ABSTRACT

@#Telephone follow-up is one of the important ways to follow up patients. High-quality follow-up can benefit both doctors and patients. However, clinical research-related follow-up is often faced with problems such as time-consuming, laborious and poor patient compliance. The authors belong to a team that has been committed to the study of patient-reported outcomes for a long time. The team has carried out long-term follow-up of symptoms, daily function and postoperative complications of more than 1 000 patients after lung cancer surgery, and accumulated certain experience. In this paper, the experience of telephone follow-up was summarized and discussed with relevant literatures from the aspects of clarifying the purpose of clinical research follow-up, understanding the needs of patients in follow-up, and using follow-up skills.

ABSTRACT

Since the appearance of urethral stent, it has become an alternative treatment for urethral stricture except for urethral reconstructive surgery, and can be used safely in some patients with poor efficacy of urethral reconstructive surgery or unable to receive reconstructive surgery. This article summarizes the urethral stent in the application of bladder neck, prostatic urethra, ball, membrane urethra, penile urethra stricture, and postoperative hypospadias. Most study revealed that urethral stent could significantly relieve symptoms, improve the maximum urinary flow rate, reduce the residual urine volume, improve patients' quality of life, but the long-term effect need more follow-up and research evidence.

ABSTRACT

OBJECTIVE@#To explore a new way to modify the UPPP and to assess the operative efficacy of modified-coblation assisted UPPP (M-CAUP) on patients with severe obstructive sleep apnea-hypopnea syndrome (OSAHS).@*METHOD@#A retrospective analysis was made on surgical therapeutic effect of M-CAUP performed in our hospital.@*INCLUSION CRITERIA@#87 adult inpatients with severe OSAHS were involved in the study whose preoperative AHI were beyond 30/h and LSaO2 were under or equal to 85%. All those had the primary level obstruction within the retropalatal region as determined by PSG and airway pressure fluctuation monitoring (ApneaGraph 200, MRA-Medical Ltd, Gloucestershire UK) . The surgical approach of M-CAUP: (1) bilateral tonsillectomy by using Arthrocare EVac 70 T&A Wand. (2) dissection of velum palatinum space by using Arthrocare EVac 70 T&A Wand, ablation of fat tissue to removed it from the space, avoiding injure levator palatini muscle and tensor palatini muscle and preserving musculus uvula, maintaining the normal structure of oropharynx. (3) ablation of channel of soft palate by using Arthrocare EVac 55 T&A Wand to reduce the volume of soft palate moderately. (4) plasty: suturing the tonsilar fossa to close the space, interrupted suture two sides of palatine arch and uvula. All patients were followed up for six to eighteen months postoperatively and received PSG.@*RESULT@#After M-CAUP, the pharyngeal cavity was enlarged while the basic structure of oropharynx was maintained and no nasopharyngeal refluxing occurred. The subjective symptoms were also improved evidently after operation. After operation, the AHI and Epworth sleepiness scale (ESS) decreased while LSaO2 increased. The reduction of AHI and ESS had statistical significance (P < 0.01), and the rise of LSaO2 also had statistical significance (P < 0.01). The total effective ratio of the operation was 89.7%.@*CONCLUSION@#Despite of various treatment, the operative efficacy of conventional UPPP was almost not ideal on severe OSAHS due to complex pathogeny. It was proved that M-CAUP was an effective surgical treatment of severe OSAHS with less blood loss. The operation was minimally invasive and maintained the normal functions of palatopharynx in principle. It could be applied in clinical practice.

ABSTRACT

OBJECTIVE@#To study the expression of nuclear factor kappa B p65 in hyperlipemia model of mice, and the relationship between hyperlipemia and deaf.@*METHOD@#Twenty mice were divided into two group. The hyperlipemia diet group was established in ten mice,and the normal diet group was served as normal control. Six weeks later, immunohistostaining was used to detected the express of NF-kappa B p65 in all mouse cochlear. ABR threshold was obtained from both normal group and hyperlipemia group.@*RESULT@#Immunoreactivity NF-kappa B p65 in mouse cochlea of hyperlipemia was localized in the organ of Corti, tectorial membrane, stria vascularis, spiral ligament, spiral ganglion and nerve fibers. The NF-kappa B p65 expression was markedly increased in mouse cochlea of hyperlipemia ABR threshold was significant difference between hyperlipemia group mice and control group mice (P < 0.01).@*CONCLUSION@#The expression of NF-kappa B p65 in mouse cochlea can be induced by hyperlipemia. And ABR threshold increased in hyperlipemia group mice. This shows that hyperlipidemia can damage acouesthesia of mice.

ABSTRACT

OBJECTIVE@#It was aimed to investigate the effect of plasma radiofrequency ablation on obstructive sleep apnea hypopnea syndrome.@*METHOD@#Fifty-four patients with OSAHS were treated with plasma radiofrequency ablation on soft palate, tonsil, uvula and root of tongue in light of needs.@*RESULT@#All these cases were reevaluated with PSG by the end of postoperative period lasted for three month. Among them, 6 were cured, 24 had significant therapeutic effect, 19 had effective outcome, and 5 had no effective, with a total effective rate of 90.74%. No serious complications occurred.@*CONCLUSION@#This method was safe and effective, and patients had mild lesion, less bleeding, less painful and the advantages of simple operation.

