ABSTRACT

Objective To investigate the expression of TP53, PDGF and EGFR in primary astrocytomas, and analyze their correlation with clinicopathological features and prognosis. Methods We analyzed retrospectively the clinicopathological data of 90 patients with primary astrocytoma. The expressions of TP53, PDGF and EGFR in primary astrocytoma tissue samples were detected by immunohistochemistry. The survival of patients was followed up and Cox regression analysis was used to determine the prognostic factors. Results TP53 was expressed in the nucleus, PDGF and EGFR were expressed in the cytoplasm and cell membrane. The positive expression rates of TP53, PDGF and EGFR were 58.89%, 51.11% and 48.89%, significantly higher than those in normal brain tissues (all P < 0.05); the positive expression rate of TP53 in patients with tumor size ≥3 cm was higher than that in patients with tumor size < 3 cm. The positive expression rates of TP53, PDGF and EGFR in patients with WHO stages Ⅲ-Ⅳ were higher than those in patients with WHO stageⅠ-Ⅱ(all P < 0.05); the survival time of patients with positive expression of TP53, PDGF and EGFR were shorter than those of negative expression (all P < 0.05). Cox regression analysis found that WHO staging, TP53, PDGF and EGFR were all factors influencing the prognosis of primary astrocytomas patients. Conclusion TP53, PDGF and EGFR are highly expressed in primary astrocytomas and closely related to tumor progression. They are factors that affect the prognosis of patients.

ABSTRACT

Objective To explore the expression of REG4 and survivin in colorectal cancer, and to analyze the relationship with clinicopathologic features. Methods The expression of REG4 and survivin in 92 colorectal cancer and corresponding normal mucosa was evaluated by immunohistochemistry combining tissue microarray. The relationship between REG4 and survivin and clinicopathologic parameters were statistically assessed. Results The positive expression of REG4 and survivin in colorectal cancer were 53.3% (49/92) and 64.1% (59/92). The positive rates were 8.6% (8/92) and 3.3% (3/92) in the normal mucosa. The level of both REG4 and survivin in colorectal cancer was significantly higher than that in normal mucosa (P 0.05). REG4 expression was positively associated with survivin (r =0.208, P<0.05). Conclusion REG4 and survivin were upregulated in colorectal carcinoma, and may be involved in the occurrence of colorectal cancer.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the methods and results of imaging guided puncture and drainage in the treatment of otogenic brain abscess.</p><p><b>METHODS</b>Reviewed and analyzed four cases of otogenic brain abscess treated by the technique of imaging guided puncture and drainage from May 2010 to October 2013, all of the four cases were male with age ranged from 21 years old to 59 years old, among whom three cases were under 25 years old. All patients had history of middle ear cholesteatoma and were found single brain abscess. Three cases were cerebrum abscess and one case was cerebellum abscess.</p><p><b>RESULTS</b>The three cases with temporal lobe abscess received skull puncture drainage drilling, and one case with cerebellar abscess used the mastoid cavity puncture drainage. Four cases were punctured successfully in one time with the assistance of image navigation, 6-15 ml of pus was drainged out. After imaging showed pus cavity closure, middle ear surgery was performed to thoroughly remove cholesteatoma. The pus bacteria was cultured, which found three cases infected with staphylococcus aureus, Streptococcus mitis and proteusbacillus vulgaris respectively. The imaging showed brain abscess closed after once puncture drainage, four cases were cured without complications. Postoperative follow-up of one year with good mastoid cavity epithelialization.</p><p><b>CONCLUSION</b>Image guidance assisted puncture can drainage brain abscess with accurate positioning, less trauma, short operation time, and high safety, which could be choosen for otogenic abscess patient.</p>

ABSTRACT

Kaino-sinus syndrome is one new complication of endoscopic sinus surgery. The patient had an operation of endoscopic surgery, because he was suffered from an illness of frontal nasal inverted papilloma again. After operation he was suffering pain in left epicanthal folds and frontal part, and had dry scab in nasal cavity when he breathed cold air. The symptoms could relieve by blocking prenaris. We found he had a big aperture of frontal sinus and dry scab in nasal endoscopic examination. His symptoms disappeared after cleaning and washing accessory nasal cavity by salt water.

