ABSTRACT

OBJECTIVE To explore the value of providing pharmaceutical service related to risdiplam in direct-to-patient (DTP) pharmacies. METHODS The follow-up data of spinal muscular atrophy (SMA) patients who purchased and used risdiplam from Shangyao Yunjiankang Yiyao Pharmacy (Shanghai) Co., Ltd. from May 2021 to January 2023 were collected. The medication information, therapeutic efficacy and the occurrence of adverse events were retrospectively analyzed. RESULTS A total of 42 prescriptions were checked by pharmacists in the DTP pharmacies, and 7 prescriptions were found to be unreasonable (16.7%, 7/42), which were corrected after the timely intervention. During the follow-up management, pharmacists replied to 4 patients (9.5%, 4/42) regarding medication consultation about medication requirements and adverse events. Two patients with type Ⅰ SMA experienced adverse events: one of them presented with fever and the other presented with skin dryness with darkening. Both of them were grade Ⅰ toxic reactions and generally did not require clinical treatment. Considering that the patient sustained low-grade fever for a long time, the pharmacist suggested symptomatic treatment under the guidance of the doctor. CONCLUSIONS Pharmacists in DTP pharmacies conducting follow-up management of risdiplam use for rare disease SMA patients can help promote rational, standardized medication for patients.

ABSTRACT

Spinal nerve injury causes mechanical allodynia and structural imbalance of neurotransmission, which were typically associated with calcium overload. Storeoperated calcium entry (SOCE) is considered crucial elements-mediating intracellular calcium homeostasis, ion channel activity, and synaptic plasticity. However, the underlying mechanism of SOCE in mediating neuronal transmitter release and synaptic transmission remains ambiguous in neuropathic pain. Neuropathic rats were operated by spinal nerve ligations. Neurotransmissions were assessed by whole-cell recording in substantia gelatinosa. Immunofluorescence staining of STIM1 with neuronal and glial biomarkers in the spinal dorsal horn. The endoplasmic reticulum stress level was estimated from qRT-PCR. Intrathecal injection of SOCE antagonist SKF96365 dose-dependently alleviated mechanical allodynia in ipsilateral hind paws of neuropathic rats with ED 50 of 18 μg. Immunofluorescence staining demonstrated that STIM1 was specifically and significantly expressed in neurons but not astrocytes and microglia in the spinal dorsal horn. Bath application of SKF96365 inhibited enhanced miniature excitatory postsynaptic currents in a dosage-dependent manner without affecting miniature inhibitory postsynaptic currents. Mal-adaption of SOCE was commonly related to endoplasmic reticulum (ER) stress in the central nervous system. SKF96365 markedly suppressed ER stress levels by alleviating mRNA expression of C/ EBP homologous protein and heat shock protein 70 in neuropathic rats. Our findings suggested that nerve injury might promote SOCE-mediated calcium levels, resulting in long-term imbalance of spinal synaptic transmission and behavioral sensitization, SKF96365 produces antinociception by alleviating glutamatergic transmission and ER stress. This work demonstrated the involvement of SOCE in neuropathic pain, implying that SOCE might be a potential target for pain management.

