ABSTRACT

Objective:To clarify the influence and influence paths of stigma on the time of the healthcare-seeking decision in caregivers of elderly patients with dementia, and to provide a theoretical basis for the construction of corresponding humanistic care strategies.Methods:A total of 176 caregivers of elderly patients with dementia who visited the Affiliated Hospital of Xuzhou Medical University and Xuzhou Oriental People ′s Hospital from February 2021 to February 2022 were selected as the study subjects. The General Information Questionnaire, self-designed Scale of Stigma for Caregivers of Senile dementia patients, Multidimensional Scale of Perceived Social Support, self-designed Elderly Dementia Caregivers′ Perceived Barriers Scale for Healthcare-seeking Decision, and self-designed Scale of the Intention to Seek Healthcare for caregivers of senile dementia patients were used in the survey. AMOS 20.0 was used to establish a structural equation model for path analysis. Results:The higher the stigma of caregivers, the longer the time of the healthcare-seeking decision ( β=0.05, P<0.05). Social support, perceived barriers to the healthcare-seeking decision, and the intention to seek healthcare were the mediating variables of caregivers ′ stigma affecting the time of the healthcare-seeking decision, with a total effect of -0.04, 0.14, and 0.36, respectively, and all P<0.05. Conclusions:The stigma in caregivers of senile dementia patients is an important factor affecting the time of the healthcare-seeking decision. By improving mediating factors including social support, perceived barriers to the healthcare-seeking decision, and the intention to seek healthcare, the implementations of targeted humanistic care strategies are expected to help shorten the time of the healthcare-seeking decision.

ABSTRACT

As a novel fungal type Ⅲ polyketide synthase, CsyB from Aspergillus oryzae can sequentially accept one molecular short chain fatty acyl CoA as start unit, one molecular malonyl-CoA and one molecular acetoacetyl-CoA as extend unit to produce the short chain csypyrone B1-3. On the basis of crystal structure of CsyB, a fatty acyl CoA binding tunnel of a length of about 16 Å is located in its active center that is proposed to accept diversified start units. In order to examine the substrate diversity of CsyB, CsyB gene was introduced and expressed in Escherichia coli that contained a number of precursors of long chain fatty acyl CoA in vivo. The results of HPLC revealed that a series of long chain csypyrone derivatives were detected in the recombinant strain in comparison with the control strain. These new csypyrone compounds were preliminarily analyzed by UV-visible spectroscopy and LC-HRMS. Three hydroxylated csypyrones were intensively determined by 1D and 2D NMR experiments, especially the position of the hydroxyl group in these compounds. These results demonstrate that CsyB exhibits a broad substrate specificity, which not only can accept the long chain saturated or unsaturated fatty acyl CoA as substrate, but also accept hydroxylated long chain fatty acyl CoA.

ABSTRACT

Objective To investigate the combination application of PQRST triage formula and self-made abdominal organ projection in the triage of patients with acute abdomen. Methods From November 2016 to February 2017 combination of PQRST triage formula and self- made abdominal organ projection were applied in triage of 99 cases of patients with abdominal pain (the observation group). Traditional triage method using SOAP information collection table were applied in 83 cases of patients with abdominal pain (the control group) from July to October 2016. The accuracy rate of triage were compared between two groups. Results The accuracy rate of nurse triage for acute abdomen was 89.90% (89/99) in the observation group, and 78.31% (65/83) in the control group, the difference was statistically significant (χ2=4.66, P< 0.05). Conclusions The combination use of PQRST formula and the abdominal organ projection image in triage can improve the accuracy of emergency triage.

ABSTRACT

Objective To investigate the incidence and risk factors for surgical site infection (SSI)in clean wound orthopedics surgery.Methods SSI among patients receiving clean wound orthopedic surgery from April 1 to Sep-tember 30 in 2014 were monitored,risk factors for SSI were analyzed.Results A total of 665 patients were moni-tored,14 cases(2.11 %)of SSI occurred.Univariate analysis showed that types of skin,skin preparation modes, and ASA score were risk factors for SSI in clean wound orthopedic surgery (all P <0.05 ).Multivariate logistic analysis revealed that conventional skin preparation,ASA score ≥ grade Ⅱ,and duration of operation ≥90 minutes were independent risk factors for SSI in patients receiving clean wound orthopedic surgery (OR [95%CI ],3.96 [1 .07-14.70],6.45[1 .97-21 .11],and 4.08[1 .35 -12.30]respectively).Conclusion The improved method of skin preparation,grade I ASA score,and shortening duration of operation can reduce the incidence of SSI in clean wound orthopedic surgery,on the basis of improving the autoimmunization of patients,strengthening prevention and control of healthcare-associated infection can reduce the incidence of SSI in clean wound orthopedic surgery.