ABSTRACT

OBJECTIVE@#Research for surgical methods and experience of sinus fibrous dysplasia treatment by auxiliary nasal endoscopy image navigation.@*METHOD@#Retrospective analysis of the records of 4 patients from March 2006 to December 2011 who were diagnosed as fibrous dysplasia of nasal sinuses, 2 male and 2 female, aged from 19 yrs to 55 yrs and average age was 38 yrs; 3 of them were destroyed of sinus floor, the other who had the bone fiber of sphenoidal sinus. By using 16 multi slice spiral CT and 3D reconstruction, the 4 cases were all treated by drill grinding abnormal fibrous tissue in the nasal sinus cavities with auxiliary nasal endoscopy image navigation, to achieve contour in the bone fiber cavities.@*RESULT@#Lesions of 4 cases were accurately found and grinded under auxiliary nasal endoscopy image navigation. For 3 patients, headache, nasal congestion, facial discomfort and other symptoms have gradually disappeared after operation. However, one patient with sinus floor destroyed suffered facial swelling and pain 3 month after operation, with ineffective conservation treatment outcomes. After CT re-scan, it showed that there were a large number of bone residue in maxillary sinus during the re-operation. The reason was that the surgical cavity was not rinsed off which leaded to maxillary sinus congestion. With the methods of expansion of the maxillary sinus, flushing the cavity, and postoperative nasal and maxillary sinus rinse, symptoms disappeared 1 month later without recurrence.@*CONCLUSION@#Patients with fibrous dysplasia of nasal sinuses are treated thoroughly, securely, and effectually, by drill grinding abnormal fibrous tissue in the nasal sinus cavities with auxiliary nasal endoscopy image navigation, to achieve contour in the bone fiber cavities.

OBJECTIVE@#To investigate the differences of IL-4, IFN-gamma gene promoter methylation of allergic rhinitis patients between Uygur and Han people of Xinjiang.@*METHOD@#Methylation-specific PCR (MSP) detected IL-4, IFN-gamma gene methylation of each of 50 patients with allergic rhinitis in Uygur and Han.@*RESULT@#Complete IL-4 gene promoter methylation rate was 44% (22/50) and 48% (24/50) in Uygur and Han AR patients, un-methylation was 26% (13/50) and 22% (11/50), coexistence rate of methylation and un-methylation was 30% (15/50) and 30% (15/50). Complete IFN-gamma gene promoter methylation rate was 12% (6/50) and 16% (8/50) in Uygur and Han AR patients, un-methylation was 8% (4/50) and 10% (5/50), coexistence rate of methylation and unmethylated was 80% (40/50) and 74% (37/50). Distribution of IL-4 gene methylation between Han and Uygur AR patients had no statistically significant (P > 0.05). Distribution of IFN-gamma gene methylation between Han and Uygur AR patients had no statistically significant (P > 0.05).@*CONCLUSION@#There is no difference of IL-4, IFN-gamma gene methylation in patients between the Han and Uygur.

ABSTRACT

OBJECTIVE@#To investigate the advantages of image guidance system in endoscopic optic nerve decompression of traumatic occlusion optic neuropathy affiliated with cerebrospinal rhinorrhea.@*METHOD@#Retrospective review of 15 traumatic occlusion optic neuropathy affiliated with cerebrospinal rhinorrhea at our department between June 2006 and June 2010. Witch were performed endoscopic optic nerve decompression and cerebrospinal rhinorrhea euplastic by image guidance system.@*RESULT@#After 3 months to 1 year follow-up, All the cases with cerebrospinal rhinorrhea euplastic were successful. Two cases recovered to 0.3-0.6 in visual activity. Two cases to 0.1-0.3. One case was less than 0.1. Two cases could see hand movement and 2 cases had light perception. Total effective rate was 60 percent (9/15).@*CONCLUSION@#Image guidance system combined with endoscopy provides accurate localization and identifies the operative borders and critical anatomical structure of skull base, optic nerve and internal carotid artery,and also clearly indicate their adjacent relations,also decreases surgical invasions and complications. Image guidance system can improve the accuracy and safety, particularly in local anatomic structure due to the trauma caused by changes in the relationship. It is a safety and effective therapy method.

