ABSTRACT

Objective:To study the changes in hemorheology in cirrhotic patients with portal hypertension before and after splenectomy.Methods:The data on cirrhotic patients with portal hypertension and hypersplenism who underwent splenectomy from January 2018 to May 2019 in the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine was analyzed. The hemorheology indexes were monitored during the perioperative period, and portal vein thrombosis was monitored by Color Doppler ultrasound.Results:A total of 30 patients were included, including 18 males and 12 females; aged 12.0-62.0 (31.7±14.0) years. One week after splenectomy, portal vein thrombosis was found in all the patients and it disappeared after heparin treatment. The leukocyte and bilirubin levels significantly improved after splenectomy ( P<0.05). The viscosity of high, medium and low cut blood, platelet and D-dimer significantly increased ( P<0.05). Conclusions:After splenectomy, blood viscosity and platelets of cirrhotic patients with portal hypertension and hypersplenism increased, and portal vein thrombosis formed easily.

ABSTRACT

Objective To evaluate the diagnosis and treatment of transcatheter small bowel radiography in intestinal obstruction.Methods From January 2016 to December 2016,150 cases of adhesive small intestine obstruction were divided into small bowel radiography group and control group.The long-term results were collected after 6-month follow-up.Results First aerofluxus time [(72 ± 5) h vs.(109 ±7) h],defecation [(89 ±8)h vs.(139 ± 17)h],extubation [(81 ± 18)h vs.(105 ± 17)h] and first oral intake [(84 ±6)h vs.(109 ±9)h] in catheter group were shorter compared with control group (all P < 0.05).The accuracy of determining obstruction site were superior in catheter group than control group (92% vs.63%,P < 0.05).Conclusion Endoscopic transnasal ileus tube significantly relieves symptoms in the adhesive intestinal obstruction,the transcatheter small intestinal radiography helps determine the obstruction site.

ABSTRACT

Objective To study the hemodynamic changes before and after splenectomy plus periesophagogastric devascularization in patients with splenic artery steal syndrome (SASS) so as to provide a theoretical basis for the treatment of portal hypertension concomitant with hypersplenism.Methods The database of 30 patients who underwent splenectomy plus devascularization from January 2015 to January 2017 was studied retrospectively.Color Doppler ultrasound was performed to detect hemodynamic changes in the hepatic artery,splenic artery,splenic vein and portal vein in patients with portal hypertension and in healthy controls.The outcomes between preoperative and postoperative biochemical indicators were compared.Results The hemodynamic indexes of splenic artery,splenic vein and portal vein in the study group were significantly higher than those in the control group (P < 0.05).The peak systolic velocity [(35.3 ± 5.1) cm/s vs.(53.1 ±7.0) cm/s] and blood flow [(175.9 ±30.5) ml/min vs.(297.0 ±48.3) ml/min)] of the hepatic artery were significantly lower than those of the control group (P < 0.05).The peak systolic velocity [(60.7 ± 11.9) cm/s vs.(35.3 ±5.1) cm/s] and blood flow [(388.8 ±79.6) ml/min vs.(175.86 ±30.46) ml/min] of the hepatic artery increased significantly after operation (P < 0.05).After splenectomy,the levels of the leucocyte [(7.9 ± 3.8) × 109/L vs.(2.8 ± 1.4) × 109/L)],thrombocyte [(491.3±194.9) × 109/L vs.(47.4 ± 16.0) × 109/L)],bilirubin [(15.0 ± 10.6) μmol/L vs.(24.4±13.8) μmol/L)] and transaminase [(32.94±8.57) U/L vs.(43.37 ±16.59) U/L)] recovered when compared with the preoperative data (P < 0.05).Conclusions SASS was detected in cirrhotic patients who had portal hypertension concomitant with hypersplenism.Splenectomy plus periesophagogastric devascularization were effective and safe for cirrhotic patients with hypersplenism and concomitant esophageal and gastric variceal bleeding.

ABSTRACT

BACKGROUND:MicroRNAs (miRNAs) are widely involved in regulation of physiological processes, such as human development, cel proliferation, differentiation, and apoptosis, angiogenesis and lipid metabolism. MiRNAs also play an important regulating role in the pathological process of femoral head necrosis. At present, the research about the effect of icarin on miRNA expression in glucocorticoid- induced avascular necrosis is stil in the exploratory stage, and the specific targets, possible regulation mechanism and signaling pathway remain unclear. OBJECTIVE:To explore the effect of icarin on miRNA expression of bone microvascular endothelial cels in steroids-induced human femoral head lesionsin vitro. METHODS: Bone microvascular endothelial cels in cancelous bone of the femoral head were isolated and harvested in vitro. Icarin preconditioning preceded establishment of models of glucocorticoid-induced bone microvascular endothelial cel injury. Differential expression profiles and transcriptomes in glucocorticoid and normal groups were tested by miRNA microarrays. The most differentialy expressed miR-23b and miR-339 in microarray analysis were further confirmed by real-time quantitative PCR, Meanwhile the effects of icarin on the expression of miR-23b-5p and miR-339-5p were detected. RESULTS AND CONCLUSION:According to the microarray analysis, one miRNA was up-regulated and four mi RNAs were down-regulated in the glucocorticoid group (fold > 2,P < 0.05). Results of RT-qPCR revealed that miR-23b was down-regulated and miR-339 up-regulated in the glucocorticoid group, which were in agreement with the microarray analysis (P < 0.05). Icarin pretreatment effectively prevented the imbalances of miR-23b expression induced by glucocorticoid (P < 0.01). These findings indicate that Icarin may participate in the pathological process of steroid-induced femoral head necrosis through regulating the expression of miR-23b.