ABSTRACT

OBJECTIVE:To investigate antitumor pharmaceutical care ability a nd the current status of direct to patient (DTP) pharmacy in China ,and to provide reference for building a quality evaluation system of DTP pharmacy which focuses on the pharmaceutical care. METHODS :From Sept. to Dec. 2019, an electronic questionnaire survey was conducted amongpharmacists of all of 126 listed DTP pharmacies and involved wang_star@163.com education background ,professional knowledge sources andpharmaceutical care ability (such as prescription checking and E-mail:dujoan-88@163.com dispensing,medication consultation and guidance ,medication management,follow-up),pharmaceutical knowledge of antitumor medicine ,etc. DTP pharmacies in C hina were inspected on site , and their pharmaceutical care management were evaluated ,and impression of on-site inspection were discribed with questionnaire survey. The current situation of oncology pharmaceutical care ability of DTP pharmacy in China was evaluated and corresponding suggestions were put forward. RESULTS :A total of 400 questionnaires were sent out ,and 392 valid questionnaires were collected with recovery rate of 98.0%. The cancer pharmacists in DTP pharmacy were mainly from East China ,mainly in Jiangxi (73 persons,18.62%),Anhui(68 persons,17.35%). The pharmacists were mainly junior college or below (229 persons,58.42%), followed by undergraduate (158 persons,40.31%). The working years were more than 5 years(241 persons,61.48%). The majors were mostly pharmaceutics (123 persons,31.38%),followed by pharmacy (including pharmacology )(81 persons, 20.66%). The regular knowledge sources of pharmacists in DTP pharmacy included learning materials (302 person times , 77.04%),textbooks(250 person times ,63.78%)and Wechat push (206 person times ,52.55%);the latest knowledge sources included training courses (347 person times ,88.52%),Wechat push (211 person times ,56.38%)and academic conferences (162 person times ,41.33%). The expected training methods included online video courses (344 person times ,87.76%)and offline courses(248 person times ,63.27%). In terms of prescription checking and dispensing ,there were mainly problems such as incomplete medical documents and materials ;82.91% and 64.54% of pharmacists needed routine medical records and discharge summary,and 91.58% of pharmacists took the doctor ’s handwritten prescription as the basis for dispensing new anti-tumor drugs ; pharmacists paid less attention to allergy history ,genetic history and ADR in the field of drug consultation and guidance. According to DTP pharmacists ,the difficulties in the process of drug treatment management included patient ’s ADR monitoring (239 person times,60.97%),evaluating the rationality of drug use (222 person times ,56.63%),making a complete drug use record (219 person times ,55.87%). The main follow-up visits of pharmacists were telephone (334 person times ,85.20%)and dispensing site (313 person times ,79.85%). And 333 pharmacists(84.95%)thought it was necessary to make an appointment for dispensing ;the score rates of professional knowledge examination of antitumor pharmacy for the respondents were 40.56%-84.69%. In the aspect of pharmaceutical care management ,the main points of deduction were drug evaluation ,content and process of drug treatment management,drug follow-up and record ,and the proportion of deduction was 93%,86% and 64% respectively. In the on-site inspection,it was found that the pharmacist had a single source of working knowledge ,did not fully grasp the contents of the evaluation forms such as ADR classification standard ,and prescription checking did not meet the requirements. CONCLUSIONS : DTP pharmacy in China develops rapidly under the guidance of relevant policies ,but its pharmaceutical care is still in the basic stage,which is far behind the requirements of providing whole-course pharmaceutical care for patients. Cardiac surgery with cardiopulmonary bypass is prone to severe bleeding and often requires blood product transfusion. However, blood products are limited, expensive, and can cause adverse reactions in recipients. Therefore, in recent years, increased research has been conducted on the use of platelet-rich plasmapheresis in such types of cardiac surgery, to reduce the destruction of the blood from cardiopulmonary bypass and to prevent blood loss.

ABSTRACT

<p><b>OBJECTIVE</b>To assess the beneficial and adverse effects of Wendan Decoction (温胆汤, WDD) for the treatment of schizophrenia.</p><p><b>METHODS</b>Five electronic databases were searched until May 2014, including the Chinese National Knowledge Infrastructure, the Chinese Biomedical Literature Database, the Chinese Scientist Journal Database, PubMed, and the Cochrane Central Register of Controlled Trials in the Cochrane Library. The randomized controlled trials (RCTs) testing WDD against placebo, antipsychotic drugs, or WDD combined with antipsychotic drugs against antipsychotic drugs alone were included. Study selection, data extraction, quality assessment, and data analyses were conducted according to the Cochrane standards.</p><p><b>RESULTS</b>Thirteen RCTs (involving 1,174 patients) were included and the methodological quality was evaluated as generally low. The pooled results showed that WDD combined with antipsychotic drugs were more effective in clinical comprehensive effect, Positive and Negative Syndrome Scale (PANSS) scores and Brief Psychiatric Rating Scale scores compared with antipsychotic drugs alone. However, WDD had less effectiveness compared with antipsychotics in clinical comprehensive effect; and WDD was not different from antipsychotic drugs for PANSS scores. The side effects were significantly reduced in the intervention group compared with the control group.</p><p><b>CONCLUSIONS</b>WDD appears to be effective on improving symptoms in patients with schizophrenia. However, due to poor methodological quality in the majority of the included trials, the potential benefit from WDD needs to be confirmed in rigorous trials and the design and reporting of trials should follow the international standards.</p>

ABSTRACT

Objective:To optimize the extraction technology of compound Xiongdan capsules. Methods: The effects of water a-mount, decoction duration and decoction times on the water extraction technology were investigated by orthogonal design using the ex-tract derivation rate and the content of ginsenoside Rg1 , Re and Rb1 as the indices. Results:The optimal water extraction conditions for ginseng and Nidus vespa were as follows:decocting three times with 8-fold of water, and extracting for one hour each time. The extract was condensed until the relative density was 1.20(80-90℃), and then dried and crushed into fine powder. The other herbs were crushed into fine powder and screened by No. 5 meshes. The above powder was blended and well mixed with starch, and then packed into capsules. Conclusion:The optimized water extraction technology is stable, feasible and reproducible, which provide reference for industrial production.