ABSTRACT

Objective:To investigate the surgical treatment of adult Hirschsprung's disease (AHD). Methods: A retrospective clinical analysis was proceeded in 7 cases of AHD treated in our hospital from January 2003 to May 2008. There were 3 males and 4 females with an age ranged from 18 to 53 years,including three cases with general segment type,two cases with short segment type,one case with long segment type and one case with whole segment type. All cases were received the modified Duhamel operation. Results: All cases had satisfactory outcome without serious complications,such as soiling, blind pouch syndrome, fecal incontinence and sexual disorder in male patients. Pelvic hydrops occurred in one case and anastomotic inflammation occurred in another case,and both were cured by intensive therapy. Conclusion: The Modified Duhamel Operation is an effective and safe procedure for AHD. Through this procedure the postoperative recurrence rate is reduced, as well the sexual function and the defecate function are remained .

ABSTRACT

OBJECTIVE To find the best way to evaluate the hearing threshold with the objectivethresholds of auditory brainstem response(ABR) and 40Hz auditory event related potential(40Hz AERP) . METHODS Pure tone audiometry,ABR and 40Hz AERP detection were performed in 120 recipients from whom 86 ears were available to be analyzed. According to the styles of configuration of hearing,we divided the 86 ears into 3 groups. There were 37 ears in flat style group,29 ears in descent style group,20 ears in ascent style group. The values were statistically analyzed with SPSS software. Correlations and multiple linear regression analysis were used in each group separately and three kind of mathematic models were established to estimate the subjective auditory thresholds. Then the models were simplified and tested. At last the models were checked out in 10 new recipients separately. RESULTS The pure tone audiometric threshold had good liner correlations with ABR and 40Hz AERP in three groups(R2=0.932,0.944,0.934) . In the flat style group,the subjective average auditory threshold can be estimated by ABR threshold. The predict mathematic model of the descent style group was 1/3(40HzAERP) +2/3ABR and the model of the ascent style group was 2/3(40HzAERP) +1/3ABR. Mean square deviation test showed equation manners were better than ABR means. CONCLUSION We can predict the subjective average auditory threshold more accurately than ABR only from ABR threshold and 40Hz AERP values with the predict mathematic models in young child and patients difficult to be test with subjective audiometry.

ABSTRACT

OBJECTIVE To investigate the feasibility of determining the maximum comfortabl level by using the acoustically evoked stapedius reflex threshold via cochlear implant. METHODS Eleve cases of Nucleus C124 cochlear implant recipients operated in our hospital were selected(8-39 years old, who can decide TIC value by themselves with the postoperative time ranging from device initiation to 1 year after operation),of those four cases were diagnosed as bilateral large vestibular aqueduct syndrome,1 case as common cavity dysplasia,.neural response telemetry of these five patients with inner ear dysplasia were very high or barely measurable.Real-MCL level and acoustically evoked stapedius reflex threshold under activated condition were measured respectively. The stimulating sound was 110dB of white noise.RESULTS The two group values of ASRT and real-MCL were found to be consistant .There is no significant difference statistically (P>0.05). The thresholds of ASR increase with the increasing of real-MCL at the beginning of device initiation. CONCLUSION The acoustically evoked stapedius reflex threshold may reflect (ASRT) the maximum comfortable stimulation level under activated conditions in patients with cochlear implants.it is simple, reliable and may be used to determine the MCL of child cochlear implanted patients.

ABSTRACT

<p><b>OBJECTIVE</b>To improve the closure rate of large nasal septal perforations.</p><p><b>METHODS</b>Using an endoscope, complex mucoperichondrial/mucoperiosteal flaps on one side and free tissue graft on the other, we designed a procedure to repair large nasal septal perforations.</p><p><b>RESULTS</b>In our series, 8 patients were operated on with this procedure, resulting in complete closure of the perforation and subsequent relief of symptoms.</p><p><b>CONCLUSION</b>This technique may be used as an alter $native for the repair of large nasal septal perforations.</p>
