacute toxicity of anticancer drugs was evaluated. Result:The degree of malnutrition in the observation group was lower than that in the control group (<italic>Z</italic>=2.401,<italic>P</italic><0.01). The levels of TB, ALB and PAB in the observation group were higher than those in the control group (<italic>P</italic><0.01). The CD4<sup>+</sup>, Treg and CD4<sup>+</sup>/CD8<sup>+</sup> levels in the observation group were higher than those in the control group (<italic>P</italic><0.01). The CD8<sup>+</sup>, Th17 and Th17/Treg levels were lower than those in the control group (<italic>P</italic><0.01). Besides, the levels of IgM and IgA in the observation group were higher than those in the control group (<italic>P</italic><0.01). The PG-SGA score and weight loss in the observation group were lower than those in the control group (<italic>P</italic><0.01). The BMI and FFMI data of the observation group were higher than those of the control group (<italic>P</italic><0.05). The scores of PFS-R and Qi-blood deficiency syndrome were lower than those of the control group (<italic>P</italic><0.01). The incidence of nausea and vomiting in the observation group was 45.45% (25/55), lower than 65.45% (36/55) in the control group (<italic>χ</italic><sup>2</sup>=4.452,<italic>P</italic><0.05). Conclusion:Modified Bazhentang can be used to assist gastric cancer patients with NAC, which can improve nutritional status and immune function, promote immune balance, reduce clinical symptoms and fatigue, and reduce chemotherapy toxicity and side effects, so it is worthy of clinical use.

ABSTRACT

Atherosclerosis is one of the most common diseases that threaten human health. How to effectively inhibit atherosclerosis, extend the survival time and improve the quality of life has become one of the most urgent issues to be solved clinically. Mongolian medicine, with a long history of managing human diseases, is an important part in traditional Chinese medicine (TCM) and has distinct ethnic characteristics. It has been gradually formed and developed by absorbing some theories of Tibetan medicine, Indian medicine and relevant knowledge of TCM. Mongolian medicine has many advantages, including but not limited to, low toxicity and diverse structure. However, the action mechanism of Mongolian medicine in preventing and managing atherosclerosis has yet to be fully clarified, which has been a major obstacle for further promotion and application of Mongolian medicine in clinical settings. In this review, the up-to-date research findings on Mongolian medicine were collected, analyzed and summarized, and the anti-atherogenic action mechanism of Mongolian medicine were reviewed from the aspects of anti-inflammatory, lipid-lowering, anti-oxidative stress, vascular endothelial cell protection, and inhibition of vascular smooth muscle cell proliferation and migration.

ABSTRACT

Caveolin-1 (CAV-1) is related to inflammation, oxidative damage, and immunity. In order to obtain a series of dibenzoylmethane halophenols with strong anti-inflammatory and antioxidant effects targeting CAV-1, twenty-nine target compounds were therefore synthesized by Baker-Ventaraman rearrangement and demethylation reaction, starting from the substituted benzoyl chloride and o-hydroxyacetophenone, and their interactions with CAV-1 were investigated by BLI technique. Their in vitro anti-inflammatory and antioxidant properties were also evaluated. The results showed that compounds A6, A17, A18, and A29 not only specifically bind to CAV-1, but also present strong anti-inflammatory and antioxidant effects. These results suggest that this class of compounds can affect the signaling pathways related to inflammation and oxidative stress by directly acting on CAV-1. In particular, these compounds exhibit the most significantly inhibitory effects on IL-1β and COX-2 release. IL-1β plays a key regulatory role in the development of arthritis. Therefore, it is worth expecting for the application of such compounds in the prevention and treatment of arthritis.