ABSTRACT

OBJECTIVE@#To investigate the long-term outcome and clinical value of utilizing plasma radio frequency ablation under indirect laryngoscope in treatment of recurrence of adenoidal hypertrophy.@*METHOD@#Fifty patients with recurrence of adenoidal hypertrophy were subjected to the operation designed by our group.@*RESULT@#The period of follow-up was more then 2 years. All patients were free of snore postoperatively; conductive hearing loss was improved as well.@*CONCLUSION@#Operation with plasma radiofrequency ablation had advantages of direct and clear view of surgical area, accurate remove of adenoids, non-bleeding, avoiding damage of normal nasopharyngeal structure structures, minimal invasion, high safety, and little complication, in treatment of recurrence of adenoidal hypertrophy.

ABSTRACT

To investigate the enhancement of immunological activity of CpG ODN by chitosan gene carrier in mice, the effect of lymphocyte proliferation was detected in mice by using MTT, the levels of IgG and cytokines (IL-2 and IL-12) in serum were measured by ELISA and peripheral blood T lymphocyte subsets CD4(+), CD8(+) were analyzed by flow cytometry. Our results showed that spleen lymphocytes isolated from the CS-CpG ODN group of mice showed the strongest proliferation (SI=1.551), and the levels of IgG, IL-2 and IL-12 in serum were higher than those of other groups. Compared with the immunization with CpG ODN, the immunization with CS-CpG ODN gene carrier was more efficient in up-regulating the percentage of CD4(+)T cells and the ratio of CD4(+)/CD8(+) of mice. It was concluded that CS gene carrier of CpG ODN was much more effective in improving immunity of CpG ODN in mice.

ABSTRACT

The relationship between the hypertension and the aging process of hearing organ was investigated. Twenty Wistar 3-month old rats and 20 Wistar 12-month old rats, 20 spontaneously hypertensive rat stroke-prone (SHRSP) 3-month old rats and 20 SHRSP 12-month old rats free of middle ear infections as observed under otomicroscopy, with normal tympanic membrane and auricle reflex, were selected to be divided into two experimental groups and two control groups respectively. The tail artery blood pressure was measured non-invasively. The threshold of auditory brain-stem response (ABR) was measured by Spirit evoked potential meter. The LDH and ChE staining in the inner ear was performed and the optical density was analyzed by the HPIAS analysis system. The results showed that there was no difference in the ABR thresholds, the activities of LDH and ChE between Wistar 3-month old group and SHRSP 3-month old group (P > 0.05). The mean value of ABR threshold and the activities of LDH and ChE in the Wistar 12-month old group at relevant sections were significantly greater than those in the two 3-month old groups (P < 0.05), whereas the mean value of ABR threshold and the activities of LDH and ChE in the SHRSP 12-month old group at relevant sections were significantly higher than those in the 3-month old control group (P < 0.01). It was concluded that presbycusis existed in the Wistar 12-month old group rats. The glycogenosis and the abnormal secretion of neural transmitter were discerned after hypertension. All the above factors may worsen the aging of the hearing system.

ABSTRACT

Objective:To elucidate possible functional subdivisions in the guinea pig accessory olfactory bulb.Method:The guinea pig accessory olfactory bulbs were cut in sagittal slice.Bipolar tungsten electrodes were inserted into anterior or posterior vomeronasal nerve layers and single square-pulses were delivered through the eletrodes to activate afferent fibres.Glass microelectrodes were used to record extracellular field potentials of anterior or posterior external plexiform layers.Result:A single shook of the VNL provoked a characteristic damped oscillatory field potential and the oscillation in the pAOB was more distinct in wave form and longer in duration than those in the aAOB.The stimulation of anterior VNL elicited field potentials exelusively in the anterior region of EPL,whereas shocks to the posterior VNL provoked oscillatory responses only within the posterior EPL.Conclusion:The accessory olfactory bulb in the guinea-pig is distinctly segregated into the anterior and posterior subdivisions and an anatomical boundary exists in both regions.The results suggested that there are at least two different input-output pathways in vomeronasal systems.

ABSTRACT

AIM: To explore the properties of the acetylcholine(ACh)-sensitive potassium channel in type Ⅱ vestibular hair cells(VHCs Ⅱ) in mice saccular macula and the modulation effect of calcium ions.METHODS: Under the whole-cell patch mode,the pharmacology properties of ACh-sensitive potassium channel and the modulation of calcium ions on ACh-sensitive potassium channel were investigated.RESULTS: Following extracellular perfusion of ACh,VHCs Ⅱ displayed a slow and sustained outward current,which was sensitive to tetraethylammonium(TEA,5 mmol/L) and charybdotoxin(CTX,100 nmol/L),but not sensitive to 4-aminopyride(4-AP,15 ?mol/L).ACh-sensitive potassium current was inhibited by intracellular application of ethylene glycol-bis(B-aminoethylether)-N,N,N',N'-tetraacetic-acid(EGTA,5 mmol/L) and extracellular perfusion of Cd2+ and Ni2+,respectively.Intracellular application of heparin(8 g/L) failed to inhibit ACh-sensitive potassium current.CONCLUSION: Extracellular application of ACh activates the big conductance,calcium-dependent potassium current(BK) in VHCs Ⅱ of mice,which is potently modulated by extracellular Ca2+ ions.However,intracellular IP3-dependent Ca2+ ions release mechanism is not involved in the activation of the ACh-sensitive BK channel.