ABSTRACT

Objective:To promote the rational drug use in the patients treated with ambulatory chemotherapy. Methods:A model of integrated pharmaceutical care was established using such service as the real-time prescription examination, configuration standardi-zation, active care and prescription review. Results:The integrated pharmaceutical care could effectively reduce the unreasonable drug use, optimize drug use structure, lower the medical expenses and provide supporting data for clinical decision-making. Conclusion:The development of integrated pharmaceutical care in the patients treated with ambulatory chemotherapy is an important field for phar-macists displaying professional skills, which can promote the rational drug use. The treatment efficacy and life quality of cancer patients can be improved with the cooperation of doctors, nurses and pharmacists.

ABSTRACT

The acupoints and meridians selected in the treatment of depression with acupuncture and moxibustion recorded in the domestic clinical literatures from 2001 to 2011 were summarized and analyzed so as to provide the references for the treatment of depression with acupuncture and moxibustion and discuss the relationship between meridians and mental state. SPSS 17.0 statistical analyzing software was used and the statistical description of enumeration data was applied for statistical analysis. It was found that all of the twelve regular meridians and the eight extraordinary meridians were involved in the treatment of depression with acupuncture and moxibustion, of which the application frequency of the Governor Vessel was the highest and that of the Foot-Taiyang Meridian, Foot-Yangming Meridian, Foot-Shaoyang Meridian and the Conception Vessel were followed. It is concluded that depression is Yin disease treated by regulating Yang meridians and it is found that the Governor Vessel, three foot yang-meridians and the Conception Vessel are closely related to mental disorders.

ABSTRACT

<p><b>BACKGROUND</b>Elevated fibrinogen (Fg) level is a known risk factor for ischemic stroke. There are few clinical trials on oral fibrinogen-depleting therapies for secondary ischemic stroke prevention. We aimed to assess the effects of one-year therapy with oral lumbrokinase enteric-coated capsules on secondary ischemic stroke prevention.</p><p><b>METHODS</b>This is a multicenter, randomized, parallel group and controlled study that began treatment in hospitalized patients with ischemic stroke and continued for 12 months. Patients were randomized to either the control group that received the standard stroke treatment or the fibrinogen-depleting group that received the standard stroke treatment plus enteric-coated lumbrokinase capsules. The NIH Stroke Scale scores (NIHSSs) and plasma Fg level were recorded. The carotid artery intima-media thickness (IMT) and status of plaques were examined through carotid ultrasound examination. Primary outcomes included all-cause mortality, any event of recurrent ischemic stroke/transient ischemic attack (TIA), hemorrhagic stroke, myocardial infarction and angina, and other noncerebral ischemia or hemorrhage. Kaplan-Meier survival analysis and the Long-rank test were used to compare total vascular end point incidence between the two groups. Comparison of median values between two groups was done by the Student t test, one-way analysis of variance (ANOVA), or non-parametric rank sum test.</p><p><b>RESULTS</b>A total of 310 patients were enrolled, 192 patients in the treatment group and 118 patients in the control group. Compared to the control group, the treatment group showed favorable outcomes in the Fg level, carotid IMT, the detection rate of vulnerable plaques, the volume of carotid plaques, NIHSS scores, and incidence of total vascular (6.78% and 2.08%, respectively) and cerebral vascular events (5.93% and 1.04%, respectively) (P < 0.05). In the treatment group, the volume of carotid plaques was significantly related to the carotid IMT, the plaque diameter, width and number (P = 0.000, 0.000, 0.000, 0.022; F = 13.51, 2.52, 11.33, -3.29, but there was a weak correlation with the Fg level (P = 0.056). After 1-year therapy, the incidence of overall vascular end points was reduced by 4.7%.</p><p><b>CONCLUSION</b>Long-term oral fibrinogen-depleting therapy may be beneficial for secondary ischemic stroke prevention.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To elucidate the roles of monocyte chemotactic factors (MCP-1, RANTES and Fractalkine) on the vulnerability of atherosclerotic plaques in patients with stable (SAP) and unstable angina pectoris (UAP).</p><p><b>METHODS</b>Patients with SAP (n = 50) and UAP (n = 50) underwent coronary angiography (CAG) and intravenous ultrasound (IVUS) were included in the study. Monocyte chemotaxis was assayed by the transwell chamber. Concentrations of hs-CRP, MCP-1, RANTES and Fractalkine were measured by Enzyme-linked-immunosorbent assay (ELISA). mRNA expression of MCP-1, RANTES and Fractalkine in the monocytes was detected by RT-PCR.</p><p><b>RESULTS</b>IVUS evidenced soft lipid plaques in 48% UAP patients and in 16% SAP patients (P < 0.05). SAP patients had mainly fibrous and mixed plaques. Plaque burden and vascular remodeling index were significantly higher in UAP patients than in SAP patients (P < 0.01). The averaged number of migrated monocytes in the UAP patients were higher than that in patients with SAP (P < 0.01). Concentration of hs-CRP, MCP-1, RANTES and Fractalkine were significantly higher in UAP patients than those of SAP patients (P < 0.05 or P < 0.01). mRNA expression of MCP-1, RANTES and Fractalkine in patients with UAP was significantly higher than those of SAP patients (P < 0.05).</p><p><b>CONCLUSION</b>Upregulated monocyte chemotactic factors (MCP-1, RANTES and Fractalkine) might promote coronary plaque vulnerability in UAP patients.</p>