ABSTRACT

BACKGROUND@#Antiviral therapy can lead to regression of fibrosis in chronic hepatitis B (CHB), but it has a limited effect on cirrhosis. Chinese medicines (CMs), particularly Fuzheng Huayu Tablet (, FZHY), have an antifibrotic effect in patients with CHB.@*OBJECTIVE@#To observe the safety and efficacy of adjunctive FZHY in patients with hepatitis B virus (HBV) cirrhosis, this study was designed as a randomized, placebo-controlled, double-blind, parallel assignment, multicenter trial at 20 centers in China. The total 700 naive patients will be enrolled with compensate cirrhosis due to HBV, and randomly assigned into 2 groups, receiving entecavir (0.5 mg, daily) and FZHY placebo (1.6 g, 3 times a day), or entecavir (0.5 mg, daily) and FZHY (1.6 g, 3 times a day), respectively. The primary endpoint was histological improvement at week 48. The secondary outcome is the decline values of liver fibrosis using the noninvasive methods from baseline to week 48 in each arm of the study. Adverse events such as stomach upset, headache, fatigue, dizziness, nausea will be strictly recorded.@*DISCUSSION@#Through this study, we hope to generate a solid evidence for the therapeutic strategy of HBV cirrhosis with a combination of anti-viral such as ETV and anti-fibrotic herbal product such as FZHY. ABSTRACT

The present study reports a rare case of Taenia saginata infection, which was initially diagnosed as acute cholecystitis in a Tibetan patient at the Qinghai-Tibetan Plateau pastoral area, China. A 45-year-old female was initially diagnosed with acute cholecystitis at a hospital in China. She had a slight fever, weight loss and constipation and complained of pain in the upper abdomen and left back areas. Increase of monocyte, eosinophil and basophil levels were shown. Taenia sp. eggs were detected in a fecal examination. An adult tapeworm approximately 146 cm in length, whitish-yellow color, was collected from the patient after treatment with traditional Chinese medicine. The adult tapeworm had a scolex and proglottids with genital pores. The scolex was rectangular shape with 4 suckers and rostellum without hooklet. The cox1 gene sequence shared 99.5-99.8% homology with that of T. saginata from other regions in China. The patient was diagnosed finally infected with T. saginata by morphological and molecular charateristics.

ABSTRACT

@#AIM:To evaluate the efficacy and safety of dexamethasone intravitreal implant for the treatment of noninfectious refractory uveitis and analyze its influence on application and dose of systemic corticosteroids and immunosuppressive agent.<p>METHODS: This was a retrospective, case series study conducted between January 2018 and September 2019, the patients with noninfectious refractory uveitis and treated with intravitreal implantation of dexamethasone intravitreal implant were included. The best corrected visual acuity(BCVA), central retinal thickness(CRT), vitreous haze score, and intraocular pressure(IOP)at 1mo, 2mo, 4mo and 6mo after the implantation of dexamethasone intravitreal implant were compared with the baseline. Meanwhile, the dosage of corticosteroids and immunosuppressive agent used before and after the implantation of dexamethasone was also evaluated. The data was displayed as mean±standard deviation and analyzed using the paired <i>t</i>-test.<p>RESULTS: A total of 19 patients(21 eyes)were included in the present study. The mean baseline BCVA of the included patients was 0.671±0.469, while the mean CRT was 369.667±177.100μm, and the mean intraocular pressure(IOP)was 14.252±3.181mmHg. One month, 2mo, 4mo and 6mo after the injection of Ozrudex, BCVA was significantly improved compared with the baseline(<i>P</i><0.05), while the CRT and vitreous haze score were significantly reduced(<i>P</i><0.05,<i> P</i><0.05). During the follow-up period, IOP of several patients significantly increased, but normal IOP can be observed at the end of the follow-up for all included patients. At baseline, 6 patients(6 eyes)were treated with systemic corticosteroids with an average dosage of 35.8mg/d, while only 2 patients were treated with systemic corticosteroids at the end of the 6mo follow-up(5mg/d). Apart from 3 eyes with Behcet's disease which showed recurrence of macular edema between 4-6mo, no other patient underwent that. <p>CONCLUSION: Dexamethasone intravitreal implantis considered to be a safe and effective approach for non-infectious refractory uveitis which can reduce systemic medication, with significant improvement of BCVA, reduction of CRT, vitreous haze score and the dosage of systemic corticosteroids.