ABSTRACT

Objective:To investigate the criterion-related validity of Menopause-Specific Quality of Life Questionnaire (MENQOL)-Chinese version and to evaluate the effect of menopausal symptoms on health related quality of life. Methods:This study was a cross-sectional survey. Three communities were randomly chosenin Changsha,and then 340 menopausal women aged 45-55 years were randomly chosen from the documented data of the 3 community health service centers. They were required to fill out 4 questionnaires: demographic questionnaire, MENQOL-Chinese version, Kupperman Index (KI) and World Health Organization Quality of Life (WHQOL)-BREF. Correlation analysis was used to measure the criterion-related validity. Results: MENQOL-Chinese version subscales (vasomotor, psycho-social, sexual and physical) and KI total score were positively correlated (r=0.800,0.751, 0.607, 0.906 respectively); while negatively correlated with WHOQOL-BREF total score (r =-0.694,-0.851,-0.585,-0.873 respectively); MENQOL-Chinese version subscales (vasomotor, psycho-social, sexual and physical) were signiifcantly correlated with WHOQOL-BREF subscales (physical, psychological, social relationship, environment), and the physical domain was the highest among the correlation coeffcients (r=-0.915). Conclusion:MENQOL-Chinese version shows relatively high criterion-related validity compared with KI and WHOQOL-BREF, which can be widely used to measure the quality of life of menopause women in China.

ABSTRACT

Succinic acid is an important C4 platform chemical in the synthesis of many commodity and special chemicals. In the present work, different compounds were evaluated for succinic acid production by Actinobacillus succinogenes GXAS 137. Important parameters were screened by the single factor experiment and Plackeet-Burman design. Subsequently, the highest production of succinic acid was approached by the path of steepest ascent. Then, the optimum values of the parameters were obtained by Box-Behnken design. The results show that the important parameters were glucose, yeast extract and MgCO3 concentrations. The optimum condition was as follows (g/L): glucose 70.00, yeast extract 9.20 and MgCO3 58.10. Succinic acid yield reached 47.64 g/L at the optimal condition. Succinic acid increased by 29.14% than that before the optimization (36.89 g/L). Response surface methodology was proven to be a powerful tool to optimize succinic acid production.

ABSTRACT

We optimized the conditions of mixed fermentation of very high gravity ethanol with cassava flour and sugarcane juice. Based on the single factor experiment, we screened the important parameters for very high gravity ethanol fermentation with cassava flour and sugarcane juice by the Plackeet-burman design. Then, we obtained the optimum values of the important parameters by the orthogonal experiments: the mixing ratio of cassava flour to sugarcane juice, 1:5; initial pH of fermentation, 4.0-4.5; the concentrations of urea and MgSO4, 0.25% and 0.04% (W/W), respectively. Finally, we used a gradient temperature control strategy with the optimized conditions, and ethanol concentration of 17.84% (V/V) and fermentation efficiency of 91.82% were achieved, correspondingly.

ABSTRACT

In order to assist doctors in making the diagnosis of mammographic masses, a method is proposed in this paper. Twenty-two features are extracted from each queried region of interest (ROI). A k-nearest neighbor (KNN) algorithm is used to retrieve similar images from database, and further calculate the mutual information (MI) between the queried image and the images which are in the retrieval results, so as to improve the retrieval performance. Finally, the scheme takes the first nine images with the highest MI scores as the final retrieval results. With the purpose of providing available decision-making information of diagnostic aids, we compare and analyze three calculating methods of decision index. The experiment results show that this method is better than the method using KNN only, and this method improves the accuracy of diagnosis effectively.

ABSTRACT

We optimized the conditions of simultaneous saccharification and fermentation (SSF) from cassava flour into high-concentration ethanol by thermophilic yeast GXASY-10. Based on the single factor experiment, we screened the important parameters by Plackeet-burman design. We used the path of steepest ascent to approach to the biggest region of ethanol production subsequently. Then, we obtained the optimum values of the parameters by Box-Behnken design. The results showed that the important parameters were the liquefaction time, glucosidase dosages and initial concentration of cassava flour (substrate). The optimum technical conditions were as follows: liquefaction time 35 min, glucosidase dosages 1.21 AGU/g substrate and initial substrate concentration 37.62%. Under such optimum conditions, the ethanol yield of 20 L fermentor reached 16.07% (V/W) after 48 h fermentation at 37 degrees C and 100 r/min. The ethanol content increased 33% than that under the original condition.

ABSTRACT

The regions suitable for growing cassava include five provinces in Southern China, with Guangxi alone accounting for over 65% of the total cassava production in the country. In this article, the state-of-the-art development of fuel ethanol production from cassava in China is illustrated by the construction of the cassava fuel ethanol plant with its annual production capacity of 200 000 metric tons. And in the meantime, problems and challenges encountered in the development of China's cassava fuel ethanol are highlighted and the strategies to address them are proposed.