ABSTRACT

Objective To estimate the performance of Wood's lamp examination in the diagnosis of superficial cutaneous fungal infections. Methods Totally, 129 patients, who were diagnosed with superficial cutaneous fungal infections according to clinical medical history and signs, were enrolled in this study. Wood's lamp examination of lesions was carried out. Cutaneous samples were obtained from the patients and subjected to microscopic examination and fungal culture. Results Wood's lamp examination was positive in 84% and 85.7% of patients with tinea versicolor and malassezia folliculitis, respectively; among these patients positive for Wood's lamp examination, 92.9% were positive for fungal culture, and 87.5% for microscopic examination. In patients clinically diagnosed with tinea manus and pedis, tinea corporis or tinea cruris, 8.3% were positive for Wood's lamp examination, while 85.4% were positive for fungal examination. There was a high consistency between Wood's lamp examination and fungal examination in patients with tinea versicolor and malassezia folliculitis, but not in those with tinea manus and pedis, tinea corporis or tinea cruris. Conclusions Wood's lamp examination shows a high specificity and sensitivity and is useful in the diagnosis of tinea versicolor and malassezia folliculitis, but seems unapplicable for the diagnosis of tinea manus and pedis, tinea corporis or tinea cruris.

ABSTRACT

Objective To explore the efficacy of low dosage of olanzapine combined with fluoxetine in the treatment of depression.Methods A 8-week study was conducted in 130 patients met the diagnostic criteria for de- pression.Subjects were randomly assigned to two groups:fluoxetine(20mg/d)alone and olanzapine(2.5~5 mg/d) plus fluoxetine(20mg/d).They were evaluated with Hamilton depression scale(HAMD).Hamilton anxiety scale (HAMA)at baseline,the 1 week,2 weeks,4 weeks and 8 weeks subsequently.Results(1)There were significant differences in the total scores and reduction rates of HAMD between two groups in every interview.(2)The combi- nation group had greater reduction in depressive and anxiety symptoms than that in fluoxetine group.(3)The re- sponse rate in combination group was higher than that of fluoxetine group in 1 week,2 weeks and 4 weeks.There were no significant differences in response and remission rate between combination group and fluoxetine group.Con- clusion The combination of olanzapine with fluoxetine demonstrated a rapid,effective antidepressant action.

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the clinical diagnosis of auditory neuropathy by retrospective review of the articles published in Chinese Journals in the past 10 years.</p><p><b>METHODS</b>The key word of "auditory neuropathy" was used to search all of the articles published in Chinese Journals by the Chinese Bio-medicine Literature Data Base and Chinese Journal Net from 1995 to 2004. The selected subjects of auditory function examination, diagnosis standards were analyzed, respectively.</p><p><b>RESULTS</b>Forty-two articles were collected, and 23 articles were clinical researches and case reports. There were 573 patients (1146 ears) in all series, and 480 ears (42%) were accords with the standards of diagnosis of auditory neuropathy. Others were accompanied by cochlea, brain stem and peripheral nerve disorders.</p><p><b>CONCLUSION</b>The clinical manifestation and hearing examination on auditory neuropathy in Chinese journal literatures is inconsistent with the diagnosis standards, moreover, it is necessary to further explore the pathological location and unify the diagnosis standards of auditory neuropathy.</p>

AIM: To investigate the current situation and future trend of use of antineoplastics.METHODS: By using the methods of order of sum of money and order of frequency,the antineoplastics,annually used in Rujin Hospital's tumor ward,were analysed.RESULTS: 30 different kinds of antineoplastics for injection and 12 different kinds of tablets that used to assist the chemotherapy were often used.Some new chemotherapeutants were at the top of the order of sum of money and tablets were often used,too.CONCLUSION: It is important to speed up the development of Chinese domestic chemotherapeutants,cut down the expenses for chemotherapy,steadily improve the rationality and effectiveness of the medicine used.

OBJECTIVE:To discuss the contents and ways of clinic pharmacy work.METHODS:The different focuses of clinical pharmacy work between China and Japan was compared based on the pertinent data obtained from conference,visiting and learning,and self practice.RESULTS & CONCLUSION:Clinic pharmacy work calls for details and pragmatic attitude,and the elementary clinical pharmacy work is much needed by patients,which thus should be carried out in hospitals at all levels.