ABSTRACT

Objective To investigate the protective effect of aloe polysaccharide on experimental colitis mucosal. Methods The TNBS-induced experimental rat colitis model was successfully established and were treated by aloe polysaccharide, and enteric-coated tablets with sulfasalazine (SASP). Results DAI score, gross morphology score , histopathologic score and MPO in the model group were significantly higher than that in the normal group (P < 0.01), while those indicators were significantly lower after treatment (P < 0.01), IL-12 and IFN-γ mRNA and protein expressions in the model group were significantly higher than that in the normal group (P < 0.01), and IL-12 and IFN-γ expression levels in treatment group were significantly lower than that in the model group (P < 0.01), IL-4 and IL-13 mRNA and protein expression levels in the model group were significantly lower than that in the normal group (P < 0.01), and IL-4 and IL-13 expression in treatment group was significantly higher than that in the model group (P < 0.01). Conclusion Aloe polysaccharide on experimental colitis in rats has a good protective effect , of which mechanism may be reduced by IFN-γ and IL-12 levels, increased IL-4, IL-13 levels, and correct the imbalance of Thl/Th2.

ABSTRACT

<p><b>BACKGROUND</b>The acetabular teardrop is often used to guide acetabular component placement in total hip arthroplasty (THA). Placing the lower acetabular component aspect at the same level as the lower teardrop edge was assumed to restore the hip center of rotation. Here we radiographically analyzed the relationship between cup center and normal contralateral acetabulum center height on unilateral THA using this placement method.</p><p><b>METHODS</b>A total of 106 unilateral THA cases with normal contralateral acetabula were reviewed and the vertical and horizontal distances in relation to the lower acetabular teardrop edge from both hip joint centers, cup inclination, and anteversion were measured radiographically. The paired t-test was used to compare left and right hip center heights. Scatter plots and Pearson's correlation coefficients were used to evaluate differences in hip center heights, cup anteversion, inclination angles, and medialized cup center distance compared to the contralateral hip joint.</p><p><b>RESULTS</b>Cup center height was significantly greater (P < 0.01) than contralateral hip joint center height (93.4% in the 0-5 mm range, 6.6% >5 mm). There was a weak correlation between hip center height difference and inclination (r = 0.376, P < 0.01) and between difference and anteversion (r = 0.310, P < 0.01) but no correlation between difference and outer cup diameter (r = 0.184, P = 0.058) or difference and medialized cup center distance (r = -0.098, P = 0.318).</p><p><b>CONCLUSIONS</b>Although this method did not exactly replicate anatomic hip center height, the clinical significance of cup center height and anatomic hip center height differences is negligible. This acetabular component placement method has high simplicity, reliability, and stability.</p>

ABSTRACT

Purpose To study the effect of China cobra venom active factor(CCVAF) from China cobra venom on endothelial cells and its mechanism.Methods MTT experiment was adopted to evaluate the effect of CCVAF on bovine arteria pulmonalis vascular endothelial cells(BAVEC).The Eosin-Coomassie brillient blue and rhodamine-phalloidin method was used for actin cytoskeleton.Flow cytometry for [Ca~(2+)]_i and spectrophotometry were used for lactate dehydrogenase(LDH) and nitrogen oxide(NO) levels in cell culture supernatant.Results CCVAF(0.625-20 μg/mL) inhibited the proliferation of BAVEC in dose-dependent manner,and IC50 of CCVAF on BAVEC was 2.45 μg/mL. After CCVAF and BAVEC coincubation, it was showed that regression of intercellular conjunctions and disorder of F-actin distribution occurred. The content of [Ca~(2+)]_i, [LDH] and [NO] increased respectively.Conclusion CCVAF can inhibit BAVEC proliferation and it maybe associated with the change of cytoskeleton and increasing of [Ca~(2+)]_i,[LDH] aod [NO].

ABSTRACT

Aim To develop King cobra antivenom from the egg yolks of immunized hens,and study dynamic expression of IgY in egg yolks.Methods chickens(white Leghorn) were immunized with detoxicated King cobra venom by formaldehyde ,Egg yolk antibody (IgY) was isotated by thiophilic interaction chromatography and identified by SDS-PAGE. Activity of IgY was evaluated by enzyme-linked immunoserbent assay(ELISA) and Double immuodiffusion. Protein was measured using folin-lowry method. Results Specific antivenom could be detected in the yolk laid by the hens 9 d after immunization, At the 60th day after primary immunization ,ELISA titers reached 1∶ 100 000,and 97.5 mg IgY?ml -1 yolk was obtained from thiophilic interaction chromatography. IgY was highly specific, No cross reactivity was observed among IgY and agkistrodon acutus venom and vipera venom ;Little cross reactivity was shown with cobra genus venoms.Conclusion King cobra IgY was obtained and purified from the egg yolks of immunized hens,The dynamic expression of IgY was manitered during the course of immunization,Further investigation is needed.

ABSTRACT

AIM: To observe the mechanism of intracellu la r signal transduction that fraction F of naja naja atra venom inhibits plate let aggregation. METHODS: Tests were divided into six groups: (1) blank group; (2 ) control group and (3)-(6) ADP plus fraction F group (doses of fraction F were 100, 30, 10, 3 mg/L, respectively). Protein tyrosine phosphorylation in platelet s was assayed by Western blotting and platelet aggregation was assayed by nephel omete r. RESULTS: Fraction F significantly inhibited molecular masses (MW ) 76, 66 and 37.5 kD protein tyrosine phosphorylation in platelet that induced by ADP in a dose-dependent manner, in which 30 and 100 mg/L dose group showed ob viously different effects when compared to control group (P
