ABSTRACT

Objective To make comparison of duration of subarachnoid block with intrathecal ropivacaine between gravidas with diabetes mellitus and non-diabetic pregnancy, to evaluate the sensitivity of parturient with diabetes to ropivacaine. Methods 75 parturients who were presenting for elective cesarean section were randomly divided into pregestational diabetes mellitus group (group P, n=15) , gestational diabetes mellitus group (group G, n=30) and non-diabetic parturients group (group N, n=30). After entering the operating room, parturients were given spinal anesthesia spinal at the L3~4 interspace with 0.5% hyperbaric ropivacaine 3 ml with left lateral decubitus position. To determine the level of sensory block by 10 g monofilament and evaluate the motor block with modified Bromage score. To record the time T6 sensory level was obtained, the onset time of sensory block, motor block, the duration of the motor block and sensory block. Results The time T6 sensory level was obtained of Group P were significantly shortened (P < 0.001). Compared with Group N and Group G, the duration of sensory (P < 0.001) and motor (P < 0.001) block were significantly prolonged. Conclusion Parturients with pregestational diabetes mellitus are more sensitive to 0.5% hyperbaric ropivacaine compared to non-diabetic parturients. Compared with non-diabetic parturients, there are no difference in the sensitivity of parturient with gestational diabetes mellitus to 0.5%hyperbaric ropivacaine.

ABSTRACT

The porous scaffolds for bone tissue engineering were prepared by foam impregnation. The magnesium and aluminum acid phosphates were used as bonder and the hydroxyapatite ((Ca10 (PO4)6(OH)2, HA) powder as raw materials. Scanning electron microscopy (SEM) examination indicated that the 3D interconnected porous structure of the organic foam was replicated well by the scaffolds calcined at high temperature and the structural requirement of tissue engineering was satisfied. XRD analysis showed that the scaffold was composed of HA and Ca7Mg2P6O24 while calcined at 1150 degrees C for shorter time and of (Ca, Mg)3(PO4)2 when the time prolonged to 2 h. There was no peak of CaO found in the scaffolds by XRD. According to the culture in vitro, the scaffold possesses good biocompatibility and certain degree of degradability.

ABSTRACT

To investigate the ability of composite graft of osteobalsts and deproteinized bone-titanium mesh (DPB-TM) scaffold to repair cranial bone defect. 30 rabbits were randomly divided into 3 groups. The passage 3 fetal rabbit osteoblasts were seeded into porous DPB-TM scaffolds at the density of 5 x 10(6) ml(-1) as the experimental group. The same defects were respectively reconstructed by DPB-TM or osteoblasts as the control groups. After 12 weeks, the result was evaluated by three-dimensional computed tomographic scanning, gross inspection, scanning electron microscopy, histological examination and mechanics test, respectively. In the experimental group, bone trabecula was observed to pass the defect and interface was mixed. No demarcation between the region of the bone defect and the normal bone was observed. There was plenty of new bone on the scaffold. Part of the scaffold was absorbed. In view of mechanics, the intensity of artificial bone (18.93+/-1.12 MPa) was higher than that of normal bone (16.96+/-1.60 MPa) (P<0.05). In the control groups, only fibrous tissue was observed in the defect region, there was no new bone formation. The tissue engineering bone constructed by osteoblasts and DPB-TM scaffold can be applied to the repair of bone defect.

ABSTRACT

70% . The patients were mechanically ventilated. VT was set at 10 ml? kg-1 and respiratory rate at 12 bpm. End-tidal PCO2 was maintained at 30-35 mm Hg. End-tidal PCO2, SpO2, ECG and BP were continuously monitored. The respiratory function and mechanics were measured using respiratory monitor CP-100 (BICORE) before and after pneumoperitoneum (intra-abdominal pressure reached 15 mm Hg) and 5,10,15,20,25,30,35 min after vecuronium. The respiratory parameters measured included inspiratory and expiratory VT ( VTi, VTe) , minute ventilation (VE), respiratory rate (RR), peak inspiratory and expiratory flow rate (PIFR,PEFR), PEEP, auto-PEEP, pressure-time product (PTP), inspiratory-time ratio (TI/TTOT ), rate/VT ratio, average airway pressure (PAWM), esophageal pressure (PES), peak inspiratory pressure (PIP), dynamic compliance (CDyn), airway resistance (RAw ) and work of breathing. Results There was no significant difference in the effect of different doses of vecuronium on respiratory function and mechanics including all parameters measured at all time points among the three groups. Mean airway pressure, esophageal pressure peak inspiratory pressure, airway resistance and work of breathing increased significantly but dynamic compliance decreased significantly after CO2 penumoperitoneum. Conclusion Intra-abdominal CO2 insufflation significantly attect respiratory mecnanics. me effect of pneumoperitoneum on respiratory mechanics can not be prevented or attenuated by increasing the dose of muscle relaxant.

ABSTRACT

Objective To investigate the effect of rocuronium of on the function of acetylcholine receptor (AchR)/channel in cone neurons of hippocampal cortex in rats. Methods SD rats of both sexes (2-5 days old) were killed by decapitation. Brain was immediately removed and hippocampal cortex was cut into slices which were incubated in decalcified and magnesium-free artificial cerebro-spinal fluid (CSF) and mechanically dissociated and then digested in artificial CFS containing 2.5% trypsin. Cell suspension was prepared. The AchR/channel function was studied in the absence and presence of various concentrations of rocuronium (0, 0.05, 0.1, 0.2, 0.4mmol? L-1) and/or atropine 0.1 mmol ?L-1 using patch clamp technique. Results (1) The conductance, open probability (P0) and short term open constant (?01) were significantly higher in R-0.05 group than those in the control group (R-0) , but there was no significant difference in long term open constant (?02) , short term close constant( (?c1) and long term close constant(?c2) between the two groups. The conductance, P0 ,?01 ,?02 , ?c1, and ?c2 were significantly longer or higher in R-0.0, 0.2 and 0.4 groups than those in control group. (2) There was no significant difference in the conductance, P0 , ?01 , ?02 , ?c1 , and ?c2, between R-0.5 and control group but those in R-0.0, 0.2 and 0.4 groups were significantly higher or longer than control group, when rocuronium was used with atropine. Conclusions AchR/channel ( muscarinic and nicotinic ) in cone neuron of hippocampal cortex were excited by different concentrations of rocuronium. The results suggest that adverse effect may be induced on CNS by rocuronium in case it permeates the injured blood-brain barrier.
